    PROFILE OF INDIVIDUALS WITH NEUROLOGICAL DISORDERS ASSISTED BY A PREHOSPITAL MOBILE EMERGENCY CARE SERVICE

    This article aims to characterize the epidemiological profile of individuals with neurological disorders assisted by a prehospital mobile emergency care service. Exploratory and descriptive study with quantitative approach. Data was collected from January to June 2016, in an emergency and urgent care referral center in the state of Rio Grande do Norte. Seventy-three (73) neurological disorders were analyzed: 38 (52.1%) patients were male individuals, 43 (58.9%) were aged between 68 and 101 years. Regarding the characteristics of the neurological condition, 59 (80.8%) patients had a cerebrovascular accident (CVA), eight (11%) had seizures, three (4.1%) had low back pain related to spinal cord trauma and three (4.1%) patients had other types of neurological conditions. As for the response time to care, the average time was 31 minutes. The epidemiological profile identified was elderly men who had CVA (stroke) and whose care needs were not responded within an appropriate time.Artículo que objetiva caracterizar el perfil epidemiológico de víctimas de afecciones neurológicas atendidas por un Servicio de Atención Móvil de Urgencia. Estudio exploratorio, descriptivo, de abordaje cuantitativo de datos. La recolección se realizó de enero a junio de 2016, en hospital de referencia en atención de urgencias y emergencias del estado de Rio Grande do Norte. Fueron analizados 73 sucesos neurológicos, 38 (52,1%) en pacientes masculinos; 43 (58,9%) pertenecientes a faja etaria de 68 a 101 años. Respecto a características de la afección, 59 (80,8%) sufrieron Accidente Cerebrovascular, 8 (11%) crisis convulsiva, 3 (4,1%) lumbalgia asociada a lesión medular, y 3 (4,1%) fueron de otra naturaleza neurológica. Acerca del tiempo de respuesta, el promedio fue de 31 minutos. El perfil epidemiológico identificado fue de hombres ancianos, que sufrieron Accidente Cerebrovascular y que no recibieron atención en el tiempo de respuesta adecuado.Este artigo tem por objetivo caracterizar o perfil epidemiológico das vítimas de afecções neurológicas atendidas por um Serviço de Atendimento Móvel de Urgência. Trata-se de estudo exploratório, descritivo e de abordagem quantitativa dos dados. A coleta foi realizada no período de janeiro a junho de 2016, em um hospital referência em atendimentos de urgência e emergência do estado do Rio Grande do Norte. Foram analisadas 73 ocorrências neurológicas, 38 (52,1%) do sexo masculino, 43 (58,9%) entre a faixa etária de 68 a 101 anos. Quanto à característica da afecção neurológica, 59 (80,8%) sofreram Acidente Vascular Encefálico, oito (11%) crise convulsiva, três (4,1%) lombalgia associada a lesão medular e três (4,1%) eram de outra natureza neurológica. Quanto ao tempo resposta para o atendimento, a média foi de 31 minutos.     Objetivo: Determinar a prevalência dos marcadores sorológicos para hepatites B (VHB) e C (VHC) em portadores de Diabetes Mellitus tipo 2 (DM2) e analisar potenciais fatores de risco associados. Métodos: Estudo prospectivo transversal, realizado de janeiro a outubro de 2019, em unidade de saúde de nível secundário em Maceió–AL. Foram incluídos 304 pacientes portadores de DM2, maiores de 18 anos, de ambos os sexos, sendo 212 (69,7%) do sexo feminino, média de idade de 59,3 ± 10,1. Para análise sorológica das hepatites B e C foram realizados: HBsAg, anti-HBc IgG, anti-HBs e anti-VHC por meio de quimioluminescência. Os fatores de risco foram avaliados através de questionário estruturado e enzimas hepáticas: AST, ALT e GGT. Resultados: Não foi caracterizado nenhum caso de hepatite B aguda ou crônica. Verificou-se que 53 (17,4%) dos pacientes apresentaram contato prévio com VHB (anti-HBc reativo), 67 (22%) padrão de imunidade para essa infecção (anti-HBs reativo), sendo 32 (10,5%) com padrão vacinal, e 4 (1,3%) casos anti-VHC positivo. Elevação da GGT foi verificada em 137 (47,9%). Conclusão: O estudo evidenciou elevada prevalência (17,4%) de marcadores do VHB em pacientes diabéticos, entretanto 82,6% não apresentaram contato prévio, o que implica maior suscetibilidade ao vírus e ressalta a importância do controle de fatores de risco e da estratégia de vacinação nestes pacientes.     Protocol Proposal For The Care Of The Person With Venous Ulcer

    Objective: To propose a care protocol for the care of the person with a venous ulcer in highly complex services. Methods and results: This is a methodological study, in three stages: literature review, validation of content and validation in the clinical context. The literature review was carried out from June to August/2011, being the basis for the construction of the Protocol for Venous Ulcers. The content validation included 53 judges (44 nurses, 8 physicians and 1 physiotherapist) selected through the Lattes platform to evaluate the items of the protocol. Validation in the clinical context occurred at the University Hospital Onofre Lopes, in Natal/RN with four judges (nurses), who worked in pairs, evaluating 32 patients with venous ulcers. The protocol was validated with 15 categories: sociodemographic data; anamnesis; examinations; ulcer characteristics; care with the lesion and perilesional area; medicines used to treat the lesion; evaluation and treatment of pain; surgical treatment of chronic venous disease; recurrence prevention (clinical and educational strategies); reference; counter-reference; and quality of life. Conclusion: The validated protocol regarding content and clinical context was applicable. Its implementation is a viable measure that assists in the reorientation of the team in high complexity services, aiming at wound healing and restoration of the patient´s integral health.     Quality Of Care Indicators Of A Pre-Hospital Mobile Emergency: A Review

    Objective: To identify the quality of care indicators in pre-hospital mobile emergency services. Methods and results: This study is a systematic review conducted in December/2015 in eight databases using the keywords: quality indicators in health care; pre-hospital care; and quality, access, and evaluation of the health care. Seventeen studies were selected and the following indicators were identified: conservation of the ambulances; physical structure; comfort in the ambulance; material resources; safety for the patient/professional; continuing education; response time; professional remuneration; professional/patient satisfaction; access; host; humanization; performed service; safety demonstrated by the team; privacy to the patient; guidelines on care; relationship between the professional/patient; opportunity of the patient to complaint. Conclusion: The establishment the of the quality of care indicators the in mobile emergency services will allow the construction of instruments to evaluate this type of service to search for excellence results in mobile emergency services

    Clinical Reasoning: Instrument for Clinical Decisions of Nursing Students in the Care Process

    Objective: Report the experience of the Nursing graduation students regarding the high technological density services, implying the clinical reasoning practice as a tool in the care process.   Method: It is a descriptive study of the type: experience report of nursing students from a public University, located on Northeast of Brazil, focusing the practical activities accomplished during the period from May, 23 to June, 13 of 2016.   Results: The experience provided relevant results for the nursing students during the practical activities, being structured on the following axes: work process of the nurse; developed activities; and practical formation.   Conclusion: The experience lived by the nursing students was marked as a tool that widen and sizes about the several abilities and skills that the nursing professional must have.
