    硫酸氢氯吡格雷联合西洛他唑治疗糖尿病四肢并发症的临床疗效观察

    Objective: To observe efficacy and safety of domestic clopidogrel cilostazol in treatment of diabetic limbs complications. Methods: 120 cases of diabetic patients with limb complications were randomly selected, including 60 cases of lower extremity arterial disease (PAD) and 60 patients with diabetic peripheral neuropathy (DPN) patients. According to the random number list, they were randomly divided into treatment group of 60 patients, DPN and PAD were given mecobalamin 0. 5mg / times , lipoic acid 500mg / d, alprostadil 10ug / d, DPN patients while giving epalrestat 50mg / times. The control group while giving cilostazol 0.1g, 2 times / d, plus clopidogrel treatment groups 150mg / d in the control group, based on continuous treatment for 8 weeks. Clinical evaluation of the two groups was made and conduction velocity of MNCV and SNCV was compared. Results: Analysis of DPN subgroup showed that the treatment group was efficient (73.4% vs 46.7%, P = 0.035), total efficiency (93.3% vs 73.3%, P = 0.038) were significantly higher, PAD subgroup analysis showed the efficiency of the treatment group was (70% vs 43.3%, P = 0.035), total efficiency (93.3% vs 73.3%, P = 0.038) were significantly higher. MNCV CKS nerve (54.3 ± 4.8 vs 50.5 ± 5.1) and the peroneal nerve (34.7 ± 3.2 vs 30.9 ± 3.4) conduction velocity improved more significantly. SNCV CKS nerve (43.4 ± 4.9 vs 38.2 ± 3.8) and the peroneal nerve (35.1 ± 5.6 vs 32.9 ± 5.4) conduction velocity, the treatment group improved significantly than the control group. PAD subgroup analysis, ABI degree of improvement (0.9 ± 0.3 vs 0.8 ± 0.3), dorsal blood flow (35.2±3.6 vs31.5±4.2), popliteal artery blood flow (64.2±5.6 vs 58.4±7.4) were better than the control group. Compared with clopidogrel, cilostazol did not increase in adverse events during treatment process. Conclusion: Clopidogrel cilostazol treatment of diabetic limb complications becomes safe and effective treatment of DPN and PAD, which can be used as a combination therapy.目的  观察国产氯吡格雷联合西洛他唑治疗糖尿病四肢并发症的有效性及安全性。方法  随机入选120例糖尿病四肢并发症患者,其中糖尿病周围神经病变(DPN)患者60例,下肢动脉病变(PAD)60例。按照随机数列表,随机分为治疗组60例,DPN及PAD均给予甲钴胺0. 5mg /次,硫辛酸500mg/d,前列地尔10ug/d,DPN患者同时给予依帕司他50mg /次。对照组同时给予西洛他唑0.1g,2次/d,治疗组在对照组基础上加上氯吡格雷首次600mg,后150mg/d连续7d,后改为75mg/d连续8周。评价两组的临床疗效及MNCV和SNCV传导速度。结果  DPN亚组分析显示,治疗组显效率(73.4% vs 46.7%, P=0.035)、总有效率(93.3% vs 73.3%,P=0.038)显著高于对照组,PAD亚组分析显示,治疗组显效率(70% vs 43.3%, P=0.035)、总有效率(93.3% vs 73.3%,P=0.038)显著高于对照组。MNCV中正中神经(54.3±4.8 vs 50.5±5.1)及腓总神经(34.7±3.2 vs 30.9±3.4)传导速度改善更为明显。SNCV中正中神经(43.4±4.9 vs 38.2±3.8)及腓总神经(35.1±5.6 vs 32.9±5.4)传导速度,治疗组改善亦较对照组更明显。PAD亚组分析显示,ABI改善程度(0.9±0.3 vs 0.8±0.3)、足背动脉血流速度(35.2±3.6 vs31.5±4.2),腘动脉血流量(64.2±5.6 vs 58.4±7.4)改善均优于对照组。治疗过程中氯吡格雷相比于西洛他唑并不增加不良事件。结论  氯吡格雷联合西洛他唑治疗糖尿病四肢并发症变安全、有效,可作为治疗DPN及PAD的联合治疗方案

    Expression of Human Leukocyte Antigen G is associated with Prognosis in Nasopharyngeal Carcinoma

    Human leukocyte antigen G (HLA-G) has multiple immune regulatory functions including the induction of immune tolerance in malignancies. The roles of HLA-G have not been investigated in nasopharyngeal carcinoma (NPC). This study is aimed to evaluate the role of HLA-G as prognostic factor for NPC patients as well as its role in the immune regulation. Western assays showed high HLA-G expression in NPC cell lines, but low in the immortalized nasopharyngeal epithelial cell line NP69. HLA-G protein was further detected in 79.2% of 552 NPC specimens with immunohistochemistry (IHC), but not in normal nasopharyngeal epithelium tissue. Moreover, high expression of HLA-G predicted poor survival of NPC patients and positively correlated with tumor N classification and recurrence or metastasis. Multivariate analysis indicated that HLA-G was an independent and unfavorable prognostic factor. Furthermore, the presence of CD68+macrophages and IL-10 were also examined, which are two prognostic markers of NPC and important factors for regulating immune surveillance. The correlations of HLA-G with these two immune factors were revealed in NPC tissues. Taken together, our results suggest that HLA-G is an independent biomarker for NPC prognosis, and HLA-G might contribute to NPC progression, which might jointly regulate immune surveillance in NPC together with macrophages and IL-10

    Association between prospective registration and overall reporting and methodological quality of systematic reviews: a meta-epidemiological study

    Objective: To investigate the differences in main characteristics, reporting and methodological quality between prospectively registered and non-registered systematic reviews. Methods: PubMed was searched to identify systematic reviews of randomized controlled trials published in 2015 in English. After title and abstract screening, potentially relevant reviews were divided into three groups: registered non-Cochrane reviews, Cochrane reviews, and non-registered reviews. For each group, random number tables were generated in Microsoft Excel, and the first 50 eligible studies from each group were randomly selected. Data of interest from systematic reviews were extracted. Regression analyses were conducted to explore the association between total R-AMSTAR or PRISMA scores and the selected characteristics of systematic reviews. Results: The conducting and reporting of literature search in registered reviews were superior to non-registered reviews. Differences in nine of the 11 R-AMSTAR items were statistically significant between registered and non-registered reviews. The total R-AMSTAR score of registered reviews was higher than non-registered reviews (MD=4.82, 95%CI: 3.70, 5.94). Sensitivity analysis by excluding the registration related item presented similar result (MD=4.34, 95%CI: 3.28, 5.40). Total PRISMA scores of registered reviews were significantly higher than non-registered reviews (all reviews: MD=1.47, 95%CI: 0.64-2.30; non-Cochrane reviews: MD=1.49, 95%CI: 0.56-2.42). However, the difference in the total PRISMA score was no longer statistically significant after excluding the item related to registration (item 5). Regression analyses showed similar results. Conclusions: Prospective registration may at least indirectly improve the overall methodological quality of systematic reviews, although its impact on the overall reporting quality was not significant

    Controllable Spin-Resolved Photon Emission Enhanced by Slow-Light Mode in Photonic Crystal Waveguides on Chip

    We report the slow-light enhanced spin-resolved in-plane emission from a single quantum dot (QD) in a photonic crystal waveguide (PCW). The slow light dispersions in PCWs are designed to match the emission wavelengths of single QDs. The resonance between two spin states emitted from a single QD and a slow light mode of a waveguide is investigated under a magnetic field with Faraday configuration. Two spin states of a single QD experience different degrees of enhancement as their emission wavelengths are shifted by combining diamagnetic and Zeeman effects with an optical excitation power control. A circular polarization degree up to 0.81 is achieved by changing the off-resonant excitation power. Strongly polarized photon emission enhanced by a slow light mode shows great potential to attain controllable spin-resolved photon sources for integrated optical quantum networks on chip.Comment: 7 pages,5 figure

    Wedelolactone Attenuates Pulmonary Fibrosis Partly Through Activating AMPK and Regulating Raf-MAPKs Signaling Pathway

    Pulmonary fibrosis is common in a variety of inflammatory lung diseases, there is currently no effective clinical drug treatment. It has been reported that the ethanol extract of Eclipta prostrata L. can improve the lung collagen deposition and fibrosis pathology induced by bleomycin (BLM) in mice. In the present study, we studied whether wedelolactone (WEL), a major coumarin ingredient of E. prostrata, provided protection against BLM-induced pulmonary fibrosis. ICR or C57/BL6 strain mice were treated with BLM to establish lung fibrosis model. WEL (2 or 10 mg/kg) was given daily via intragastric administration for 2 weeks starting at 7-day after intratracheal instillation. WEL at 10 mg/kg significantly reduced BLM-induced inflammatory cells infiltration, pro-inflammatory factors expression, and collagen deposition in lung tissues. Additionally, treatment with WEL also impaired BLM-induced increases in fibrotic marker expression (collagen I and α-SMA) and decrease in an anti-fibrotic marker (E-cadherin). Treatment with WEL significantly prevented BLM-induced increase in TGF-β1 and Smad2/3 phosphorylation in the lungs. WEL administration (10 mg/kg) also significantly promoted AMPK activation compared to model group in BLM-treated mice. Further investigation indicated that activation of AMPK by WEL can suppressed the transdifferentiation of primary lung fibroblasts and the epithelial mesenchymal transition (EMT) of alveolar epithelial cells, the inhibitive effects of WEL was significantly blocked by an AMPK inhibitor (compound C) in vitro. Together, these results suggest that activation of AMPK by WEL followed by reduction in TGFβ1/Raf-MAPK signaling pathways may have a therapeutic potential in pulmonary fibrosis
