    Twin Pregnancy in a Woman with Uterus Didelphys

    Uterus didelphys is one of the congenital uterine anomalies due to defective medial fusion of mullerian ducts. This anomaly is known to have poor reproductive outcome and women with this condition often have to be treated for infertility. Multiple gestation is rare with this condition. An 18 years old primigravida presenting with threatened abortion at eight weeks, was found to have uterus didelphys. She was managed conservatively, aborted one of the fetuses at 16weeks of gestation, and went till term to deliver a healthy baby by cesarean section

    Should Magnesium Sulphate Prophylaxis be Used in all Cases of Severe Preeclampsia?

    A case control study, undertaken in the department of Obstetrics and Gynecology in a tertiary referral centre, was designed to administer standard dose of magnesium sulphate to 50 randomly selected severely preeclamptic women in labor (Group 1). 48 women who formed the control group did not receive the drug (Group 2). The admission – delivery interval, rate of Cesarean section showed no statistically significant difference. Four patients developed convulsions in Group 1 in contrast to twelve patients in Group 2. Efficacy of magnesium sulphate at preventing eclampsia was calculated as 68%. Mild respiratory depression occurred in two cases and oliguria in eight cases in Group 1. Both the conditions improved on suspending further doses of magnesium sulphate. No patient had post partum haemorrhage in Group 1 although 4% patients had it in Group 2. Neonatal outcome was comparable in both the groups. Hence magnesium sulphate may be regarded as a fairly safe and effective prophylactic agent for eclampsia when used in severe preeclampsia in labo

    Benign vulvar growths are rare. Initially these growths remain small and asymtomatic. Due to social stigma, young women in this stage often prefer to conceal the pathology. With considerable growth, apart from physical complaints, they might cause psychological problems of the subjects and even social withdrawal. Urgent detection and subsequent removal of the growth along with counselling is warranted to improve the patient’s self-esteem

    Unsafe abortion represents a preventable yet major cause for maternal mortality in India. A majority of these abortions are performed confidentially. Complications occur in a large portion of these cases and ultimately require tertiary care. However, patients and their relatives often fail to disclose the abortion despite the critical state of the patients. This scenario creates considerable confusion for diagnosis and treatment and can lead to further complications. This case report of complications associated with an unsafe abortion in India highlights the need for clinicians to consider the possibility of an undisclosed abortion when treating any morbid woman of reproductive age

    Neuropilin-2 regulates androgen-receptor transcriptional activity in advanced prostate cancer

    Aberrant transcriptional activity of androgen receptor (AR) is one of the dominant mechanisms for developing of castration-resistant prostate cancer (CRPC). Analyzing AR-transcriptional complex related to CRPC is therefore important towards understanding the mechanism of therapy-resistance. While studying its mechanism, we observed that a transmembrane protein called neuropilin-2 (NRP2) plays a contributory role in forming a novel AR-transcriptional complex containing nuclear pore proteins. Using immunogold electron microscopy, high-resolution confocal microscopy, chromatin immunoprecipitation, proteomics, and other biochemical techniques, we delineated the molecular mechanism of how a specific splice variant of NRP2 becomes sumoylated upon ligand stimulation and translocates to the inner nuclear membrane. This splice variant of NRP2 then stabilizes the complex between AR and nuclear pore proteins to promote CRPC specific gene expression. Both full-length and splice variants of AR have been identified in this specific transcriptional complex. In vitro cell line-based assays indicated that depletion of NRP2 not only destabilizes the AR-nuclear pore protein interaction but also inhibits the transcriptional activities of AR. Using an in vivo bone metastasis model, we showed that the inhibition of NRP2 led to the sensitization of CRPC cells toward established anti-AR therapies such as enzalutamide. Overall, our finding emphasize the importance of combinatorial inhibition of NRP2 and AR as an effective therapeutic strategy against treatment refractory prostate cancer

    Going beyond base-pairs: topology-based characterization of base-multiplets in RNA

    Identification and characterization of base-multiplets, which are essentially mediated by base-pairing interactions, can provide insights into the diversity in the structure and dynamics of complex functional RNAs, and thus facilitate hypothesis driven biological research. The necessary nomenclature scheme, an extension of the geometric classification scheme for base-pairs by Leontis and Westhof, is however available only for base-triplets. In the absence of information on topology, this scheme is not applicable to quartets and higher order multiplets. Here we propose a topology-based classification scheme which, in conjunction with a graph-based algorithm, can be used for the automated identification and characterization of higher order base-multiplets in RNA structures. Here, the RNA structure is represented as a graph, where nodes represent nucleotides and edges represent base-pairing connectivity. Sets of connected components (of n nodes) within these graphs constitute subgraphs representing multiplets of ``n'' nucleotides. The different topological variants of the RNA multiplets thus correspond to different nonisomorphic forms of these subgraphs. To annotate RNA base-multiplets unambiguously, we propose a set of topology-based nomenclature rules for quartets, which are extendable to higher multiplets. We also demonstrate the utility of our approach toward the identification and annotation of higher order RNA multiplets, by investigating the occurrence contexts of selected examples in order to gain insights regarding their probable functional roles
