Audit of surgical delay in relationship to outcome after proximal femoral fracture.
To ascertain the influence of surgical delay on outcome after proximal femoral fracture in elderly females, a cohort study of patients presenting in 1987 was compared to 1989/90. Organisational changes in the intervening period were introduced to reduce delay to surgical intervention. Two hundred and eighty females aged 65 years and over presenting from the local catchment area of an acute inner-city teaching hospital were enrolled in the study. Seventy-nine patients received surgery in 1987 and 186 in 1989/90. The one year mortality was 34% and 26% respectively. The proportion receiving surgery within 24 hours rose from 34% in 1987 to 57% in 1989/90. The relative hazard of the group receiving surgery on day 2 in comparison to day 1 was 1.7 (95% CI 1.0 to 2.9) when adjusted for co-variance of age and mental score. Medically fit elderly patients presenting with proximal femoral fracture have improved survival with early surgery within 24 hours of admission. Improvements in the organisation of hospital care will result in important benefits for the increasing number of elderly females presenting with proximal femoral fracture
Comparison of nursing home residents admitted from home or hospital.
A growing elderly population coupled with a reduction in hospital long term care has led to an increase in the independent nursing home sector. This is an expensive resource. Proper placement is therefore essential to ensure its efficient use. Prior to the introduction of care management there was no standard assessment procedure for admission to nursing home care from different sources. A nursing home population (n = 624) in North and West Belfast was studied and mental scores, levels of disability, and the source of admission to the nursing home recorded. Residents admitted from geriatric medical units (n = 132) were compared with those from general medical and surgical wards (n = 168) and those from home (n = 243). Residents who were admitted from a geriatric unit were the most disabled, those admitted from home were the least and those from general wards had intermediate levels of disability (p < 0.005). This is likely to be the result of different assessment procedures for prospective nursing home residents. With the introduction of care management, it is hoped that standardised assessment will follow. The roles of different medical specialists in this process is not yet clear. Further study is needed to assess the appropriateness of placement in nursing homes under care management
Origins of Mass
Newtonian mechanics posited mass as a primary quality of matter, incapable of
further elucidation. We now see Newtonian mass as an emergent property. Most of
the mass of standard matter, by far, arises dynamically, from back-reaction of
the color gluon fields of quantum chromodynamics (QCD). The equations for
massless particles support extra symmetries - specifically scale, chiral, and
gauge symmetries. The consistency of the standard model relies on a high degree
of underlying gauge and chiral symmetry, so the observed non-zero masses of
many elementary particles ( and bosons, quarks, and leptons) requires
spontaneous symmetry breaking. Superconductivity is a prototype for spontaneous
symmetry breaking and for mass-generation, since photons acquire mass inside
superconductors. A conceptually similar but more intricate form of
all-pervasive (i.e. cosmic) superconductivity, in the context of the
electroweak standard model, gives us a successful, economical account of
and boson masses. It also allows a phenomenologically successful, though
profligate, accommodation of quark and lepton masses. The new cosmic
superconductivity, when implemented in a straightforward, minimal way, suggests
the existence of a remarkable new particle, the so-called Higgs particle. The
mass of the Higgs particle itself is not explained in the theory, but appears
as a free parameter. Earlier results suggested, and recent observations at the
Large Hadron Collider (LHC) may indicate, the actual existence of the Higgs
particle, with mass GeV. In addition to consolidating our
understanding of the origin of mass, a Higgs particle with
GeV could provide an important clue to the future, as it is consistent with

Flavour constraints on scenarios with two or three heavy squark generations
We re-assess constraints from flavour-changing neutral currents in the kaon
system on supersymmetric scenarios with a light gluino, two heavy generations
of squarks and a lighter third generation. We compute for the first time limits
in scenarios with three heavy squark families, taking into account QCD
corrections at the next-to-leading order. We compare our limits with those in
the case of two heavy families. We use the mass insertion approximation and
consider contributions from gluino exchange to constrain the mixing between the
first and second squark generation. While it is not possible to perform a
general analysis, we assess the relevance of each kind of flavour- and
CP-violating parameters. We also provide ready to use magic numbers for the
computation of the Wilson coefficients at 2 GeV for these scenarios.Comment: 23 pages, 14 figures; v3: matches published version (contains

The , interaction in finite volume and the resonance
In this work the interaction of the coupled channels and
in an SU(4) extrapolation of the chiral unitary theory, where the
resonance appears as dynamically generated from that
interaction, is extended to produce results in finite volume. Energy levels in
the finite box are evaluated and, assuming that they would correspond to
lattice results, the inverse problem of determining the phase shifts in the
infinite volume from the lattice results is solved. We observe that it is
possible to obtain accurate phase shifts and the position of the
resonance, but it requires the explicit consideration of the
two coupled channels. We also observe that some of the energy levels in the box
are attached to the closed channel, such that their use to induce the phase shifts via L\"uscher's formula leads to incorrect results.Comment: 10 pages, 13 figures, accepted for publication in Eur. Phys. J.
Flavor Violating Higgs Decays
We study a class of nonstandard interactions of the newly discovered 125 GeV
Higgs-like resonance that are especially interesting probes of new physics:
flavor violating Higgs couplings to leptons and quarks. These interaction can
arise in many frameworks of new physics at the electroweak scale such as two
Higgs doublet models, extra dimensions, or models of compositeness. We rederive
constraints on flavor violating Higgs couplings using data on rare decays,
electric and magnetic dipole moments, and meson oscillations. We confirm that
flavor violating Higgs boson decays to leptons can be sizeable with, e.g., h ->
tau mu and h -> tau e branching ratios of order 10% perfectly allowed by low
energy constraints. We estimate the current LHC limits on h -> tau mu and h ->
tau e decays by recasting existing searches for the SM Higgs in the tau-tau
channel and find that these bounds are already stronger than those from rare
tau decays. We also show that these limits can be improved significantly with
dedicated searches and we outline a possible search strategy. Flavor violating
Higgs decays therefore present an opportunity for discovery of new physics
which in some cases may be easier to access experimentally than flavor

'To live and die [for] Dixie': Irish civilians and the Confederate States of America
Around 20,000 Irishmen served in the Confederate army in the Civil War. As a result, they left behind, in various Southern towns and cities, large numbers of friends, family, and community leaders. As with native-born Confederates, Irish civilian support was crucial to Irish participation in the Confederate military effort. Also, Irish civilians served in various supporting roles: in factories and hospitals, on railroads and diplomatic missions, and as boosters for the cause. They also, however, suffered in bombardments, sieges, and the blockade. Usually poorer than their native neighbours, they could not afford to become 'refugees' and move away from the centres of conflict. This essay, based on research from manuscript collections, contemporary newspapers, British Consular records, and Federal military records, will examine the role of Irish civilians in the Confederacy, and assess the role this activity had on their integration into Southern communities. It will also look at Irish civilians in the defeat of the Confederacy, particularly when they came under Union occupation. Initial research shows that Irish civilians were not as upset as other whites in the South about Union victory. They welcomed a return to normalcy, and often 'collaborated' with Union authorities. Also, Irish desertion rates in the Confederate army were particularly high, and I will attempt to gauge whether Irish civilians played a role in this. All of the research in this paper will thus be put in the context of the Drew Gilpin Faust/Gary Gallagher debate on the influence of the Confederate homefront on military performance. By studying the Irish civilian experience one can assess how strong the Confederate national experiment was. Was it a nation without a nationalism
Polarized semi-inclusive electroweak structure functions at next-to-leading-order
We present a next-to-leading order (NLO) computation of the full set of
polarized and unpolarized electroweak semi-inclusive DIS (SIDIS) structure
functions, whose knowledge is crucial for a precise extraction of polarized
parton distributions. We focus on the phenomenology of the polarized structure
functions for the kinematical conditions that could be reached in an
Electron-Ion-Collider.
We show that the NLO corrections are sizeable, particularly in the small-
range. We test the sensitivity of these structure functions on certain quark
distributions and compare it to the situation of inclusive DIS and
electromagnetic SIDIS.Comment: 17 pages, 5 figure
Higgs Boson Mass in Low Scale Gauge Mediation Models
We consider low scale gauge mediation models with a very light gravitino
m_{3/2}~16 eV, in the light of recent experimental hints on the Higgs boson
mass. The light gravitino is very interesting since there is no gravitino
over-production problem, but it seems difficult to explain the Higgs boson mass
of ~125 GeV. This is because of the conflict between the light gravitino mass
and heavy SUSY particle masses needed for producing the relatively heavy Higgs
boson mass. We consider two possible extensions in this paper: a singlet
extension of the Higgs sector, and strongly coupled gauge mediation. We show
that there is a large parameter space, in both scenarios, where the Higgs boson
mass of ~125 GeV is explained without any conflict with such a very light
