Pleural mesothelioma and lung cancer risks in relation to occupational history and asbestos lung burden.
BACKGROUND: We have conducted a population-based study of pleural mesothelioma patients with occupational histories and measured asbestos lung burdens in occupationally exposed workers and in the general population. The relationship between lung burden and risk, particularly at environmental exposure levels, will enable future mesothelioma rates in people born after 1965 who never installed asbestos to be predicted from their asbestos lung burdens. ”m were counted by transmission electron microscopy in lung samples obtained from 133 patients with mesothelioma and 262 patients with lung cancer. ORs for mesothelioma were converted to lifetime risks. RESULTS: Lifetime mesothelioma risk is approximately 0.02% per 1000 amphibole fibres per gram of dry lung tissue over a more than 100-fold range, from 1 to 4 in the most heavily exposed building workers to less than 1 in 500 in most of the population. The asbestos fibres counted were amosite (75%), crocidolite (18%), other amphiboles (5%) and chrysotile (2%). CONCLUSIONS: The approximate linearity of the dose-response together with lung burden measurements in younger people will provide reasonably reliable predictions of future mesothelioma rates in those born since 1965 whose risks cannot yet be seen in national rates. Burdens in those born more recently will indicate the continuing occupational and environmental hazards under current asbestos control regulations. Our results confirm the major contribution of amosite to UK mesothelioma incidence and the substantial contribution of non-occupational exposure, particularly in women
Interpreting intraplate tectonics for seismic hazard : a UK historical perspective
It is notoriously difficult to construct seismic source models for probabilistic seismic hazard assessment in intraplate areas on the basis of geological information, and many practitioners have given up the task in favour of purely seismicity-based models. This risks losing potentially valuable information in regions where the earthquake catalogue is short compared to the seismic cycle. It is interesting to survey how attitudes to this issue have evolved over the past 30 years. This paper takes the UK as an example, and traces the evolution of seismic source models through generations of hazard studies. It is found that in the UK, while the earliest studies did not consider regional tectonics in any way, there has been a gradual evolution towards more tectonically based models. Prevention is better than cure, but...: Preventive medication as a risk to ordinariness?
Preventive health remains at the forefront of public health concerns; recent initiatives, such as the NHS health check, may lead to recommendations for medication in response to the identification of 'at risk' individuals. Little is known about lay views of preventive medication. This paper uses the case of aspirin as a prophylactic against heart disease to explore views among people invited to screening for a trial investigating the efficacy of such an approach. Qualitative interviews (N=46) and focus groups (N=5, participants 31) revealed dilemmas about preventive medication in the form of clashes between norms: first, in general terms, assumptions about the benefit of prevention were complicated by dislike of medication; second, the individual duty to engage in prevention was complicated by the need not to be over involved with one's own health; third, the potential appeal of this alternative approach to health promotion was complicated by unease about the implications of encouraging irresponsible behaviour among others. Simple strong glass forming models: mean-field solution with activation
We introduce simple models, inspired by previous models for froths and
covalent glasses, with trivial equilibrium properties but dynamical behaviour
characteristic of strong glass forming systems. These models are also a
generalization of backgammon or urn models to a non--constant number of
particles, where entropic barriers are replaced by energy barriers, allowing
for the existence of activated processes. We formulate a mean--field version of
the models, which keeps most of the features of the finite dimensional ones,
and solve analytically the out--of--equilibrium dynamics in the low temperature

This paper offers a brief consideration of how narrative, in the form of peopleâs own stories, potentially figures in health and social care provision as part of the impulse towards patient-centred care. The rise of the epistemological legitimacy of patientsâ stories is sketched here. The paper draws upon relevant literature and original writing to consider the ways in which stories can mislead as well as illuminate the process of making individual treatment care plans
Testing Tablet Computers in Nursing Education: A Comprehensive Evaluation Framework
Background: Tablet Computers (TCs) and other mobile digital devices are rapidly changing the way we communicate and access information in our personal and professional lives. Scarce research exists regarding their effectiveness in promoting the learning of health professionals. This paper describes the evaluation framework used in a study to test TCs in a post-diploma baccalaureate nursing program in the Gulf Cooperation Council (GCC) state of Qatar.Purpose: The evaluation framework was structured around 10 objectives designed to assess the impact of TC integration into the evidence-based practice (EBP) and reflective practice (RP) components of a scholarship course. Evaluation variables included perceptions of knowledge, confidence, comfort, satisfaction and technical skill before and after the 7-week TC implementation; studentsâ usage patterns and attitudes about the usefulness of TCs in promoting their learning related to EBP and RP were also examined; in addition, studentsâ views about the impact of TCs on the learning environment and their engagement in the learning process were sought.Methods: A mixed method descriptive design was used to assess outcomes of interest. Qualitative methods (focus groups, participant observation, field notes and reflective journals) were used to capture subjective perspectives of TC users. Quantitative methods (pre-test/posttest, activity logs and skills labs) were used to assess change in knowledge, attitude and technical proficiency over time.Results: The evaluation framework used to assess process and outcome variables in this study combined structural, philosophical, theoretical, pedagogical and methodological elements. These included the logic model, participatory action, theory-based course concepts, as well as a learning taxonomy involving cognitive, affective and psychomotor competencies.Conclusion: The value of a comprehensive evaluation plan executed in tandem with TC implementation is highlighted
The Retail FX Trader: Random Trading and the Negative Sum Game
With the internet boom of early 2000 making access to trading the Foreign Exchange (FX) market far simpler for members of the general public, the growth of 'retail' FX trading continues, with daily transaction volumes as high as $200 billion. Potential new entrants to the retail FX trading world may come from the recent UK pension deregulations, further increasing the volumes. The attraction of FX trading is that it offers high returns and whilst it has been understood that it is high-risk in nature, the rewards are seen as being commensurately high for the 'skilled and knowledgeable' trader who has an edge over other market participants. This paper analyses a number of independent sources of data and previous research, to examine the profitability of the Retail FX trader and compares the results with that of a simulated random trading models. This paper finds evidence to suggest that whilst approximately 20% of traders can expect to end up with a profitable account, around 40% might expect their account to be subject to a margin call. This paper finds a strong correlation between the overall profitability of traders and impact of the cost of the bid-ask spread, whilst finding little if any evidence that retail FX traders, when viewed as a group, are achieving results better than that from random trading
Statistical Mechanics of Two-dimensional Foams
The methods of statistical mechanics are applied to two-dimensional foams
under macroscopic agitation. A new variable -- the total cell curvature -- is
introduced, which plays the role of energy in conventional statistical
thermodynamics. The probability distribution of the number of sides for a cell
of given area is derived. This expression allows to correlate the distribution
of sides ("topological disorder") to the distribution of sizes ("geometrical
disorder") in a foam. The model predictions agree well with available
experimental data
Gauss Sums and Quantum Mechanics
By adapting Feynman's sum over paths method to a quantum mechanical system
whose phase space is a torus, a new proof of the Landsberg-Schaar identity for
quadratic Gauss sums is given. In contrast to existing non-elementary proofs,
which use infinite sums and a limiting process or contour integration, only
finite sums are involved. The toroidal nature of the classical phase space
leads to discrete position and momentum, and hence discrete time. The
corresponding `path integrals' are finite sums whose normalisations are derived
and which are shown to intertwine cyclicity and discreteness to give a finite

Decay of Correlations in a Topological Glass
In this paper we continue the study of a topological glassy system. The state
space of the model is given by all triangulations of a sphere with nodes,
half of which are red and half are blue. Red nodes want to have 5 neighbors
while blue ones want 7. Energies of nodes with other numbers of neighbors are
supposed to be positive. The dynamics is that of flipping the diagonal between
two adjacent triangles, with a temperature dependent probability. We consider
the system at very low temperatures.
We concentrate on several new aspects of this model: Starting from a detailed
description of the stationary state, we conclude that pairs of defects (nodes
with the "wrong" degree) move with very high mobility along 1-dimensional
paths. As they wander around, they encounter single defects, which they then
move "sideways" with a geometrically defined probability. This induces a
diffusive motion of the single defects. If they meet, they annihilate, lowering
the energy of the system. We both estimate the decay of energy to equilibrium,
