¿Qué hay de nuevo en la osteosíntesis?
Los sistemas para la fijación interna de las fracturas se encuentran en continua evolución. Los
actuales conocimientos en cuanto a materiales, biomecánica y biología de la consolidación de las fracturas dan
pie al diseño de nuevas osteosíntesis. Uno de los problemas más frecuentes y de difícil solución es el tratamiento
quirúrgico de las fracturas osteoporóticas. Los fracasos en la fijación y sucesivas reintervenciones ocasionan
complicaciones severas en estos pacientes. En el presente trabajo se revisan algunas de las novedades en los sistemas
de fijación de fracturas, la mayoría de ellas dirigidas al tratamiento de pacientes con fracturas osteoporóticas.Systems for internal fixation of fractures are still evolving. Current knowledge in materials, biomechanics
and biology of fracture healing becomes in new internal fixation devices. One of the most common
problems and more difficult to solve, is the surgical treatment of osteoporotic fractures. Fixation failure and

Nueva técnica de sutura artroscópica transósea del manguito de los rotadores: estudio anatómico
Se ha realizado un estudio anatómico experimental sobre cadáver que ha fundamentado el desarrollo
de una técnica alternativa de reparación artroscópica del manguito rotador, puramente transósea. Se han utilizado
10 hombros de cadáver fresco congelado. Mediante una guía externa diseñada específicamente para esta técnica,
se han efectuado perforaciones en el troquíter con diferentes angulaciones. En cada uno de los ángulos se ha medido
la distancia al nervio axilar, así como el grosor de la pastilla ósea de troquíter obtenida. El análisis de los datos ha
mostrado una distribución normal de los valores, y diferencias estadísticamente significativas entre los resultados
obtenidos en cada ángulo de colocación de la guía externa (p<0.001). Conclusiones: La sutura artroscópica transósea
del manguito rotador, realizada mediante una guía externa a través del deltoides, es una técnica factible, que puede
llevarse a cabo con márgenes de seguridad respecto a una hipotética lesión del nervio axilar.An anatomical study with ten fresh shoulder specimens was carried out in order to establish the safest
angle to perform a new arthroscopic technnique for true transosseous rotator cuff repair. With an external guide
designed for this study, several osseous tunnels were made through the greater tuberosity using different angles. The
minimum distance from the drill to the axillary nerve was measured with each angle, as well as the thickness of the
osseous bridge in the greater tuberosity. Statistically significant differences were observed between the measurements
obtained with each angle of the external guide (p<0.001). From our data it is concluded that transosseous arthroscopic
repair of the rotator cuff with an external guide is a feasible technique and can be performed with minimal risk
for the axillary nerve
Resultados de la autotransfusión postoperatoria en artroplastia total de rodilla
Introducción. El propósito es evaluar si la utilización de los drenajes autotransfusores en la artroplastia
total de rodilla primaria minimiza la caída de la hemoglobina en el postoperatorio, así como la necesidad
de transfusiones sanguíneas alogénicas. Material y métodos. Estudio retrospectivo, analítico observacional que
incluyó 100 pacientes intervenidos de artroplastia de rodilla entre enero 2014 y enero 2015 en el Hospital Dr.
Peset. Compara tanto los parámetros analíticos de sangrado (hemoglobina y hematocrito) como la necesidad de
transfusión sanguínea entre pacientes reinfundidos y no reinfundidos. Resultados. Se reinfundieron 50 pacientes
con un volumen medio de 505 ml. La necesidad de transfusión sanguínea alogénica en los reinfundidos fue de
4% mientras que en los no reinfundidos fue de 24%. La caída media de hemoglobina postquirúrgica fue 1,12 g/
dl menor en el grupo reinfundido. Conclusiones. El redón autotransfusor es una técnica segura que mejora el
rendimiento analítico postransfusional y, por consiguiente, reduce el número de transfusiones alogénicas.Introduction. The purpose is to evaluate the use of autotransfusors drains in total knee arthroplasty
primary minimizes the drop in hemoglobin in the postoperative period, and the need for allogeneic blood
transfusions. Material and methods. A retrospective, analitic and observational study, have been included 100
patients undergoing knee arthroplasty between January 2014 and January 2015 at the Dr. Peset Hospital. Compare
the analytical parameters of bleeding (hemoglobin and hematocrit) and the need for blood transfusion among
patients and not reinfused reinfused. Results. Fifty patients with Total Knee Arthroplasty (TKA) were reinfused
with a mean volume of 505 ml. The need for allogenic blood transfusions was 4%, while in patients not reinfused
was 24%. The average drop in postoperative hemoglobin was 1.12 g / dl lower in the group reinfused. Conclusions.
The autotransfusion drain is a safe technique which improves post-transfusional values and therefore
reduces the number of allogenic blood transfusions
Estudio baropodométrico en pacientes tratados mediante artroplastia total de rodilla
La patología artrósica de la rodilla condiciona una disminución de la carga del miembro afecto y
un patrón de apoyo con predominio generalmente del arco externo. El propósito de este estudio es determinar si
existe una modificación significativa de este patrón tras la implantación de una artroplastia total de rodilla (ATR)
primaria, recogiendo las presiones plantares de ambos pies mediante baropodometría electrónica previamente y 4
meses tras la cirugía, tanto en estática como en dinámica. Así, se analizan las presiones plantares de 30 pacientes,
21 mujeres y 9 hombres, intervenidos de ATR primaria en el Hospital Universitario Doctor Peset entre los años
2013 y 2015. También se ha determinado si existe una corrección del eje anatómico tras la cirugía. Utilizando test
no paramétricos (test de Wilcoxon para datos apareados, U de Mann-Whitney), observamos una modificación
significativa de dicho ángulo desde valores medianos de 1.2º de varo a 4.9º de valgo en el postoperatorio, una
disminución mediana de la presión plantar en ambos pies y una redistribución de la carga axial, aumentando en
el miembro intervenido. Concluimos que, 4 meses tras la cirugía, se produce una tendencia a la “normalización”
de la pisada en pacientes en los que se ha implantado una ATR, condicionado tanto por la mejora del dolor y la
funcionalidad como por la realineación del eje.Knee osteoarthritis determines a decrease of the load of the affected limb and a pattern usually

in this pattern is? after implantation of a primary total knee arthroplasty (TKA), measuring plantar pressures
in both feet by electronic baropodometry previously and 4 months after surgery, in static and dynamic. Plantar
pressures of 30 patients are analyzed, who underwent primary TKA at the University Hospital Doctor Peset
between 2013 and 2015. In addition, it has also been determined if there is a correction on the anatomical axis
after surgery. Using nonparametric test (Wilcoxon test for paired data, U Mann Whitney), we observed a significant
change in the angle from median values of 1.2º varus to 4.9º valgus postoperatively, a median decrease of
plantar pressure in both feet and a redistribution of axial load, increasing in the operated limb. We conclude that,
4 months after surgery, there is a tendency to “normalization” of the tread in patients undergoing TKA. This is
conditioned by improving pain and function as axis realignment occurs
Artritis séptica por streptococcus pneumoniae: reporte de un caso
La artritis neumocócica es infrecuente en adultos, predomina en pacientes pediátricos. La neumonía
y la bacteriemia son las manifestaciones más frecuentes, la afectación articular según series recientes tiene
una prevalencia menor al 1% en sujetos menores de 50 años. Se presenta el caso de una paciente de 60 años con
LES (Lupus Eritematoso Sistémico) la cual presentó monoartritis séptica de rodilla derecha causada por Streptococcus

Necrosis avascular de cabeza femoral en pacientes con enfermedad de Gaucher tipo I : tratamiento mediante artroplastia total de cadera no cementada
El 80% de los pacientes con enfermedad de Gaucher presentan manifestaciones óseas, siendo la necrosis avascular de la cabeza femoral una de las que mayor limitación produce y la primera causa de intervención quirúrgica osteoarticular. La artroplastia total de cadera cementada ha sido el tratamiento de elección en los casos con mayor deterioro de la articulación, pero sus resultados a medio y largo plazo se han visto ensombrecidos por la elevada tasa de aflojamiento. La terapia enzimática sustitutiva ha disminuido las complicaciones postoperatorias, además de permitir el uso de componentes no cementados con resultados similares a las prótesis implantadas por otros motivos. Presentamos una serie de 5 artroplastias de cadera no cementadas, con un seguimiento medio de 12.4 años (3.4-30), en 4 pacientes con enfermedad de Gaucher tipo I y osteonecrosis de la cabeza femoral. Solo se observó un caso de aflojamiento aséptico a los 18 años de la intervención
Manejo perioperatorio de fármacos anticoagulantes y antiagregantes en el paciente con fractura de cadera
La fractura de cadera eleva de manera significativa en los estudios publicados la morbilidad y la
mortalidad en los pacientes ancianos que la padecen. La participación en el proceso de fármacos que alteran la
hemostasia se ha convertido en uno de los aspectos más importantes y controvertidos del manejo perioperatorio
del paciente con fractura de cadera. La anticoagulación o antiagregación plaquetaria de estos pacientes tiene unas
implicaciones tanto en la práctica anestésica como quirúrgica que hacen necesaria la búsqueda de un equilibrio
entre el riesgo hemorrágico y trombótico de los mismos. Con la elaboración del presente protocolo, en línea con
las últimas evidencias científicas publicadas, se sientan las bases para avanzar en el tratamiento de este grupo de
pacientes. Una atención multidisciplinar especializada, una fijación precoz de la fractura antes de las 48 horas, y
un efectivo programa de rehabilitación pueden disminuir las complicaciones del proceso.Hip fracture rises significantly in published studies morbidity and mortality in elderly patients.
Participation in the process of hemostasis-altering drugs has become one of the most important and controversial
perioperative management of patients with hip fracture aspects. Anticoagulation or antiplatelet of these patients
have some implications as a surgical anesthetic practice that make it necessary to seek a balance between bleeding
and thrombotic risk thereof. With the development of this protocol, in line with the latest scientific evidence
published, the stage is set to advance the treatment of this patient group. A specialized multidisciplinary care,
early fracture fixation within 48 hours and an effective rehabilitation program can reduce complications of that
process

The use of the intramedullary nail in extracapsular fractures of the proximal third of the femur is the most frequent stabilization system. In recent years, the biomechanical complications that affect this system have been increasing.A retrospective radiographic study of intertrochanteric fractures was performed followed by an analysis of the biomechanical complications in zone 1 of the cephalic screw. Of the 548 cases, 85 cases (15.5%) of complications were observed, with "back-out" being the most frequent minor complication and "cut-out" the most frequent major complication. 41% of the total fractures correspond to type 31-A2.3 AO / OTA. Infección periprotésica en pacientes ancianos tratados mediante hemiartroplastia de cadera tras fractura intracapsular ¿Es el uso de cemento impregnado con altas dosis de antibiótico es un factor asociado a su disminución?.
Objective: to study whether the use of cement with a high dose of antibiotics is a factor associated with a lower infection rate in hip hemiarthroplasty after femoral neck fracture.Material and methods: a retrospective cohort study of 144 consecutive patients treated surgically for a cervical hip fracture with cemented hip hemiarthroplasty between February 2015 and February 2017. In 88 (61.1%) high dose antibiotic cement was used, defined as a concentration ? 5% of the total weight (Group 1) and in 56 (38.9%) cement without antibiotics was used (Group 2). The primary outcome variable was periprosthetic infection.Results: there were 2 (1.38%) infections that were classified as deep, both in group 1. One of patients had a superficial infection in group 2. We did not find differences in the rate of infections between the two groups. Among the intra-surgical complications, we found two failures of the cement-prosthesis interface (non-adherence of the cement to the stem at the end of the setting time), both cases occurred in group 1. There was no allergic or toxic reaction. The mean survival time of all operated patients was 2.7 years (95% CI: 2.4-2.9) at the end of follow-up. We found no differences in survival when stratified by the use or not of antibiotics in cement, p = 0.874.Conclusion:In our experience, in patients undergoing hip hemiarthroplasties the use of cement with a high dose of antibiotic is not associated with lower infection rate when intravenous antibiotic prophylaxis is correct
Lipoma intraóseo, un tumor óseo benigno infrecuente : presentación de dos casos intervenidos quirúrgicamente y revisión de la literatura
Intraosseous lipomas are rare and benign tumors of unclear etiology. Most of them are asymptomatic, being accidentally diagnosed during imaging tests performed for other reasons and they usually have unspecific clinical presentations. Radiographically, they can be confused with other entities as non-ossifying fibromas or simple cysts. Methods. We report two cases, a 39-year-old and a 25-year-old Caucasian female patients, referred to our hospital with a mass in the proximal third of the right tibia and distal femur respectively with slowly progressive growth. In conventional X-ray images and computed tomography, a well-defined, expansive intraosseous lytic lesion without cortical disruption was observed in both cases. Magnetic resonance imaging demonstrated predominance of lipomatous tissue signal, being compatible with intraosseous lipoma. The patients underwent excision of the tumor, confirming the diagnosis of intraosseous lipoma after pathological study. Results. In the immediate postoperative period, swelling and pain decreased and at the 6th-month postoperative follow-up the patients are asymptomatic and without radiographic signs of recurrence. Discussion. Intraosseous lipomas are tumors that can go easily unnoticed, being difficult to diagnose on conventional X-ray images. Because of that, other imaging tests, such as CT and MRI, are necessary for diagnosis and histopathology for confirmation. Conservative treatment in asymptomatic patients and surgery in symptomatic cases is recommended. Recurrence and malignant transformation of these tumors is rare
