    Unsteady pressure measurements on a biconvex airfoil in a transonic oscillating cascade

    Flush-mounted dynamic pressure transducers were installed on the center airfoil of a transonic oscillating cascade to measure the unsteady aerodynamic response as nine airfroils were simultaneously driven to provide 1.2 deg of pitching motion about the midchord. Initial tests were performed at an incidence and angle of 0 deg and A Mach number of 0.65 in order to obtain results in a shock-free compressible flowfield. Subsequent tests were performed at an incidence angle of 7 deg and Mach number of 0.8 in order to observe the surface pressures with an oscillating shock near the leading edge of the airfoil. Results are presented for interblade phase angles of 90 and -90 deg and at blade oscillatory frequencies of 200 and 500 Hz (semi-chord reduced frequencies up to about 0.5 at a Mach number of 0.8). Results from the zero-incidence cascade are compared with a classical unsteady flat-plate analysis. Flow visualization results depicting the shock motion on the airfoils in the high-incidence cascade are discussed. The airfoil pressure data are tabulated

    Patients hearing command hallucinations to harm whose only self-management strategies are to obey these commands, can represent serious safety concerns on inpatient psychiatric units. A comprehensive evidence-based program teaching self-management of auditory hallucinations on inpatient psychiatric units is described that includes five components: suggestions for staff education; patient self-assessment tools; an interview guide and safety protocol; a course to teach strategies for managing distressing voices and commands to harm; suggestions to improve staff communication; and a plan to extend the program from inpatient care settings into the community by sharing materials with community case managers and caregivers when patients are discharged

    Experimental investigation of unsteady flows at large incidence angles in a linear oscillating cascade

    The aerodynamics of a cascade of airfoils oscillating in torsion about the midchord is investigated experimentally at a large mean incidence angle and, for reference, at a low mean incidence angle. The airfoil section is representative of a modern, low aspect ratio, fan blade tip section. Time-dependent airfoil surface pressure measurements were made for reduced frequencies up to 0.8 for out-of-phase oscillations at Mach numbers up to 0.8 and chordal incidence angles of 0 deg and 10 deg. For the 10 deg chordal incidence angle, a separation bubble formed at the leading edge of the suction surface. The separated flow field was found to have a dramatic effect on the chordwise distribution of the unsteady pressure. In this region, substantial deviations from the attached flow data were found with the deviations becoming less apparent in the aft region of the airfoil for all reduced frequencies. In particular, near the leading edge the separated flow had a strong destabilizing influence while the attached flow had a strong stabilizing influence

    Behavioral Management of Auditory Hallucinations: Implementation and Evaluation of a 10-Week Course

    A 10-session behavioral course for self-management of auditory hallucinations in patients with schizophrenia has demonstrated positive outcomes. This article evaluates both the course’s implementation and benefits to patients attending the course. Teleconferencing, electronic media, and 26 monthly conference calls were used to educate six advanced practice nurses (APNs) at six sites about the course implementation. Thirty-two patients within the U.S. Department of Veterans Affairs participated in the course. All of the APNs reported course helpfulness, improved communication with patients about voices, and improved harm assessment. Of the patients, 96% found the course helpful: 67% no longer heard voices to harm self or others, and 60% had improved auditory hallucination intensity scores. The project demonstrated successful implementation and practice integration with APNs’ activities corresponding to Rogers’ stages of innovation adoption. Facilitators and barriers to implementation are also described

    Chinese herbal recipe versus diclofenac in symptomatic treatment of osteoarthritis of the knee: a randomized controlled trial [ISRCTN70292892]

    BACKGROUND: Duhuo Jisheng Wan (DJW) is perhaps the best known and most widely used Chinese herbal recipe for arthralgia, but the clinical study to verify its efficacy is lacking. The purpose of this study was to compare the efficacy of DJW versus diclofenac in symptomatic treatment of osteoarthritis (OA) of the knee. METHODS: This study was a randomized, double-blind, double-dummy, controlled trial. The 200 patients suffering from OA of the knee, were randomized into the DJW and diclofenac group. The patients were evaluated after a run-in period of one week (week 0) and then weekly during 4 weeks of treatment. The clinical assessments included visual analog scale (VAS) score that assessed pain and stiffness, Lequesne's functional index, time for climbing up 10 steps, as well as physician's and patients' overall opinions on improvement. RESULTS: Ninety four patients in each group completed the study. In the first few weeks of treatment, the mean changes in some variables (VAS, which assessed walking pain, standing pain and stiffness, as well as Lequesne's functional index) of the DJW group were significantly lower than those of the diclofenac group. Afterwards, these mean changes became no different throughout the study. Most of the physician's and patients' overall opinions on improvement at each time point did not significantly differ between the two groups. Approximately 30% of patients in both groups experienced mild adverse events. CONCLUSION: DJW demonstrates clinically comparable efficacy to diclofenac after 4 weeks of treatment. However, the slow onset of action as well as approximately equal rate of adverse events to diclofenac might limit its alternative role in treatment of OA of the knee
