    Cholesterol Primer for Health Professionals

    As research continues in the field of coronary artery disease, more information is revealed about various etiological factors. Emerging lipoprotein risk factors have been identified and are now starting to surface as instrumental in the cause and prevention of coronary artery disease. In order to conduct comprehensive cholesterol screening programs and counseling sessions a health professional must have a thorough understanding of lipid metabolism. Recent changes in cholesterol guidelines make it necessary to have a review that addresses the specifics of lipid management. A health professional needs an appropriate knowledge base to be able to understand a major coronary artery disease risk factor and thereby more effectively educate the public about lipid management and coronary artery disease risk reduction. Therefore, the purpose of this article is to review the role of cholesterol in both normal physiological functioning and disease causation and to examine the research concerning new emerging cholesterol risk factors

    A Review of Weight Control Strategies and Their Effects on the Regulation of Hormonal Balance

    The estimated prevalence of obesity in the USA is 72.5 million adults with costs attributed to obesity more than 147 billion dollars per year. Though caloric restriction has been used extensively in weight control studies, short-term success has been difficult to achieve, with long-term success of weight control being even more elusive. Therefore, novel approaches are needed to control the rates of obesity that are occurring globally. The purpose of this paper is to provide a synopsis of how exercise, sleep, psychological stress, and meal frequency and composition affect levels of ghrelin, cortisol, insulin GLP-1, and leptin and weight control. We will provide information regarding how hormones respond to various lifestyle factors which may affect appetite control, hunger, satiety, and weight control

    Applying the Theory of Planned Behavior to Sedentariness and Stress

    Objective: The purpose of this study was to evaluate the predictive value of the theory of planned behavior (TPB) in the sedentary behavior (SB) of young and middle-aged U.S. adults. Relationships between SB over a six-week period were examined using socio-demographic characteristics, TPB constructs, and a stress variable. Methods: Participants (n=45, mean age=31 years, 70% female, 83% White) completed surveys that included sociodemographic information, TPB constructs, and the Weekly Stress Inventory. Participants wore an activity monitor for six weeks and completed the stress inventory once weekly over the study period. A longitudinal model was estimated to determine the relationship between TPB constructs, socio-demographic characteristics, and stress level with SB across the six weeks. Results: Activity monitors revealed participants were sedentary for approximately 11 waking hours per day (SD=1.4). Bivariate analyses indicated a small effect between subjective norms and SB. Model fit indices modestly supported TPB constructs in explaining SB (i.e., a 2.3% reduction in person-level error variance); and a modest relationship between greater stress and less SB (i.e., additional 1.4% reduction in person-level error variance). Conclusions: Results cautiously support continued exploration of the TPB in SB research. Like most behaviors, the TPB alone may not fully explain SB. Future research should continue to explore theoretical determinants of SB, expand to include other theoretical models; and include diverse populations. More research is needed to understand the relationship between SB and stress. Practitioners are encouraged to consider both SB and stress in holistic efforts to improve the health of adults

    Effects of Calcium Pyruvate Supplementation During Training On Body Composition, Exercise Capacity, and Metabolic Responses To Exercise

    Objective: We evaluated the effects of calcium pyruvate supplementation during training on body composition and metabolic responses to exercise. Method: Twenty-three untrained females were matched and assigned to ingest in a double blind and randomized manner either 5 g of calcium pyruvate (PYR) or a placebo (PL) twice daily for 30 d while participating in a supervised exercise program. Prior to and following supplementation, subjects had body composition determined via hydrodensiometry; performed a maximal cardiopulmonary exercise test; and performed a 45-min walk test at 70% of pre-training VO2 max in which fasting pre- and post exercise blood samples determined. Results: No significant differences were observed between groups in energy intake or training volume. Univariate repeated measures ANOVA revealed that subjects in the PYR group gained less weight (PL 1.2 ± 0.3, PYR 0.3 ± 0.3 kg, P = 0.04), lost more fat (PL 1.1 ± 0.5; PYR −0.4 ± 0.5 kg, P = 0.03), and tended to lose a greater percentage of body fat (PL 1.0 ± 0.7; PYR −0.65 ± 0.6%, P = 0.07), with no differences observed in fat-free mass (PL 0.1 ± 0.5; PYR 0.7 ± 0.3 kg, P = 0.29). However, these changes were not significant when body composition data were analyzed by MANOVA (P = 0.16). There was some evidence that PYR may negate some of the beneficial effects of exercise on HDL values. No significant differences were observed between groups in maximal exercise responses or metabolic responses to submaximal walking. Conclusions: Results indicate that PYR supplementation during training does not significantly affect body composition or exercise performance and may negatively affect some blood lipid levels

    Associations of Objectively Measured Light-Intensity Activity on Metabolic Risk Factors in Female Adults

    Metabolic syndrome (MetSy) has been defined as a collection of metabolic irregularities that increase the risk of cardiovascular disease, type 2 diabetes, and all-cause mortality. Women are at an even greater risk for developing MetSy when compared to men with prevalence increasing with age. Moderate to vigorous physical activity (MVPA) has shown consistent association with decreases in risk of MetSy; however, light physical activity (LPA) has been studied less often in comparison. PURPOSE: The purpose of this study was to examine relationships between accelerometer-determined LPA and MetSy risk factors (waist circumference (WC), glucose, high density lipoprotein (HDL), triglycerides, systolic blood pressure (SBP), and diastolic blood pressure (DPB)) across women of different age groups. METHODS: A cross-sectional study design was used to examine 68 female participants, ages 20-65, employed in sedentary careers working in an urban area in the south central United States. Venous blood samples were taken from the antecubital area of the arm from all participants who had fasted at least eight hours prior to their scheduled appointment times. Physical activity data was collected for seven days via hip-worn accelerometers with LPA being measured based on Freedson cut-points (100-1951 counts per minute). MetSy was diagnosed based on the criteria set forth by the National Cholesterol Education Program in 2002. Data were analyzed using regression analysis with SPSS version 20.0. Between-subject analysis was conducted for four age groups (group1=20-35; group2=36-45; group3=46-55; group4=55 and older) using LPA, WC, glucose, HDL, triglycerides, SBP, and DPB. RESULTS: Between-subject regression analysis showed HDL as a significant MetSy risk factor in a full model (p=0.02) and a model with age groups as the only independent variable (p=0.00). Post-hoc Tukey tests showed significance differences in HDL among groups 4 and 2 (confidence interval (CI): 4.03, 35.92; p=0.00), and groups 4 and 3 (CI: 6.06, 39.63; p=0.00). Significance dropped below the apriori level of 0.05 in a model based on LPA. Post-hoc Tukey tests revealed non-significant (α\u3e0.05) associations between LPA levels (high, medium, low) and all MetSy values. CONCLUSION: The relationship between LPA and MetSy risk factors has been studied less frequently than MVPA in relation to MetSy risk factors. Based on these findings LPA is not significantly related with these risk factors. Future studies should continue to explore the relationship between LPA and MetSy risk factors among both men and woman, and in different settings

    Association between Total Body Composition and VO2max in Individuals with Mid-Spectrum Chronic Kidney Disease

    Total body composition (TBC), a measure of body fat percentage (%BF), lean body mass (LBM), and bone mineral content (BMC), can be used as a predictor of cardiovascular fitness. Prior studies have established a relationship between TBC and VO2max in healthy individuals over 35 years of age. However, this relationship is poorly understood in chronic disease populations. PURPOSE: To assess the relationship between TBC and cardiovascular fitness in a clinical population of adults with mid-spectrum (stages G2, G3a, and G3b) chronic kidney disease (CKD). METHODS: A cross-sectional analysis was conducted among 24 subjects diagnosed with mid-spectrum CKD. Nine males and 15 females with an average age of 62.25±9.2 years and a glomerular filtration rate (GFR) of 54.38±9.04 ml/min/1.73m2 completed the study. Subjects completed a health screening, dual-energy x-ray absorptiometry (DEXA) scan, and underwent VO2max testing on a treadmill using a modified Bruce protocol. Normality tests, descriptive statistics, Pearson’s correlations, t-tests, and ANOVAs were conducted in SAS v.9.4. RESULTS: The average %BF was 36.28±8.47%, LBM was 117.16±31.32lbs., BMC was 2308.74±735.19g., and VO2max was 20.13±5.04ml/kg/min. VO2max was positively correlated with BMC and LBM (r=0.65, p=0.001 and r=0.75, p\u3c0.001, respectively) and negatively correlated with %BF (r= -0.80, p\u3c0.001). Individuals in later stages of CKD had lower LBM, BMC, and VO2max (p=0.017, p=0.001, and p=0.007, respectively), yet there was no association of CKD stage with %BF or age (p=0.210 and p=0.107). CONCLUSION: TBC was found to be significantly associated with cardiovascular fitness in the study sample. Higher BMD and LBM values were associated with higher VO2max whereas higher %BF was associated with a lower VO2max in individuals with mid-spectrum CKD. Progression of CKD stage was associated with lower LBM, BMC, and VO2max values, indicating a graded effect of CKD stage on cardiovascular fitness

    Effects of Fish Oil Supplementation on Post-Resistance Exercise Muscle Soreness in Untrained Females

    The anti-inflammatory properties of omega-3 fatty acids contained in fish oil may help alleviate symptoms of delayed onset of muscle soreness (DOMS) and improve recovery from exercise. PURPOSE: To examine the effects of fish oil supplementation on the time-course of post-resistance exercise muscle soreness in young untrained females. METHODS: Seventeen non-resistance trained females (age 23.6 ± 3.0 y, % fat 27.4 ± 3.2, weight 60.2 ± 9.6 kg) were randomized into one of two groups: fish oil (6 g/d; 5:1 EPA:DHA) or placebo (6 g/d corn/soy oil). After consuming the supplements for one week, participants underwent a single bout of resistance exercise designed to induce muscle damage. Subjects performed 10 sets to failure of biceps curl machine and leg extensions using 50% of the previously measured 1-repetition maximum. Over the next week, subjective muscle soreness of the upper and lower body was measured via a grounded 10-cm visual analog scale. At 48 hours and 1 week post-exercise, subjects performed a test to determine soreness during functional movements. The comparison-wise error rate was set at p \u3c 0.10. RESULTS: Muscle soreness increased significantly in both groups and peaked at 48 hours post-exercise. The fish oil group perceived less muscle soreness in the lower body than the placebo group (p= 0.06), but there was no difference in the upper body (p= 0.27). The fish oil group reported less perceived soreness during functional movements (p= 0.07 for upper and lower body soreness). Effect sizes, indicating the reduction in muscle soreness that may be attributed to fish oil (Δ effect size between groups), was 0.75 (95% CI: -0.70 – 2.20) for the arms and 0.77 (-0.76 – 2.33) for the legs. The effect size for the functional tests was 0.63 (-0.71 – 1.97) for the arms and 0.57 (-0.85 – 1.99) for the legs. CONCLUSION: Supplementing the diet with 6 grams per day of fish oil may alleviate the muscle soreness experienced after resistance training in young untrained females, but additional studies with larger sample sizes should be conducted to confirm these findings
