    Il presente contributo affronta il tema del riconoscimento e validazione delle competenze chiave europee in contesto lavorativo e nelle situazioni di lavoro e produzione. La ricerca-intervento presentata, condotta in un distretto di artigianato industriale della moda di lusso italiano, illustra alcuni fattori per un\u2019efficace progettazione e conduzione del dispositivo al fine di giungere all\u2019auspicata trasformazione delle pratiche, delle rappresentazioni dei ruoli organizzativi e del genere professionale: la tipologia di apprendimenti, prevalentemente taciti e situati, la necessit\ue0 di repertori contestualizzati alle situazioni di lavoro, la personalizzazione dell\u2019accompagnamento in funzione delle capacit\ue0 riflessive e di concettualizzazione degli attori aziendali e, non ultimo, la valorizzazione della dimensione collettiva.     \uc0 partir de l\u2019examen des r\ue9sultats de deux recherches-actions r\ue9alis\ue9es dans les secteurs de la chaussure de luxe et de l\u2019activit\ue9 artisanale de fabrication de dentelle, cet article interroge les processus d\u2019hybridation de deux logiques cat\ue9goriales pour l\u2019\ue9dification des r\ue9f\ue9rentiels de comp\ue9tences : le mod\ue8le des comp\ue9tences technico-professionnelles et celui des comp\ue9tences transverses.     The validation of the competences acquired other than by formal means is a current preoccupation at national and European policies level, an opportunity for the young and the adult people alike, a desire, but at the same time a reality of everyday life. The optimal conducting of the validation of nonformal and informal learning entails, according to the European documents, the supporting of candidates. The article presents, in its first part three case studies from different national contexts, Romania, France and Italy, focused on the counselling activity, In its second part, the article analyses the perception of a counselling framework possibly to be implemented in the human resources departments of Romanian companies as a complementary manner to the counselling offered by accredited evaluation centres

    ackground: Neoplasms of the bilio-pancreatic tract are often disgnosed in an advanced stage. Palliation of these patients is focused on resolution of biliary obstruction. Methods: From 1 January 2003 to 31 January 2009 we managed 39 patients with a palliative procedure, 8 of them were treated by a surgical by-pass, the endoscopic procedures were 21, while the transepatic percutaneous drainages were 12. Discussion: Surgical palliation is reserved for patients with a good performance status and expected survival of over 6 mounths, it is also indicated for whom, affected by pancreatic carcinoma, developed duodenal obstruction associated to jaundice. If the biliary obstruction is near the papillary region the treatment of choice is endoscopic drainage, if the obstruction is distal these region the way of choice is the transepatic one. Metallic stents have a greater initial cost but a less risk of obstruction than the plastic stents, these are indicated in patients with a short survival expectance. Conclusions: Endoscopic drainage is the gold standard of treatment for obstructive jaundice, percutaneous biliary decompressione is the treatment of choice for palliation of distal bile obstruction

    Essential thrombocythemia (ET) is rare in children, and little or no information is available about clonality or JAK2 mutations. However, the analyses in this work prove useful for the diagnosis of adult myeloproliferative disorders (MPDs). We evaluated the clonality status and V617FJAK2 mutation in 20 children affected by ET and compared them with 47 consecutive adult ET cases. Clonality was evaluated on the DNA of granulocytes and on the RNA of platelets. V617FJAK2 was analyzed by sequencing tests, allele-specific polymerase chain reaction (PCR), and digestion by BsaXI. A monoclonal pattern was found in 4 (28.5%) of 14 children and in 45% of informative adults. Heterozygous V617FJAK2 was found less frequently in children than in adults (P < .009). Only 2 girls showed both the V617FJAK2 mutation and a monoclonal pattern; one of them was the only child presenting a major thrombotic complication. In contrast to adults, most children with ET do not show either a clonal disorder or the V617FJAK2 mutation

    Background: Staging laparoscopy may be usefull avoiding unnecessary laparotomy in patints with advanced cancer. Methods: In the last 16 months diagnostic laparoscopy was performed in 8 patients. There were no major complications, no mortality and no trocar site metastasis were observed. Discussion: Staging laparoscopy has a great role in case of advanced pancreatic cancer to reduce the diagnosis of unresectable disease made by unnecessary laparotomy. Laparoscopic exploration can be proposed for advanced gastric cancer that can be associated with an high incidence of peritoneal involvement. Laparoscopy is also usefull to evaluate candidates for cytoreduction of peritoneal carcinomatosis and hypertermic intraperitoneal chemotherapy or evaluating advanced stage of ovarian cancer

    Surgery for breast cancer has changed in the last few decades in favor of more conservative approaches, without compromising loco\u2010regional control and survival. Common immediate complications fol \u2010lowing breast surgery are hematoma, seroma, and wound infection.1Traditionally, surgeons have implemented the use of closed\u2010suction drains in this setting with the aim of preventing these complications, which can cause discomfort, morbidity, increased follow\u2010up visits,and possible delay in the beginning of adjuvant therapies.2 On the other hand, potential benefits of performing breast procedures without using a drain have been increasingly considered.3 There is lack of modern and official guidelines on use of drains in breast sur \u2010gery, with no consensus regarding their management and the appro\u2010priate use of antibiotics in this setting. Almost all reports agree withthe use of drains after breast procedures, especially if they are asso\u2010ciated with reconstruction or axillar lymph node dissection (ALND), but an extreme variability is reported regarding practice patterns.4A national survey regarding the management of drains afterbreast cancer surgery was sent by email on January 2018 to all Breast Centers (BC) registered with Senonetwork Italia. Data col \u2010lected by the administrative office of Senonetwork were de\u2010iden\u2010tified and exported for statistical analysis. A Steering Committeecomposed of general surgeons and plastic surgeons was appointedto coordinate the survey, which consisted of 22 multiple choicequestions, designed to inquire about surgeons' demographics, use of drains after breast procedures with and without reconstructionand/or ALND, antibiotic use and to characterize drain managementin their clinical practice
