Immunotherapy and cardiovascular diseases: novel avenues for immunotherapeutic approaches
As current therapies for cardiovascular disease (CVD), predominantly based on lipid lowering, still face an unacceptable residual risk, novel treatment strategies are being explored. Besides lipids, inflammatory processes play a major role in the pathogenesis of atherosclerosis, the underlying cause of the majority of CVD. The first clinical trials targeting the interleukin-1 beta-inflammasome axis have shown that targeting this pathway is successful in reducing cardiovascular events but did not decrease overall CVD mortality. Hence, novel and improved immunotherapeutics to treat CVD are being awaited
Ocular Refraction at Birth and Its Development During the First Year of Life in a Large Cohort of Babies in a Single Center in Northern Italy
The purpose of this study was to investigate refraction at birth and during the first year of life in a large cohort of babies born in a single center in Northern Italy. We also aimed to analyze refractive errors in relation to the gestational age at birth. An observational ophthalmological assessment was performed within 24 h of birth on 12,427 newborns. Refraction was examined using streak retinoscopy after the administration of tropicamide (1%). Values in the range of between +0.50 ≤ D ≤ +4.00 were defined as physiological refraction at birth. Newborns with refraction values outside of the physiological range were followed up during the first year of life. Comparative analyses were conducted in a subgroup of babies with known gestational ages. The following distribution of refraction at birth was recorded: 88.03% of the babies had physiological refraction, 5.03% had moderate hyperopia, 2.14% had severe hyperopia, 3.4%, had emmetropia, 0.45%, had myopia, 0.94% had astigmatism, and 0.01% had anisometropia. By the end of the first year of life, we observed reductions in hyperopia and astigmatism, and stabilization of myopia. Preterm babies had a four-fold higher risk of congenital myopia and a three-fold higher risk of congenital emmetropia as compared to term babies. Refraction profiles obtained at birth changed during the first year of life, leading to a normalization of the refraction values. Gestational age at birth affected the incidence of refractive errors and amblyopia
Widening use of dexamethasone implant for the treatment of macular edema
Sustained-release intravitreal 0.7 mg dexamethasone (DEX) implant is approved in Europe for the treatment of macular edema related to diabetic retinopathy, branch retinal vein occlusion, central retinal vein occlusion, and non-infectious uveitis. The implant is formulated in a biodegradable copolymer to release the active ingredient within the vitreous chamber for up to 6 months after an intravitreal injection, allowing a prolonged interval of efficacy between injections with a good safety profile. Various other ocular pathologies with inflammatory etiopathogeneses associated with macular edema have been treated by DEX implant, including neovascular age-related macular degeneration, Irvine–Gass syndrome, vasoproliferative retinal tumors, retinal telangiectasia, Coats’ disease, radiation maculopathy, retinitis pigmentosa, and macular edema secondary to scleral buckling and pars plana vitrectomy. We undertook a review to provide a comprehensive collection of all of the diseases that benefit from the use of the sustained-release DEX implant, alone or in combination with concomitant therapies. A MEDLINE search revealed lack of randomized controlled trials related to these indications. Therefore we included and analyzed all available studies (retrospective and prospective, comparative and non-comparative, randomized and nonrandomized, single center and multicenter, and case report). There are reports in the literature of the use of DEX implant across a range of macular edema-related pathologies, with their clinical experience supporting the use of DEX implant on a case-by-case basis with the aim of improving patient outcomes in many macular pathologies. As many of the reported macular pathologies are difficult to treat, a new treatment option that has a beneficial influence on the clinical course of the disease may be useful in clinical practice
AVHRR Automated detection of volcanic clouds.
A new satellite‐based technique has recently been proposed which seems suitable for an automatic detection of volcanic clouds in daytime conditions. In this paper the robustness of such a new approach, in particular in detecting early eruptive clouds, is evaluated, on several eruptive events at Mt Etna, by using five years of Advanced Very High Resolution Radiometer (AVHRR) data. The detection scheme is discussed together with its possible extension to night‐time monitoring and the improvements expected by its application to the next generation of satellite sensors (in particular Spinning Enhanced Visible and Infrared Imager (SEVIRI)) with enhanced spectral and temporal resolution. The proposed approach seems to overcome the limitations related to other proposed methods which, in some conditions (very fresh eruptive clouds, cold‐backgrounds, etc.), give false or missed detection and will no longer be applicable to the next generation of Geostationary Operational Environmental Satellites (GOES) due to the planned reduction of their thermal infrared channels until 2010
Thermal Performance Investigation by Infrared Analysis of Mini Pulsating Heat Pipe
A promising solution in the field of passive two-phase heat transfer devices is represented by Pulsating Heat Pipes (PHPs). They are undoubtedly appealing due to the high heat transfer capability, efficient thermal control, adaptability and low cost. In the last years they are raising concern for space applications that are characterised by extreme environmental conditions, strictly constrains in terms of compactness, reliability and the need to dissipate efficiently heat in microgravity conditions. In this study, the thermal performance of oscillating heat pipes that consists of extra-thin metallic pipes are investigated: the adoption of metallic pipes with an inner diameter less than 0.4 mm permits to couple flexibility and compactness with high heat transfer performance. HFC-134a is used as working fluid. Many authors have investigated the pulsating behaviour of this type of heat transfer devices only considering the average temperature of the evaporator and condenser. In this work, to deeply investigate the oscillating behaviour of the proposed PHP, it is adopted an approach based on the study of the local temperature distributions on the wall of the PHP, acquired with a high-speed and high-resolution infrared camera. The local analysis of the temperature trends is of fundamental importance in the understanding of the complex phenomena that govern the pulsating field
Positive effects of robotic exoskeleton training of upper limb reaching movements after stroke
This study, conducted in a group of nine chronic patients with right-side hemiparesis after stroke, investigated the
effects of a robotic-assisted rehabilitation training with an upper limb robotic exoskeleton for the restoration of
motor function in spatial reaching movements. The robotic assisted rehabilitation training was administered for a
period of 6 weeks including reaching and spatial antigravity movements. To assess the carry-over of the observed
improvements in movement during training into improved function, a kinesiologic assessment of the effects of the
training was performed by means of motion and dynamic electromyographic analysis of reaching movements
performed before and after training. The same kinesiologic measurements were performed in a healthy control
group of seven volunteers, to determine a benchmark for the experimental observations in the patients’ group.
Moreover degree of functional impairment at the enrolment and discharge was measured by clinical evaluation
with upper limb Fugl-Meyer Assessment scale (FMA, 0–66 points), Modified Ashworth scale (MA, 0–60 pts) and
active ranges of motion. The robot aided training induced, independently by time of stroke, statistical significant
improvements of kinesiologic (movement time, smoothness of motion) and clinical (4.6 ± 4.2 increase in FMA,
3.2 ± 2.1 decrease in MA) parameters, as a result of the increased active ranges of motion and improved cocontraction
index for shoulder extension/flexion. Kinesiologic parameters correlated significantly with clinical
assessment values, and their changes after the training were affected by the direction of motion (inward vs.
outward movement) and position of target to be reached (ipsilateral, central and contralateral peripersonal space).
These changes can be explained as a result of the motor recovery induced by the robotic training, in terms of
regained ability to execute single joint movements and of improved interjoint coordination of elbow and shoulder
joints
Efficacy of Three Different Prophylactic Treatments for Postoperative Nausea and Vomiting after Vitrectomy: A Randomized Clinical Trial
Postoperative nausea and vomiting (PONV) after vitreoretinal surgery may potentially be associated with severe complications, such as suprachoroidal hemorrhage. The purpose of the present multicenter clinical trial (NCT02386059) was to assess the efficacy of three different prophylactic treatments for PONV after vitrectomy under local anesthesia. Patients undergoing primary vitrectomy were randomized to the control arm or to one of the treatment arms (4 mg ondansetron, 4 mg dexamethasone, combination of the two drugs). The primary outcome measure was the proportion of complete response (no nausea, no vomiting, no retching, and no use of antiemetic rescue medication) during 24 h after vitrectomy. Secondary outcomes included the severity standardized score of PONV, postoperative pain standardized score, and rate of ocular and non-ocular adverse events. Baseline demographics of the 1287 patients were comparable between the four arms. The combined therapy group showed a statistically significant lower incidence of PONV compared to the placebo and monotherapy (p < 0.001). PONV severity was also reduced in the combination group compared to the others (p < 0.001). Postoperative pain scores and adverse events were comparable among the four groups. Combined therapy with dexamethasone and ondansetron was the most effective treatment for reducing the incidence and severity of PONV in patients undergoing vitrectomy under local anesthesia
