    INSTITUTIONAL ASPECTS OF THE CLUSTER DEVELOPMENT OF THE REGIONAL ECONOMIC SYSTEM

    Purpose: The heterogeneity of the economic space has intensified in recent years in the process of transition and laying the foundations of a market economy in the CIS countries, as well as in the EU regions, due to its expansion. This unevenness was manifested as a result of different starting levels of economies, different resource endowments, the choice of their targets and priorities for socio-economic development, and the increased authority of local authorities. Methodology: This is analytical-logical research that has been done through content analysis and documentary and library research. Result: Problems of market fiasco in the regions, as well as the important role of local institutions in regional development are also noted in the EU and serve as a strong argument for the need for government intervention in regional policy, primarily from the standpoint of regulating the innovation sphere, financial assistance to create jobs or clusters capable of self-development. Applications: This research can be used for universities, teachers, and students.     Early infant feeding and adiposity risk: from infancy to adulthood

    Introduction: Systematic reviews suggest that a longer duration of breast-feeding is associated with a reduction in the risk of later overweight and obesity. Most studies examining breast-feeding in relation to adiposity have not used longitudinal analysis. In our study, we aimed to examine early infant feeding and adiposity risk in a longitudinal cohort from birth to young adulthood using new as well as published data. Methods: Data from the Western Australian Pregnancy Cohort (Raine) Study in Perth, W.A., Australia, were used to examine associations between breast-feeding and measures of adiposity at 1, 2, 3, 6, 8, 10, 14, 17, and 20 years. Results: Breast-feeding was measured in a number of ways. Longer breast-feeding (in months) was associated with reductions in weight z-scores between birth and 1 year (ÎČ = -0.027; p \u3c 0.001) in the adjusted analysis. At 3 years, breast-feeding for \u3c4 months increased the odds of infants experiencing early rapid growth (OR 2.05; 95% CI 1.43-2.94; p \u3c 0.001). From 1 to 8 years, children breast-fed for ≀4 months compared to ≄12 months had a significantly greater probability of exceeding the 95th percentile of weight. The age at which breast-feeding was stopped and a milk other than breast milk was introduced (introduction of formula milk) played a significant role in the trajectory of the BMI from birth to 14 years; the 4-month cutoff point was consistently associated with a higher BMI trajectory. Introduction of a milk other than breast milk before 6 months compared to at 6 months or later was a risk factor for being overweight or obese at 20 years of age (OR 1.47; 95% CI 1.12-1.93; p = 0.005). Discussion: Breast-feeding until 6 months of age and beyond should be encouraged and is recommended for protection against increased adiposity in childhood, adolescence, and young adulthood. Adverse long-term effects of early growth acceleration are fundamental in later overweight and obesity. Formula feeding stimulates a higher postnatal growth velocity, whereas breast-feeding promotes slower growth and a reduced likelihood of overweight and obesity.     Adolescent dietary patterns are associated with lifestyle family psycho-social factors

    Background/ Objectives: Dietary intake during adolescence contributes to lifelong eating habits and the development of early risk factors for disease in adulthood. Few studies have examined the dietary patterns of adolescents and the social and environmental factors that may affect them during this life stage. The present study describes dietary patterns in a cohort of adolescents and examines their associations with socioeconomic factors, as well as parental and adolescent risk factor behaviours. Design: A semi quantitative FFQ was used to assess study adolescents’ usual dietary intake over the previous year. Information was collected on family functioning and various socio economic and risk factor variables via questionnaire. Adolescents visited the clinic for anthropometric measurements. Setting: The Western Australian Pregnancy Cohort Study (Raine Study), Perth, Western Australia. Subjects: Adolescents (n 1631) aged 14 years from a pregnancy cohort study. Results: Factor analysis identified two distinct dietary patterns that differed predominantly in fat and sugar intakes. The ‘Western’ pattern consisted of high intakes of take away foods, soft drinks, confectionery, French fries, refined grains, full fat dairy products and processed meats. The ‘Healthy’’ pattern included high intakes of whole grains, fruit, vegetables, legumes and fish. ANOVA showed that the ‘Western’ dietary pattern was positively associated with greater television viewing and having a parent that smoked, and was inversely associated with family income. The ‘Healthy’’ pattern was positively associated with female gender, greater maternal education, better family functioning and being in a two parent family, and was inversely associated with television viewing. Conclusions: This study suggests that both lifestyle factors and family psycho social environment are related to eating patterns in Australian adolescents

    Effects of muscle strength and endurance on blood pressure and related cardiometabolic risk factors from childhood to adolescence

    Objective: This study aimed to examine the evolution of relationships between measures of muscle strength and endurance with individual cardiometabolic risk factors from childhood to late adolescence in a prospective population-based cohort. Methods: Participants from the Western Australian Pregnancy Cohort (Raine) Study at ages 10, 14 and 17 were analysed, using longitudinal linear mixed model analyses. Results: Handgrip strength after adjusting for the confounding effects of BMI was positively associated with SBP, but not DBP. The association between handgrip strength and SBP was stronger in men than women at all time points [coefficient (women): 0.18, P < 0.001; sex × handgrip strength coefficient: 0.09, P = 0.002]. The association was strongest at 10 years and significantly attenuated over time (year × handgrip coefficient from 10 to 14 years: −0.11, P = 0.003; year × handgrip coefficient from 10 to 17 years: −0.19, P ≀ 0.001). After the inclusion of BMI as a confounder, handgrip strength was significantly negatively associated with homeostatic model assessment of insulin resistance and high-sensitivity C-reactive protein over time in both sexes. Back muscle endurance was positively associated with SBP, but not DBP, after adjustment for the confounding effects of BMI (coefficient: 0.01, P = 0.002). There were small, albeit significant, inverse associations between back muscle endurance and log homeostatic model assessment of insulin resistance and log high-sensitivity C-reactive protein. Conclusion: The positive association between handgrip strength and back muscle endurance with SBP throughout childhood and adolescence contrasts with beneficial effects on other related traditional cardiometabolic risk factors. Mechanisms underlying these paradoxical effects on SBP warrant further investigation

    Abstract— The article shows a study of the role of supply chain management of innovative biologically valuable food products in the industrial development and economy of the region. The analysis of the process of updating the assortment of food products based on the introduction of innovative developments and production principles conducive to the release of a healthy diet product (innovative product) is presented. The main provisions of the concept of healthy food products, the category of “innovative food products” are analyzed. The article systematizes the definitions of the concept of “innovative product”. Based on the results of the study, features, functional properties and characteristic features of an innovative food product are determined. Based on the concept of “innovative food product”, the concepts of “new food product”, “improved food product”, and “modified food product” are formulated. A classification model of innovative food products is presented. Based on it, it is shown that the development, production and sale of an innovative food product two functions: economic and social. The study made it possible to formulate the main criteria that make it possible to attribute a food product to a group of innovative food products. The article describes the characteristics of an innovative food product, describes market and consumer properties. The author's definitions of concepts are given: “innovative food product”, “new food product”, “improved food product”, “modified food product”. A classification of innovative food products is proposed

    Low vitamin D levels are associated with symptoms of depression in young adult males

    Objective: Results from studies examining associations between serum 25-hydroxyvitamin D (25(OH)D) concentrations and depressive symptoms are equivocal. We investigated the relationship between serum 25(OH)D concentrations and symptoms of depression, anxiety and stress in a cross-sectional analysis of a population-based sample of young adults participating in the Western Australian Pregnancy Cohort (Raine) Study. Methods: Participants provided a blood sample at the 20-year follow-up (March 2010-April 2012) for the measurement of serum 25(OH)D concentrations. Mental health symptoms were assessed using the 21-item Depression Anxiety Stress Scales (DASS-21). Associations between serum 25(OH)D concentrations and total DASS-21 scores and subscale scores of depression, anxiety and stress were explored in males and females using negative binomial regression, adjusting for age, race, body mass index (BMI) and physical activity (n=735). Models examining subscale scores were also adjusted for the other subscale scores. Results: After adjusting for confounders, an increase in serum 25(OH)D concentrations of 10 nmol/L decreased total DASS-21 scores in males by 9% (rate ratio (RR) 0.91; 95%CI 0.87,0.95; p<0.001) and depression subscale scores in males by 8% (RR 0.92; 95%CI 0.87,0.96; p=0.001). However, in adjusted models there were no significant associations between serum 25(OH)D concentrations and symptoms of anxiety and stress in males. There were no significant associations between serum 25(OH)D concentrations and symptoms of depression, anxiety and stress in females. Conclusions: We found an association between serum 25(OH)D concentrations and symptoms of depression, but not anxiety and stress, in males. Randomised controlled trials are necessary to determine any benefit of vitamin D supplementation in the prevention and treatment of depressive symptoms in young adults

    A comparison of outcomes with angiotensin-converting-enzyme inhibitors and diuretics for hypertension in the elderly

    Copyright © 2003 Massachusetts Medical Society. All rights reserved.Background Treatment of hypertension with diuretics, beta-blockers, or both leads to improved outcomes. It has been postulated that agents that inhibit the renin–angiotensin system confer benefit beyond the reduction of blood pressure alone. We compared the outcomes in older subjects with hypertension who were treated with angiotensin-converting–enzyme (ACE) inhibitors with the outcomes in those treated with diuretic agents. Methods We conducted a prospective, randomized, open-label study with blinded assessment of end points in 6083 subjects with hypertension who were 65 to 84 years of age and received health care at 1594 family practices. Subjects were followed for a median of 4.1 years, and the total numbers of cardiovascular events in the two treatment groups were compared with the use of multivariate proportional-hazards models. Results At base line, the treatment groups were well matched in terms of age, sex, and blood pressure. By the end of the study, blood pressure had decreased to a similar extent in both groups (a decrease of 26/12 mm Hg). There were 695 cardiovascular events or deaths from any cause in the ACE-inhibitor group (56.1 per 1000 patient-years) and 736 cardiovascular events or deaths from any cause in the diuretic group (59.8 per 1000 patient-years; the hazard ratio for a cardiovascular event or death with ACE-inhibitor treatment was 0.89 [95 percent confidence interval, 0.79 to 1.00]; P=0.05). Among male subjects, the hazard ratio was 0.83 (95 percent confidence interval, 0.71 to 0.97; P=0.02); among female subjects, the hazard ratio was 1.00 (95 percent confidence interval, 0.83 to 1.21; P=0.98); the P value for the interaction between sex and treatment-group assignment was 0.15. The rates of nonfatal cardiovascular events and myocardial infarctions decreased with ACE-inhibitor treatment, whereas a similar number of strokes occurred in each group (although there were more fatal strokes in the ACE-inhibitor group). Conclusions Initiation of antihypertensive treatment involving ACE inhibitors in older subjects, particularly men, appears to lead to better outcomes than treatment with diuretic agents, despite similar reductions of blood pressure.Lindon M.H.
