The use of a tailored surgical technique for minimally invasive esophagectomy
ObjectiveUncertainty exists among surgeons as to whether minimally invasive esophagectomy (MIE) is a comparable operation to open esophagectomy (OE). The surgical technique and oncologic dissection should not be degraded when using a minimally invasive approach.MethodsWe reviewed a single hospital’s experience with both OE and MIE. From 2000 to 2010, 257 patients underwent esophagectomy by 1 of 3 surgical techniques: transhiatal, Ivor Lewis, or 3-hole.ResultsOf the 257 patients (median age, 67 years; range, 58–74), 92 underwent MIE. Both groups were comparable in terms of gender, age, comorbidities, surgical technique, and induction chemotherapy and radiotherapy. The overall median follow-up was 29.5 months (range, 9.9–61.5). The MIE group had a significantly shorter operative time (MIE vs OE, 330 vs 365 minutes, P = .04), length of stay (MIE vs OE, 9 vs 12 days, P < .01), intensive care unit admission rate (MIE vs OE, 55% vs 81%, P < .01), intensive care unit length of stay (MIE vs OE, 1 vs 2 days, P < .01), and estimated blood loss (MIE vs OE, 100 vs 400 mL, P < .01). More lymph nodes were harvested in the MIE group than in the OE group (17 vs 11 nodes, P < .01). There were insignificant differences in 30-day mortality (MIE vs OE, 2.2% vs 3.0%; P = .93) and overall survival (P = .19), as well as in the rates of all complications, except pneumonia (MIE vs OE, 2% vs 13%; P = .01).ConclusionsA thoracic surgeon can safely tailor the MIE to a patient’s anatomy and oncologic demands while maintaining equivalent survival
Irradiation of Nf1 mutant mouse models of spinal plexiform neurofibromas drives pathologic progression and decreases survival
Background: Genetically susceptible individuals can develop malignancies after irradiation of normal tissues. In the context of therapeutic irradiation, it is not known whether irradiating benign neoplasms in susceptible individuals promotes neoplastic transformation and worse clinical outcomes. Individuals with Neurofibromatosis 1 (NF1) are susceptible to both radiation-induced second malignancies and spontaneous progression of plexiform neurofibromas (PNs) to malignant peripheral nerve sheath tumors (MPNSTs). The role of radiotherapy in the treatment of benign neoplasms such as PNs is unclear.
Methods: To test whether radiotherapy promotes neoplastic progression of PNs and reduces overall survival, we administered spinal irradiation (SI) to conditional knockout mouse models of NF1-associated PNs in 2 germline contexts: Nf1 fllfl ; PostnCre + and Nf1 fl/- ; PostnCre + . Both genotypes develop extensive Nf1 null spinal PNs, modeling PNs in NF1 patients. A total of 101 mice were randomized to 0 Gy, 15 Gy (3 Gy Ă— 5), or 30 Gy (3 Gy Ă— 10) of spine-focused, fractionated SI and aged until signs of illness.
Results: SI decreased survival in both Nf1 fllfl mice and Nf1 fl/- mice, with the worst overall survival occurring in Nf1 fl/- mice receiving 30 Gy. SI was also associated with increasing worrisome histologic features along the PN-MPNST continuum in PNs irradiated to higher radiation doses.
Conclusions: This preclinical study provides experimental evidence that irradiation of pre-existing PNs reduces survival and may shift PNs to higher grade neoplasms

Safety and effectiveness of bariatric surgery: Roux-en-y gastric bypass is superior to gastric banding in the management of morbidly obese patients: a response
The recent article by Guller, Klein, Hagen was reviewed and discussed by the authors of this response to critically analyze the validity of the conclusions, at a time when patients and providers depend on peer reviewed data to guide their health care choices. The authors of this response all have high volume bariatric surgery practices encompassing experience with both gastric bypass and gastric banding, and have made significant contributions to the peer reviewed literature. We examined the assumptions of the paper, reviewed the main articles cited, provided more evidence from articles that were included in the materials and methods of the paper, but not cited, and challenge the conclusion that Roux-en-Y gastric bypass is superior to gastric banding. The paper by Guller et al was subject to significant bias. The authors did not demonstrate an understanding of gastric banding, selectively included data with unfavorable results towards gastric banding, did not provide equal critique to the literature on gastric bypass, and deliberately excluded much of the favorable data on gastric banding. The paper's conclusion that gastric bypass is the procedure of choice is biased, unsubstantiated, not supported by the current literature and represents a disservice to the scientific and health care community

Nach aktueller Rechtsprechung ist die Begutachtung der Schuldfähigkeit und Massnahmenindikation Personen mit Facharzttitel für Psychiatrie und Psychotherapie vorbehalten. Der damit verbundene Ausschluss von Psycholog:innen ist angesichts der Entwicklungen in den letzten zehn Jahren nicht gerechtfertigt. Die Autor:innen zeigen auf, unter welchen Weiterbildungsbedingungen Psycholog:innen als Sachverständige geeignet sind.

L'exclusion des psychologues, qui en découle, ne se justifie pas au vu des évolutions de ces dix dernières années. Brigatinib causes tumor shrinkage in both NF2-deficient meningioma and schwannoma through inhibition of multiple tyrosine kinases but not ALK
Neurofibromatosis Type 2 (NF2) is an autosomal dominant genetic syndrome caused by mutations in the NF2 tumor suppressor gene resulting in multiple schwannomas and meningiomas. There are no FDA approved therapies for these tumors and their relentless progression results in high rates of morbidity and mortality. Through a combination of high throughput screens, preclinical in vivo modeling, and evaluation of the kinome en masse, we identified actionable drug targets and efficacious experimental therapeutics for the treatment of NF2 related schwannomas and meningiomas. These efforts identified brigatinib (ALUNBRIG®), an FDA-approved inhibitor of multiple tyrosine kinases including ALK, to be a potent inhibitor of tumor growth in established NF2 deficient xenograft meningiomas and a genetically engineered murine model of spontaneous NF2 schwannomas. Surprisingly, neither meningioma nor schwannoma cells express ALK. Instead, we demonstrate that brigatinib inhibited multiple tyrosine kinases, including EphA2, Fer and focal adhesion kinase 1 (FAK1). These data demonstrate the power of the de novo unbiased approach for drug discovery and represents a major step forward in the advancement of therapeutics for the treatment of NF2 related malignancies

Gerade in börsennotierten Gesellschaften nehmen zunehmend institutionelle Investoren und von ihnen beauftragte Stimmrechtsberater maßgeblichen Einfluss auf die Abstimmungsergebnisse auf der Hauptversammlung. Die Rolle institutioneller Investoren als Eigentümer börsennotierter Gesellschaften in Deutschland
Die Anzahl börsennotierter Unternehmen in Deutschland ist seit 2001 deutlich zurückgegangen und lag im Jahr 2018 mit 403 Unternehmen um 46% unter der Anzahl von 2001 (748). Maßstab ist hier die Anzahl der im CDAX gelisteten Unternehmen mit Sitz in Deutschland. Im gleichen Zeitraum verringerte sich unter den Unternehmen des Prime Standard der Anteil jener Unternehmen deutlich, die über einen Großaktionär mit einem Anteilsbesitz von mindestens 25% verfügen. Besonders stark ausgeprägt war dieser Rückgang bei Ankeraktionären mit mindestens 50-prozentiger Beteiligungsquote (von 34% 2001 auf 28% 2018). Mit dieser Entwicklung geht ein erhöhter Anteil an Streubesitz an den betreffenden Unternehmen einher. Eine stärkere Bedeutung institutioneller Investoren mit nennenswerten Beteiligungsquoten, insbesondere jenseits der Meldeschwelle von 3%, ist jedoch nur für den DAX-30 eindeutig festzustellen. Auch ein relativer Bedeutungszuwachs von passiven (ETFs, Indexfonds), in Relation zu aktiv gemanagten Fonds, ist insbesondere für den DAX-30 festzustellen. Hier fällt der Anstieg der Beteiligungsquote passiver Fonds von 1,3% (2007) auf 8,1% (2017) jedoch durchaus erheblich aus. Institutionelle Investoren aus dem angelsächsischen Raum, speziell aus den Vereinigten Staaten, konnten ihre Beteiligungsquoten an den Unternehmen des Prima Standard seit 2007 signifikant steigern. Besonders attraktiv scheinen für institutionelle Anleger aus empirischer Sicht solche Unternehmen zu sein, die relativ groß sind (gemessen an der logarithmierten Mitarbeiterzahl), die über relativ wenige Paketaktionäre verfügen, die aus besonders profitablen Industrien stammen und die über eher geringe Sachanlagequoten verfügen
