    The Value of Cytological Examination in the Diagnosis of Noninvasive Thyroid Neoplasm with Papillary-like Nuclear Features (NIFTP)

    Since the introduction of the concept that the noninvasive encapsulated follicular variant of papillary thyroid carcinoma does not warrant a diagnosis of carcinoma and shall be renamed noninvasive follicular thyroid neoplasm with papillary-like nuclear features (NIFTP), many questions have arisen among thyroid cytopathologists concerning the implications of this changing paradigm on the cytological classification of thyroid nodules. Specifically, whether it is possible to diagnose NIFTP by fine-needle aspiration, how will the risk of malignancy of the diagnostic categories of thyroid cytopathology be affected, and which changes will be made to the current cytological workup of thyroid nodules. The aim of our work was to review published literature to analyze how cytopathologists are dealing with this changing paradigm in thyroid pathology and how, in turn, it is affecting their current practice

    A ceRNA approach may unveil unexpected contributors to deletion syndromes, the model of 5q- syndrome

    In genomic deletions, gene haploinsufficiency might directly configure a specific disease phenotype. Nevertheless, in some cases no functional association can be identified between haploinsufficient genes and the deletion-associated phenotype. Transcripts can act as microRNA sponges. The reduction of transcripts from the hemizygous region may increase the availability of specific microRNAs, which in turn may exert in-trans regulation of target genes outside the deleted region, eventually contributing to the phenotype. Here we prospect a competing endogenous RNA (ceRNA) approach for the identification of candidate genes target of epigenetic regulation in deletion syndromes. As a model, we analyzed the 5q- myelodysplastic syndrome. Genes in haploinsufficiency within the common 5q deleted region in CD34+ blasts were identified in silico. Using the miRWalk 2.0 platform, we predicted microRNAs whose availability, and thus activity, could be enhanced by the deletion, and performed a genomewide analysis of the genes outside the 5q deleted region that could be targeted by the predicted miRNAs. The analysis pointed to two genes with altered expression in 5q- transcriptome, which have never been related with 5q- before. The prospected approach allows investigating the global transcriptional effect of genomic deletions, possibly prompting discovery of unsuspected contributors in the deletion-associated phenotype. Moreover, it may help in functionally characterizing previously reported unexpected interactions

    Induction of heat shock protein expression in cervical epithelial cells by human semen.

    OBJECTIVE: The 70kD heat shock protein (Hsp70), induced when cells are subjected to environmental stress, prevents the denaturation and incorrect folding of polypeptides and may expedite replication and transmission of DNA and RNA viruses. We analyzed whether messenger RNA (mRNA) for Hsp70 was expressed following exposure of a cultured human cervical cell line (HeLa cells) to human semen or in cervical cells from sexually active women. STUDY DESIGN: HeLa cells were co-cultured with a 1:50 dilution of semen from four men or with purified spermatozoa or cell-free seminal fluid. Endocervical swabs were acquired at mid-cycle from 53 women. Heat shock protein 70 mRNA was detected by a reverse transcriptase-polymerase chain reaction utilizing specific primer pairs and analysis on agarose gels. In cervical cells Hsp70 mRNA was measured identically followed by hybridization with an Hsp70-specific internal probe and detection by enzyme-linked immunosorbent assay (ELISA). Cervical immunoglobulin A (IgA) antibodies to the human Hsp70 were determined by ELISA. RESULTS: HeLa cell-semen co-culture resulted in the induction of Hsp70 mRNA. In addition, cell-free seminal plasma and motile sperm incubated individually with HeLa cells also induced this mRNA. Heat shock protein 70 mRNA was detected in 28 (52.8%) of 53 endocervical samples obtained from women at various time points following intercourse. The percentage of samples expressing this mRNA was 37.5% at less than 10 hours, 64.3% at 10 hours, 70% at 11 hours, and between 36% and 50% at later times after semen exposure. The detection of cervical IgA antibodies to the Hsp70 was highly associated with Hsp70 gene transcription. CONCLUSION: Human semen induces transcription of Hsp70 in cervical epithelial cells

    Identifying Future Hydrographic Survey Priorities: A Quantitative Uncertainty Based Approach

    There is no universal standard methodology for assessing the validity of hydrographic survey data and charted information as they age. NOAA’s current method is the Hydrographic Health Model (HHM), a risk-based approach that incorporates crucial maritime variables and heuristic changeability terms through the history and frequency of large storms, tidal currents, and anthropogenic obstructions of a given area. Here we propose a quantitative approach evaluated in Chesapeake Bay and the Delmarva Peninsula that supports uncertainty-based estimates of chart health through alternative methodologies of calculating the initial state of historic hydrographic data and modeling how those change through time.No existe una metodología estándar universal para evaluar la validez de los datos de los levantamientos hidrográficos y de la información cartográfica a medida que maduran. El método actual de la NOAA es el Modelo Hidrográfico de Salud (HHM), un enfoque basado en el riesgo que incorpora variables marítimas cruciales y términos de variabilidad heurística a través de la historia y de la frecuencia de grandes tormentas, corrientes de marea y obstrucciones antropógenas de una zona determinada. Aquí proponemos un enfoque cuantitativo evaluado en la bahía de Chesapeake y en la península de Delmarva, que apoya, basadas en la incertidumbre, las estimaciones de la salud cartográfica mediante metodologías alternativas de cálculo del estado inicial de los datos hidrográficos históricos y de la modelización de cómo cambian dichos datos a través del tiempo.Il n’existe pas de méthode standard universelle permettant d’évaluer la validité des données des levés hydrographiques et des informations cartographiées quand elles deviennent anciennes.     Detection rate of FNA cytology in medullary thyroid carcinoma. a meta-analysis

    Background: The early detection of medullary thyroid carcinoma (MTC) can improve patient prognosis, because histological stage and patient age at diagnosis are highly relevant prognostic factors. As a consequence, delay in the diagnosis and/or incomplete surgical treatment should correlate with a poorer prognosis for patients. Few papers have evaluated the specific capability of fine-needle aspiration cytology (FNAC) to detect MTC, and small series have been reported. This study conducts a meta-analysis of published data on the diagnostic performance of FNAC in MTC to provide more robust estimates. Research Design and Methods: A comprehensive computer literature search of the PubMed/MEDLINE, Embase and Scopus databases was conducted by searching for the terms 'medullary thyroid' AND 'cytology', 'FNA', 'FNAB', 'FNAC', 'fine needle' or 'fine-needle'. The search was updated until 21 March 2014, and no language restrictions were used. Results: Fifteen relevant studies and 641 MTC lesions that had undergone FNAC were included. The detection rate (DR) of FNAC in patients with MTC (diagnosed as 'MTC' or 'suspicious for MTC') on a per lesion-based analysis ranged from 12·5% to 88·2%, with a pooled estimate of 56·4% (95% CI: 52·6-60·1%). The included studies were statistically heterogeneous in their estimates of DR (I-square >50%). Egger's regression intercept for DR pooling was 0·03 (95% CI: -3·1 to 3·2, P = 0·9). The study that reported the largest MTC series had a DR of 45%. Data on immunohistochemistry for calcitonin in diagnosing MTC were inconsistent for the meta-analysis. Conclusions: The presented meta-analysis demonstrates that FNAC is able to detect approximately one-half of MTC lesions. These findings suggest that other techniques may be needed in combination with FNAC to diagnose MTC and avoid false negative results. © 2014 John Wiley & Sons Ltd

    A durable coating to prevent stress corrosion effects on the surface strength of annealed glass

    The durability of an innovative polymeric coating recently developed by the authors to prevent stress corrosion in annealed glass is herein examined. The coating, having functional graded properties through the thickness, is optimised to provide a very good adhesion with glass and an excellent hydrophobic behavior on the side exposed to the environment, thus creating a good barrier to humidity, which is the triggering agent for stress corrosion. Three scenarios are analysed in terms of ageing: (i) cyclic loading, accomplished by subjecting coated samples to repetitive loading; (ii) natural weathering, performed by exposing coated samples to atmospheric agents; (iii) artificial weathering, carried out by exposing coated specimens to fluorescent UV lamps, heat and water. The durability of the coating is assessed indirectly, on the base of its residual effectiveness in preventing stress corrosion, by comparing the bending strength, obtained with the coaxial double ring test, of aged coated glass specimens with that of un-coated and freshly coated specimens. The obtained results prove that the proposed formulation is almost insensitive to cyclic loading, maintains a very good performance in case of natural weathering, whereas is slightly more sensitive to artificial weathering

    Correlated electron-hole plasma in organometal perovskites

    Organic-inorganic perovskites are a class of solution-processed semiconductors holding promise for the realization of low-cost efficient solar cells and on-chip lasers. Despite the recent attention they have attracted, fundamental aspects of the photophysics underlying device operation still remain elusive. Here we use photoluminescence and transmission spectroscopy to show that photoexcitations give rise to a conducting plasma of unbound but Coulomb-correlated electron-hole pairs at all excitations of interest for light-energy conversion and stimulated optical amplification. The conductive nature of the photoexcited plasma has crucial consequences for perovskite-based devices: in solar cells, it ensures efficient charge separation and ambipolar transport while, concerning lasing, it provides a low threshold for light amplification and justifies a favourable outlook for the demonstration of an electrically driven laser. We find a significant trap density, whose cross-section for carrier capture is however low, yielding a minor impact on device performance

    The second version of The Bethesda System for Reporting Thyroid Cytopathology endorsed the introduction of non-invasive follicular thyroid neoplasms with papillary-like nuclear features (NIFTP) as a distinct entity with low malignant potential into clinical practice. Consequently, the risk of malignancy (ROM) of cytological diagnoses has changed, but the magnitude of the change remains uncertain. The present systematic review was undertaken to obtain more robust information about the true impact of NIFTP on the ROM among patients undergoing surgery following a fine-needle aspiration cytology (FNAC) diagnosis of suspicious for malignancy (Bethesda V) or malignant (Bethesda VI). As they are managed surgically, these two diagnostic categories are the primary entities that are clinically impacted by the advent of NIFTP. Systematic review and meta-analysis. A comprehensive literature search of online databases was performed in November 2018. The search was conducted looking for data of histologically proven NIFTP with preoperative FNAC. One-hundred fifty-seven articles were identified and nine were included in the study. Overall, there were 13,752 thyroidectomies with a cancer prevalence of 45.7%. When NIFTP was considered non-malignant, the pooled risk difference for ROM was 5.5%. Applying meta-analysis, the pooled prevalence of NIFTP among nodules with FNAC of Bethesda V or Bethesda VI was 14 and 3%, respectively. This meta-analysis shows that the inclusion of NIFTP leads to a reduction in the ROM for the Bethesda V and Bethesda VI FNAC diagnostic categories by 14 and 3%, respectively. Clinicians should be aware of these data to avoid overtreatment
