Self-assembly of Tetraurea Calix[4]arenes
We have elaborated a multistep strategy to synthesize ABAB-type tetraureas. There are overall nine steps but they involve very simple chemistry. The sequence starts with a 1,3-dialkylation and this is the step in which a difference between distal phenolic units is introduced. The selective ipso-nitration in the next step is based on the difference in reactivity between free phenolic units and alkylated ones.
The direct reaction of tetraamino calixarene with tolylisocyanate appears not to be an appropriate method to synthesize 1,3-ditolylurea calixarenes but can be used to get tetraureas of ABBB- and AABB-types in two steps with yields of about 60%.
A complete regioselective dimerization was obtained with mono-loop derivatives in which two adjacent
urea residues are covalently connected. As predicted/expected the loop prevents the formation of one regioisomer, and only the dimer in which the open-chain residue slips through the loop is formed. To synthesize mono-loop tetraureas 1,2-diBoc protected tetraamino calixarene was acylated with activated di-urethanes under high dilution conditions.
Di-loop compounds were synthesized by two different ways. In the reaction of tetraamine and di-urethanes the yield is about 30-40%. The second method is based on the metathesis reaction within a suitable heterodimer. For this strategy, tetraurea derivatives with residues which have terminal double bonds were prepared. The exclusive formation of the heterodimer with tetratosylurea as template is the key point in this strategy. Metathesis followed by hydrogenation give exceptionally good yields (> 80%) of the loop compounds.
All the NMR data for di-loop compounds confirm that the loops prevent the interaction of the urea residues which are connected and thus, as
expected, the di-loop derivatives do not form homodimers. The heterodimer between di-loop compounds and tetratolylurea (open-chain tetraureas) was the only species observed for a 1:1 mixture in benzene or chloroform.
The rational synthesis of bis-[2]catenanes was a consequence of the selective formation of one regioisomer of mono-loop derivatives and the exclusive formation of heterodimers by di-loop derivatives. The formation of interlocking-ring in the synthesis of bis-[2]catenanes is an additional evidence that one open-chain residue slips through the loop in mono- or di-loop derivatives. Exceptionally good yields in the synthesis of bis-[2]catenanes are due to the high preorganization in the dimer which undergoes the metathesis. This preorganization decreases the number of the wrong connections and favors the new connections to be formed. Although the procedure for working up the reaction mixture should be still improved, these results are promising.
A C2-symmetrical bis-[2]catenane was
successfully resolved by column chromatography using a chiral stationary phase. Thus it should be possible to separate a larger amount to obtain pure enantiomers for further studies
Evaluation of medical decision errors during the transition period to telemedicine
The context of the Coronavirus pandemic has fundamentally changed the way we approach medical services. Beyond setting up new technological possibilities, it has propelled telemedicine to become a reality, bringing undeniable practical benefits. The questions that arise are both justified and worrying: can digitalization replace a direct interpersonal relationship that involves a physical examination, while preserving the quality of the medical act and the degree of patient satisfaction? Isn\u27t there a risk that the digitization of the medical record will cancel out the deep human character of classical medicine that has evolved since the time of Hippocrates? Should the implementation of telemedicine be the state-of-art of modern medicine, in accordance with the co-evolution of digital technology? It is hard to believe that once used in this period, telemedicine will be abandoned. However, telemedicine must be analyzed not only in the short term but also in the long term, in order to be able to evaluate both its usefulness and possible deficiencies
Oral pathology induced by excess or deficiency of glucocorticoids in adults
Oral manifestations are present both in Cushing\u27s syndrome and in adrenal insufficiency. Possible oro-dental pathology in patients with Cushing\u27s syndrome include jawbone loss, tooth loss, and periodontal diseases. Professional societies did not include Cushing\u27s syndrome as being part of systemic diseases associated with loss of periodontal supporting tissues. The comorbidities of Cushing\u27s syndrome such as obesity, osteoporosis, and diabetes are conditions that influence periodontal attachment apparatus. In patients with adrenal primary insufficiency, the most specific sign is the melanic pigmentation of the skin and mucosal surfaces due to increments of corticotropin and pro-opiomelanocortin peptide levels that occur as a result of decreased cortisol feedback. The oral mucosa develops black plaques that can also be present on the gums, palate, tongue, and lips. The pallor may occur in patients with adrenocortical insufficiency secondary to corticotropin deficiency. Patients with primary adrenal insufficiency need to increase their glucocorticoid doses during physical activity, intercurrent illnesses, surgery, and medical procedures. Current evidence indicates that routine, nonsurgical, or minor surgical procedures do not need supplemental glucocorticoids in diagnosed patients who are in a stable condition. However, for major oral surgery, glucocorticoid supplementation is necessary for the surgery day and for at least one postoperative day

Ekspresja adiponektyny w otyłości trzewnej jest istotnym wyznacznikiem insulinooporności w otyłości olbrzymiej
Introduction: Visceral adiposity is associated with decreased serum adiponectin levels, peripheral resistance to insulin and an increased risk of cardio-metabolic complications. However, the link between adiponectin expression in visceral adipose tissue (VAT), its serum levels and metabolic protection is controversial. The aim of this study was to investigate the relationship between the adiponectin gene expression in VAT and clinical and metabolic parameters in patients with severe obesity.
Material and Methods: This is a cross-sectional study that included 51 severely obese patients (age 43.24±11.29 years, BMI 45.13±8.67 kg/m2), extensively evaluated clinically and biologically (metabolic tests, serum adiponectin measurements, HOMA-IR) before bariatric surgery. Omental adipose tissue was sampled during the intervention and the relative quantification of adiponectin gene expression was performed by real-time PCR, using beta-actin as reference gene.
Results. Adiponectin mRNA in VAT was significantly higher in obese insulin-sensitive patients than in the rest of obese patients (p < 0.05) and negatively correlated with HOMA-IR (r =-0.354, p=0.016) and uric acid (r =-0.304, p=0.045). After adjustment for gender, TG/HDL ratio and uric acid, adiponectin expresion (β= -0.439, p=0.001), waist circumference (β=0.467, p=0.001) and serum adiponectin (β =-0.339, p=0.011) remained significantly associated with HOMA-IR, together explaining more than 50% of its variation.
Conclusions. In severely obese patients, adiponectin gene expression in VAT is negatively correlated with serum levels of uric acid and is an independent determinant, together with anthropometric parameters of visceral obesity and serum adiponectin levels, of insulin resistance.Wstęp: Otyłość trzewna związana jest ze zmniejszonym stężeniem adiponektyny w surowicy krwi, obwodową opornością na działanie insuliny oraz ze zwiększonym ryzykiem powikłań sercowo-metabolicznych. Jednak związek między ekspresją adiponektyny w trzewnej tkance tłuszczowej, jej stężeniem w surowicy krwi a ochroną metaboliczną jest kwestią sporną. Celem niniejszej pracy było zbadanie związku między ekspresją genu adiponektyny w trzewnej tkance tłuszczowej a klinicznymi i metabolicznymi parametrami pacjentów ze znaczną otyłością.
Materiał i metody: To przekrojowe badanie obejmowało 51 znacznie otyłych pacjentów (wiek 43,24 ± 11,29 roku, BMI 45,13 ± 8,67 kg/m2), szczegółowo ocenionych pod względem klinicznym i biologicznym (testy metaboliczne, pomiary stężenia adiponektyny w surowicy krwi, wskaźnik HOMA-IR) przed operacją bariatryczną. Podczas operacji pobrano tkankę tłuszczową sieci. Względna ocena ilościowa ekspresji genu adiponektyny była przeprowadzona metodą PCR w czasie rzeczywistym.
Wyniki: Poziom mRNA adiponektyny w trzewnej tkance tłuszczowej był znacząco wyższy u otyłych pacjentów wrażliwych na insulinę niż u pozostałych otyłych pacjentów (p < 0,05) oraz ujemnie skorelowany ze wskaźnikiem HOMA-IR (r = –0,354, p = 0,016) i kwasem moczowym (r = –0,304, p = 0.045). Po uwzględnieniu płci, wskaźnika TG/HDL i kwasu moczowego, ekspresja adiponektyny (β = –0,439, p = 0,001), obwód talii (β = 0,467, p = 0,001) i poziom adiponektyny w surowicy krwi (β = –0,339, p = 0,011) pozostały istotnie związane ze wskaźnikiem HOMA-IR, łącznie wyjaśniając ponad 50% jego wariancji.
Wnioski: W przypadku znacznie otyłych pacjentów ekspresja genu adiponektyny w trzewnej tkance tłuszczowej jest ujemnie skorelowana ze stężeniem kwasu moczowego w surowicy krwi i razem z antropometrycznymi parametrami otyłości trzewnej oraz stężeniem adiponektyny w surowicy krwi jest niezależnym wyznacznikiem insulinooporności
Comparative Assessment of Phenolic Compounds from Authentic Wine Varieties from North-Western Romania from the 2021-2022 Harvest
Wine is one of the most consumed beverages in the world. Romania, as a country, has a history in vineyards and wine making. This present study analyzed the phenolic compounds of 5 varieties of wine from Romania (Fetească Regală Muscat Ottonel, Riesling Italian, Pinot Gris and Merlot). The results suggested that some of the wines present a wide category of phenolic compounds where others present low values and some of the compounds are absent as detected with a developed HPLC-DAD-ESI+ method. Phenolic compounds were identified after the retention time. Merlot wine presented, in most cases, the highest values for phenolic compounds, with a few exceptions. As a conclusion, romanian wines are rich in phenolic compounds that possess a series of beneficial properties

Spinal ultrasound – Identification of the normal structures
The incomplete ossification of the spinal processes allows the ultrasonographic evaluation of the spinal cord and the adjacent structures in neonates and small infants. The paper describes the ultrasonographic examination of the spinal structures and the normal appearance of the spinal cord, the structures within the spinal canal, and the bony and muscular adjacent structures. Sagittal and axial sections at cervical, thoracic, lumbar, and sacral levels are described. There are also mentioned findings in the M mode and Doppler examinations
Orbital complications of acute rhinosinusitis
Introduction. Rhinosinusitis is the infection and inflammation of at least one of the 4 paranasal sinuses (frontal, maxillary, ethmoid and sphenoid). Their correct diagnosis is generally determined by examining the patient clinically and by rhinosinusal endoscopic examination. There are also cases that show complications or continued evolution despite the correct treatment, and then we will use some imaging investigations to find out more about affection(radiography of anterior sinuses of the face, sinus computerized tomography, sinus magnetic resonance imaging). In particular, acute rhinosinusitis should be carefully managed to avoid complications such as the local ones: orbital cellulitis, orbital abscess, osteomyelitis, cavernous sinus thrombosis; and intracranial complications: meningitis, epidural abscess, subdural abscess, cerebral abscess.Materials and methods. There will be presented all orbital complications according to the cases treated in the ENT Clinic of the Coltea Clinical Hospital.Conclusions. Diagnosis of rhinosinusitis is largely clinical and endoscopic.When complications of the condition arise, these should be investigated imagistically to determine their exact extent and to institute the correct treatment as soon as possible. The complications of rhinosinusitis are medical and surgical life-threatening emergencies, which is why in order to diagnose and correct and quickly institute therapy requires a multidisciplinary approach
