Saúde ambiental e saúde do trabalhador na atenção primária à saúde, no SUS: oportunidades e desafios Environmental and workers' health, within the framework of primary health care in the Brazilian National Health System (SUS): opportunities and challenges

O texto busca contribuir para a discussão dos entrecruzamentos entre os campos da saúde ambiental e da saúde do trabalhador, referenciada no cenário brasileiro das relações produção/trabalho, ambiente e saúde e nas mudanças na organização do SUS, com destaque para o papel da atenção primária à saúde (APS), e se destina a contribuir para as discussões no processo de preparação da 1ª Conferência Nacional de Saúde Ambiental (1ª CNSA), prevista para ser realizada em dezembro de 2009. São descritos, de modo sintético, aspectos históricos e conceituais desses campos, algumas das características compartilhadas e as ações esperadas do sistema de saúde, com destaque para o papel da APS e a importância do diálogo com o movimento social. Finalizando, são identificados pontos para uma agenda de trabalho comum.<br>This paper has the purpose of contributing to the discussion of the crossing areas between Environmental Health and Workers´ Health, in the Brazilian context of Labor, Production, Environment and Health. This paper emerges in the context of the current organizational changes of the Brazilian National Health System (SUS), with a major focus on Primary Health Care, having in mind, also, the preparation of the 1st National Environmental Health Conference (1ª CNSA) to be held in December of 2009. So, historical and conceptual aspects of those fields are described in a summarized manner, as well as some shared features and expected actions of the Health System, with emphasis to the role of Primary Health Care and to the importance of the dialogue with the social movement. Finally, some topics for a common agenda were identified by the authors

Saúde e ambiente no Brasil: desenvolvimento, território e iniqüidade social Health and environment in Brazil: development, territory, and social iniquity

As autoras apresentam uma abordagem das iniqüidades sócio-ambientais no Brasil, decorrentes do modelo de desenvolvimento, no contexto atual da globalização, e as perspectivas da luta social. A realização histórica da ideologia do desenvolvimento tem implicado em profundas transformações do território, com sérias repercussões sobre a saúde das populações e dos ecossistemas, as quais apenas muito recentemente começam a ser reconhecidas como problemas na agenda social. A tentativa de conciliar soluções para a crise social e a crise ambiental resultou na formulação da proposta de desenvolvimento sustentável - incorporada em alguns novos discursos no campo da saúde coletiva - cuja análise crítica é o ponto de partida deste artigo. O território foi examinado como um operador útil para analisar a expressão da ligação entre saúde, ambiente e desenvolvimento, revelador do paradoxo entre as potencialidades naturais e humanas do Brasil e sua pobreza, espelho da injustiça ambiental e da violação de direitos humanos. Trata-se de um conceito expandido de ordenação territorial, importante para criar a possibilidade de se compreender o contexto de modernização e de possibilidade das populações pobres conquistarem uma vida melhor.<br>The present paper deals with the subject of socio-environmental injustice in Brazil as a consequence of the country's current development model in the context of globalization and from the perspective of social struggle. Over time the realization of the development ideology has introduced profound changes in the country's territory, with severe repercussions for population and ecosystem health that have only recently been included on the social agenda. Efforts to reconcile solutions to the social and environmental crises have converged in the concept of sustainable development, within new discourses in the field of collective health (and whose analysis provides the point of departure for this paper). Territory is explored as a useful tool for analyzing the relationship between health, environment, and development, revealing the paradox between Brazil's great natural and human potential and its poverty, reflecting environmental injustice and disregard for human rights. This expanded concept of territorial organization is important for understanding the context of modernization and the possibility for poor populations to enjoy a better life

Influência das condições socioambientais na prevalência de hipertensão arterial sistêmica em duas comunidades ribeirinhas da Amazônia, Brasil

This article discusses the influence of environmental conditions on the prevalence of systemic hypertension in two riverine communities in the Sustainable Development Reserve of Tupé, Manaus, Amazonas, Brazil, through an ecological study of multiple groups and contextual analysis carried out with the local inhabitants. To identify the environmental etiology describing the risk of disease development, the study compares demographics, incidence rates and common daily practices in these communities, using data collected in the field, between 2012 and 2014, as well as values provided by IBGE, originally from National Health Survey, 2013. The results suggest that social and environmental determinants, such as general living conditions, occupation and access to protective health care, in the investigated communities, are relevant factors in explaining the observed variability in systemic arterial hypertension (SAH) incidence rates. The study concludes by pointing out the importance and need to consider socio-environmental vulnerability in the elaboration of public health policies and in the management of environmentally protected areas. © 2018, Associacao Brasileira de Pos - Graduacao em Saude Coletiva.
