    Coming Out Ahead While Losing a Partner: The Thoroughbred Industry Stays on Course

    Purpose In many industries, firms collaborate as business partners, which helps them achieve superior outcomes and ensure survival in a crisis. Business relationships help companies access limited resources, share information and build trust within the community. This paper aims to highlight the strategies that firms can use to adapt to the loss of a business partner. Design/methodology/approach This study considers qualitative examples from what happens when a business partner disappears in the Thoroughbred horse industry. The authors draw attention to several types of partner loss due to firm bankruptcy, owner death and strategic restructuring. Findings This paper proposes a framework of strategies for surviving the loss of business partners. Specifically, surviving partners may respond by strategic distancing, relationship self-repair or reconfiguration through asset purchases or mimicry by minimizing exit risks. Practical implications The proposed framework can be used by strategists and managers to determine a course of action when faced with the loss of a business partner. Managers can quickly respond to a partner’s exit with the appropriate action to distance their business or stabilize alternate relationships. Originality/value The novel framework, informed by examples from the Thoroughbred horse industry, conceptualizes an important theoretical and practical problem.     Glyceryl trinitrate patch versus intravenous ritodrine for tocolysis in pre-term labour

    Background: Preterm birth is the single most important determinant of adverse infant outcome. Tocolytic therapy has shown beneficial effect in certain selected patients. The present study was conducted to evaluate the efficacy, maternal and fetal outcome with the use of Transdermal Glyceryl nitrate patches versus Intravenous Ritodrine tocolytic agents.Methods: Study included 50 patients of gestation 24-34 weeks in preterm labor. Group I: Glyceryl trinitrate patch releasing 10mg/24 hours (0.4mg/ hour) was applied transdermally and repeated after 2 hours if no reduction in contractions was seen. Group II: Intravenous Ritodrine infusion prepared by adding 50mg to a bottle of 5% dextrose or ringer lactate solution and infusion started at an initial rate of 0.05mg/min which was increased by 50 micrograms per minute every 15 min until contractions ceased.Results: 22 patients in group I treated with GTN and 19 patients in group II treated with Ritodrine achieved successful tocolysis. The difference was statistically insignificant (P value - 0.23). Mean Gestational age at delivery was 34.97 in GTN Group as compared to 33.24 weeks in Ritodrine Group ; difference which is statistically significant (P = 0.0004). Very few adverse effects were observed in the GTN Group. Comparatively, the Ritodrine Group had severe adverse effects requiring discontinuation of therapy. Fetal outcome was satisfactory in both groups.Conclusions: The effects of Glyceryl trinitrate and Ritodrine in the treatment of preterm labor were essentially similar. Glyceryl trinitrate is associated with lesser maternal and fetal adverse effects and appears to be a very viable, inexpensive and safer alternative to Ritodrine

    Optimality of the Fastest Available Server Policy

    We give sufficient conditions under which a policy that assigns customers to the Fastest Available Server, labelled FAS, is optimal in queueing models with multiple independent Poisson arrival processes and heterogeneous parallel exponential servers. The criterion is to minimize the long-run average cost per unit time. We obtain results for loss models and for queueing systems with a finite-capacity or infinite-capacity buffer under a head-of-the-line priority scheme. The results depend on cost assumptions, so we analyze the robustness of the cost structure and present counter-examples to illustrate when FAS is not optimal

    Unilateral absence of pulmonary artery: a radiographically occult cause of life-threatening hemoptysis

    Unilateral absence or agenesis of pulmonary artery (UAPA) is a rare congenital abnormality with an estimated prevelance of 1 in 2,00,000 adults. The entity occurs commonly in association with other congenital heart diseases like septal defects or patent ductus arteriosus. The condition usually runs a benign clinical course with patients usually presenting clinically in adulthood with history of recurrent respiratory tract infections. Two such patients presented with recurrent respiratory tract infections, breathlessness and hemoptysis. The chest radiograph of first patient was reported as normal in the referring hospital, while that of second patient showed volume loss in left lung. CT Pulmonary Angiography (CTPA) was then performed which demonstrated the absence of right and left pulmonary arteries respectively in the first and second patients. Pulmonary artery branches were reformed distally by multiple collaterals arising from systemic arteries. The entire spectrum, including embryology, imaging features and management of UAPA are discussed. UAPA remains a potential cause for life-threatening hemoptysis, due to extensive collateralization associated with the condition. It is important for radiologists to be aware of this uncommon entity in order to suspect it on a routine chest radiograph, diagnose it and map associated collaterals on CTPA and also embolize the bleeding collaterals

    Small Molecules from Nature Targeting G-Protein Coupled Cannabinoid Receptors: Potential Leads for Drug Discovery and Development

    The cannabinoid molecules are derived from Cannabis sativa plant which acts on the cannabinoid receptors types 1 and 2 (CB1 and CB2) which have been explored as potential therapeutic targets for drug discovery and development. Currently, there are numerous cannabinoid based synthetic drugs used in clinical practice like the popular ones such as nabilone, dronabinol, and Δ9-tetrahydrocannabinol mediates its action through CB1/CB2 receptors. However, these synthetic based Cannabis derived compounds are known to exert adverse psychiatric effect and have also been exploited for drug abuse. This encourages us to find out an alternative and safe drug with the least psychiatric adverse effects. In recent years, many phytocannabinoids have been isolated from plants other than Cannabis. Several studies have shown that these phytocannabinoids show affinity, potency, selectivity, and efficacy towards cannabinoid receptors and inhibit endocannabinoid metabolizing enzymes, thus reducing hyperactivity of endocannabinoid systems. Also, these naturally derived molecules possess the least adverse effects opposed to the synthetically derived cannabinoids. Therefore, the plant based cannabinoid molecules proved to be promising and emerging therapeutic alternative. The present review provides an overview of therapeutic potential of ligands and plants modulating cannabinoid receptors that may be of interest to pharmaceutical industry in search of new and safer drug discovery and development for future therapeutics

    Delayed presentation of duodenal atresia

    Background: The most common type of duodenal atresia (DA) (Type I), also known as duodenal web or membrane can present later in infancy or early childhood if the membrane or web is fenestrated. We describe six patients with delayed presentation of DA. Materials and Methods: Retrospective review of hospital records of six patients with delayed presentation of DA due to fenestrated web managed in Paediatric Surgery Department at a tertiary care institute over a period of 2 years (January 2019 to December 2020) was done. The data of these patients were analysed on the basis of age at presentation, clinical presentation, associated anomalies, radiological findings, intra-operative findings, management and postoperative course. Results: The median age at presentation was 6.5 months (range: 1 month to 10 years). There were four males and two females. The most common presentation was emesis seen in all six patients. Two patients had Down syndrome. Associated congenital anomalies were cardiac in one patient, anterior ectopic anus in one patient and malrotation of midgut in one patient. Upper gastrointestinal contrast suggested incomplete duodenal obstruction in all patients. At laparotomy, fenestrated duodenal membrane was observed in all patients – preampullary in three patients and postampullary in three patients. Lateral duodenotomy, web excision and transverse closure was done in all six patients. The postoperative period was uneventful in all patients and mean duration of hospital stay was 9 days. Conclusion: Fenestrated duodenal webs present a diagnostic challenge to the paediatric surgeons because of delayed and variable clinical presentation. A modification of the present classification of DA has been proposed in this study which would help in better reporting of epidemiology and aid in early diagnosis of this congenital anomaly

    Background Hypospadias is one of the most common genital birth defects. There are around 300 various techniques available for the repair of hypospadias. This study aims to compare the reported outcomes of Tubularized incised plate urethroplasty (TIP) and Grafted TIP (GTIP) repair in children undergoing primary hypospadias repair.Methods This meta-analysisadhered to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, and we framed our research question using the population, intervention, control and outcomes format. We conducted comprehensive electronic searches across various databases, employing a Boolean search strategy with predefined search terms. Only randomized controlled trials (RCTs) were included for quantitative analysis.Results Totally, 10 RCTs met our inclusion criteria for quantitative analysis. The results indicated that urethrocutaneous fistula, glans dehiscence, and stricture rates were comparable between the two groups. The incidence of meatal stenosis was found to be significantly lower in the GTIP group with a relative risk (RR) of 0.32 (95% confidence interval (CI) 0.15 to 0.67).Conclusion The coucomes UCF, glans dehiscence, and stricture rates were comparable between the two groups. Notably, the incidence of meatal stenosis was found to be significantly lower in the grafted TIP group. In terms of operative time, our quantitative synthesis demonstrated that the TIP group had a shorter operative time than the GTIP group with significant heterogeneity
