    Stone free rate and clinical complications in patients submitted to retrograde intrarenal surgery (RIRS): Our experience in 571 consecutive cases

    Introduction: The purpose of this study is to report the stone free rate (SFR) and clinical complications in patients submitted to retrograde intrarenal surgery (RIRS). Materials and methods: A total of 571 procedures of upper urinary stones treated using flexible ureteroscopy and holmium laser lithotripsy from January 2014 to February 2020 have been analyzed. Overall SFR was evaluated after 3 months following the procedure by means of a non-contrast computed tomography. Success was considered as stone-free status or ≤ 0.4 cm fragments. Results: The overall SFR was 92.3% in group 1 (stone size: 1 ≤ 2 cm), 56.7% in group 3 (stone size: 2-3 cm) and 69.6% in group 4 (multiple stones). Post-operative complications, according to the Clavien- Dindo (CD) classification system, were recorded in 32 (5.6%) procedures. The major complications recorded were: one case of subcapsular hematoma (SRH) associated with pulmonary embolism two days after the procedure (CD Grade IIIa) treated conservatively and one case of hemorrhagic shock 2 hour with multiple renal bleedings requiring urgent nephrectomy (CD Grade IVA). Conclusions: The RIRS is an effective and safe procedure with a high SFR significantly correlated with the stone size; at the same time, RIRS could be characterized by severe clinical complications that require rapid diagnosis and prompt treatment

    The Activity of Special Continuity Care Units in the City of Florence During the COVID-19 Pandemic

    Objectives: Worldwide, countries adopted different strategies in primary care (PC) to cope with the COVID-19 pandemic. This study aims to describe and evaluate the functions and activity load of a specific PC organizational model called “Special Continuity Care Units” (SCCU) in Florence, Italy, and to investigate the characteristics of the COVID-19 patients assisted by the service.Methods: The retrospective cross-sectional design used daily updated reports by SCCU team members to evaluate the activity load. The retrospective cohort study analyzed data of the demographics, clinical characteristics, and process outcomes of patients assisted during the second pandemic wave.Results: The analysis shows how the service activity load changed along with the epidemiological trend. Regarding people assisted by the SCCU, the median follow-up duration of symptoms was 6 days; male gender and being symptomatic were predictors of hospitalization.Conclusion: Some key characteristics can be described as indispensable in PC services facing health emergencies: model flexibility, the availability of resources, networking among services to enhance coordination and resource optimization, and close collaboration with general practitioners

    Pro-Environmental Behaviors: Determinants and Obstacles among Italian University Students

    The awareness of citizens concerning the health risks caused by environmental pollution is growing, but studies on determinants of pro-environmental behaviors have rarely examined health-related aspects. In this study, we investigated these determinants using data from a large survey among Italian university students (15 Universities: 4778 filled questionnaires). Besides the health-related aspects, represented by environmental health risk perception and functional health literacy, we considered social and demographic characteristics (gender, area of residence, sources of information, trust in institutional and non-institutional subjects, and students' capacity of positive actions, indicated as internal locus of control). The attitudes towards pro-environmental behaviors were positive for more than 70% of students and positively related with health risk perception, internal locus of control, and health literacy. The correspondence between the positive attitudes towards pro-environmental behaviors and the real adoption of such behaviors was approximately 20% for most behaviors, except for the separate collection of waste (60%). Such a discrepancy can be attributable to external obstacles (i.e., lack of time, costs, lack of support). The health-related aspects were linked to the pro-environmental attitudes, but to a lesser extent to pro-environmental behaviors, owing to the complexity of their determinants. However, they should be taken in account in planning education interventions

    Knowledge Spillovers in the Technology vs. Product Market Spaces: a Firm-level Investigation

    This study aims at exploring empirically the relationship between knowledge spillovers and firm performance. The investigation is carried out on a firm-level panel data set covering 31 OECD countries in the 2009-2018 period. Exploiting and adapting Bloom, Schankerman and Van Reenen's (2013) empirical framework, we estimate the effect of knowledge spillovers coming from technological neighbours and product market rivals on a range of firm performance indicators (market value, patent counts, productivity). We further analyse heterogeneity in the emergence of the effects, by re-estimating our models on subsamples of our firm population, identified by employment size cohorts or by clusters of technologically homogeneous sectors. At the full sample level, we observe a positive effect of technological spillovers on each of the outcome variables, while product market rivalry effects are insignificant. The split sample analysis highlights the presence of firm size- and sector-specific dynamics in the estimated effects, suggesting the need for an explicit account of their extent and of the factors which cause them

    Lights and shadows of COVID-19, Technology and Industry 4.0

    Scientific discoveries and technologies played a significant role in the digital revolution that occurred over the last years. But what is their role in the turmoil brought by the current pandemic? The aim of this paper is to show how digital technologies are operating during this first phase of the spreading of COVID-19. The study analyses and debates the current and potential role of digital technologies, focusing on their influence in the industrial and social fields. More specifically we used the blogging platform "Medium", which has seen an exponential growth in its production of articles over the last couple of months. Even if different from esteemed scientific sources, this platform provides a structure that suits our analysis. We searched how many times digital technologies are mentioned in articles regarding Coronavirus and, after collecting these articles, we collected page tags (comparable to "keywords" in scientific articles) and classified them (technology tags and non-technology tags), to create a graph showing the relation between them. This network allowed us to acknowledge and picture how technologies are currently debated. This was the starting point to discuss the key implications for an imminent future, and question about the impact on industry, society and labour market. What are the opportunities or threats of using technologies of Industry 4.0? Which are the needs rising because of the pandemic and how can technologies help in their fulfillment? How will the industrial scenario change after this pandemic? How will the labour market be affected? How can technologies be advantageous in the emerging social challenges

    Acute Scrotum Following Traumatic Spermatic Cord Hematoma: A Case Report and Review

    Acute scrotum constitutes the most common urological emergency secondary to spermatic cord torsion, testicular trauma, orchiepididymitis and hernias. We report a very rare case of unique traumatic spermatic cord hematoma following scrotum injury occurred during a football match. Clinical exam showed an increased volume of the left spermatic cord; the color Doppler ultrasound (CDU) demonstrated left testicular ischemia secondary to a large spermatic cord hematoma that needs surgical exploration. Spermatic cord hematoma rarely induces acute scrotum, however it could be treated conservatively surgery is mandatory when pain is persistent or testicular ischemia is confirmed by CDU
