Early assessment of vestibular function after unilateral cochlear implant surgery
Introduction : Cochlear implantation (CI) has been reported to negatively effect on the vestibular function. The study of the vestibular function has variably been conducted by different types of diagnostic tools. The combined use of modern, rapidly performable diagnostic tools could reveal useful for standardizing the evaluation protocol.
Methods: In a group of 28 subjects undergoing CI, the video Head Impulse Test (vHIT), the cervical Vestibular Evoked Myogenic Potentials (cVEMPS) and the short-form of Dizziness Handicap Inventory (DHI) questionnaire were investigated pre-operatively and post-operatively (implant on and off) in both the implanted and the contralateral, non-implanted ear. All surgeries were performed with a round window approach (RWA), except for three otosclerosis cases were the extended RWA (eRWA) was used.
Results: The vHIT of the lateral semicircular canal showed a pre-operative vestibular involvement in nearly 50% of the cases, whilst the three canals were contemporarily affected in only 14% of them. In all the hypo-functional subjects, cVEMPs were absent. A low VOR gain in all the investigated SSCC was found in 4 subjects (14%). In those subjects, (21.7%) in whom cVEMPs were pre-operatively present and normal in the operated side, absence of response was post-operatives recorded.
Discussion/Conclusion: The vestibular protocol applied for the study showed to be appropriate for distinguishing between the CI operated and the non-operated ear. In this regard, cVEMPs showed to be more sensitive than vHIT for revealing a vestibular sufferance after CI, although without statistical significance. Finally, the use of the RWA surgery was apparently not avoiding signs of vestibular impairment to occur
Rhamnolipids: diversity of structures, microbial origins and roles
Rhamnolipids are glycolipidic biosurfactants produced by various bacterial species. They were initially found as exoproducts of the opportunistic pathogen Pseudomonas aeruginosa and described as a mixture of four congeners: α-L-rhamnopyranosyl-α-L-rhamnopyranosyl-β-hydroxydecanoyl-β-hydroxydecanoate (Rha-Rha-C10-C10), α-L-rhamnopyranosyl-α-L-rhamnopyranosyl-β-hydroxydecanoate (Rha-Rha-C10), as well as their mono-rhamnolipid congeners Rha-C10-C10 and Rha-C10. The development of more sensitive analytical techniques has lead to the further discovery of a wide diversity of rhamnolipid congeners and homologues (about 60) that are produced at different concentrations by various Pseudomonas species and by bacteria belonging to other families, classes, or even phyla. For example, various Burkholderia species have been shown to produce rhamnolipids that have longer alkyl chains than those produced by P. aeruginosa. In P. aeruginosa, three genes, carried on two distinct operons, code for the enzymes responsible for the final steps of rhamnolipid synthesis: one operon carries the rhlAB genes and the other rhlC. Genes highly similar to rhlA, rhlB, and rhlC have also been found in various Burkholderia species but grouped within one putative operon, and they have been shown to be required for rhamnolipid production as well. The exact physiological function of these secondary metabolites is still unclear. Most identified activities are derived from the surface activity, wetting ability, detergency, and other amphipathic-related properties of these molecules. Indeed, rhamnolipids promote the uptake and biodegradation of poorly soluble substrates, act as immune modulators and virulence factors, have antimicrobial activities, and are involved in surface motility and in bacterial biofilm development
Molecular Cloud Populations in the Context of Their Host Galaxy Environments: A Multiwavelength Perspective
We present a rich, multiwavelength, multiscale database built around the PHANGS-ALMA CO (2 - 1) survey and ancillary data. We use this database to present the distributions of molecular cloud populations and subgalactic environments in 80 PHANGS galaxies, to characterize the relationship between population-averaged cloud properties and host galaxy properties, and to assess key timescales relevant to molecular cloud evolution and star formation. We show that PHANGS probes a wide range of kpc-scale gas, stellar, and star formation rate (SFR) surface densities, as well as orbital velocities and shear. The population-averaged cloud properties in each aperture correlate strongly with both local environmental properties and host galaxy global properties. Leveraging a variable selection analysis, we find that the kpc-scale surface densities of molecular gas and SFR tend to possess the most predictive power for the population-averaged cloud properties. Once their variations are controlled for, galaxy global properties contain little additional information, which implies that the apparent galaxy-to-galaxy variations in cloud populations are likely mediated by kpc-scale environmental conditions. We further estimate a suite of important timescales from our multiwavelength measurements. The cloud-scale freefall time and turbulence crossing time are ∼5-20 Myr, comparable to previous cloud lifetime estimates. The timescales for orbital motion, shearing, and cloud-cloud collisions are longer, ∼100 Myr. The molecular gas depletion time is 1-3 Gyr and shows weak to no correlations with the other timescales in our data. We publish our measurements online, and expect them to have broad utility to future studies of molecular clouds and star formation

The article “Rosai-Dorfman disease with isolated laryngeal involvement” in the October 2012 issue10 is very interesting, and we found it useful in helping us reach the diagnosis in a case of Rosai-Dorfman disease (RDD) of the larynx. Our experience with this case led us to formulate an opinion about the correct interpretation of the rarity of this localization in the larynx.
Reports indicate that 43% of patients with RDD exhibit extranodal disease, with the head and neck region being one of the most common extranodal areas involved.2,3 Within the head and neck, the most common locations are the nasal cavity, paranasal sinuses, and orbit; laryngeal involvement is extremely rare, as only 16 cases have been reported in the literature.4,5 Our opinion is that localization of RDD in the larynx is rare, as indicated by a review of the literature, but it can be explained by the anatomy of the larynx itself.
The larynx, despite the absence of lymph nodes, has endolymphatic tissue, which is located in the soft tissues, as part of the MALT (mucosa-associated lymphoid tissue) system. The MALT system is present in different locations of the human body, predominantly in the submucosal region, and is an important part of the immune system. In the larynx, the MALT system is prevalent in the supraglottic region and is present in each of the subsites of the larynx, as well.6 RDD is therefore localized in the intranodal and extranodal (MALT system) lymphatic system.
According to the literature, RDD of the larynx is rare, but when you consider that 43% of patients with an extranodal manifestation of RDD have involvement of the MALT system, which is widely represented in the larynx, it is our opinion that the larynx should be considered as a possible location of the disease. Hence, it is not uncommon, and therefore should not be considered a rare organ disease
Sudden sensorineural hearing loss: A vascular cause? Analysis of prothrombotic risk factor in head and neck.
Objective: This aim of this study was to determine the prevalence of thrombophilic risk factors in sudden sensorineural hearing loss, central retinal vein occlusion, and stroke associated with small vessel disease, with the purpose of investigating and reinforcing the vascular hypothesis in the pathogenesis of sudden sensorineural hearing loss. Design: Case-control study. Genetic and acquired risk factors of these three groups were compared with healthy controls. Study sample: Forty-nine, 60, and 101 patients affected respectively by sudden sensorineural hearing loss, central retinal vein occlusion, or stroke associated with small vessel disease, enrolled during a three-year period were compared with 210 healthy controls. Results: The frequency of hyperhomocysteinemia (homocysteine ≥ 15 μmol/L) was higher in each disease group than in controls. A statically significant, albeit weak, correlation between the MTHFR C677T mutation and hyperhomocysteinemia was found in all three diseases. Conclusions: Hyperhomocysteinemia proved to be a risk factor for sudden sensorineural hearing loss. Based on these results, we propose to analyse homocysteine in sudden sensorineural hearing loss patients and, if its values are high, to evaluate the presence of MTHFR C677T mutation
PHANGS-ALMA: Arcsecond CO(2-1) Imaging of Nearby Star-forming Galaxies
We present PHANGS-ALMA, the first survey to map CO J = 2 -> 1 line emission at similar to 1 '' similar to 100 pc spatial resolution from a representative sample of 90 nearby (d less than or similar to 20 Mpc) galaxies that lie on or near the z = 0 "main sequence" of star-forming galaxies. CO line emission traces the bulk distribution of molecular gas, which is the cold, star-forming phase of the interstellar medium. At the resolution achieved by PHANGS-ALMA, each beam reaches the size of a typical individual giant molecular cloud, so that these data can be used to measure the demographics, life cycle, and physical state of molecular clouds across the population of galaxies where the majority of stars form at z = 0. This paper describes the scientific motivation and background for the survey, sample selection, global properties of the targets, Atacama Large Millimeter/submillimeter Array (ALMA) observations, and characteristics of the delivered data and derived data products. As the ALMA sample serves as the parent sample for parallel surveys with MUSE on the Very Large Telescope, the Hubble Space Telescope, AstroSat, the Very Large Array, and other facilities, we include a detailed discussion of the sample selection. We detail the estimation of galaxy mass, size, star formation rate, CO luminosity, and other properties, compare estimates using different systems and provide best-estimate integrated measurements for each target. We also report the design and execution of the ALMA observations, which combine a Cycle 5 Large Program, a series of smaller programs, and archival observations. Finally, we present the first 1 '' resolution atlas of CO emission from nearby galaxies and describe the properties and contents of the first PHANGS-ALMA public data release
