    Exploring spatial feedbacks between adaptation policies and internal migration patterns due to sea-level rise

    Abstract Climate change-induced sea-level rise will lead to an increase in internal migration, whose intensity and spatial patterns will depend on the amount of sea-level rise; future socioeconomic development; and adaptation strategies pursued to reduce exposure and vulnerability to sea-level rise. To explore spatial feedbacks between these drivers, we combine sea-level rise projections, socioeconomic projections, and assumptions on adaptation policies in a spatially-explicit model (‘CONCLUDE’). Using the Mediterranean region as a case study, we find up to 20 million sea-level rise-related internal migrants by 2100 if no adaptation policies are implemented, with approximately three times higher migration in southern and eastern Mediterranean countries compared to northern Mediterranean countries. We show that adaptation policies can reduce the number of internal migrants by a factor of 1.4 to 9, depending on the type of strategies pursued; the implementation of hard protection measures may even lead to migration towards protected coastlines. Overall, spatial migration patterns are robust across all scenarios, with out-migration from a narrow coastal strip and in-migration widely spread across urban settings. However, the type of migration (e.g. proactive/reactive, managed/autonomous) depends on future socioeconomic developments that drive adaptive capacity, calling for decision-making that goes well beyond coastal issues

    Variations in locoregional therapy in postmenopausal patients with early breast cancer treated in different countries

    BACKGROUND: The Tamoxifen and Exemestane Adjuvant Multinational (TEAM) trial is an international randomized trial evaluating the efficacy and safety of exemestane, alone or following tamoxifen. The large number of patients already recruited offered the opportunity to explore locoregional treatment practices between countries. METHODS: Patients were enrolled in Belgium, France, Germany, Greece, Ireland, Japan, the Netherlands, the UK and the USA. The core protocol had minor differences in eligibility criteria between countries, reflecting variations in national guidelines and practice regarding adjuvant endocrine therapy. RESULTS: Between 2001 and 2006, 9779 patients of mean(s.d.) age 64(9) years were randomized. Some 58.4 per cent had T1 tumours (range between countries 36.8-75.9 per cent; P < 0.001) and 47.3 per cent were axillary node positive (range 25.9-84.6 per cent; P < 0.001). Independent factors for type of breast surgery were country, age, tumour status and calendar year of surgery. After breast-conserving surgery, radiotherapy was given to 93.2 per cent of patients, 86.0 per cent in the USA and 100 per cent in France. Axillary lymph node dissection was performed in 82.0 (range 74.6-99.1) per cent. CONCLUSION: Despite international consensus guidelines, wide global variations were observed in treatment practices of early breast cancer. There should be further efforts to optimize locoregional treatment for breast cancer worldwide.Surgical oncolog

    The effect of exemestane and tamoxifen on bone health within the Tamoxifen Exemestane Adjuvant Multinational (TEAM) trial: a meta-analysis of the US, German, Netherlands, and Belgium sub-studies

    PURPOSE: We performed a meta-analysis of three sub-studies of the randomized Tamoxifen Exemestane Adjuvant Multinational (TEAM) trial to determine the effects of exemestane and tamoxifen on bone health. METHODS: Patients received exemestane or tamoxifen as adjuvant therapy for hormone receptor-positive breast cancer. Bone mineral density (BMD) was assessed at baseline and after 12 and 24 months of treatment. Bone turnover markers were also measured. RESULTS: Patients receiving tamoxifen showed a mean increase from baseline in lumbar spine BMD of 1.2% at month 12 and 0.2% at month 24. Patients receiving exemestane showed a mean decrease from baseline of 2.6% after 12 months and 3.5% after 24 months. There were significant differences in the changes in lumbar spine BMD between treatment groups (P < 0.0001 at both time points). Changes in BMD from baseline at the total hip were also significantly different between exemestane and tamoxifen (P < 0.05 at both time points). Bone turnover markers decreased from baseline with tamoxifen and increased with exemestane. CONCLUSIONS: Exemestane resulted in decreases in BMD and increases in bone turnover markers.     Middle to Late Quaternary palaeolandscapes of the central Azraq Basin, Jordan: Deciphering discontinuous records of human-environment dynamics at the arid margin of the Levant

    The Azraq oasis in the Eastern Desert of Jordan has produced considerable stone artefacts attributed to the early Palaeolithic, yet relatively few data are available regarding the chronology and palaeoenvironmental contexts of the remains. In this study, we present stratigraphic, sedimentological, and micropalaeontological analyses of the Late Acheulean site SM1 located within the former Shishan Marsh, which we combine with geochronological and sedimentological data from 13 neighbouring geological exposures to reconstruct landscape evolution at the western margin of the Shishan Marsh. We then discuss the Late Quaternary palaeolandscapes of the Greater Azraq Oasis Area over the past c. 350 ka. Our work demonstrates that the central Azraq Basin experienced three local wetting-drying cycles since the late Middle Pleistocene that would have dramatically shifted the quantity and distribution of freshwater resources, ranging from expansive wetland landscapes to desert refugia characterised by isolated spring pools—changes that would have significantly impacted the mobility decisions and settlement patterns of Palaeolithic inhabitants. Our study highlights that developing long-term records of human-environment dynamics in arid environments requires a mosaic approach to palaeoenvironmental reconstruction that is nested within a well-developed understanding of landscape evolution
