Could Dietary Goals and Climate Change Mitigation Be Achieved Through Optimized Diet? The Experience of Modeling the National Food Consumption Data in Italy
Objective: The aim of this study is to define a healthy and sustainable diet model with low GHGE, fulfilling dietary requirements, and considering current Italian food consumption patterns. Design: A duly designed database was developed, linking food nutritional composition and GHGE based on 921 food items consumed in Italy according to the last national food consumption survey (INRAN-SCAI 2005–2006). Linear programming was used to develop new diet plans separately for males and females, aged 18–60 years (n = 2,098 subjects), in order to minimize GHGE. The program is based on dietary goals and acceptability constraints as well as on 13 nutrient requirement constraints aiming to reach a healthy and acceptable diet for the Italian population. Results: Diet optimization resulted in a nutritionally adequate pattern minimizing GHGE values (4.0 vs. 1.9 kg CO2e/day for males and 3.2 vs. 1.6 kg CO2e/day for females). In both sexes, the nutrient intake of the optimized diet was at the established lower bound for cholesterol and calcium and at the established upper bound for free sugar and fiber. In males, intake of zinc was at the established lower bound whereas iron was at the established upper bound. Consumption of red meat and fruit and vegetables was at the established lower and upper bound, respectively, in both males and females. Despite the decrease in meat consumption, especially red meat, in the optimized diet with respect to the observed diet, levels of iron intake in females increased by 10% (10.3 vs. 11.3 mg/day) but remained below the adequate intake established in Italian national DRIs. Conclusions: An attainable healthy dietary pattern was developed that would lead to the reduction of GHGE by 48% for males and by 50% for females with respect to current food consumption in the Italian adult population. Health-promoting dietary patterns can substantially contribute to achieve related Sustainable Development Goals
Early and late improvement of global and regional left ventricular function after transcatheter aortic valve implantation in patients with severe aortic stenosis: an echocardiographic study.
The recent development of transcatheter aortic valve implantation (TAVI) for severe aortic stenosis (AS) treatment offers a viable option for high-risk patient categories. Our aim is to evaluate whether 2D strain and strain rate can detect subtle improvement in global and regional LV systolic function immediately after TAVI. 2D conventional and 2D strain (speckle analysis) echocardiography was performed before, at discharge and after three months in thirty three patients with severe AS. After TAVI, we assessed by conventional echocardiography an immediate reduction of transaortic peak pressure gradient (p<0.0001), of mean pressure gradient (p<0.0001) and a concomitant increase in aortic valve area (AVA: 1.08±0.31 cm(2)/m(2); p<0.0001). 2D longitudinal systolic strain showed a significant improvement in all patients, both at septal and lateral level, as early as 72 h after procedure (septal: -14.2±5.1 vs -16.7±3.7%, p<0.001; lateral: -9.4±3.9 vs -13.1±4.5%, p<0.001; respectively) and continued at 3 months follow-up (septal: -18.1±4.6%, p<0.0001; lateral: -14.8±4.4%, p<0.0001; respectively). Conventional echocardiography after TAVI proved a significant reduction of LV end-systolic volume and of LV mass with a mild improvement of LV ejection fraction (EF) (51.2±11.8 vs 52.9±6.4%; p<0.02) only after three months. 2D strain seems to be able to detect subtle changes in LV systolic function occurring early and late after TAVI in severe AS, while all conventional echo parameters seem to be less effective for this purpose. Further investigations are needed to prove the real prognostic impact of these echocardiographic findings
Wigs, disguises and child's play : solidarity in teacher education
It is generally acknowledged that much contemporary education takes place within a dominant audit culture, in which accountability becomes a powerful driver of educational practices. In this culture both pupils and teachers risk being configured as a means to an assessment and target-driven end: pupils are schooled within a particular paradigm of education. The article discusses some ethical issues raised by such schooling, particularly the tensions arising for teachers, and by implication, teacher educators who prepare and support teachers for work in situations where vocational aims and beliefs may be in in conflict with instrumentalist aims. The article offers De Certeau’s concept of ‘la perruque’ to suggest an opening to playful engagement for human ends in education, as a way of contending with and managing the tensions generated. I use the concept to recover a concept of solidarity for teacher educators and teachers to enable ethical teaching in difficult times
Psychopathological profile in COVID-19 patients including healthcare workers: the implications
OBJECTIVE: The effects of COVID-19 seem to extend beyond the physical pain and is showing psychiatric implications as well. Moreover, psychopathological implications seem to last also after patients' discharge. Our goal is to investigate the psychological impact and psychopathological outcome of patients affected by COVID-19.PATIENTS AND METHODS: We have engaged 34 patients with COVID-19 conditions [eight of them were healthcare workers patients (HCW)] hospitalized at "Policlinico Gemelli Foundation" of Rome, Italy. All patients were evaluated through the Impact of Event Scale-Revised (IES-R) and the Symptom Checklist 90-R (SCL-90-R) first, during their hospitalization (baseline), and then, after 4 months from hospital discharge (follow-up), through phone interviews.RESULTS: At baseline, 82% of patients revealed from mild to severe psychological impact of COVID-19, according to the IES-R. At follow-up, the mean IES-R total score was significantly decreased (p<0.001) even if almost half (46.6%) of our cohort still showed it. HCW patients showed a significantly higher score than other patients at IES-R scale, both at baseline (p=0.005) and at follow-up (p<0.001). Moreover, at 4 months from discharge, they showed a significantly higher percentage of moderate and severe distress (p=0.015). In addition to this. at follow-up, our cohort of patients showed an increase of anxiety symptoms, even if not significant compared to baseline (46.7% vs. 35.3% respectively; p=1.000). and HCW patients suffered more sleep disorders (p=0.019) and anxiety symptoms (p=0.019) compared to other patients.CONCLUSIONS: We indicate the importance of assessing psychopathology of COVID-19 survivors, monitoring their changes over time, and providing psychological support to improve their psychological well-being
Volcanic CO2 tracks the incubation period of basaltic paroxysms
The ordinarily benign activity of basaltic volcanoes is periodically interrupted by violent paroxysmal explosions ranging in size from Hawaiian to Plinian in the most extreme examples. These paroxysms often occur suddenly and with limited or no precursors, leaving their causal mechanisms still incompletely understood. Two such events took place in summer 2019 at Stromboli, a volcano otherwise known for its persistent mild open-vent activity, resulting in one fatality and damage to infrastructure. Here, we use a post hoc analysis and reinterpretation of volcanic gas compositions and fluxes acquired at Stromboli to show that the two paroxysms were preceded by detectable escalations in volcanic plume CO2 degassing weeks to months beforehand. Our results demonstrate that volcanic gas CO2 is a key driver of explosions and that the preparatory periods ahead of explosions in basaltic systems can be captured by precursory CO2 leakage from deeply stored mafic magma
Severe Aortic Stenosis and Myocardial Function: Diagnostic and Prognostic Usefulness of Ultrasonic Integrated Backscatter Analysis
Background— The aim of this study was to assess the myocardial reflectivity pattern in severe aortic valve stenosis through the use of integrated backscatter (IBS) analysis. Patients with aortic stenosis (AS) were carefully selected in the Department of Cardiology.
Methods and Results— Thirty-five subjects (AS: valve orifice ≤1 cm2; 12 female; mean age, 71.8±6.2 years) and 25 healthy subjects were studied. All subjects of the study had conventional 2D-Doppler echocardiography and IBS. Backscatter signal was sampled at the septum and posterior wall levels. Patients with AS were divided into 2 groups: 16 patients with initial signs of congestive heart failure and a depressed left ventricular systolic function (DSF) (ejection fraction [EF] range, 35% to 50%) and 19 asymptomatic patients with normal left ventricular systolic function (NSF) (EF >50%). Myocardial echo intensity (pericardium related) was significantly higher at the septum and posterior wall levels in DSF than in NSF and in control subjects. IBS variation, as an expression of variation of the signal, appeared to be significantly lower in AS with DSF than in NSF and in control subjects, at both the septum and posterior wall levels. Patients with DSF underwent aortic valve replacement, and, during surgical intervention, a septal myocardial biopsy was made for evaluation of myocardium/fibrosis ratio. Abnormally increased echo intensity was detected in left ventricular pressure overload by severe aortic stenosis and correlated with increase of myocardial collagen content (operating biopsy).
Conclusions— One year after aortic valve replacement, we observed a significant reduction of left ventricular mass, and, only if pericardial indexed IBS value (reduction of interstitial fibrosis) decreased, it was possible to observe an improvement of EF and of IBS variation
Zermelo Navigation in the Quantum Brachistochrone
We analyse the optimal times for implementing unitary quantum gates in a constrained finite dimensional controlled quantum system. The family of constraints studied is that the permitted set of (time dependent) Hamiltonians is the unit ball of a norm induced by an inner product on su(n). We also consider a generalisation of this to arbitrary norms. We construct a Randers metric, by applying a theorem of Shen on Zermelo navigation, the geodesics of which are the time optimal trajectories compatible with the prescribed constraint. We determine all geodesics and the corresponding time optimal Hamiltonian for a specific constraint on the control i.e. k (Tr(Hc(t)^2) = 1 for any given value of k > 0. Some of the results of Carlini et. al. are re-derived using alternative methods. A first order system of differential equations for the optimal Hamiltonian is obtained and shown to be of the form of the Euler Poincare equations. We illustrate that this method can form a methodology for determining which physical substrates are effective at supporting the implementation of fast quantum computation
Ground deformation reveals the scale-invariant conduit dynamics driving explosive basaltic eruptions
The mild activity of basaltic volcanoes is punctuated by violent explosive eruptions that occur without obvious precursors. Modelling the source processes of these sudden blasts is challenging. Here, we use two decades of ground deformation (tilt) records from Stromboli volcano to shed light, with unprecedented detail, on the short-term (minute-scale) conduit processes that drive such violent volcanic eruptions. We find that explosive eruptions, with source parameters spanning seven orders of magnitude, all share a common pre-blast ground inflation trend. We explain this exponential inflation using a model in which pressure build-up is caused by the rapid expansion of volatile-rich magma rising from depth into a shallow (<400m) resident magma conduit. We show that the duration and amplitude of this inflation trend scales with the eruption magnitude, indicating that the explosive dynamics obey the same (scale-invariant) conduit process. This scale-invariance of pre-explosion ground deformation may usher in a new era of short-term eruption forecasting
