Perforación percutánea de la válvula pulmonar en recién nacidos. Evolución del abordaje transfemoral al transyugular

Depending on the patient’s anatomy, different therapeutic strategies—surgical or trancatheter—can be planned.
The objective of this study was to describe the results of transcatheter pulmonary valve perforation in patients with PA/IVS in a single tertiary center, and compare transjugular to transfemoral approach. The need for additional source of pulmonary flow (ductal stenting or systemic-to-pulmonary artery fistula) at follow-up was reviewed to identify possible risk factors associated with this reintervention.
Methods: patients with PA/IVS referred for transcatheter pulmonary valve perforation as first-line therapy from February 2004 through May 2022 were included. Technical procedural details, total procedural and fluoroscopy times, and demographic and echocardiographic data were studied.
Results: A total of 22 patients were included. Procedure was successful in 20 cases (91%). The rate of complications was 2/22 (9%). No deaths were reported. The transjugular and transfemoral approaches were equally safe and effective. The total median procedural (n = 20) and fluoroscopy times (n = 16), however, were shorter in the transjugular compared to the transfemoral approach (85 min vs 156 min, and 31 min vs 62 min, respectively), which reached statistical significance. At follow-up, 8/20 (40%) patients needed additional flow (4 ductal stenting, 4 systemic-to-pulmonary artery shunts). No significant risk factors regarding this reintervention were reported.
Conclusions: Transcatheter mechanical pulmonary valve perforation may be feasible in expert hands and properly selected patients being an attractive alternative to surgery. In our own experience, transjugular approach seems to simplify the procedure, and reduces procedural and fluoroscopy times.Introducción y objetivos: La atresia pulmonar con septo íntegro (APSI) es una cardiopatía congénita cianosante infrecuente que por su gravedad requiere un tratamiento en época neonatal. En función de la anatomía del ventrículo derecho y de la circulación coronaria, se pueden plantear distintas estrategias. El objetivo fue describir los resultados de la perforación valvular pulmonar percutánea de los pacientes con diagnóstico de APSI en un centro terciario. Se comparó el abordaje transfemoral con el transyugular. Se revisó la necesidad de flujo adicional (fístula sistémico-pulmonar o stent ductal) en el seguimiento, procurando identificar posibles factores de riesgo asociados a esta reintervención.
Métodos: Se incluyeron todos los pacientes con APSI tratados con perforación percutánea de la válvula pulmonar como primera opción terapéutica desde febrero de 2004 hasta mayo de 2022. Se estudiaron los detalles técnicos del cateterismo, los tiempos de procedimiento y de escopia, y variables demográficas y ecocardiográficas.
Resultados: Se incluyeron 22 pacientes y el procedimiento fue exitoso en 20 (91%). Se presentaron complicaciones en 2 pacientes (9%). No hubo fallecimientos. Los abordajes transyugular y transfemoral mostraron una eficacia y una seguridad parecidas. Los tiempos medianos de procedimiento (n = 20) y de escopia (n = 16) fueron menores en los pacientes con acceso transyugular que en aquellos con acceso transfemoral (85 frente a 156 y 31 frente a 62 minutos, respectivamente), con significación estadística. En el seguimiento, 8 pacientes (40%) necesitaron flujo adicional (4 stent ductal y 4 fístula sistémico-pulmonar). No se encontró ningún factor asociado significativamente a esta reintervención.
Conclusiones: La perforación percutánea de la válvula pulmonar puede ser factible en manos expertas y en pacientes bien seleccionados, y representa una alternativa atractiva a la cirugía. En nuestra experiencia, el abordaje transyugular parece simplificar el procedimiento, reduciendo los tiempos de intervencionismo y de escopia
Spanish cardiac catheterization in congenital heart diseases registry. First official report from the ACI-SEC and the GTH-SECPCC (2020)
Introduction and objectives: The Interventional Cardiology Association of the Spanish Society of Cardiology (ACI-SEC) and the Spanish Society of Pediatric Cardiology Working Group on Interventional Cardiology (GTH-SECPCC) introduce their annual activity report for 2020, the starting year of the pandemic of coronavirus disease (COVID-19). Methods: All Spanish centers with cath labs and interventional activity in congenital heart diseases were invited to participate. Data were collected online, and analyzed by an external company together with members from the ACI-SEC and the GTH-SECPCC. Results: A total of 16 centers participated (all of them public) including 30 cath labs experienced in the management of congenital heart diseases, 7 of them (23.3%) dedicated exclusively to pediatric patients. A total of 1046 diagnostic studies, and 1468 interventional cardiac catheterizations were registered. The interventional procedures were considered successful in 93.4% of the cases with rates of major procedural complications and mortality of 2%, and 0.1%, respectively. The most frequent procedures were atrial septal defect closure (377 cases), pulmonary angioplasty (244 cases), and the percutaneous closure of the patent ductus arteriosus (199 cases). Conclusions: This report is the first publication from the Spanish Cardiac Catheterization in Congenital Heart Diseases Registry. The data recorded are conditioned by the COVID-19 pandemic. Diagnostic cardiac catheterization still plays a key role in this field. Most interventional techniques have reported excellent security and efficacy rates
Aneurismas coronarios gigantes en la enfermedad de Kawasaki incompleta
La relación de la enfermedad de Kawasaki con la formación de aneurismas y estenosis coronarias es bien conocida, sobre todo en pacientes que no han recibido tratamiento con gammaglobulinas. Se presenta el caso de una paciente controlada por un conducto arterioso permeable, en cuya ecocardiografía de control a los 2 años de vida se observaron aneurismas coronarios gigantes en ambas coronarias. Se le realizaron angiografía por tomografía computarizada (angio-TC) y posterior cateterismo, que confirmaron el hallazgo. Revisando los antecedentes, ingresó con 9 meses de vida por un cuadro febril, compatible con pielonefritis, con buena respuesta al tratamiento antibiótico, que realmente pudo corresponder a una enfermedad de Kawasaki incompleta por la fiebre y la descamación posterior. Se inició entonces tratamiento antiagregante y anticoagulante, que se mantiene, de momento, de forma indefinida. A los 6 años de edad se repitieron la ecocardiografía (figura 1, eje corto; Ao: aorta, 1A: flecha derecha: aneurisma coronaria izquierda; flecha izquierda: aneurisma coronaria derecha; flecha 1B: aneurisma coronaria derecha), la angio-TC (figura 2, flechas derechas: aneurisma coronaria izquierda; flechas izquierdas: aneurisma coronaria derecha) y el cateterismo, que mostraron 2 aneurismas calcificados en la coronaria derecha de 17 × 8,8 mm y 7,3 × 5,3 mm (figura 3A,B y..

Registro español de intervencionismo en cardiopatías congénitas. Primer informe oficial de la ACI-SEC y el GTH-SECPCC (2020)
RESUMEN Introducción y objetivos: La Asociación de Cardiología Intervencionista de la Sociedad Española de Cardiología (ACI-SEC) y el Grupo de Trabajo de Hemodinámica de la Sociedad Española de Cardiología Pediátrica y Cardiopatías Congénitas (GTH-SECPCC) presentan el informe de actividad hemodinámica en cardiopatías congénitas de 2020, año de inicio de la pandemia de la enfermedad coronavírica de 2019 (COVID-19). Métodos: Se invitó a participar a los centros españoles con laboratorio de hemodinámica y actividad intervencionista en cardiopatías congénitas. La recogida de datos se realizó telemáticamente; una empresa externa, junto con miembros de la ACI-SEC y el GTH-SECPCC, los analizó. Resultados: Participaron 16 centros (todos públicos), que acumulan 30 salas de hemodinámica con actividad en cardiopatías congénitas, 7 (23,3%) de ellas con dedicación exclusiva a pacientes pediátricos. Se registraron 1.046 estudios diagnósticos y 1.468 cateterismos intervencionistas. Los procedimientos terapéuticos fueron exitosos en el 94,9%, con una tasa de complicaciones mayores del 2% y una mortalidad del 0,1%. Las técnicas más frecuentes fueron el cierre de comunicación interauricular (377 casos), la angioplastia pulmonar (244 casos) y el cierre de ductus arterioso (199 casos). Conclusiones: El presente trabajo representa la primera publicación del Registro Español de Intervencionismo en Cardiopatías Congénitas. La casuística registrada está condicionada por la pandemia de la COVID-19. Los cateterismos diagnósticos siguen teniendo un papel relevante en esta actividad. Para la mayoría de las técnicas intervencionistas se han reportado excelentes datos de seguridad y eficacia
