    Informing the development of an online self-management program for men living with HIV: a needs assessment

    Background: The aim of this mixed methods study was to conduct a multifaceted needs assessment to inform the development of an online self-management program for men living with HIV. The objectives were to describe the health-related quality of life for men living with HIV, the impact of living with HIV, and the perceived problem areas and service and support needs of these men. The needs assessment was conducted in accordance with the PRECEDE model for health promotion program planning.Methods: A survey assessing the quality of life of men living with HIV (n = 72) was conducted and results were compared to Australian normative data. Focus groups were also undertaken with men living with HIV (n = 11) and a multidisciplinary team of service providers working in the area of HIV (n = 11). Focus groups enabled an in-depth description of the impact of HIV on quality of life and perceived problem areas in daily life.Results: HIV-positive men experience significantly lower quality of life when compared with Australian normative data, particularly in those domains concerned with social and emotional aspects of quality of life. Qualitative focus groups yielded an overarching theme ‘The psychosocial impact of HIV’ which contained three sub-themes; (1) Life before and after HIV – a changed identity and its repercussions; (2) Resilience and the importance of social support; (3) Negotiating the practicalities – intimate relationships and disclosure.Conclusions: The findings from this needs assessment highlight the need to target socio-emotional contexts of HIV positive men’s daily lives to improve quality of life and well-being. Intervention priorities for the proposed online self-management program include: (1) managing the emotional impact of HIV; (2) disclosing HIV status to family and friends; (3) maintaining social connectedness; (4) managing HIV within intimate relationships; and (5) disclosure of HIV status to intimate partners

    A Single-Rate Context-Dependent Learning Process Underlies Rapid Adaptation to Familiar Object Dynamics

    Motor learning has been extensively studied using dynamic (force-field) perturbations. These induce movement errors that result in adaptive changes to the motor commands. Several state-space models have been developed to explain how trial-by-trial errors drive the progressive adaptation observed in such studies. These models have been applied to adaptation involving novel dynamics, which typically occurs over tens to hundreds of trials, and which appears to be mediated by a dual-rate adaptation process. In contrast, when manipulating objects with familiar dynamics, subjects adapt rapidly within a few trials. Here, we apply state-space models to familiar dynamics, asking whether adaptation is mediated by a single-rate or dual-rate process. Previously, we reported a task in which subjects rotate an object with known dynamics. By presenting the object at different visual orientations, adaptation was shown to be context-specific, with limited generalization to novel orientations. Here we show that a multiple-context state-space model, with a generalization function tuned to visual object orientation, can reproduce the time-course of adaptation and de-adaptation as well as the observed context-dependent behavior. In contrast to the dual-rate process associated with novel dynamics, we show that a single-rate process mediates adaptation to familiar object dynamics. The model predicts that during exposure to the object across multiple orientations, there will be a degree of independence for adaptation and de-adaptation within each context, and that the states associated with all contexts will slowly de-adapt during exposure in one particular context. We confirm these predictions in two new experiments. Results of the current study thus highlight similarities and differences in the processes engaged during exposure to novel versus familiar dynamics. In both cases, adaptation is mediated by multiple context-specific representations. In the case of familiar object dynamics, however, the representations can be engaged based on visual context, and are updated by a single-rate process

    The positive outlook study- a randomised controlled trial evaluating the effectiveness of an online self-management program targeting psychosocial issues for men living with HIV: a study protocol

    Background: The emergence of HIV as a chronic condition means that people living with HIV are required to takemore responsibility for the self-management of their condition, including making physical, emotional and socialadjustments. This paper describes the design and evaluation of Positive Outlook, an online program aiming toenhance the self-management skills of gay men living with HIV.Methods/design: This study is designed as a randomised controlled trial in which men living with HIV in Australiawill be assigned to either an intervention group or usual care control group. The intervention group willparticipate in the online group program ‘Positive Outlook’. The program is based on self-efficacy theory and uses aself-management approach to enhance skills, confidence and abilities to manage the psychosocial issues associatedwith HIV in daily life. Participants will access the program for a minimum of 90 minutes per week over seven weeks.Primary outcomes are domain specific self-efficacy, HIV related quality of life, and outcomes of health education.Secondary outcomes include: depression, anxiety and stress; general health and quality of life; adjustment to HIV;and social support. Data collection will take place at baseline, completion of the intervention (or eight weeks postrandomisation) and at 12 week follow-up.Discussion: Results of the Positive Outlook study will provide information regarding the effectiveness of onlinegroup programs improving health related outcomes for men living with HIV

    High and low levels of an NTRK2-driven genetic profile affect motor- and cognition-associated frontal gray matter in prodromal Huntington’s disease

    This study assessed how BDNF (brain-derived neurotrophic factor) and other genes involved in its signaling influence brain structure and clinical functioning in pre-diagnosis Huntington’s disease (HD). Parallel independent component analysis (pICA), a multivariate method for identifying correlated patterns in multimodal datasets, was applied to gray matter concentration (GMC) and genomic data from a sizeable PREDICT-HD prodromal cohort (N = 715). pICA identified a genetic component highlighting NTRK2, which encodes BDNF’s TrkB receptor, that correlated with a GMC component including supplementary motor, precentral/premotor cortex, and other frontal areas (p < 0.001); this association appeared to be driven by participants with high or low levels of the genetic profile. The frontal GMC profile correlated with cognitive and motor variables (Trail Making Test A (p = 0.03); Stroop Color (p = 0.017); Stroop Interference (p = 0.04); Symbol Digit Modalities Test (p = 0.031); Total Motor Score (p = 0.01)). A top-weighted NTRK2 variant (rs2277193) was protectively associated with Trail Making Test B (p = 0.007); greater minor allele numbers were linked to a better performance. These results support the idea of a protective role of NTRK2 in prodromal HD, particularly in individuals with certain genotypes, and suggest that this gene may influence the preservation of frontal gray matter that is important for clinical functioning.This project was supported by 1U01NS082074 (V.C. and J.T., co-principal investigators) from the National Institutes of Health, National Institute of Neurological Disorders and Stroke. However, when considering the range and complexity of the processes that are involved in motor learning, even the mere mortals among us exhibit abilities that are impressive. We exercise these abilities when taking up new activities - whether it is snowboarding or ballroom dancing - but also engage in substantial motor learning on a daily basis as we adapt to changes in our environment, manipulate new objects and refine existing skills.     The Dance of Cystic Fibrosis: Experiences of Living with Cystic Fibrosis as an Adult

    Aim: This study sought to understand the experience of living with cystic fibrosis as an adult.Background: Cystic fibrosis is one of the most difficult chronic diseases to manage long‐term because of numerous challenges faced on a day‐to‐day basis. The majority of studies focus on improving symptom response to new treatment regimens with the hope of prolonging life. Yet few qualitative studies explore the experience of adults with cystic fibrosis. What is missing from the current literature are the voices of people living with cystic fibrosis, especially as they age.Design: This study follows a hermeneutic phenomenology design guided by Martin Heidegger’s philosophy.Methods: Semi‐structured interviews were conducted with 9 adults living with cystic fibrosis. Hermeneutic phenomenology guided the data interpretation. This manuscript follows COREQ reporting guidelines.Results: Hermeneutic analysis revealed the overarching theme The Dance of Cystic Fibrosis which is supported by five themes: 1) The Paradox of Control, 2) Living Deaths, 3) Dancing with Death, 4) Relearning to Dance, and 5) Role of the Dance Partner.Conclusions: The experience of living with cystic fibrosis as an adult is multidimensional and unique to each person. Despite the uniqueness, there is a shared experience that manifested as The Dance of Cystic Fibrosis . As life expectancy continues to increase for persons with cystic fibrosis, it is essential that researchers and healthcare professionals intentionally consider the life prolonging effects of the treatment regimen alongside the persons’ experience with those effects.Relevance to Clinical Practice: Nurses at all levels of practice should be prepared to address the multifaceted experience of living with cystic fibrosis through thoughtful incorporation of open‐ended questions. This allows patients to share their experience with nurses, augmenting their practice of delivering holistic care

    Interactions between new and pre-existing dynamics in bimanual movement control

    Motor skills are commonly acquired through practice. This process not only involves acquisition of the particular task demands but also requires overcoming pre-existing modes. In the present study, interactions between new and intrinsic dynamics were evaluated. Accordingly, bimanual finger tapping with a 2:1 ratio was performed according to two training schedules: continuous (consecutive trials) and interrupted (non-consecutive trials with intermediate 1:1 in-phase performances). In addition, in-phase and anti-phase were probed before and after training. Behavioural output was assessed by means of temporal accuracy and variability, whereas neural activation patterns were determined by EEG coherence. Results showed that continuous practice resulted in improved performance with reduced coherence across the motor network. For interrupted practice, behavioural execution ameliorated, although it was inferior to performance with continuous practice. In terms of neural changes, the degree of intrahemispheric and midline connectivity did not reduce with interrupted practice, whereas interhemispheric connectivity increased. This signifies that short-term motor consolidation of the 2:1 task was disrupted due to intermediate performance of the in-phase mode. Furthermore, the probed in-phase and anti-phase pattern showed no behavioural changes, although neural alterations occurred that depended on training schedule and coordination mode. Overall, the observations illustrate bidirectional interactions between new and inherent dynamics during motor acquisition, raising issues about effective methods for learning skills and scheduling of practices in neurorehabilitation
