    Process capability indices and X , R control chart limit adjustments by taking into account measurement system errors

    This paper investigates the effects of measurement system variability arising from evaluations of process capabilities. In this work, relationships between true process capability indices, Cp,True and Cpm,True, and their corresponding observed indices, Cp,Obs and Cpm,Obs, are developed depending on the levels of type I and II errors. Moreover, the method for adjusting control chart limits used to monitor the process is proposed based on measurement system variability. An industrial case study is used to highlight the findings of this investigation and to discuss the adjustment levels that should be conducted depending on the values of type I and type II errors

    Neurocognitive outcome after preterm birth: Interest of the follow-up and the systematic evaluation

    ObjectivePreterm children can experience cognitive and behavioral difficulties being able to be responsible for school difficulties going to the academic failure. The aim of this study was to assess the cognitive process while insisting on the early screening from the preschool age.MethodsThe data arise from the study of files and from the neuropsychological evaluations realized with the premature children followed in a regular way to the service. The premature children with or without motor disabilities more 4 and a half-years, old deficit integrated pre-school and ordinary school were included. The children with severe disabilities in upper limbs and the children having a mental deficiency were excluded.Results30 middle-aged children 7 years 5 months have been included. The prematurity is between 27–34. The born term has an effect on the performances in particular on attention and visuo-spatial capacities.ConclusionThe prematurity is a risk factor of the school future of the child. There is specially a negative impact on visuo-spatial and visuo-motor processes and those children present social and behavioral difficulties. It is mandatory to include the neuropsychological evaluation in any follow-up of premature child thanks to tests validated in the Tunisian context. It remains of great importance to identify effective interventions to improve the long-term neurocognitive outcomes

    L’aspergillome sphenoïdal : a propos d’un cas sphenoidalaspergilloma : a case report

    Introduction : L’aspergillose sphénoïdale isolée est une pathologie rare potentiellement grave vu le risque d’extension neuro-méningée et orbitaire. malgré qu’elle est plus fréquente chez les immuno-déprimés, l’atteinte de l’immunocompétent a été décrite. a travers une observation, nous rapportons les caractéristiques clinico-radiologiques, thérapeutiques et évolutives de l’aspergillome sphénoïdal.Observutin : Homme de 35 ans a consulté pour des céphalées rétro-orbitaires évoluant depuis 3 mois. L’examen était normal à part une rhinorrhée purulente postérieure. Le scanner a montré un processus hétérogène comblant le sinus sphénoïdal associé à une lyse osseuse. Une sphénoïdotomiepar voie endonasale a permis de confirmer le diagnostic et de nettoyer les lésions aspergillaires. Un traitement par voriconazol a été associé avec évolution favorable après un recul de 18 mois.Conclusion : L’aspergillome sphénoïdale est l’un des diagnostics différentiel des tumeurs sphénoïdales chez l’immunocompétent. Le traitement est essentiellement chirurgical avec de très bons résultats. L’adjonction d’un antifongique systémique est controversée chez l’immunocompétent.Mots- clefs : aspergillome, sphénoïde, imagerie, Tumeur, chirurgieObjective : Isolated sphenoidalaspergillosis is rare but potentially grave because of the risk of neuro-meningeal and orbitalextension. althoughitis more common in immuno-compromised, the achievement of immunocompetent has been described. Through an observation, we report the clinic-radiological, therapeutic and evolutionary featuresof sphenoidalaspergilloma.Case-report : male 35 years consulted for retro-orbital headache lasting for 3 months. The exam was normal except a purulent rhinorrhea. The CT scan showed a heterogeneous process filling the sphenoid sinus associated with bone loss. Sphenoidotomy by an endonasal approach confirmed the diagnosis of aspergillosis lesions. Treatment with voriconazole was associated with favorable outcome after 18 months.Conclusion : Sphenoidal as pergilloma is a differential diagnosis of tumors in the immuno-competent. Treatment is mainly surgical with good results. The addition of a systemic antifungal agent is controversial in the immunocompetent.Keywords : aspergilloma, sphenoïd, imagery, tumor, surger

    Leiomyome agressif naso-sinusien : a propos d’un cas et revue de la litterature

    Introduction : Les léiomyomes sont des tumeurs bénignes issues des fibres musculaires lisses. La localisation au niveau des cavités nasosinusiennes est très rare.Cas clinique : Il s’agit d’une patiente âgée de 59 ans, qui a consulté pour obstruction nasale et épistaxis gauches. L’endoscopie nasale a montré une formation comblant totalement la fosse nasale gauche. L’imagerie (TDM, IRM) a objectivé un volumineux processus expansif tissulaire naso-ethmoïdo-maxillaire, avec extension intra-orbitaire et endocrânienne. La biopsie confirmé qu’il s’agit d’un léiomyome à localisation nasosinusienne. La patiente a été opérée par voie paralatéronasale élargie. Elle a eu une exérèse tumorale complète et une reconstruction de la perte de substance osseuse par autogreffe. Les suites opératoires étaient simples. Aucune récidive n’a été notée après un recul de 1 an.Conclusion : La localisation nasosinusienne du léiomyome est très rare. L’imagerie est indispensable dans le cadre du bilan d’extension loco-régional. Seul une exérèse chirurgicale complète permettra d’éviter toute récidive aussi bien à court qu’à long terme.Mots-clés : léiomyome, fosses nasales, sinus, tomodensitométrie, imagerie par résonance magnétique, chirurgie sinusienneIntroduction: Leiomyomas are benign tumors arising from smooth muscle fibers. The location at the sinonasal cavities is very rare.Case report: This is a female patient aged 59, who consulted for left nasal obstruction and epistaxis. Nasal endoscopy showed a tumor completely filling the left nasal cavity. Imaging (CT, MRI) has objectified a large expansive tissularnaso-maxillary-ethmoidal process with intraorbital and intracranial extension. The biopsy confirmed that this was a leiomyoma with sinonasal localization. The patient was operated by extended paralatéronasale approach. She had a complete tumor resection and reconstruction of the bone defect with autograft. The postoperative course was uneventful. No recurrence was observed after a follow-up period of 1 year.Conclusion: The sinonasal location of leiomyoma is very rare. Imaging is essential for locoregional staging.
