Az esszenciális zsĂrsavak metabolizmusának zavarai Ă©s terápiás befolyásulásuk lehetĹ‘sĂ©gei a gyermekgyĂłgyászati gyakorlatban = Disturbances of essential fatty acid metabolism and potential therapeutic interventions in the paediatric praxis
Szignifikáns fordĂtott korreláciĂłt találtunk a nĹ‘i tej arachidonsav- Ă©s dokozahexĂ©nsavtartalma valamint a 18 szĂ©natomot tartalmazĂł transz izomĂ©r zsĂrsavak között, ami arra utal, hogy az anyai Ă©trendbe bekerĂĽlĹ‘ transz izomĂ©r zsĂrsavak károsan befolyásolhatják az ĂşjszĂĽlött esszenciális zsĂrsavellátottságát. This observation indicates that maternal dietary intake of trans isomeric fatty acids may untowardly influence the availability of essential fatty acids for the infant. We found significantly higher values of arachidonic and docosahexaenoic acids in milk samples of mothers of preterm than in those of full-term infants. This observation provides further ground for the utilisation of the own mothers' milk in the nutrition of preterm infants. We revealed significant correlations between long-chain polyunsaturated fatty acids in cord vessel wall lipids and neurodevelopment of the offspring both at the age of 3 and 18 months. These data indicate that fatty acid status of expecting women may influence the neurodevelopment of their children at least up to the age of 1.5 years. We demonstrated significant correlations between the -866 G/A polymorphism of uncoupling protein gene and fatty acid status in obese children; this relationship should be taken into consideration at planning studies on modifying fatty acid intake in obese subjects. Az omega-3 zsĂrsav fogyasztásának növelĂ©se kedvezĹ‘ hatásĂş lehet a fokozott cardiovascularis kockázatĂş szemĂ©lyek, köztĂĽk az elhĂzottak számára. A kedvezĹ‘ hatások lĂ©trejöttĂ©nek feltĂ©tele az omega-3 zsĂrsavaknak a szervezet plazmalipideibe törtĂ©nĹ‘ beĂ©pĂĽlĂ©se, azonban a gyermekkorban ehhez szĂĽksĂ©ges szupplementáciĂłs dĂłzisra vonatkozĂłan igen kevĂ©s adat áll rendelkezĂ©sre. A plazmalipidek zsĂrsavösszetĂ©telĂ©t nagy felbontĂłkĂ©pessĂ©gű kapilláris gáz-folyadĂ©k kromatográfiával határoztuk meg.
Eredmények:
Az alfa-linolĂ©nsav-Ă©rtĂ©kek emelkedĹ‘ tendenciája volt megfigyelhetĹ‘ az alfa-linolĂ©nsavval törtĂ©nĹ‘ szupplementáciĂłt követĹ‘en a foszfolipid-, a triacil-glicerin- Ă©s a szterol-Ă©szter-frakciĂłban egyaránt, azonban egyik frakciĂłban sem volt a változás szignifikáns. Következtetés:
Az elhĂzott gyermekekben az Ă©trend 6 hĂ©ten keresztĂĽl napi 1000 mg alfa-linolĂ©nsavval törtĂ©nĹ‘ kiegĂ©szĂtĂ©se csak a plazma nem Ă©szterifikált zsĂrsavaiban növelte szignifikánsan az omega-3 zsĂrsavak arányát, az Ă©szterifikált frakciĂłkban nem volt szignifikáns hatás. ElhĂzott gyermekekben az omega-3-zsĂrsavellátottság befolyásolásához az alfa-linolĂ©nsav-szupplementáciĂł dĂłzisának 1 g/nap fölĂ© törtĂ©nĹ‘ növelĂ©se tűnik szĂĽksĂ©gesnek.

Introduction:
Enhanced dietary intake of omega-3 fatty acids may benefit persons with increased cardiovascular risk, among them obese subjects. Incorporation of omega-3 fatty acids into the plasma lipids is a prerequisite to achieve the favorable effects; however, only very few data are available on the dose of omega-3 fatty acid supplementation in children. The aim of our study was to examine the effects of the consumption of a diet supplemented with 1000 mg alpha-linolenic acid daily on plasma lipids in obese children.
Methods:
In this two times six-week-long, placebo-controlled, crossover study, 9 obese children (age: 13.1 [2.5] years, body mass index: 31.2 [6.2] kg/m
2
), median [IQR]) incorporated into their diet one egg and one meatball (50 g) per day from hens fed diets containing flaxseed oil, i.e. supplementary dietary intake of 1000 mg alpha-linolenic acid per day was provided. The fatty acid composition of plasma lipids was determined by high-resolution gas-liquid chromatography.
Results:
Tendencies of increase were observed in the alpha-linolenic acid content of plasma lipids in the phospholipid, triacyl-glicerine and sterol-ester fractions after the supplementation with alpha-linolenic acid. In the non-esterified fatty acid fraction, the values of alpha-linolenic acid were significantly higher after the supplementation (0.11 [0.08] versus 0.14 [0.20], % weight/weight,

< 0.05), indicating the beginning of the accumulation of alpha-linolenic acid in plasma lipids.
Conclusion:
In obese children a six-week-long supplementation of the diet with 1000 mg alpha-linolenic acid per day increased significantly the contribution of omega-3 fatty acids only to the non-esterified fatty acids of plasma lipids, but had no significant effect on the esterified fractions. Increase of the dose of supplementation may be needed to influence omega-3 fatty acid status in obese children
