    Clinical evaluation of dental alignment and leveling with three different

    INTRODUCTION: A wide variety of orthodontic wires made of different alloys is available to be used in orthodontic practice and may produce different clinical responses during tooth movement. OBJECTIVE: This research evaluated the alignment and leveling of lower dental arches after the use of three types of orthodontic wires. METHODS: A sample of 36 patients was randomly divided into 3 groups: stainless steel, multistranded steel and superelastic nickel-titanium, according to the first leveling arches used. In order to observe differences in tooth position and axial inclination of the lower incisors, all patients had lateral cephalometric radiographs taken before the insertion of the first arches and 2 months later. The irregularity index and the curve of Spee were measured, compared between groups and considered influential on the proclination of incisors during the initial phase of alignment and leveling. The Reflex microscope was used to measure the irregularity index, whereas the ANOVA analysis of variance was used to verify differences between groups with regard to the degree of dental alignment and leveling. RESULTS: There were significant differences between groups only at T2 for the irregularity index. CONCLUSION: The NiTi and multistranded steel wires showed greater aligning capacity when compared with stainless steel wires.INTRODUÇÃO: uma grande variedade de fios ortodônticos, compostos por diferentes ligas, está disponível para utilização ortodôntica, podendo gerar respostas clínicas diversas, durante a movimentação dentária. OBJETIVO: este trabalho visa a avaliar o alinhamento e nivelamento das arcadas dentárias inferiores, após a utilização de três tipos de fios ortodônticos. MÉTODOS: uma amostra de 36 pacientes foi aleatoriamente dividida em três grupos, de acordo com os primeiros arcos utilizados para o alinhamento e nivelamento: aço inoxidável convencional, aço multifilamentado e de níquel-titânio superelástico. Para se avaliar as diferenças relacionadas ao posicionamento dentário e inclinação axial dos incisivos inferiores, foram obtidas radiografias cefalométricas de perfil, de todos os pacientes, em duas fases do tratamento: antes da inserção dos primeiros arcos e dois meses após a inserção dos mesmos. O índice de irregularidade dentária (IID) e a profundidade inicial de curva de Spee, fatores influentes sobre a projeção de incisivos durante a fase inicial de alinhamento e nivelamento, foram avaliados e comparados intra- e intergrupos. Para a medição do IID, foi utilizado o Reflex Microscope, possibilitando a mensuração do grau de alinhamento e nivelamento dentário em terceira dimensão. A análise de variância (ANOVA) foi utilizada para se avaliar as diferenças intergrupos em relação ao grau de alinhamento e nivelamento dentário. RESULTADOS: diferenças estatisticamente significativas intergrupos somente foram encontradas em T2, em relação ao índice de irregularidade dentária, já que os fios de NiTi e aço multifilamentado apresentaram maior capacidade de alinhamento do que os fios de aço inoxidável convencionais

    A case report of a 10 years and 11 months patient indicated for treatment of a skeletal Class II due to a maxillary protrusion and deep overbite. The patient expressed great worry related to his self-image because his large overjet resulting in soft tissue profile imbalance due to 6mm of overjet and 8mm of overbite. Therapeutic procedures are described for promotion the facial and cephalometric alterations caused by the Kloehn extra-oral traction and J Hook, moreover the four premolars extraction used to correct the Class II deep overbite malocclusion.Relato de caso clínico de um paciente com 10 anos e 11 meses portador de maloclusão esquelética e dentária de Classe II e sobremordida exagerada. O paciente apresentava grande comprometimento estético e baixa auto-estima devido ao acentuado overjet 6mm e overbite 8mm, o que prejudicava sua harmonia facial. São descritos os procedimentos terapêuticos realizados para o tratamento da maloclusão e as alterações faciais e esqueléticas obtidas, promovidas pelos aparelhos extra-oral de tração cervical e J Hook, além das exodontias de quatro pré-molares para correção de da sobremordida profunda

    Orthodontic preparation for surgical treatment of skeletal Class III malocclusion involves joint planning with an oral and maxillofacial surgeon to address the functional and esthetic needs of the patient. In order to allow surgical manipulation of the jaws in the preoperative phase, the need to achieve a negative overjet through incisor decompensation often leads the orthodontist to extract the upper first premolars. This report illustrates an orthodontic preparation case where due to specific factors inherent in the patient's psychological makeup retroclination of the upper incisors and proclination of the mandibular incisors was achieved without removing any teeth. This case was presented to the Brazilian Board of Orthodontics and Facial Orthopedics (BBO) in partial fulfillment of the requirements for obtaining the BBO Diploma.<br>O preparo ortodôntico para tratamento cirúrgico do padrão esquelético de Classe III envolve o planejamento em conjunto com o cirurgião bucomaxilofacial, com o objetivo de solucionar as necessidades funcionais e estéticas do paciente. A fim de permitir a manipulação cirúrgica das bases ósseas, a obtenção de overjet negativo por meio da descompensação dos incisivos, na fase pré-cirúrgica, leva, com frequência, o ortodontista a optar pela exodontia dos primeiros pré-molares superiores. O presente relato ilustra um caso de preparo ortodôntico no qual, devido a fatores específicos inerentes à questão psicológica da paciente, a retroinclinação dos incisivos superiores e vestibularização dos incisivos inferiores foi realizada sem a remoção de elementos dentários. Esse caso foi apresentado à diretoria do Board Brasileiro de Ortodontia e Ortopedia Facial (BBO) como parte dos requisitos para a obtenção do título de Diplomado pelo BBO

    Influence of surface treatment on shear bond strength of orthodontic brackets

    INTRODUCTION: The shear bond strength of orthodontic brackets bonded to micro-hybrid and micro-particulate resins under different surface treatment methods was assessed. METHODS: Two hundred and eighty test samples were divided into 28 groups (n = 10), where 140 specimens were filled with Durafill micro-particulate resin and 140 with Charisma composite. In 140 samples, a coupling agent (silane) was applied. The surface treatment methods were: Phosphoric and hydrofluoric acid etching, sodium bicarbonate and aluminum oxide blasting, stone and burs. A Universal Instron Machine was used to apply an occlusal shear force directly to the resin composite bracket surface at a speed of 0.5 mm/min. The means were compared using analysis of variance and multivariate regression to assess the interaction between composites and surface treatment methods. RESULTS: Means and standard deviations for the groups were: Sodium bicarbonate jet 11.27±2.78; burs 9.26±3.01; stone 7.95±3.67; aluminum oxide blasting 7.04±3.21; phosphoric acid 5.82±1.90; hydrofluoric acid 4.54±2.87, and without treatment 2.75±1.49. An increase of 1.94 MPa in shear bond strength was seen in Charisma groups. Silane agent application reduced the Charisma shear bond strength by 0.68 Mpa, but increased Durafill means for bicarbonate blasting (0.83), burs (0.98) and stone drilling (0.46). CONCLUSION: The sodium bicarbonate blasting, burs and stone drilling methods produced adequate shear bond strength and may be suitable for clinical use. The Charisma micro hybrid resin composite showed higher shear bond means than Durafill micro particle composite

    Mechanical properties of NiTi and CuNiTi shape-memory wires used in orthodontic treatment. Part 1: stress-strain tests

    OBJECTIVE: This research aimed to compare, through traction tests, eight types of superelastic and heat-activated NiTi archwires, by six trade companies (GAC, TP, Ormco, Masel, Morelli and Unitek) to those with addition of copper (CuNiTi 27ºC and 35ºC, Ormco). METHODS: The tests were performed in an EMIC mechanical testing machine, model DL10000, capacity of 10 tons, at the Military Institute of Engineering (IME). RESULTS: The results showed that, generally, heat-activated NiTi archwires presented slighter deactivation loadings in relation to superelastic. Among the archwires that presented deactivation loadings biologically more adequate are the heat-activated by GAC and by Unitek. Among the superelastic NiTi, the CuNiTi 27ºC by Ormco were the ones that presented slighter deactivation loadings, being statistically (ANOVA) similar, to the ones presented by the heat-activated NiTi archwires by Unitek. When compared the CuNiTi 27ºC and 35ºC archwires, it was observed that the 27ºC presented deactivation forces of, nearly, &#8531; of the presented by the 35ºC. CONCLUSION: It was concluded that the CuNiTi 35ºC archwires presented deactivation loadings biologically less favorable in relation to the other heat-activated NiTi archwires, associated to lower percentage of deformation, on the constant baselines of deactivation, showing less adequate mechanical behavior, under traction, in relation to the other archwires

    OBJETIVO: este estudo clínico prospectivo avaliou as alterações dentoesqueléticas em pacientes portadores de más oclusões de Classe II esquelética e dentária, tratados com o aparelho ortopédico funcional Twin Block (TB). MÉTODOS: a amostra foi dividida em dois grupos de 19 indivíduos cada: o grupo TB, com idade média de 9 anos e 6 meses (d.p. = 10 meses); e o grupo controle, com idade média de 9 anos e 9 meses (d.p. = 13 meses), ambos situados na fase de pré-surto de crescimento puberal. O teste t de Student não paramétrico mostrou que a amostra era homogênea para os valores cefalométricos ao início do estudo. Radiografias cefalométricas ao início do estudo (T1) e ao final dos 12 meses (T2) foram obtidas de todos os pacientes. Os testes de Wilcoxon e Mann-Whitney foram utilizados para avaliar as mudanças intragrupos e intergrupos em T1 e T2. RESULTADOS: a relação molar de Classe I foi obtida em 15 indivíduos do grupo tratado, enquanto nenhuma modificação ocorreu no grupo controle. Não foram observadas alterações na maxila e no padrão vertical de crescimento facial. Um aumento significativo do comprimento total mandibular e um deslocamento anterior da mandíbula ocorreram no grupo tratado (pOBJECTIVES: This prospective clinical study evaluated dentoskeletal changes in Class II malocclusion patients after treatment with the Twin Block functional appliance. METHODS: The treatment group consisted of 19 subjects (mean age 9.5 years) and the control group consisted of 19 subjects (mean age 9.9 years) situated before the pubertal growth spurt. Unpaired Student's t test showed the sample homogeneity at the beginning of the study. Initial (T1) and one year follow-up (T2) cephalometric radiographs were obtained for all subjects. Wilcoxon test and Mann-Whitney test were used to evaluate changes intra and inter groups from T1 to T2. RESULTS: A Class I molar relationship was achieved in 15 subjects of the TB group while no modification occurred in the control group. No significant effect was observed either in the maxilla or in the vertical pattern. A significant increase in total mandibular length and an anterior displacement of the mandibular position occurred in the treated group (p<0.05) as well as an overjet reduction, influenced by significant upper incisor retroclination and lower incisor proclination (p<0.05). CONCLUSION: Class II treatment with the Twin Block appliance in Brazilian patients showed skeletal and dental effects, including increase in mandibular length and incisors compensation, respectively

    OBJECTIVE: This research aimed at comparing the qualitative chemical compositions and the surface morphology of fracture regions of eight types of Nickel (Ni) Titanium (Ti) conventional wires, superelastic and heat-activated (GAC, TP, Ormco, Masel, Morelli and Unitek), to the wires with addition of copper (CuNiTi 27oC and 35oC, Ormco) after traction test. METHODS: The analyses were performed in a scanning electronic microscope (JEOL, model JSM-5800 LV) with EDS system of microanalysis (energy dispersive spectroscopy). RESULTS : The results showed that NiTi wires presented Ni and Ti as the main elements of the alloy with minimum differences in their composition. The CuNiTi wires, however, presented Ni and Ti with a significant percentage of copper (Cu). As for surface morphology, the wires that presented the lowest wire-surface roughness were the superelastic ones by Masel and Morelli, while those that presented the greatest wire-surface roughness were the CuNiTi 27oC and 35oC ones by Ormco, due to presence of microcavity formed as a result of pulling out some particles, possibly of NiTi. 4 The fracture surfaces presented characteristics of ductile fracture, with presence of microcavities. The superelastic wires by GAC and the CuNiTi 27oC and the heat-activated ones by Unitek presented the smallest microcavities and the lowest wire-surface roughness with regard to fracture, while the CuNiTi 35oC wires presented inadequate wire-surface roughness in the fracture region. CONCLUSION: CuNiTi 35oC wires did not present better morphologic characteristics in comparison to the other wires with regard to surfaces and fracture region
