Income and consumption inequality in Poland, 1998–2008
This paper estimates a variety of inequality indices to study the evolution of income and consumption inequality in Poland between 1998 and 2008. We use robust methods to adjust for the impact of extremely large observations. We also conduct statistical tests on inequality changes using methods, which account for the complexity of the household sample design. All analyses are performed for the entire population, for rural and urban subpopulations, and for the three largest cities. The main result is that during 1998–2008 there was a statistically significant rise in economic inequalities in Poland, which depending on the inequality index, ranged from 8.7% to 19.6% in case of income distribution and from 6.5% to 12.3% in case of consumption distribution. Among the studied subpopulations, economic inequalities are both the highest and the fastest-growing in Warsaw, where consumption inequality as measured by the Gini index increased during the studied period by as much as almost 23%.income inequality, consumption inequality, Pareto model, robust estimation, statistical inference, Poland
Comparison of the efficacy of dexketoprofen and diclofenac in treatment of non-specific low back pain
Work-related loads, improper lifestyle, increasing obesity, and lack of adequate prophylaxy render low back pain (LBP) one of the most common causes of chronic pain worldwide. Objective. The aim of the study was to compare the effect of two analgesic drugs on the effectiveness of therapy measured by pain intensity. and the degree of disability during treatment of chronic low back pain syndrome Material and method. The retrospective analysis involved 185 patients undergoing treatment for chronic low back pain syndrome with dexketoprofen (DEX) and diclofenac (DIC). Patients’ gender. place of residence. cause of the pain as well as pain intensity in the visual-analogue scale (VAS) and the disability degree (Oswestry Disability Index – ODI) were analysed. Results. From the first week of treatment to the end of the observation. the DEX group exhibited significantly lower values of pain intensity on the disability index. The correlation coefficients between the parameters were significantly higher in the DEX group. Analysis of variance demonstrated that the choice of NSAIDs was the most significant factor determining the effectiveness of the treatment. Discussion. The cause of the pain and place of residence did not have any impact on the treatment efficacy. The pharmacological properties of dexketoprofen contribute to its beneficial effect on the therapy used. which validates the potential use of DEX in LBP management. Summary. The significantly increased correlation between the aforementioned parameters suggests that administration of dexketoprofen in the management of non-specific low back pain results in a more rapid return to full physical activity and therefore more prompt return to work
Active – Passive: On Preconceptions of Testing, Journal of Telecommunications and Information Technology, 2011, nr 3
In telecommunications and software engineering, testing is normally understood to be essentially active: a tester is said to stimulate, control, and enforce. Passive testing does not fit this paradigm and thus remains the niche research subject, which bears on the scope and depth of the obtained results. It is argued that such limited understanding of testing is one of its many community-bound preconceptions. It may be acceptable in the current engineering approach to testing, but can and should be challenged in order to converge on the core concepts of the proposed science of testing (“testology”). This methodological work aims at establishing that there are no fundamental reasons for admitting the dominant role of the active element in testing. To show this, external (also extra- technical) areas are consulted for insight, direct observations, and metaphors. The troublesome distinction between (passive) testing and monitoring, as well as unclear relations between testing and measurements, are also addressed
Gabapentin and tramadol for the treatment of postherpetic neuralgia. Preliminary study
Wstęp. Neuralgia po półpaścu jest zespołem bólowym występującym u 10–15%
pacjentów po przebytej infekcji spowodowanej wirusem varicella zoster, który często
nie poddaje się leczeniu. Celem pracy była ocena skuteczności gabapentyny i tramadolu
w leczeniu neuralgii po półpaścu u pacjentów wcześniej leczonych karbamazepiną.
Materiał i metody. Badaniem objęto 14 chorych. Średni czas od ustąpienia
zmian skórnych wynosił 25,8 miesiąca (SD = 16,0). U wszystkich chorych stwierdzono
ból w obszarze 2–5 segmentów międzyżebrowych, średnio 3,4 segmentu (SD = 0,7)
o natężeniu średnio 7 punktów w skali VAS (SD = 0,73). Wszystkich chorych wcześniej
leczono karbamazepiną. Po upływie 2 tygodni od momentu odstawienia karbamazepiny
rozpoczynano leczenie tramadolem w stałej dawce 200 mg/dobę i gabapentyną od dawki
100 mg/dobę, zwiększając ją codziennie o 100 mg. Leczenie uznawano za skuteczne
w przypadku zmniejszenia punktacji w sali VAS w 2., 3., 4. i 5. tygodniu leczenia.
Wyniki. Skuteczna dzienna dawka gabapentyny wynosiła 900–2400 mg, średnio
1475 mg (SD = 413). Natężenie bólu podczas ostatniego badania wynosiło średnio
2,58 punktu w skali VAS (SD = 51), a różnica w porównaniu z wartością wyjściową
- 4,42 punktu (SD = 0,9) i była istotna statystycznie (p < 0,05). Stwierdzono,
że na wielkość dawki gabapentyny wpływała liczba zajętych segmentów międzyżebrowych
χ2 = 32,0, p < 0,001. Badanie obejmowało małą grupę pacjentów, zatem
bardzo dobre wyniki leczenia, odbiegające od opisywanych w piśmiennictwie, należy
poddać weryfikacji podczas dalszych badań.
Wnioski. Zastosowanie gabapentyny i tramadolu w leczeniu neuralgii popółpaścowej
daje bardzo dobre efekty terapeutyczne. Stwierdzono dobrą tolerancję proponowanego
modelu leczenia. Wielkość skutecznej dawki gabapentyny może zależeć od obszaru
skóry, w którym stwierdzono ból neuropatyczny.Background. Postherpetic neuralgia (PHN) is a difficult to treat pain syndorme
affecting 10–15% patients with varicella zoster infection. The aim of the study
was to determine the efficacy of PHN treatment with gabapentine and tramadol.
Material and methods. Fourteen patients with PHN aged 54.1 years (SD =
7.6) were included into the study. The mean time from regression of skin lesions
was 25.8 months (SD = 16.0). All patients suffered form neurophatic pain located
in 2–5 thorax dermatoms (average 3.4, SD = 0.7), the mean pain intensity was 7
points in Visual Analogue Scale (VAS). All patients were previously treated with
carbamazepine. After 2 weeks of wash-out period, treatment with tramadol (200
mg/d) and gabapentine (100 mg/d) was initiated. Dosage of gabapentine was increased
by 100 mg every day until effective dose was reached. The effectiveness of treatment
was determined by assessment of VAS in 2, 3, 4 and 5 week of treatment.
Results. Daily effective dose of gabapentine was 900–2400 mg, mean 1475
mg (SD = 413). The mean intensity of pain during last examination was 2.58 in
VAS (SD = 51) and the difference form the initial evaluation of pain intensity
was - 4.42 points (SD = 0,9) and was statistically significant (p < 0.05). The
dosage of gabapetnine correlated with the number of affected skin dermatomes (χ2
= 32.0, p < 0.001). The treated group was small so the unexpectedly good results
should undergo further investigation.
Conclusion. The use of gabapentine with tramadol in treatment of PHN gave
good results. The effective dose of gabapentine may depend on the number of skin
dermatoms affected with neuropatic pain
Look-through approach w kontekście klauzuli beneficial owner mającej zastosowanie przy opodatkowaniu dywidend – źródła transformacji niebezpiecznej koncepcji antyabuzywnej w instrument ochrony interesów podatnika i płatnika
This article analysis an issue of the effects of a payment of dividends to an entity that cannot be considered as a beneficial owner. Against this background, an analysis of the look-through approach is presented. The axiological and normative justification of this concept is indicated. It is interesting that the actual reason for its appearance in the jurisprudence of Polish courts and administrative practice was a draft interpretative act which, incidentally, has never been approved by the Minister of Finance.Publikacja dotyczy analizy zagadnienia skutków wypłaty dywidendy do podmiotu, który nie może być uznany za beneficial owner. Na tym tle zaprezentowano analizę koncepcja look-through approach. Wskazano na aksjologiczne i normatywne uzasadnienie tej koncepcji. Interesujące jest to, że faktyczną przyczyną jej pojawienia się w orzecznictwie polskich sądów i praktyce administracyjnej był projekt aktu interpretacyjnego, który zresztą nigdy nie został zatwierdzony przez Ministra Finansów
Evaluation of the long-term effectiveness of suprascapular nerve block in the treatment of the shoulder pain syndrome - preliminary communication
Background. One of the methods of symptomatic treatment of the cervico-brachial neuralgia, called
shoulder pain syndrome (SPS), regardless of the cause of the complaint, is the suprascapular nerve block
(SNB).
The aim of the study was to evaluate the long-term effctiveness of SNB performed with lidocaine (LID) and
betametazone (BET), assessed using the Visual Analogue Scale (VAS).
Material and methods. The study covered 18 people with SPS. The prognostic block was performed with
10 ml 1% LID (the first phase - Ph I), and after obtaining VAS < 5, in the second phase (Ph II), with 10 ml
of the mixture of 1% LID and 7 mg BET. Pain intensity was assessed by VAS before the block, ½ hour after,
and every 4 weeks, to final observation in the twentieth week. Statistical analysis included descriptive
statistics, paired-samples t-Student test. The p < 0.05 was considered as statistically significant.
Results. The initial mean VAS F I results were 6.83 (SD = 0.76); after SNB they decreased to 2.69 (SD = 1.13)

occurrence was inversely proportional to the duration of the disease. During Ph II, the initial mean VAS values
were 6.34 (SD = 0.61), while after SNB, they equalled 1.4 (SD = 1.1), (p < 0.05) and pain intensity was 2,41
(SD = 1,45) at average.
The SNB efficiency is connected with a relatively easy technique of performing the procedure. The inverse
proportion of the disease duration and the period of analgesia may be related to the phenomenon of pain
chronization.
Conclusions. Suprascapular nerve block may be an effective method of the long-term treatment in SPS.
Application of the mixture of LID and BET gave a decrease in pain intensity in 88.8% of the patients.Wstęp. Jedną z metod objawowego leczenia zespołu bolesnego barku, niezależnie od przyczyny schorzenia,
jest blokada nerwu nadłopatkowego.
Celem pracy była ocena długoterminowej skuteczności blokady nerwu nadłopatkowego z użyciem mieszaniny
lidokainy i betametazonu za pomocą skali wzrokowo-analogowej (VAS).
Materiał i metody. Badaniem objęto 18 osób z zespołem bolesnego barku. Do blokady prognostycznej
użyto 10 ml 1-procentowego roztworu lidokainy (faza pierwsza - F I), a po uzyskaniu wartości poniżej 5
w skali VAS w fazie drugiej (F II) 10 ml mieszaniny 1-procentowej lidokainy i 7 mg betametazonu. Oceniano
natężenie bólu w skali VAS przed zastosowaniem blokady, pół godziny po jej wykonaniu i co 4 tygodnie,
kończąc obserwacje w 20. tygodniu. W analizie statystycznej wykorzystano statystykę opisową oraz test
t-Studenta dla grup powiązanych. Za istotne przyjęto p < 0,05.
Wyniki. Średnie wartości wstępne VAS odnotowane w F I wynosiły 6,83 (SD = 0,76) i po zastosowaniu
blokady nerwu nadłopatkowego zmniejszyły się do 2,69 (SD = 1,13) (p < 0,05), utrzymując się na poziomie
poniżej 5 przez średnio 9,28 dnia (SD = 6,65). Czas do wystąpienia bólu odpowiadającego wartości
przekraczającej 5 w skali VAS był odwrotnie proporcjonalny do długości czasu trwania choroby. Podczas F II
początkowa średnia wartość w skali VAS wyniosła 6,34 (SD = 0,61), natomiast po zastosowaniu blokady
nerwu nadłopatkowego - 1,4 (SD = 1,1, p < 0,05), a natężenie bólu - średnio 2,41 (SD = 1,45).
Skuteczność blokady nerwu nadłopatkowego wiąże się ze stosunkowo łatwą techniką wykonania tego
zabiegu. Zależność odwrotną między długością trwania choroby i okresem analgezji można wiązać ze
zjawiskiem uwalniania się bólu.
Wnioski. Blokada nerwu nadłopatkowego może być skuteczną metodą długoterminowego leczenia bólu
w przebiegu zespołu bolesnego barku.

Structural and biophysical studies of new L-asparaginase variants : lessons from random mutagenesis of the prototypic Escherichia coli Ntn-amidohydrolase
This work reports the results of random mutagenesis of the Escherichia coli class 2 l-asparaginase EcAIII belonging to the Ntn-hydrolase family. New variants of EcAIII were studied using structural, biophysical and bioinformatic methods. Activity tests revealed that the l-asparaginase activity is abolished in all analyzed mutants with the absence of Arg207, but some of them retained the ability to undergo the autoproteolytic maturation process. The results of spectroscopic studies and the determined crystal structures showed that the EcAIII fold is flexible enough to accept different types of mutations; however, these mutations may have a diverse impact on the thermal stability of the protein. The conclusions from the experiments are grouped into six lessons focused on (i) the adaptation of the EcAIII fold to new substitutions, (ii) the role of Arg207 in EcAIII activity, (iii) a network of residues necessary for autoprocessing, (iv) the complexity of the autoprocessing reaction, (v) the conformational changes observed in enzymatically inactive variants and (vi) the cooperativity of the EcAIII dimer subunits. Additionally, the structural requirements (pre-maturation checkpoints) that are necessary for the initiation of the autocleavage of Ntn-hydrolases have been classified. The findings reported in this work provide useful hints that should be considered before planning enzyme-engineering experiments aimed at the design of proteins for therapeutic applications. This is especially important for l-asparaginases that can be utilized in leukemia therapy, as alternative therapeutics are urgently needed to circumvent the severe side effects associated with the currently used enzymes
Wpływ niedokrwienia kończyn dolnych na wyniki chirurgicznej rewaskularyzacji mięśnia sercowego
Wstęp: Współistnienie choroby niedokrwiennej kończyn dolnych u pacjentów
operowanych z powodu choroby wieńcowej jest ważnym, niezależnym czynnikiem zwiększającym
ryzyko zabiegu. Nawet subkliniczna niedrożność naczyń kończyn dolnych daje ten
sam niekorzystny skutek co pełnoobjawowe niedokrwienie kończyn dolnych. Celem
pracy była ocena częstości występowania niedokrwienia kończyn u pacjentów operowanych
z powodu choroby niedokrwiennej serca oraz ocena wpływu współistnienia niedokrwienia
kończyn dolnych na wyniki wczesne pomostowania tętnic wieńcowych.
Materiał i metody: W grupie 125 pacjentów, zakwalifikowanych do chirurgicznej
rewaskularyzacji serca, wyliczono wskaźnik kostkowo-ramienny (ABI). Wśród nich
było 95 mężczyzn (76%) i 30 kobiet (24%). Pacjentów ze wskaźnikiem równym lub
mniejszym od 0,9 zakwalifikowano do grupy I, a pozostałych do grupy II.
Wyniki: Wśród 125 chorych objawowe niedokrwienie kończyn stwierdzono u
7 (5,6%) osób. Wartość ABI mniejszą lub równą 0,9 zanotowano u 32 pacjentów, co
stanowiło 25,6%. U 25 (78,2%) chorych z tej grupy nie wystąpiły objawy niedokrwienia
kończyn dolnych. W grupie I statystycznie częściej zaobserwowano: zawał okołooperacyjny
(p = 0,04), zespół niskiego rzutu (p = 0,02), migotanie przedsionków (p = 0,01),
infekcję rany mostka (p = 0,03).
Wnioski: Niedokrwienie kończyn dolnych u osób operowanych z powodu choroby
wieńcowej stwierdzono u 25,6%, z czego u 78,2% pacjentów nie występowały objawy.
Niedokrwienie kończyn dolnych w sposób istotny wpływa na zwiększenie liczby powikłań
