    Capsule Retentions and Incomplete Capsule Endoscopy Examinations: An Analysis of 2300 Examinations

    Aim. To evaluate capsule endoscopy in terms of incomplete examinations and capsule retentions and to find risk factors for these events. Material and Methods. This retrospective and consecutive study includes data from 2300 capsule enteroscopy examinations, performed at four different hospitals in Stockholm, Sweden from 2003 to 2009. Results. The frequency of incomplete examinations was 20%. Older age, male gender, suspected, and known Crohn's disease were risk factors for an incomplete examination. The PillCam capsule had the highest rate of completed examinations. Capsule retention occurred in 1.3% (n = 31). Risk factors for capsule retention were known Crohn's disease and suspected tumor. Complications of capsule retention were acute obstructive symptoms in six patients and one death related to complications after acute surgical capsule retrieval. Conclusion: Capsule endoscopy is considered a safe procedure, although obstructive symptoms and serious complications due to capsule retention can be found in a large series of patients

    Improvisation Àr konsten att komponera, uttala, arrangera eller framstÀlla nÄgonting ej förberett. Hur gör man dÄ för att lÀra sig musikalisk improvisation? Vanligen sker detta med hjÀlp av en musikteoretisk ingÄng. Detta arbete syftar dÀrför till att ta reda pÄ vad ett inlÀrningssÀtt pÄ en gehörsbaserad grund kan ha för effekter. Metoden har varit att planka olika pianisters solon pÄ samma lÄt och göra en egen inspelning, Àven den pÄ samma lÄt bÄde före och efter plankningen för att kunna jÀmföra dessa bÄda inspelningar och observera vilka resultat arbetet givit. Det har Àven gjorts en inspelning pÄ en annan liknande lÄt bÄde före och efter plankningen för att undersöka om utveckling som skett i den första lÄten Àven Àr överförbart till andra lÄtar. Resultatet tyder pÄ att ett plankningsarbete leder till utveckling inom parametrarna timing och frasering, men att andra tillvÀgagÄngssÀtt Àr nödvÀndiga för att stÀrka sÄdant som rytm och tonmaterial.

    Syftet med föreliggande studie har varit att med hjĂ€lp av kvalitativa intervjuer undersöka hur sex musiklĂ€rare inom ’en-till-en’ undervisning förhĂ„ller sig till att skapa och upprĂ€tthĂ„lla en relation gentemot deras elever i kulturskolan. Tidigare studier föreslĂ„r att lĂ€rare mĂ„ste upprĂ€tthĂ„lla en komplex tvĂ„delad relation som bĂ„de tillhandahĂ„ller den auktoritĂ€ra- och den personliga lĂ€rarrollen. Detta sĂ€tt att betrakta lĂ€rarrollen kallas för Persona, ett begrepp som Ă€r centralt i uppsatsen. LĂ€rarnas utsagor har analyserats med hjĂ€lp av tematisk analysmetod och temana som skapades presenteras och kopplas samman med den teoretiska utgĂ„ngspunkten att lĂ€rare har tvĂ„ roller att upprĂ€tthĂ„lla vad gĂ€ller relationen gentemot eleven. Resultatet visar att den personliga relationen Ă€r av stor vikt för informanterna dĂ„ den synliggör elevens styrkor och svagheter; sĂ„ledes kan lĂ€rarna individanpassa undervisningen för eleverna. Denna relation skapas och upprĂ€tthĂ„lls via lĂ€rarens intresse för eleven. Detta intresse yttrar sig bland annat genom mellanmĂ€nskliga samtal under lektionerna. Dock sĂ„ bör inte detta intresse vara lektionens enskilda fokus. Earlier studies suggest that teachers must uphold a complex two-parted relationship that involves the teacher both as an authority and in a more personal role. This way of observing the teacher role is called Persona, a term central in this essay. The statements of the teachers have been analysed with the help of thematic analysis and the themes which were created are presented and connected with the theoretical perspective that teachers have two roles to uphold when it comes to the relationship towards the student. The result of the study shows that the personal relationship is of great importance for the informants as it throws light upon the students strengths and weaknesses. This makes it possible for the teachers to customize the education to fit the student. The relationship is created and upheld on the basis of interest that the teachers have for their students. This interest makes itself shown, among other ways, through interpersonal conversations during the lessons. The teachers interest in their student shouldn’t be the single focus of the lesson though. Based on the interviews, it appears that the teachers also must provide more authoritarian and impersonal teacher roles in order for the student to actually learn anything. These two teacher roles should in other words not be seen as dichotomies, but should instead be used as a complement to each other

Oct. 2016 R E S E A R C H A R T I C L E Open Access Bleeding from gastrointestinal angioectasias is not related to bleeding disorders -a case control study Abstract Background: Angioectasias in the gastrointestinal tract can be found in up to 3% of the population. They are typically asymptomatic but may sometimes result in severe bleeding. The reasons for why some patients bleed from their angioectasias are not fully understood but it has been reported that it may be explained by an acquired von Willebrand syndrome (AVWS). This condition has similar laboratory findings to congenital von Willebrand disease with selective loss of large von Willebrand multimers. The aim of this study was to find out if AVWS or any other bleeding disorder was more common in patients with bleeding from angioectasias than in a control group. Methods: We compared bleeding tests and coagulation parameters, including von Willebrand multimers, from a group of 23 patients with anemia caused by bleeding from angioectasias, with the results from a control group lacking angioectasias. Results: No significant differences between the two groups were found in coagulation parameters, bleeding time or von Willebrand multimer levels. Conclusion: These results do not support a need for routine bleeding tests in cases of bleeding from angioectasias and do not show an overall increased risk of AVWS among these patients

    Is tramadol associated to bleeding peptic ulcer? A nationwide case-control study in hospitalized Swedish patients.

    AimsTramadol, a widely used analgesic drug, inhibits the reuptake of noradrenaline and serotonin impairing the aggregation function of thrombocytes. However, the risk for severe bleeding has previously not been studied. The aim of the present study is to investigate the association between tramadol and bleeding peptic ulcer in the Swedish population.MethodsIn this register based case-control study based on the Swedish national patient registry and prescription drug registry, we included 18 306 patients hospitalized with a first-time diagnosis of bleeding peptic ulcer. For every case, 4 matched controls were included. To investigate the temporal aspects of tramadol induced bleeding ulcer, exposure was divided into patients with newly initiated and ongoing treatment. To explore a possible confounding by indication, the effect of codeine, a drug also prescribed for the treatment of moderate pain, but not known to affect thrombocyte function, was investigated. Univariable and multivariable logistic regression was used to analyse the association between tramadol use and bleeding ulcer.ResultsTramadol was associated with an increased risk of bleeding ulcer (adjusted odds ratio (aOR) 2.1, 95% confidence interval: (2.0-2.3). The association was stronger for newly initiated treatment with tramadol 2.8 (2.5-3.2) and weaker for ongoing treatment 1.7 (1.6-1.9). Codeine was also associated with an increased risk of bleeding ulcer 1.9 (1.7-2.1) and this association was also stronger for newly initiated treatment with codeine 2.3 (2.0-2.6) and weaker for ongoing treatment 1.7 (1.5-1.9).ConclusionTreatment with tramadol was associated with an increased risk of bleeding peptic ulcer. Most of this association may be mediated by factors related to the pain condition rather than the pharmacologic effect per se
