    Liposarcoma: exploration of clinical prognostic factors for risk based stratification of therapy

    <p>Abstract</p> <p>Background</p> <p>Prognosis and optimal treatment strategies of liposarcoma have not been fully defined. The purpose of this study is to define the distinctive clinical features of liposarcomas by assessing prognostic factors.</p> <p>Methods</p> <p>Between January 1995 and May 2008, 94 liposarcoma patients who underwent surgical resection with curative intent were reviewed.</p> <p>Results</p> <p>Fifty patients (53.2%) presented with well differentiated, 22 (23.4%) myxoid, 15 (16.0%) dedifferentiated, 5 (5.3%) round cell, and 2 (2.1%) pleomorphic histology. With the median 14 cm sized of tumor burden, about half of the cases were located in the retroperitoneum (46.8%). Seventy two (76.6%) patients remained alive with 78.1%, and 67.5% of the 5- and 10-year overall survival (OS) rates, respectively. Low grade liposarcoma (well differentiated and myxoid) had a significantly prolonged OS and disease free survival (DFS) with adjuvant radiotherapy when compared with those without adjuvant radiotherapy (5-year OS, 100% vs 66.3%, P = 0.03; 1-year DFS, 92.9% <it>vs </it>50.0%, respectively, P = 0.04). Independent prognostic factors for OS were histologic variant (P = 0.001; HR, 5.1; 95% CI, 2.0 – 12.9), and margin status (P = 0.005; HR, 4.1; 95% CI, 1.6–10.5). We identified three different risk groups: group 1 (n = 66), no adverse factors; group 2, one or two adverse factors (n = 28). The 5-year OS rate for group 1, and 2 were 91.9%, 45.5%, respectively.</p> <p>Conclusion</p> <p>The histologic subtype, and margin status were independently associated with OS, and adjuvant radiotherapy seems to confer survival benefit in low grade tumors. Our prognostic model for primary liposarcoma demonstrated distinct three groups of patients with good prognostic discrimination.</p

    A pilot study to evaluate the effect of Taeumjowi-tang on obesity in Korean adults: study protocol for a randomised, double-blind, placebo-controlled, multicentre trial

    <p>Abstract</p> <p>Background</p> <p>Obesity, which is described as excessive or abnormal body fat, increases the risk of diet-related diseases. In Korea and around the world, the prevalence of obesity has grown annually from 1998 to 2008. This growth has continued despite various therapeutic efforts. The discovery of new and alternative treatments for obesity should be considered an important priority. Taeumjowi-tang (TJ001), a traditional Korean medicinal extract consisting of eight herbs, is a widely used herbal remedy for obesity in Korea. However, the efficacy and safety of TJ001 have not been fully investigated in a clinical trial. The purpose of this pilot study is to estimate obesity-related parameters and to assess the efficacy and safety of TJ001.</p> <p>Methods</p> <p>Our study is a randomised, double-blind, placebo-controlled, multicentre clinical trial of Taeumjowi-tang (TJ001). For this study, we will recruit obese Korean patients of both sexes, ages 18 to 65 years, from four university hospitals. A total of 104 subjects will be recruited. The participants will receive either 7 g of TJ001 or a placebo three times daily for 12 weeks. The primary end point will be the rate of subjects who lose at least 5% of their baseline body weight. The secondary end points will be changes in body weight, body mass index, waist circumference, hip circumference, waist/hip circumference ratio, lipid profiles, body fat composition, blood pressure, fasting glucose concentration, C-reactive protein and questionnaires related to the quality of life. The outcomes will be measured every 4 weeks. The study period will be 12 weeks and will include a total of five visits with each subject (at screening and at 0, 4, 8 and 12 weeks).</p> <p>Conclusions</p> <p>The results of our study will inform various estimates of TJ001 and will serve as the basis for a larger-scale trial.     Feeding by red-tide dinoflagellates on the cyanobacterium Synechococcus

    We investigated the feeding by 18 red-tide dinoflagellate species on the cyanobacterium Synechococcus sp. We also calculated grazing coefficients by combining the field data on abundances of the dinoflagellates Prorocentrum donghaiense and P. micans and co-occurring Synechococcus spp. with laboratory data on ingestion rates obtained in the present study. All 17 cultured red-tide dinoflagellates tested (Akashiwo sanguinea, Alexandrium catenella, A. minutum, A. tamarense, Cochlodinium polykrikoides, Gonyaulax polygramma, G. spinifera, Gymnodinium catenatum, G, impudicum, Heterocapsa rotundata, H. triquetra, Karenia brevis, Lingulodinium polyedrum, Prorocentrum donghaiense, P. minimum, P. micans, and Scrippsiella trochoidea) were able to ingest Synechococcus. Also, Synechococcus cells were observed inside the protoplasms of P, triestinum cells collected from the coastal waters off Shiwha, western Korea, during red tides dominated by the dinoflagellate in July 2005. When prey concentrations were 1.1 to 2.3 x 10(6) cells ml(-1), the ingestion rates of these cultured red-tide dinoflagellates on Synechococcus sp. (1.0 to 64.2 cells dinoflagellate(-1) h(-1)) generally increased with increasing size of the dinoflagellate predators (equivalent spherical diameters = 5.2 to 38.2 mu m). The ingestion rates of P. donghaiense and P. micans on Synechococcus sp. increased with increasing mean prey concentration, with saturation occurring at a mean prey concentration of approximately 1.1 to 1.4 x 10(6) cells ml(-1). The maximum ingestion and clearance rates of P. micans on Synechococcus sp. (38.2 cells dinoflagellate(-1) h(-1) and 4.3 mu l dinoflagellate(-1) h(-1)) were much higher than those of P. donghaiense on the same prey species (7.7 cells dinoflagellate(-1) h(-1) and 2.6 mu l dinoflagellate(-1) h(-1)). The ingestion rates of red-tide dinoflagellates on Synechococcus sp. were comparable to those of the heterotrophic nanoflagellates and ciliates on Synechococcus spp., so far reported in the literature. The calculated grazing coefficients attributable to small Prorocentrum spp. (R donghaiense + P. minimum) and P. micans on co-occurring Synechococcus spp. were up to 3.6 and 0.15 h(-1), respectively. The results of the present study suggest that red-tide dinoflagellates potentially have a considerable grazing impact on populations of Synechococcus.Y
