Long-Term Outcomes of Trabeculectomy Augmented with Mitomycin C Undertaken within the First 2 Years of Life
To evaluate the long-term effectiveness and safety of mitomycin C (MMC)âaugmented trabeculectomy undertaken within the first 2 years of life for the surgical management of glaucoma.
DESIGN: Retrospective, consecutive, noncomparative case series.
PARTICIPANTS: All children who underwent MMC-augmented trabeculectomy within 2 years of birth between May 2002 and November 2012.
METHODS: The medical records of 40 consecutive eyes of 26 children who underwent surgery by a single surgeon were reviewed. Data collected during routine clinical care were analyzed.
MAIN MEASURE OUTCOMES: Assessment of clinical outcomes included intraocular pressure (IOP), final visual acuity, bleb morphology, surgical complications (early and late), postoperative interventions, and further glaucoma surgery performed. Surgical success was defined as final IOP of 5 mmHg or more and of 21 mmHg or less, with anti-glaucoma medications (qualified success) and without (complete success), stable ocular dimensions and optic disc cupping, and no further glaucoma surgery (including needling) or loss of light perception. Surgical outcomes were evaluated using Kaplan-Meier life table analysis.
RESULTS: Forty eyes of 26 children were studied over a mean follow-up period of 62.8 months. Most cases (80%) were of primary congenital glaucoma after failed goniotomy surgery. Cumulative probabilities of survival at 1, 5, and 7 years were 78%, 67%, and 60%, respectively. Of eyes regarded as successful, 96% (25/26 eyes) had controlled IOP without topical medication and 44% achieved visual acuity of 20/40 or better. In only 1 of the 40 eyes did a cystic avascular bleb develop, with all the other eyes being non-cystic in nature (diffuse and elevated or flat) at final follow-up. Sixty-four percent (9/14 eyes) of cases regarded as failures ultimately underwent glaucoma drainage device implantation.
CONCLUSIONS: A contemporary pediatric trabeculectomy technique augmented with MMC is an effective procedure in the management of glaucoma within the first 2 years of life, as shown by the successful long-term outcomes and low incidence of sight-threatening complications. Trabeculectomy after failed goniotomy surgery or as a primary surgical intervention may offer a phakic infant with glaucoma an excellent opportunity to achieve long-term control of IOP without medications and may be associated with optimal visual outcomes
Discovery limits for a new contact interaction at future hadronic colliders with polarized beams
The production of high-transverse energy jets in hadron-hadroncollisions is
sensitive to the presence of new contact interactions between quarks. If proton
polarization were available, the measurement of some parity violating spin
asymmetries in one-jet production at large transverse energy would complement
the usual search for deviations from the expected QCD cross section. In the
same time, a unique information on the chirality structure of the new
interaction could be obtained. In this context, we compare the potentialities
of various and colliders that are planned or have been proposed,
with the additional requirement of beam polarization.Comment: Latex file, 9 pages and 1 ps fig, definition of the subprocess

Core excitations across the neutron shell gap in ÂČâ°â·Tl
The single closed-neutron-shell, one proton-hole nucleus 207Tl was populated in deep-inelastic collisions of a 208Pb beam with a 208Pb target. The yrast and near-yrast level scheme has been established up to high excitation energy, comprising an octupol
Longitudinally Polarized Photoproduction of Inclusive Hadrons Beyond the Leading Order
We present a complete next-to-leading order QCD calculation for
single-inclusive large-pT hadron production in longitudinally polarized
lepton-nucleon collisions, consistently including ``direct'' and ``resolved''
photon contributions. This process could be studied experimentally at a future
polarized lepton-proton collider like eRHIC at BNL. We examine the sensitivity
of such measurements to the so far completely unknown parton content of
circularly polarized photons.Comment: 15 pages, 7 eps figure
HLA-A, -B, -C, -DRB1, DRB3, DRB4, DRB5 and DQB1 polymorphism detected by PCR-SSP in a semi-urban HIV-positive Ugandan population.
PCR-SSP was used to HLA-type a cohort of Ugandan HIV-positive individuals. The results represent a more comprehensive description of HLA in an African population than previously described and are in concordance with data from a general Black population. Substantial differences exist between this population and Caucasoid populations in which immunological responses to HIV have been investigated; this emphasises that the main HLA-restrictive elements for HIV-specific cytotoxic T lymphocytes will most likely be different for each population
Joint system quantum descriptions arising from local quantumness
Bipartite correlations generated by non-signalling physical systems that
admit a finite-dimensional local quantum description cannot exceed the quantum
limits, i.e., they can always be interpreted as distant measurements of a
bipartite quantum state. Here we consider the effect of dropping the assumption
of finite dimensionality. Remarkably, we find that the same result holds
provided that we relax the tensor structure of space-like separated
measurements to mere commutativity. We argue why an extension of this result to
tensor representations seems unlikely
The relevance of outsourcing and leagile strategies in performance optimization of an integrated process planning and scheduling
Over the past few years growing global competition has forced the manufacturing industries to upgrade their old production strategies with the modern day approaches. As a result, recent interest has been developed towards finding an appropriate policy that could enable them to compete with others, and facilitate them to emerge as a market winner. Keeping in mind the abovementioned facts, in this paper the authors have proposed an integrated process planning and scheduling model inheriting the salient features of outsourcing, and leagile principles to compete in the existing market scenario. The paper also proposes a model based on leagile principles, where the integrated planning management has been practiced. In the present work a scheduling problem has been considered and overall minimization of makespan has been aimed. The paper shows the relevance of both the strategies in performance enhancement of the industries, in terms of their reduced makespan. The authors have also proposed a new hybrid Enhanced Swift Converging Simulated Annealing (ESCSA) algorithm, to solve the complex real-time scheduling problems. The proposed algorithm inherits the prominent features of the Genetic Algorithm (GA), Simulated Annealing (SA), and the Fuzzy Logic Controller (FLC). The ESCSA algorithm reduces the makespan significantly in less computational time and number of iterations. The efficacy of the proposed algorithm has been shown by comparing the results with GA, SA, Tabu, and hybrid Tabu-SA optimization methods
Performance optimization of a leagility inspired supply chain model: a CFGTSA algorithm based approach
Lean and agile principles have attracted considerable interest in the past few decades. Industrial sectors throughout the world are upgrading to these principles to enhance their performance, since they have been proven to be efficient in handling supply chains. However, the present market trend demands a more robust strategy incorporating the salient features of both lean and agile principles. Inspired by these, the leagility principle has emerged, encapsulating both lean and agile features. The present work proposes a leagile supply chain based model for manufacturing industries. The paper emphasizes the various aspects of leagile supply chain modeling and implementation and proposes a new Hybrid Chaos-based Fast Genetic Tabu Simulated Annealing (CFGTSA) algorithm to solve the complex scheduling problem prevailing in the leagile environment. The proposed CFGTSA algorithm is compared with the GA, SA, TS and Hybrid Tabu SA algorithms to demonstrate its efficacy in handling complex scheduling problems
Color Octet Contribution to J/psi Photoproduction Asymmetries
We investigate photoproduction asymmetries in the framework of the
NRQCD factorization approach. It is shown that the color octet contribution
leads to large uncertainties in the predicted asymmetries which rules out the
possibility to precisely measure the gluon polarization in the nucleon through
this final state. For small values of the color octet parameters being
compatible with photoproduction data it appears possible that a
measurement of asymmetries could provide a new test for the NRQCD
factorization approach, on one hand, or a measurement of the polarized gluon
distribution from low inelasticity events , on the otherComment: 12 pages, LaTeX, with 6 figs. Final version published in Phys.Rev.
