    An Integrated Oncology Data Warehouse for Clinical Decision Support and Complex Patient Cohort Identification in a Hybrid Cancer Center

    BACKGROUND: A data warehouse is a repository that centralizes and integrates data from disparate systems to provide the ability to easily access historical, consistent data. Integration of disparate source systems into one centralized location can enable rapid identification of more robust research cohorts and enable data-driven decision making. The objective of the Miami Cancer Institute (MCI) Oncology Data Warehouse (ODW) is to collect and organize data from clinical records, research, and administrative systems to support information retrieval, business intelligence, and analytics for high-level decision making for oncology patients. The design, architecture, and implementation aligns with industry best practices which includes Data Governance, Enterprise Data Modeling, and Metadata Management. METHODS: We integrated structured and unstructured data from disparate sources into one centralized data model optimized for querying known as the ODW. The ODW is modeled as a star schema, with fact tables and conformed dimension tables, and expands to a galaxy schema with constellation facts and dimensions that can snowflake to other data models as needed. Each fact table represents a subject area (i.e. pathology), that is directly related to the conformed dimension tables using surrogate and foreign keys. Conformed dimensions represent the attributes associated to the subject area (i.e. date of encounter). The source data is extracted, transformed and loaded (ETL) automatically from different databases into a set of tables. The ETL code performs incremental loads at regular prescribed intervals into two parallel storage areas, a relational database management system (RDMS) as well as a Big Data file storage system. RESULTS: An interdisciplinary team of physicians, engineers, scientists, and subject matter experts at the Miami Cancer Institute of Baptist Health South Florida, has designed, developed, and implemented the ODW with information originating from different data sources which include: Electronic Medical Record (EMR) systems, Financial Systems, Clinical Trial Management Systems, Tumor Registries, Biospecimen Repositories, Pathology synoptic reports and archives, and Next Generation Sequencing services. Structurally it is a subject-oriented, integrated collection of data leveraging conformed dimensions. The ODW is capable of connecting most business intelligence (i.e. Tableau) or statistical (i.e. SAS) tools for automated or static report development. CONCLUSION: The growing ODW enables physicians, clinical management teams, and medical analysts to systematically mine and review the molecular, genomic, and associated clinical or administrative information of patients, and identify patterns that may influence treatment decisions and potential outcomes. By implementing an innovative combination of technology tools and methods, we were able to organize enterprise information about oncology patients which can be utilized for clinical decision support and precision medicine use cases

    Congenital Hypothyroidism Long‐Term Follow‐up Project: Navigating the Rough Waters of a Multi‐Center, Multi‐State Public Health Project

    The Region 4 Midwest Genetics Collaborative, made up of seven regional states (Illinois, Indiana, Kentucky, Michigan, Minnesota, Ohio, and Wisconsin), brought together pediatric endocrinologists, state laboratory experts, public health follow‐up specialists, and parents of children with congenital hypothyroidism (CH) to identify the three‐year follow‐up management and education patterns of primary care clinicians and pediatric endocrinologists in the care of children diagnosed with CH by state newborn screening (NBS) programs. Among a number of challenges, each state had different NBS methods, data systems, public health laws, and institutional review board (IRB) requirements. Furthermore, the diagnosis of CH was complicated by the timing of the NBS sample, the gestational age, weight, and co‐morbidities at delivery. There were 409 children with CH identified through NBS in 2007 in the seven state region. The clinician of record and the parents of these children were invited to participate in a voluntary survey. Approximately 64 % of clinician surveys were collected with responses to questions relating to treatment, monitoring practices, educational resources, genetic counseling, and services provided to children with confirmed CH and their families. Nearly one‐quarter (24 %) of parents surveyed responded to questions relating to treatment, education, genetic counseling, resources, and services they received or would like to receive. De‐identified data from six of the seven states were compiled for analysis, with one state being unable to obtain IRB approval within the study timeline. The data from this collaborative effort will improve state follow‐up programs and aid in developing three‐year follow‐up guidelines for children diagnosed with CH.     Living Pictures: Lewis and the Imagination - Martha Sammons This paper focuses on Lewis\u27s use of the metaphor of pictures and living pictures in his fiction and non-fiction. This metaphor conveys his ideas about the purpose of imagination. All Lewis\u27s stories, including The Chronicles of Narnia and the three science fiction books, began by seeing mental pictures. Then the ideas began to bubble up into the story form. Like a picture, says Lewis, a story should trigger the reader\u27s imagination and emotion. In contrast, Lewis describes imagining as dropping mental images once they appear; otherwise, imagination is inhibited. He uses living pictures in his stories to represent the eternal truth that art can only reflect through static images. The secondary world created by the imagination presents the world that lies behind appearances. Because this world is even more real than the world of fact we see in space and time, both worlds are necessary parts of the whole truth. Forming the Organ of Meaning: - Bill Gorman In this paper I introduce C.S. Lewis\u27 distinction between reason as the organ of truth and imagination as the organ of meaning and give some of the background which led him to make this distinction. This is followed by an examination of the implications of Lewis\u27s distinction for Christian formation particularly in the area of worldview shaping, re-shaping ad subverting. Special attention will be given to how Lewis understood reason and imagination and how his relationship with Owen Barfield affected his understanding of the role of the imagination in the knowing process. Analyzing the Faerie World as a Model for Christian Spirituality: An Interpretation of Spiritual Progression in George MacDonald\u27s Fantasy Literature - Jeffrey W. Smith When studying the works of George MacDonald, one cannot neglect the deep spiritual purpose found throughout his writings. Indeed, it seems that MacDonald never wrote lest he proclaimed what would direct the reader to a greater perspective of the eternal realm. This study will analyze the role of the faerie world in MacDonald’s literature as a plane which his characters must enter and acknowledge as they achieve spiritual union with the divine. Spiritual progression, therefore, will be emphasized with the use of “board-crossings” in MacDonald’s novels and fairy tales. Although MacDonald’s writing should be approached with a broad knowledge of theory, a Christian reading of his work is most crucial. This proposal shall offer further insight into MacDonald’s spirituality with the effect it has had on Christian thought. Moderator: Tom Moorma

    Linearity analysis and comparison study on the epocÂź point-of-care blood analysis system in cardiopulmonary bypass patients

    The epoc¼ blood analysis system (Epocal Inc., Ottawa, Ontario, Canada) is a newly developed in vitro diagnostic hand-held analyzer for testing whole blood samples at point-of-care, which provides blood gas, electrolytes, ionized calcium, glucose, lactate, and hematocrit/calculated hemoglobin rapidly. The analytical performance of the epoc¼ system was evaluated in a tertiary hospital, see related research article “Analytical evaluation of the epoc¼ point-of-care blood analysis system in cardiopulmonary bypass patients” [1]. Data presented are the linearity analysis for 9 parameters and the comparison study in 40 cardiopulmonary bypass patients on 3 epoc¼ meters, Instrumentation Laboratory GEM4000, Abbott iSTAT, Nova CCX, and Roche Accu-Chek Inform II and Performa glucose meters
