    Synchronous testicular liposarcoma and prostate adenocarcinoma: a case report

    Gastrik ve Gastro-özefageal Bileşke Kanserlerinde HER2 Overekspresyonunun Klinik Önemi Amaç: Bu çalışmada gastrik kanser (GC) ve gastroözefageal bileşke kanserlerinde (GEJC) HER2 overekspresyon oranı, HER2 overekspresyonunun klinik, patolojik parametrelerle ilişkisi ve prognoz üzerine etkisi araştırıldı. Gereç ve Yöntem: Gastrik kanser ve GEJC tanılı 285 (202 erkek, 83 kadın) hastanın cerrahi ya da biyopsi preperatlarında HER2 ekspresyon varlığı immunohistokimyasal (IHK) yöntemle ve silver insitu hibridizasyon (SISH) yöntemiyle değerlendirildi. HER2 (+) liğinin tümörün çapı, histopatolojisi, grade i, seroza invazyonu (Sİ), lenfovasküler (LVİ) ve perinöral invazyon (PNİ), Lauren ve Borrmann tipi, tümörün lokalizasyonu, TNM evresi, izlemde lokal nüks (LN) ve metastaz (M) gelişimi ilişkisi, sağkalım (OS) üzerine etkisi araştırıldı. Bulgular: IHK skorları sırasıyla: 194 (%68.1) IHK 0, 34 (%11.9) IHK +1, 30 (%10.5) IHK +2, 27 (%9.5) IHK +3. IHK +2 olan 30 hastanın 12 de SISH (+) (%4.2), 18 inde SISH (-). IHK +3 ve IHK +2 olup SISH (+) hasta sayısı birlikte değerlendirildiğinde HER2 (+) liği %13.7 di. HER2 (+) liğiyle yaş, cinsiyet, TNM evresi, tümör çapı, tümörün lokalizasyonu LN ve M gelişimi, LVİ ve PNİ, Borrman tipi arasında ilişki yoktu. İntestinal tip tümörlerde diffüz tipe göre HER2 (+) liği daha fazlaydı (%16.7 vs %6.9, p=0.075). İyi-orta derecede diferansiye tümörlerde kötü diferansiye tümörlere göre HER2 (+) liği anlamlı oranda yüksekti (p=0.001). Pür adenokarsinomlarda %19.4, taşlı yüzük hücreli karsinomlarda %4.2, mikst adenokarsinom-taşlıyüzük hücreli karsinomlarda %10 HER2 (+) di (p=0.013). HER2 (+) liğinin OS üzerine anlamlı etkisi saptanmadı (p=0.81), ancak erken evre HER2 (+) hastaların median OS leri daha kısaydı (p=0.022). Sonuç: Gastrik kanserler ve GEJC de HER2 (+) liği tümörün histopatolojisi ve diferansiasyon derecesiyle ilişkilidir. Materials and Methods: Surgery or biopsy specimen of 285 (202 male, 83 female) patients with GC or GEJC, the presence of HER2 overexpression by immunohistochemistry and silver insitu hybridization (SISH) were evaluated. The relationship between HER2 pozitivity and tumor size, histopathology, grade, serosal invasion (SI), lenfovaskular invasion (LVI), perineural invasion (PNI), Lauren and Borrmann type, tumor location, TNM stage, the development of local recurrence and metastatisis during follow up and survival (OS) were investigated. Results: IHC scores were; 194 (68.1%) IHC 0, 34 (11.9%) IHC +1, 30 (10.5%) IHC +2, 27 (9.5%) IHC +3. 12 of 30 (4.2%) patients with IHC +2, SISH positive, and 18 patients SISH negative. The number of patients evaluated with IHC +3 or IHC +2 and SISH (+), HER2 positivity was 13.7%. There was no relationship between HER (+) and the age, gender, TNM stage, tumor size and localization, development of LN and M, LVI, PNI, Borrmann type. HER2 (+) was higher in intestinal type tumors than in diffuz type (16.7% vs 6.9%, p=0.075). HER (+) was significantly higher in well-moderate diferantiated tumors than poorly diferantiated tumors (p=0.001). 19.4% of adenocarcinomas, 4.2% of signet-ring cell carcinomas, 10% of mixed adenocarcinoma-signet ring cell carcinomas were HER (+) (p=0.013). HER2 pozitivity was no significant effect on OS (p=0.81). But the early-stage HER2 (+) patients, median OS was shorter than advanced stage (p=0.022). Conclusion: HER2 positivity is associated with the degree of tumor differantiation and histopathology in GC and GEJC. Patients with early-stage, HER2 positivity is related to poor prognosis

    Paraneoplastic cerebellar ataxia (PCA) is most frequently observed in gynaecological cancers, small cell lung cancer, breast cancer, Hodgkin's lymphoma, cancer testis or malignant thymoma. In the literature, there is no data related to the effects of PCA during pregnancy or reports on the effects of anaesthesia in patients with PCA. We present management of a pregnant woman with PCA who was suddenly unable to walk with PCA and for whom effective spinal anaesthesia was performed for an elective caesarean section with no complications

    Bilateral Choroidal Metastases from Lung Adenocarcinoma: A Case Report

    The most common malignancy of the eye is metastatic tumors, with choroidal metastases being the majority of them. In women, breast cancer is the most common cause of orbital metastases, and in men, it is lung cancer. Despite the fact that there are efficient treatment options for orbital metastases, the benefit of procedures to detect choroidal metastases is debatable due to the quick progression and poor prognosis of lung cancer. In choroidal metastases resulting from lung cancer, patients are usually presented with decreased sight. Defects in the field of vision, flashes of light and floating bodies generally follow. Treatment options of choroidal metastases include many methods including laser photocoagulation, cryotherapy, chemotherapy, radiotherapy, surgical resection, enucleation and photodynamic therapy. There are reports emphasizing radiotherapy as the most efficient treatment option. In this case report, we sum up the case of a male patient presenting with blurry vision in both eyes, who was subsequently detected to have bilateral choroidal metastatic tumor and was diagnosed with primary lung adenocarcinoma.
