Nuclear Pedigree Criteria for the Identification of Individuals Suspected to Be at Risk of an Inherited Predisposition to Gastric Cancer
Gastric cancer is the second most frequently diagnosed malignancy worldwide and therefore represents a significant healthcare burden. Environmental and genetic factors are involved in the development of gastric cancer. To date only one clear genetic predisposition has been identified involving mutations in the E-cadherin gene. The disease phenotype in patients harbouring E-cadherin mutations appears to be specifically related to diffuse gastric cancer. Little is known genetically about the other forms of gastric cancer. Since there is a growing awareness about the necessity of early intervention criteria have been developed that aid the identification of hereditary forms of gastric cancer. Treatment and its side effects in ANCA-associated vasculitides - Study based on POLVAS registry data

Celem publikacji przygotowanej przez zespół powołany przez konsultanta krajowego ds. reumatologii jest usystematyzowanie wiedzy na temat diagnostyki i terapii reumatoidalnego zapalenia stawów. Oprócz rozpoznania reumatoidalnego zapalenia stawów
na podstawie kryteriów ACR, zaproponowano kryteria diagnostyczne wczesnego reumatoidalnego i wczesnego niesklasyfikowanego
zapalenia stawów. Określono w nich przydatność oznaczania przeciwciał antycytrulinowych, ultrasonografii stawów oraz rezonansu
magnetycznego. Podstawowym celem we wczesnym postępowaniu jest ustalenie obecności i aktywności zapalenia na podstawie
wywiadu, badania fizykalnego, badań laboratoryjnych, ewentualnie USG i rezonansu magnetycznego. Postępowanie różnicujące
powinno być prowadzone w miarę postępu choroby, do czasu
ustalenia ostatecznego rozpoznania. W każdym przypadku zapale nia stawów (poza infekcyjnym) należy wdrożyć leczenie glikokortykosteroidami w dawce je opanowującej. W zależności od ustalonego rozpoznania stosowanie glikokortykosteroidów powinno być
połączone ze stosowaniem leku podstawowego, nie później jednak
niż od 4. mies. Preferowanym lekiem jest metotreksat w dawce tygodniowej 15–25 mg. W przypadku jego nietolerancji sugerowane
jest zastosowanie leflunomidu. Brak skuteczności monoterapii
którymkolwiek z nich jest wskazaniem do leczenia skojarzonego
kilkoma lekami modyfikującymi przebieg choroby. Leki antycytokinowe należy wdrożyć w przypadku nieskuteczności tego postępowania, powinno ono być połączone z podawaniem metotreksatu
w pełnych dawkach, wyjątkowo innego leku podstawowego. Leczenie rytuksymabem i abataceptem stosuje się u chorych, u których powyższe postępowanie jest nieskuteczne. Niesteroidowe leki przeciwzapalne stanowią leczenie uzupełniające tylko
w okresach zaostrzeń. Przez wszystkie lata powinna być stosowana fizjoterapia. Celem leczenia jest uzyskanie i podtrzymywanie
małej aktywności choroby ocenianej wg kryteriów DAS 28.The aim of the publication prepared by the panel formed by the
State Consultant for Rheumatology is to summarize knowledge on
diagnosis and treatment of rheumatoid arthritis. Except diagnosing
rheumatoid arthritis on the basis of ACR criteria, diagnostic criteria
for early rheumatoid and early unclassified arthritis have been
proposed. These criteria establish the usefulness of anticitruline
antibody determinations, ultrasonography of joints and magnetic
resonance. The most important procedure in early detection is to
establish the presence and activity of inflammatory process, based
on history, physical examination, laboratory tests and, possibly,
ultrasonography and magnetic resonance. Differentiation should
progress following the disease progress, until final diagnosis is
established. In each case of arthritis (except infective arthritis)
treatment with glucocorticoids should be employed, at the dose allowing suppression of symptoms. Depending on established
diagnosis, this treatment should be accompanied by first-line
medication, not later than from the 4th month. Preferred medication
is methotrexate at a weekly dose of 15-25 mg. In the case of
intolerance, it is suggested to use leflunomide. Lack of efficiency of
monotherapy with either of the above mentioned medications is an
indication for a combined treatment with several drugs modifying
the course of the disease. Anticytokine drugs should be used in the
case of inefficiency of such a procedure; this should be connected
with full doses of methotrexate or, in exceptional cases, with
another first-line drug. Treatment with rituximab and abatacept is
reserved for patients in whom the above therapy is ineffective. Nonsteroid anti-inflammatory drugs offer an adjuvant therapy option
only during exacerbations. During all these years, physiotherapy
should be continued. Treatment should aim at achieving and
maintaining low activity of the disease, evaluated according to DAS
28 criteria
Clinical characteristics of Polish patients with ANCA-associated vasculitides-retrospective analysis of POLVAS registry
Objective Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are rare small to medium-size
vessel systemic diseases. As their clinical picture, organ involvement, and factors influencing outcome may differ
between countries and geographical areas, we decided to describe a large cohort of Polish AAV patients coming from
several referral centers-members of the Scientific Consortium of the Polish Vasculitis Registry (POLVAS).
Methods We conducted a systematic multicenter retrospective study of adult patients diagnosed with AAV between
Jan 1990 and Dec 2016 to analyze their clinical picture, organ involvement, and factors influencing outcome. Patients
were enrolled to the study by nine centers (14 clinical wards) from seven Voivodeships populated by 22.3 mln inhabitants (58.2% of the Polish population).
Results Participating centers included 625 AAV patients into the registry. Their distribution was as follows: 417 patients
(66.7%) with GPA, 106 (17.0%) with MPA, and 102 (16.3%) with EGPA. Male-to-female ratios were almost 1:1 for
GPA (210/207) and MPA (54/52), but EGPA was twice more frequent among women (34/68). Clinical manifestations
and organ involvement were analyzed by clinical phenotype. Their clinical manifestations seem very similar to other
European countries, but interestingly, men with GPA appeared to follow a more severe course than the women. Fifty five
patients died. In GPA, two variables were significantly associated with death: permanent renal replacement therapy
(PRRT) and respiratory involvement (univariate analysis). In multivariate analysis, PRRT (OR = 5.3; 95% confidence interval (CI) = 2.3–12.2), respiratory involvement (OR = 3.2; 95% CI = 1.06–9.7), and, in addition, age > 65 (OR = 2.6;
95% CI = 1.05–6.6) were independently associated with death. In MPA, also three variables were observed to be
independent predictors of death: PRRT (OR = 5.7; 95% CI = 1.3–25.5), skin involvement (OR = 4.4; 95% CI = 1.02–
19.6), and age > 65 (OR = 6.3; 95% CI = 1.18–33.7).
Conclusions In this first multicenter retrospective study of the Polish AAV patients, we have shown that their demographic
characteristics, disease manifestations, and predictors of fatal outcome follow the same pattern as those from other European
countries, with men possibly suffering from more severe course of the disease

Zastosowanie sekukinumabu w leczeniu chorych na łuszczycowe zapalenie stawów — stanowisko ekspertów Polskiego Towarzystwa Reumatologicznego i Polskiego Towarzystwa Dermatologicznego
Sekukinumab jest lekiem biologicznym, ludzkim monoklonalnym przeciwciałem skierowanym przeciwko interleukinie 17A stosowanym w leczeniu chorych na łuszczycowe zapalenie stawów. Lek może być stosowany po nieskuteczności metotreksatu lub leków biologicznych a u chorych na postać osiową choroby jako pierwszy lek, po nieskuteczności niesteroidowych leków przeciwzapalnych. Ze względu na mechanizm działania odmienny od dotychczas stosowanych leków, znaczącą skuteczność i duże bezpieczeństwo stosowania wprowadzenie sekukinumabu do leczenia chorych na łuszczycowe zapalenie stawów istotnie poszerzy możliwości dostępnej farmakoterapii tej choroby.
Sekukinumab w leczeniu pacjentów z osiową spondyloatropatią — stanowisko ekspertów Polskiego Towarzystwa Reumatologicznego
Zesztywniające zapalenie stawów kręgosłupa (ZZSK) to przewlekła, autozapalna choroba reumatologiczna charakteryzująca się przewlekłym procesem zapalnym, uszkodzeniem strukturalnym stawów krzyżowo-biodrowych w badaniu radiograficznym oraz wzmożoną osteogenezą w szkielecie osiowym. Aktualne wytyczne dotyczące leczenia systemowego w ZZSK obejmują niesteroidowe leki przeciwzapalne oraz inhibitory czynnika martwicy nowotworu alfa (TNF-alpha). Inhibitory TNF stanowią bardzo skuteczną opcję terapeutyczną u pacjentów z osiową spondyloatropatią, ale część chorych nie odpowiada na stosowane leczenie lub występują u nich objawy niepożądane. W ostatnich latach zainteresowania skupiły się nad rolą IL-17 w patogenezie spondyloatropatii jako potencjalnego celu terapii. Sekukinumab jest w pełni ludzkim przeciwciałem monoklonalnym przeciwko IL-17A, którego skuteczność w leczeniu pacjentów z ZZSK potwierdzono w badaniach klinicznych. W dwóch badaniach 3 fazy sekukinumab zmniejszał aktywność choroby w stopniu porównywalnym z inhibitorami TNF z jednoczesnym dobrym profilem bezpieczeństwa. Leczenie sekukinumabem prowadziło także do istotnej poprawy jakości życia uwarunkowanej stanem zdrowia oraz do zmniejszenia stopnia niesprawności i zmęczenia u pacjentów z aktywnym ZZSK. Wyniki 2-letniej obserwacji sugerują ponadto, że terapia sekukinumabem zmniejsza progresję zmian radiograficznych w kręgosłupie, co wymaga potwierdzenia w kontrolowanym badaniu w dłuższym horyzoncie czasowym. Sekukinumab jest pierwszym lekiem biologicznym innym niż inhibitory TNF zarejestrowanym do leczenie pacjentów z ZZSK w Stanach Zjednoczonych i Unii Europejskiej

Apremilast jest inhibitorem fosfodiesterazy 4 stosowanym w leczeniu chorych na łuszczycowe zapalenie stawów. Zespół ekspertów Polskiego Towarzystwa Reumatologicznego (PTR) i Polskiego Towarzystwa Dermatologicznego (PTD) uważa, że apremilast może stanowić dodatkową opcję terapeutyczną w aktualnym schemacie postępowania terapeutycznego po stwierdzeniu nieskuteczności klasycznych syntetycznych leków modyfikujących przebieg choroby lub ich nietolerancji
An expert opinion of the Polish Cardiac Society Working Group on Pulmonary Circulation and the Polish Society for Rheumatology on the diagnosis and treatment of pulmonary hypertension in patients with connective tissue disease
Systemic connective tissue diseases (CTDs) comprise a large group of diseases that are auto-immune in nature and characterized by the involvement of multiple systems and organs. Pul-monary hypertension (PH) of various etiologies may develop in the course of CTD, including pulmonary arterial hypertension (PAH), PH secondary to the lung disease, postcapillary PH in the course of left heart disease, and chronic thromboembolic pulmonary hypertension (CTEPH). In addition, the different forms of PH may coexist with each other. Among patients with CTD, PAH occurs most commonly in those with systemic sclerosis, where it affects ap-proximately 8%–12% of patients. The prognosis in patients with untreated PAH is very poor. It is particularly important to identify the high-risk CTD-PAH population and to perform effi-cient and accurate diagnostics so that targeted therapy of the pulmonary arteries can be intro-duced. Echocardiography is used to screen for PH, but clinical and echocardiographic suspicion of PH always requires confirmation by right heart catheterization. Confirmation of PAH ena-bles the initiation of life-prolonging pharmacological treatment in this group of patients, which should be administered in referral centers.
