First International Consensus Conference on lesions of uncertain malignant potential in the breast (B3 lesions).
The purpose of this study is to obtain a consensus for the therapy of B3 lesions. The first International Consensus Conference on lesions of uncertain malignant potential in the breast (B3 lesions) including atypical ductal hyperplasia (ADH), flat epithelial atypia (FEA), classical lobular neoplasia (LN), papillary lesions (PL), benign phyllodes tumors (PT), and radial scars (RS) took place in January 2016 in Zurich, Switzerland organized by the International Breast Ultrasound School and the Swiss Minimally Invasive Breast Biopsy group-a subgroup of the Swiss Society of Senology. Consensus recommendations for the management and follow-up surveillance of these B3 lesions were developed and areas of research priorities were identified. The consensus recommendation for FEA, LN, PL, and RS diagnosed on core needle biopsy or vacuum-assisted biopsy (VAB) is to therapeutically excise the lesion seen on imaging by VAB and no longer by open surgery, with follow-up surveillance imaging for 5Â years. The consensus recommendation for ADH and PT is, with some exceptions, therapeutic first-line open surgical excision. Minimally invasive management of selected B3 lesions with therapeutic VAB is acceptable as an alternative to first-line surgical excision
Essentially English : Sherlock Holmes at the BBC
Sir Arthur Conan Doyleâs Sherlock Holmes stories, currently enjoying renewed popularity on television via the BBCâs Sherlock (2010- ), have been adapted for the screen countless times around the world. Arguably best remembered are Granadaâs long-running strand with Jeremy Brett (1984-94), and the Universal film series of the 1940s, featuring Basil Rathbone and Nigel Bruce. Less frequently cited, however, are the two series produced by the BBC between 1965 and 1968, in which first Douglas Wilmer and later Peter Cushing took on the mantle of the Baker Street detective. Typically for the time â but against the wishes of the Conan Doyle estate - these programmes adopted a multi-camera studio model; a mode of production which made them less attractive to US networks than the single camera 35mm film output of commercial rivals such as ITC. Drawing upon material from the BBCâs Written Archives Centre, this article investigates the motivations underpinning the Corporationâs refusal to accommodate the estateâs exhortations to seek American co-production and utilise single camera filming, not least of which was the BBCâs stated desire to maintain the âessentially Englishâ quality of Sherlock Holmes. This decision would have significant repercussions for the seriesâ overseas saleability, and â despite impressive viewing figures and positive audience reaction at home in the UK â helped contribute to its âforgottenâ status with regard to the television canon
Fabrication and characterization of dual function nanoscale pH-scanning ion conductance microscopy (SICM) probes for high resolution pH mapping
The easy fabrication and use of nanoscale dual function pH-scanning ion conductance microscopy (SICM) probes is reported. These probes incorporate an iridium oxide coated carbon electrode for pH measurement and an SICM barrel for distance control, enabling simultaneous pH and topography mapping. These pH-SICM probes were fabricated rapidly from laser pulled theta quartz pipets, with the pH electrode prepared by in situ carbon filling of one of the barrels by the pyrolytic decomposition of butane, followed by electrodeposition of a thin layer of hydrous iridium oxide. The other barrel was filled with an electrolyte solution and Ag/AgCl electrode as part of a conductance cell for SICM. The fabricated probes, with pH and SICM sensing elements typically on the 100 nm scale, were characterized by scanning electron microscopy, energy-dispersive X-ray spectroscopy, and various electrochemical measurements. They showed a linear super-Nernstian pH response over a range of pH (pH 2â10). The capability of the pH-SICM probe was demonstrated by detecting both pH and topographical changes during the dissolution of a calcite microcrystal in aqueous solution. This system illustrates the quantitative nature of pH-SICM imaging, because the dissolution process changes the crystal height and interfacial pH (compared to bulk), and each is sensitive to the rate. Both measurements reveal similar dissolution rates, which are in agreement with previously reported literature values measured by classical bulk methods
Adolescent Religiosity and Selective Exposure to Television
Relying on the Adolescent Media Practice Model and selective exposure theory, this study investigated whether religious adolescents watch less mature television entertainment programs than their less religious peers. Program maturity was measured using V-chip ratings, with higher maturity scores indicating content that included more sexuality, violence, and/or adult and sexual language. The responses from 1,335 16- to 18-year-olds who completed Wave 2 of the National Study of Youth and Religion (NSYR) survey were analyzed. Findings indicate that religiosity contributes to explaining the variance in television maturity means, with more religious adolescents indicating a preference for less mature television entertainment. Gender, race, income, and parentsâ monitoring of teensâ media were also found to influence television maturity. Teensâ attitudes toward premarital sex appeared to mediate the effect of religiosity on their television entertainment choices
High on-off conductance switching ratio in optically-driven self-assembled conjugated molecular systems
A new azobenzene-thiophene molecular switch is designed, synthesized and used
to form self-assembled monolayers (SAM) on gold. An "on/off" conductance ratio
up to 7x1E3 (with an average value of 1.5x1E3) is reported. The "on"
conductance state is clearly identified to the cis isomer of the azobenzene
moiety. The high "on/off" ratio is explained in terms of photo-induced,
configuration-related, changes in the electrode-molecule interface energetics
(changes in the energy position of the molecular orbitals with respect to the
Fermi energy of electrodes) in addition to changes in the tunnel barrier length
(length of the molecules). First principles DFT calculations demonstrate a
better delocalization of the frontier orbitals, as well as a stronger
electronic coupling between the azobenzene moiety and the electrode for the cis
configuration over the trans one. Measured photoionization cross-sections for
the molecules in the SAM are close to the known values for azobenzene
derivatives in solution.Comment: 1 file with main text, figure and suppementary informatio

Comparison of Outcomes of antibiotic Drugs and Appendectomy (CODA) trial: a protocol for the pragmatic randomised study of appendicitis treatment.
INTRODUCTION: Several European studies suggest that some patients with appendicitis can be treated safely with antibiotics. A portion of patients eventually undergo appendectomy within a year, with 10%-15% failing to respond in the initial period and a similar additional proportion with suspected recurrent episodes requiring appendectomy. Nearly all patients with appendicitis in the USA are still treated with surgery. A rigorous comparative effectiveness trial in the USA that is sufficiently large and pragmatic to incorporate usual variations in care and measures the patient experience is needed to determine whether antibiotics are as good as appendectomy.
OBJECTIVES: The Comparing Outcomes of Antibiotic Drugs and Appendectomy (CODA) trial for acute appendicitis aims to determine whether the antibiotic treatment strategy is non-inferior to appendectomy.
METHODS/ANALYSIS: CODA is a randomised, pragmatic non-inferiority trial that aims to recruit 1552 English-speaking and Spanish-speaking adults with imaging-confirmed appendicitis. Participants are randomised to appendectomy or 10 days of antibiotics (including an option for complete outpatient therapy). A total of 500 patients who decline randomisation but consent to follow-up will be included in a parallel observational cohort. The primary analytic outcome is quality of life (measured by the EuroQol five dimension index) at 4âweeks. Clinical adverse events, rate of eventual appendectomy, decisional regret, return to work/school, work productivity and healthcare utilisation will be compared. Planned exploratory analyses will identify subpopulations that may have a differential risk of eventual appendectomy in the antibiotic treatment arm.
ETHICS AND DISSEMINATION: This trial was approved by the University of Washington\u27s Human Subjects Division. Results from this trial will be presented in international conferences and published in peer-reviewed journals.

Overview of the 2010 Carbonaceous Aerosols and Radiative Effects Study (CARES)
Substantial uncertainties still exist in the scientific understanding of the possible interactions between urban and natural (biogenic) emissions in the production and transformation of atmospheric aerosol and the resulting impact on climate change. The US Department of Energy (DOE) Atmospheric Radiation Measurement (ARM) program\u27s Carbonaceous Aerosol and Radiative Effects Study (CARES) carried out in June 2010 in Central Valley, California, was a comprehensive effort designed to improve this understanding. The primary objective of the field study was to investigate the evolution of secondary organic and black carbon aerosols and their climate-related properties in the Sacramento urban plume as it was routinely transported into the forested Sierra Nevada foothills area. Urban aerosols and trace gases experienced significant physical and chemical transformations as they mixed with the reactive biogenic hydrocarbons emitted from the forest. Two heavily-instrumented ground sites-one within the Sacramento urban area and another about 40 km to the northeast in the foothills area-were set up to characterize the evolution of meteorological variables, trace gases, aerosol precursors, aerosol size, composition, and climate-related properties in freshly polluted and aged urban air. On selected days, the DOE G-1 aircraft was deployed to make similar measurements upwind and across the evolving Sacramento plume in the morning and again in the afternoon. The NASA B-200 aircraft, carrying remote sensing instruments, was also deployed to characterize the vertical and horizontal distribution of aerosols and aerosol optical properties within and around the plume. This overview provides: (a) the scientific background and motivation for the study, (b) the operational and logistical information pertinent to the execution of the study, (c) an overview of key observations and initial findings from the aircraft and ground-based sampling platforms, and (d) a roadmap of planned data analyses and focused modeling efforts that will facilitate the integration of new knowledge into improved representations of key aerosol processes and properties in climate models
