    Whose security is it? Elitism and the global approach to maritime security in Africa

    Africa’s marine environment and resources that lie beneath it are central to the continent’s sustainable development and actualising the ambitions set out by the African Union in its Agenda 2063, where the oceans are described as the frontier of Africa’s development. The continent’s maritime domain and resources are also attractive to foreign partners relying on its oceans to enhance their economic development and geostrategic interests. Serving the interests of all parties, especially the 38 coastal states and the Small Island Developing States (SIDS) and land-linked countries on the continent that benefit from the maritime sector, comes with challenges, some of which manifest as threats to the sustainable resource extraction and safety of those that use the maritime domain. We explored the literature, policy documents and maritime security reports database, together with our experiences as African maritime governance and security experts, to critically examine maritime security in Africa and unravel how extra-regional actors have securitised maritime threats. We show how the selective framing of what constitutes threats and associated resourcing of responses to counter them, often dictated by foreign interests, is an elite project that undermines a holistic notion of maritime security that would benefit the African people.Publisher PDFPeer reviewe

    UK parents' attitudes towards meningococcal group B (MenB) vaccination: a qualitative analysis

    OBJECTIVES: (1) To explore existing knowledge of, and attitudes, to group B meningococcal disease and serogroup B meningococcal (MenB) vaccine among parents of young children. (2) To seek views on their information needs. DESIGN: Cross-sectional qualitative study using individual and group interviews conducted in February and March 2015, prior to the introduction of MenB vaccine (Bexsero) into the UK childhood immunisation schedule. SETTING: Community centres, mother and toddler groups, parents’ homes and workplaces in London and Yorkshire. PARTICIPANTS: 60 parents of children under 2 years of age recruited via mother and baby groups and via an advert posted to a midwife-led Facebook group. RESULTS: Although recognising the severity of meningitis and septicaemia, parents’ knowledge of group B meningococcal disease and MenB vaccine was poor. While nervous about fever, most said they would take their child for MenB vaccination despite its link to fever. Most parents had liquid paracetamol at home. Many were willing to administer it after MenB vaccination as a preventive measure, although some had concerns. There were mixed views on the acceptability of four vaccinations at the 12-month booster visit; some preferred one visit, while others favoured spreading the vaccines over two visits. Parents were clear on the information they required before attending the immunisation appointment. CONCLUSIONS: The successful implementation of the MenB vaccination programme depends on its acceptance by parents. In view of parents’ recognition of the severity of meningitis and septicaemia, and successful introduction of other vaccines to prevent bacterial meningitis and septicaemia, the MenB vaccination programme is likely to be successful. However, the need for additional injections, the likelihood of post-immunisation fever and its management are issues about which parents will need information and reassurance from healthcare professionals.     In Vivo Metal Substitutions in Metal Sequestering Subcellular Compartments: X-Ray Mapping in Cryosections

    Qualitative digital X-ray mapping techniques were employed to determine the distributions of essential and non-essential elements in three invertebrate models : (1) Pb, Zn, Cd, Cu, Fe in thin cryosections of the hepatopancreas of the terrestrial isopod, Oniscus asellus; (2) Pb, Zn, Cd, Ca in thin cryosections of the chloragogenous tissue of the earthworm, Lumbricus rubellus; and (3) As in air-dried smears and thin cryosections of chloragogen in L. rubellus. Four general conclusions were drawn from the results of these studies: (a) non-essential elements can accumulate, distribute and be compartmentalized because they, or the organo-complexes that they form, act as mimics of essential elements with which they share to a greater or lesser extent certain chemical affinities; (b) thermodynamic considerations notwithstanding, the influence of biological factors on the sequestration and fates of certain elements (e.g., arsenic) is profound through modifications of redox states and organo-compound formation; (c) X-ray mapping, combined with anhydrous preparative procedures, yields unbiased information concerning the relative spatial distributions of several elements in structurally heterogeneous sampling fields , although the morphological characterization of (occasionally unsuspected) subcellular compartments may be constrained by the intrinsic quality of the preparation; and (d) X-ray microanalysis yields codistribution data, when integrated with biochemical information from other sources, which give strong pointers to the identity of binding ligands and of the valence state of sequestered cations

    Infrared spectroscopic studies of hydrogen bonding in substituted nitrophenols: substituent and solvent effects

    A detailed infrared spectroscopic study of the substituted phenols 2-cyano-4,6-dinitrophenol and 4-cyano-2,6-dinitrophenol has been carried out (in several different solvents) in order to investigate the substituent and solvent effects on their intra- and inter-molecular hydrogen bonding properties. In benzene or dichloromethane it is found that both isomers form strong intramolecular hydrogen bonds with the 2-cyano (2CN) isomer having a stronger intramolecular interaction (in accordance with the higher pKa). The 4-cyano (4CN) isomer shows two distinct NO2 groups and exchange between the two possible hydrogen bonding sites is probably slow on the infrared time-scale. In protic solvents such as methanol the intramolecular hydrogen bonds are broken (more easily for the 4CN isomer) by intermolecular hydrogen bonding to the solvent. The differential “reactivity” towards methanol may be associated with steric congestion in the 4CN isomer leading to the forcing of at least one of the NO2 groups out of the aromatic plane. The use of mixed solvents (benzene-methanol) has established that the two hydrogen bonded species are observed together and that a high concentration of methanol is required to drive the equilibrium towards the intermolecular hydrogen bonded species. In dimethyl sulphoxide the behaviour of the two isomers is even more interesting. The 4CN isomer is ionised to produce the corresponding phenolate. However the 2CN isomer remains neutral (but highly solvated). We attribute this difference to the requirement for the 4CN isomer to allow the 2- and 6-NO2 groups to recover planarity with the aromatic ring. The energy compensation involved in this process is clearly sufficient to break a stronger intramolecular hydrogen bond.info:eu-repo/semantics/publishedVersio

    The formation of adducts with 1 :2 and 1:3 stoichiometry by methoxide addition to nitro-activated anisoles has been examined spectrophotometrically. For these equilibria the ‘basicity’ of sodium methoxide solutions in methanol is appreciably greater than that of corresponding potassium methoxide solutions. This is in contrast with other measures of basicity and is attributed to the association of the multi-charged adducts with cations which is stronger with sodium than with potassium ions.info:eu-repo/semantics/publishedVersio

    Group A streptococcal vaccine delivery by immunization with a self-adjuvanting M protein-based lipid core peptide construct

    Background & objectives: To develop a broad strain coverage GAS vaccine, several strategies have been investigated which included multi-epitope approaches as well as targeting the M protein conserved C-region. These approaches, however, have relied on the use of adjuvants that are toxic for human application. The development of safe and effective adjuvants for human use is a key issue in the development of effective vaccines. In this study, we investigated the lipid polylysine core peptide (LCP) system as a self-adjuvanting GAS vaccine delivery approach. Methods: An LCP-GAS construct was synthesised incorporating multiple copies of a protective peptide epitope (J8) from the conserved carboxy terminal C-repeat region of the M protein. B10.BR mice were immunized parenterally with the LCP-J8 construct, with or without conventional adjuvant, prior to the assessment of immunogenicity and the induction of serum opsonic antibodies. Results: Our data demonstrated immunogenicity of LCP-J8 when coadministered in complete Freund's adjuvant (CFA), or administered in the absence of conventional adjuvant. In both cases, immunization led to the induction of high-titre J8 peptide-specific serum IgG antibody responses, and the induction of heterologous opsonic antibodies that did not cross-react with human heart tissue proteins. Interpretation & conclusion: These data indicated the potential of a novel self-adjuvanting LCP vaccine delivery system incorporating a synthetic GAS M protein C-region peptide immunogen in the induction of broadly protective immune responses, and pointed to the potential application of this system in human vaccine development against infectious diseases

    Implementation of a novel antimicrobial stewardship strategy for rural facilities utilising telehealth

    A significant portion of healthcare takes place in small hospitals, and many are located in rural and regional areas. Facilities in these regions frequently do not have adequate resources to implement an onsite antimicrobial stewardship programme and there are limited data relating to their implementation and effectiveness. We present an innovative model of providing a specialist telehealth antimicrobial stewardship service utilising a centralised service (Queensland Statewide Antimicrobial Stewardship Program) to a rural Hospital and Health Service. Results of a 2-year post-implementation follow-up showed an improvement in adherence to guidelines [33.7% (95% CI 27.0–40.4%) vs. 54.1% (95% CI 48.7–59.5%)] and appropriateness of antimicrobial prescribing [49.0% (95% CI 42.2–55.9%) vs. 67.5% (95% CI 62.7–72.4%) (P < 0.001). This finding was sustained after adjustment for hospitals, with improvement occurring sequentially across the years for adherence to guidelines [adjusted odds ratio (aOR) = 2.44, 95% CI 1.70–3.51] and appropriateness of prescribing (aOR = 2.48, 95% CI 1.70–3.61). There was a decrease in mean total antibiotic use (DDDs/1000 patient-days) between the years 2016 (52.82, 95% CI 44.09–61.54) and 2018 (39.74, 95% CI 32.76–46.73), however this did not reach statistical significance. Additionally, there was a decrease in mean hospital length of stay (days) from 2016 (3.74, 95% CI 3.08–4.41) to 2018 (2.55, 95% CI 1.98–3.12), although this was not statistically significant. New telehealth-based models of antimicrobial stewardship can be effective in improving prescribing in rural areas. Programmes similar to ours should be considered for rural facilities
