    Membrane Mucin Muc4 promotes blood cell association with tumor cells and mediates efficient metastasis in a mouse model of breast cancer

    Mucin-4 (Muc4) is a large cell surface glycoprotein implicated in the protection and lubrication of epithelial structures. Previous studies suggest that aberrantly expressed Muc4 can influence the adhesiveness, proliferation, viability and invasiveness of cultured tumor cells, as well as the growth rate and metastatic efficiency of xenografted tumors. Although it has been suggested that one of the major mechanisms by which Muc4 potentiates tumor progression is via its engagement of the ErbB2/HER2 receptor tyrosine kinase, other mechanisms exist and remain to be delineated. Moreover, the requirement for endogenous Muc4 for tumor growth progression has not been previously explored in the context of gene ablation. To assess the contribution of endogenous Muc4 to mammary tumor growth properties, we first created a genetically engineered mouse line lacking functional Muc4 (Muc4 ko), and then crossed these animals with the NDL (Neu DeLetion mutant) model of ErbB2-induced mammary tumorigenesis. We observed that Muc4 ko animals are fertile and develop normally, and adult mice exhibit no overt tissue abnormalities. In tumor studies, we observed that although some markers of tumor growth such as vascularity and cyclin D1 expression are suppressed, primary mammary tumors from Muc4 ko /NDL female mice exhibit similar latencies and growth rates as Muc4 wt /NDL animals. However, the presence of lung metastases is markedly suppressed in Muc4 ko /NDL mice. Interestingly, histological analysis of lung lesions from Muc4 ko /NDL mice revealed a reduced association of disseminated cells with platelets and white blood cells. Moreover, isolated cells derived from Muc4 ko /NDL tumors interact with fewer blood cells when injected directly into the vasculature or diluted into blood from wild type mice. We further observed that blood cells more efficiently promote the viability of non-adherent Muc4 wt /NDL cells than Muc4 ko /NDL cells. Together, our observations suggest that Muc4 may facilitate metastasis by promoting the association of circulating tumor cells with blood cells to augment tumor cell survival in circulation

    Use of molecular tools to identify patients with indolent breast cancers with ultralow risk over 2 decades

    IMPORTANCE The frequency of cancers with indolent behavior has increased with screening. Better tools to identify indolent tumors are needed to avoid overtreatment. OBJECTIVE To determine if a multigene classifier is associated with indolent behavior of invasive breast cancers in women followed for 2 decades. DESIGN, SETTING, AND PARTICIPANTS This is a secondary analysis of a randomized clinical trial of tamoxifen vs no systemic therapy, with more than 20-year follow-up. An indolent threshold (ultralow risk) of the US Food and Drug Administration–cleared MammaPrint 70-gene expression score was established above which no breast cancer deaths occurred after 15 years in the absence of systemic therapy. Immunohistochemical markers (n = 727 women) and Agilent microarrays, for MammaPrint risk scoring (n = 652 women), were performed from formalin-fixed paraffin-embedded primary tumor blocks. Participants were postmenopausal women with clinically detected node-negative breast cancers treated with mastectomy or lumpectomy and radiation enrolled in the Stockholm tamoxifen (STO-3) trial, 1976 to 1990. EXPOSURES After 2 years of tamoxifen vs no systemic therapy, regardless of hormone receptor status, patients without relapse who reconsented were further randomized to 3 additional years or none. MAIN OUTCOMES AND MEASURES Breast cancer–specific survival assessed by Kaplan-Meier analyses and multivariate Cox proportional hazard modeling, adjusted for treatment, patient age, year of diagnosis, tumor size, grade, hormone receptors, and ERBB2/HER2 and Ki67 status. RESULTS In this secondary analysis of node-negative postmenopausal women, conducted in the era before mammography screening, among the 652 women with MammaPrint scoring available (median age, 62.8 years of age), 377 (58%) and 275 (42%) were MammaPrint low and high risk, respectively, while 98 (15%) were ultralow risk. At 20 years, women with 70-gene high and low tumors but not ultralow tumors had a significantly higher risk of disease-specific death compared with ultralow-risk patients by Cox analysis (hazard ratios, 4.73 [95% CI, 1.38-16.22] and 4.54 [95% CI, 1.40-14.80], respectively). There were no deaths in the ultralow-risk tamoxifen-treated arm at 15 years, and these patients had a 20-year disease-specific survival rate of 97%, whereas for untreated patients the survival rate was 94%. Recursive partitioning identified ultralow risk as the most significant predictor of good outcome. In tumors “not ultralow risk,” tumor size greater than 2 cm was the most predictive of outcome. CONCLUSIONS AND RELEVANCE The ultralow-risk threshold of the 70-gene MammaPrint assay can identify patients whose long-term systemic risk of death from breast cancer after surgery alone is exceedingly low

    Intratumor heterogeneity of the estrogen receptor and the long-term risk of fatal Breast cancer

    Background: Breast cancer patients with estrogen receptor (ER)–positive disease have a continuous long-term risk for fatal Breast cancer, but the biological factors influencing this risk are unknown. We aimed to determine whether high intratumor heterogeneity of ER predicts an increased long-term risk (25 years) of fatal Breast cancer. Methods: The STO-3 trial enrolled 1780 postmenopausal lymph node–negative Breast cancer patients randomly assigned to receive adjuvant tamoxifen vs not. The fraction of cancer cells for each ER intensity level was scored by Breast cancer pathologists, and intratumor heterogeneity of ER was calculated using Rao’s quadratic entropy and categorized into high and low heterogeneity using a predefined cutoff at the second tertile (67%). Long-term Breast cancer-specific survival analyses by in-tra-tumor heterogeneity of ER were performed using Kaplan-Meier and multivariable Cox proportional hazard modeling adjusting for patient and tumor characteristics. Results: A statistically significant difference in long-term survival by high vs low intratumor heterogeneity of ER was seen for all ER-positive patients (P < .001) and for patients with luminal A subtype tumors (P ¼ .01). In multivariable analyses, patients with high intratumor heterogeneity of ER had a twofold increased long-term risk as compared with patients with low intratumor heterogeneity (ER-positive: hazard ratio [HR] ¼ 1.98, 95% confidence interval [CI] ¼ 1.31 to 3.00; luminal A subtype tumors: HR ¼ 2.43, 95% CI ¼ 1.18 to 4.99). Conclusions: Patients with high intratumor heterogeneity of ER had an increased long-term risk of fatal Breast cancer. Interestingly, a similar long-term risk increase was seen in patients with luminal A subtype tumors. Our findings suggest that intratumor heterogeneity of ER is an independent long-term prognosticator with potential to change clinical management, especially for patients with luminal A tumors

    Na prevenção da ocorrência ou na interrupção da evolução de enfermidades infecto-transmissíveis comuns aos animais e aos seres humanos, como é o caso da salmonelose, o uso de um desinfetante capaz de agir sobre o agente causal quando em vida livre, no ambiente, exerce grande importância. No entanto, a resistência microbiana, intrínseca ou adquirida, pode apresentar-se como um limitante no uso deste instrumento sanitário. Objetivando monitorar a sensibilidade da Salmonella Typhimurium, 96 amostras isoladas de suínos abatidos no Estado do Rio Grande do Sul,Brasil, foram confrontadas com dois compostos químicos desinfetantes (origem comercial) de uso freqüente em ambientes de produção animal e de transformação de seus subprodutos: um quaternário de amônio e o iodofor. Foram usadas as concentrações indicadas pelo fabricante e uma menor para simular possível situação de sub-concentração. O método de verificação foi o de diluição através do teste de suspensão, observando a inativação bacteriana nos tempos de contato 5, 15, 30 e 60 minutos. Como resultados obtidos, todas as amostras foram inativadas quando utilizado o composto quaternário de amônio, em ambas as concentrações. Frente ao iodofor, 4 (quatro) amostras mostraram-se resistentes a este composto na concentração indicada e 59 frente à sub-concentração. Conclui-se ser necessário, seja para a eleição ou para o monitoramento da eficácia, o confronto dos desinfetantes/anti-sépticos com bactérias presentes nos ambientes específicos de produção animal ou mesmo nos de transformação de seus subprodutos.<br>For prevention of infectious diseases common to man and animals such as salmonellosis, the successful use of disinfectants is of great importance. However, intrinsic or acquired resistance presented by microorganism against these compounds may constitute a limiting aspect in disinfections protocols. This study was aimed at monitoring the sensitivity of 96 Salmonella Typhimurium strains isolated from slaughter pigs in the state of Rio Grande do Sul, Brazil. The isolates were tested against quaternary ammonium and iodophor, which represent two commercial disinfectants commonly used in animal production. The tested disinfectants were used in the concentration recommended by the fabricant and in a sub-concentration in order to simulate a possible field situation. Dilution suspension tests were conducted, observing the inactivation of each S.Typhimurium isolate after 5, 15, 30 and 60 minutes of contact with each compound. All tested isolates were inactivated by the quaternary ammonium compound in both concentrations. Four isolates revealed resistant to iodophor in the recommended concentration and 59 isolates when a sub-concentration was tested. The testing of resistance against disinfectants in microorganisms present on farm and in food processing plants might be an important step on monitoring the effectiveness of adopted disinfections protocols

    A colibacilose, causada por Escherichia coli, é a enfermidade entérica de maior impacto na produção de suínos, podendo levar à morte do animal. Esta bactéria possui grande capacidade de desenvolver resistência a múltiplos antimicrobianos e a desinfetantes. Desta forma, estudos que abordem mecanismos de resistência e perfil de amostras de campo tornam-se necessários. E. coli é amplamente utilizada como modelo de estudos que exploram a resistência intrínseca e extrínseca a multidrogas. Neste trabalho, buscou-se verificar o perfil de sensibilidade de 62 isolados de E. coli de suínos frente a três desinfetantes e a 13 antimicrobianos. Ainda, em 31 destes isolados foi pesquisada a presença de mecanismo de efluxo. Dos três desinfetantes avaliados, o cloreto de alquil dimetil benzil amônio+poliexietilenonilfenileter foi o que se mostrou mais eficaz (100%), seguido do glutaraldeído+cloreto de alquil dimetil benzil amônio (95,2%) e do cloreto de alquil dimetil benzil amônio (88,8%). Dentre os antimicrobianos testados, observou-se maior resistência para a tetraciclina (62,2%) e maior sensibilidade para o florfenicol (88,6%). A alta sensibilidade dos isolados frente aos desinfetantes pode estar relacionada à ausência de mecanismo de efluxo. O índice de resistência múltipla médio aos antimicrobianos foi de 0,52, o que demonstra um perfil multirresistente dos isolados, conduzindo para a necessidade do uso racional destas drogas em suinocultura
