1.1 Myosin binding protein C mutation in hypertrophic cardiomyopathy
provokes mitochondrial H2O2 by a mismatch between ADP-induced
oxidation and Ca2+-induced regeneration of NADPH
Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiac disease and is
frequently caused by mutations in sarcomeric proteins, in particular of the cardiac myosinbinding
protein C (Mybpc3). Many mutations have in common that they increase
myofilament Ca2+ sensitivity, typically inducing diastolic dysfunction. While Ca2+
sensitization of the myofilaments explains diastolic dysfunction, it is incompletely resolved
how these mutations induce cardiac hypertrophy and arrhythmias. Reactive oxygen species
(ROS) are involved in the pathophysiology of hypertrophy and arrhythmias, and an important
ROS source are mitochondria. Elimination of H2O2 (but not .O2-) in mitochondria is governed
by enzymes that require NADPH, which is in equilibrium with NADH and regenerated by the
Krebs cycle. Here, we show that HCM mutations that increase myofilament Ca2+ sensitivity
generate bioenergetic mismatch and oxidative stress during β-adrenergic stimulation.
An elevation of work per se induces oxidation of NAD(P)H, since ADP accelerates the
donation of electrons to the ETC by NADH. Physiological elevations of cardiac work are
mediated by b-adrenergic stimulation, which increases cytosolic Ca2+ ([Ca2+]c) transients and
mitochondrial Ca2+ ([Ca2+]m) accumulation. Elevated [Ca2+]m stimulates the Krebs cycle,
accelerating NAD(P)H regeneration to equilibrate its redox state. Since myofilament Ca2+
hypersensitivity, as seen in HCM, induces contraction (and thus, work) at lower [Ca2+]c, we
hypothesized that this creates an imbalance between ADP-induced oxidation and Ca2+-
dependent regeneration of NAD(P)H, favoring mitochondrial H2O2 emission. The ensuing
overflow of ROS from mitochondria then causes arrhythmias. For any given sarcomere
shortening that produces work and consumes ATP, less Ca2+ stimulates the Krebs cycle to
maintain mitochondrial NADH. This mismatch reverses the mitochondrial transhydrogenase
(NNT) to regenerate NADH from NADPH, supporting ATP production at the cost of
NADPH-dependent antioxidative capacity.Cardiac myocytes were isolated from a Mybpc3-targeted Knock-In mouse model (KI)
carrying a point mutation frequently associated with HCM and increased myofilament Ca2+
sensitivity, and wild-type littermates (WT). Myocytes were exposed to a protocol that
simulates a physiological increase in workload by applying the b-adrenergic agonist
isoprenaline (10 nM) and increasing the (electrical field-) stimulation frequency from 0.5 to 5
Hz for 1 min (Iso/5Hz). During this protocol, the redox states of NAD(P)H/NAD(P)+ and
FADH2/FAD were determined within the same cells by their autofluorescences, respectively
(WT, n=25; KI, n=20). Furthermore, fluorescence of the superoxide (.O2-) indicator MitoSOX
(n=24/25) or the H2O2 indicator DCF (n=43/25) were monitored, and sarcomere
length/shortening was recorded. Finally, [Ca2+]c and [Ca2+]m were monitored simultaneously
by a patch-clamp based protocol (performed by Dr. M. Kohlhaas) with similar conditions
(n=14/9).
As predicted by increased myofilament Ca2+ sensitivity, diastolic sarcomere length was
shorter in KI compared to WT myocytes, and after iso/5Hz, systolic sarcomere shortening
increased more in KI than in WT (p<0.0001, respectively). In contrast, no differences in
diastolic and systolic [Ca2+]c and [Ca2+]m nor in the efficacy of mitochondrial Ca2+ uptake
were observed, as indicated by the ratio of [Ca2+]m/[Ca2+]c. Redox states of
NAD(P)H/NAD(P)+ and FADH2/FAD were slightly more oxidized at baseline in KI vs. WT
myocytes, which was aggravated after Iso/5Hz (p<0.05). While mitochondrial .O2- formation
was similar, H2O2 emission increased in KI compared to WT myocytes during redox state
oxidations in response to Iso/5Hz (p<0.05). In contrast to WT myocytes, KI myocytes did not
survive an extended 3-min protocol of Iso/5Hz.
In conclusion, we discovered a novel mechanism by which increased Ca2+ sensitivity of
myofilaments in HCM induces mitochondrial ROS emission during b-adrenergic stimulation
by a mismatch between ADP-induced oxidation and Ca2+-induced regeneration of the
mitochondrial redox state. This mechanism may play an important pathophysiological role for

Die hypertrophe Kardiomyopathie (HCM) ist die häufigste hereditäre Herzerkrankung und
wird meistens durch Mutationen in den fĂĽr die Sarkomerproteine kodierenden Genen
verursacht, insbesondere des Myosin-bindenden Proteins C (Mybpc3). Während vermehrte
Ca2+-Sensitivität der Myofilamente die diastolische Dysfunktion erklärt, ist es allerdings
unvollständig geklärt, wie diese Mutationen kardiale Hypertrophie und Arrhythmien
induzieren. Reaktive Sauerstoffspezies (ROS) sind in die Pathophysiologie der Hypertrophie
und Arrhythmien involviert, und Mitochondrien sind die Hauptquelle von ROS. Die
Eliminierung von H2O2 (aber nicht ·O2-) in Mitochondrien wird durch Enzyme reguliert, die
NADPH benötigen, welches im Gleichgewicht mit NADH steht und durch den Citratzyklus
regeneriert wird. Hier zeigen wir, dass HCM Mutationen, die die myofilamentäre Ca2+-
Sensitivität erhöhen, bioenergetisches Ungleichgewicht und oxidativen Stress während einer
β-adrenergen Stimulation erzeugen.
Eine Erhöhung der Arbeitslast per se induziert die Oxidation von NAD(P)H, da ADP die
Abgabe von Elektronen an die Atmungskette fördert. Eine physiologische Erhöhung der
kardialen Arbeit wird durch β-adrenerge Stimulation vermittelt, die die zytosolischen Ca2+
([Ca2+]c) Transienten und mitochondriale Ca2+ ([Ca2+]m) Akkumulation erhöht. Gesteigertes
[Ca2+]m stimuliert den Citratzyklus und beschleunigt die NAD(P)H Regeneration, um den
Redox-Status auszugleichen. Die bei HCM erhöhte myofilamentäre Ca2+-Sensitivität führt zu
einer verstärkten Kontraktion und damit Arbeit bei niedrigeren oder gleichen [Ca2+]c. Folglich
spekulierten wir, dass dieser Umstand ein Ungleichgewicht zwischen ADP-induzierter
Oxidation und Ca2+-abhängiger Regeneration von NAD(P)H enstehen lässt, welches die
mitochondriale H2O2 Emission begĂĽnstigt. Die darauffolgende mitochondriale
Überproduktion von ROS könnte dann kardiale Arrhythmien verursachen. In Folge jeder
einzelner SarkomerverkĂĽrzung, die Arbeit produziert und ATP verbraucht, stimuliert weniger
Ca2+ den Citratzyklus, um NADH zu regenerieren. Dieses Ungleichgewicht dreht die
Reaktionsrichtung der mitochondrialen Transhydrogenase (NNT) um, mit dem Ziel, das
benötigte NADH aus NADPH zu regenerieren - die ATP-Produktion wird somit auf Kosten
der antioxidativen Kapazität gefördert. Kardiale Myozyten wurden aus einem Mausmodel mit genetischem Knock-In (KI) einer beim
Menschen vorkommenden Mutation des Mybpc3 Gens isoliert, die eine gesteigerte Ca2+-
Sensitivität der Myofilamente verursacht. Als Kontrolle dienten gleichaltrige Wildtyp-
Geschwistertiere. Die Myozyten wurden einem Protokoll ausgesetzt, das einen
physiologischen Arbeitslastanstieg durch Anwendung des β-adrenergen Agonisten
Isoprenalin (10 nM) und Zunahme der elektrischen Feldstimulationsfrequenz von 0,5 auf 5 Hz
für eine Minute simulierte (Iso/5Hz). Während dieses Protokolls wurde der mitochondriale
Redoxstatus von NAD(P)H/NAD(P)+ und FADH2/FAD anhand der entsprechenden
Autofluoreszenzen aufgezeichnet (WT, n=25; KI, n=20). DarĂĽber hinaus wurde die
Fluoreszenz des Superoxid (.O2-)-Indikators MitoSOX (n=24/25) und des H2O2-Indikators
DCF (n=43/25) beobachtet und darüber hinaus die Länge und die Verkürzung der Sarkomere
aufgezeichnet. Am Ende wurden [Ca2+]c and [Ca2+]m gleichzeitig anhand eines Patch-Clampbasierten
Protokolls unter ähnlichen Voraussetzungen aufgezeichnet (durch Dr. M. Kohlhaas;
n=14/9).
Bedingt durch die erhöhte Ca2+-Sensitivität der Myofilamente war die diastolische
Sarkomerlänge kürzer bei den KI im Vergleich zu den Wildtyp Myozten. Nach Iso/5Hz nahm
die Sarkomerverkürzung der KI stärker zu als die der WT (p<0.0001). Im Gegensatz hierzu
wurden weder Unterschiede zwischen diastolischem und systolischem [Ca2+]c und [Ca2+]m,
noch in der Wirksamkeit der mitochondrialen Ca2+ Aufnahme festgestellt, abgebildet in dem
Verhältnis von [Ca2+]c und [Ca2+]m. Der mitochondriale Redoxstatus von NAD(P)H/NAD(P)+
und FADH2/FAD war zu Beginn leicht oxidierter bei den KI verglichen mit den WT
Myozyten, was sich nach Iso/5Hz weiter verstärkte (p<0.05). Im Gegensatz zu den WT
Myozyten überlebten die KI Myozten kein verlängertes 3-Minuten Protokoll von Iso/5Hz.
Zusammenfassend haben wir einen neuen Mechanismus entdeckt, bei dem durch die
gesteigerte Ca2+-Sensibilität der Myofilamente bei HCM mitochondriale ROS-Produktion
während β-adrenerger Stimulation induziert wird. Dies geschieht durch ein Ungleichgewicht
zwischen ADP-induzierter Oxidation und Ca2+-induzierter Regeneration des mitochondrialen
Redoxstatus. Dieser Mechanismus könnte eine wichtige pathophysiologische Rolle in der
Entstehung von Hypertrophie und Arrhythmien bei der HCM spielen

Expression of TRAIL, IP-10, and CRP in children with suspected COVID-19 and real-life impact of a computational signature on clinical decision-making: a prospective cohort study
Purpose We evaluated the host-response marker score “BV” and its components TRAIL, IP-10, and CRP in SARS-CoV-2
positive children, and estimated the potential impact on clinical decision-making.
Methods We prospectively analyzed levels of TRAIL, IP-10, CRP, and the BV score, in children with suspected COVID19. Classifcation of infectious etiology was performed by an expert panel. We used a 5-point-questionnaire to evaluate the
intention to treat with antibiotics before and after receiving test results.
Results We screened 111 children, of whom 6 (5.4%) were positive for SARS-CoV-2. A total of 53 children were included
for the exploratory analysis. Median age was 3.1 years (interquartile range [IQR] 1.3–4.3), and 54.7% (n=29) were girls.
A viral and a bacterial biomarker pattern was found in 27/53 (50.9%) and 15/53 (28.3%), respectively. BV scores difered
between COVID-19, children with other viral infections, and children with bacterial infections (medians 29.5 vs. 9 vs. 66;
p=0.0006). Similarly, median TRAIL levels were diferent (65.5 vs. 110 vs. 78; p=0.037). We found no diferences in IP-10
levels (555 vs. 504 vs. 285; p=0.22). We found a concordance between physicians’ “unlikely intention to treat” children with
a viral test result in most cases (n=19/24, 79.2%). When physicians expressed a “likely intention to treat” (n=15), BV test
revealed 5 bacterial, viral, and equivocal scores each. Antibiotics were withheld in three cases (20%). Overall, 27/42 (64%)
of pediatricians appraised the BV test positively, and considered it helpful in clinical practice.
Conclusion Host-response based categorization of infectious diseases might help to overcome diagnostic uncertainty, support clinical decision-making and reduce unnecessary antibiotic treatment
Nabelvenenkatheter- und periphere zentrale katheterassoziierte Komplikationen bei FrĂĽhgeborenen mit einem Geburtsgewicht < 1250 g : Ergebnisse einer Umfrage in Ă–sterreich und Deutschland
Background and objective
Umbilical venous catheters (UVC) and peripherally inserted central catheters (PICC) are commonly used in preterm infants but have been associated with a number of serious complications. We performed a survey in Austria and Germany to assess the use of UVCs and PICCs in preterm infants with a birth weight < 1250 g and associated rates of catheter-related adverse events.
Methods
Electronic survey of participating centers of the NeoVitaA trial. Main outcome parameter was the reported rates of UVC- and PICC-associated complications (infection, thrombosis, emboli, organ injury, arrhythmia, dislocation, miscellaneous).
Results
In total, 20 neonatal intensive care units (NICU) providing maximal intensive care in Austria and Germany (level I) were contacted, with a senior neonatologist response rate of 12/20 (60%). The reported rates for UVC with a dwell time of 1–10 days were bacterial infection: 4.2 ± 3.4% (range 0–10%); thrombosis: 7.3 ± 7.1% (0–20%); emboli: 0.9 ± 2.0% (0–5%); organ injury: 1.1 ± 1.9% (0–5%); cardiac arrhythmia: 2.2 ± 2.5% (0–5%); and dislocation: 5.4 ± 8.7% (0–30%); and for PICCs with a dwell time of 1–14 days bacterial infection: 15.0 ± 3.4% (range 2.5–30%); thrombosis; 4.3 ± 3.5% (0–10%); emboli: 0.8 ± 1.6% (0–5%); organ injury: 1.5 ± 2.3% (0–5%); cardiac arrhythmia: 1.5 ± 2.3% (0–5%), and dislocation: 8.5 ± 4.6% (0–30%).
Conclusion
The catheter-related complication rates reported in this survey differed between UVCs and PICCs and were higher than those reported in the literature. To generate more reliable data on this clinically important issue, we plan to perform a large prospective multicenter randomized controlled trial investigating the non-inferiority of a prolonged UVC dwell time (up to 10 days) against the early change (up to 5 days) to a PICC.Hintergrund und Ziel
Nabelvenenkatheter („umbilical venous catheters“ [UVC]) und periphere zentrale Venenkatheter (PICC) werden häufig bei Frühgeborenen eingesetzt, sind jedoch mit einer Reihe von schwerwiegenden Komplikationen verbunden. In Österreich und Deutschland wurde eine Umfrage durchgeführt, um die Verwendung von UVC und PICC bei Frühgeborenen mit einem Geburtsgewicht < 1250 g und die damit verbundenen Raten von katheterbedingten unerwünschten Ereignissen zu bewerten.
Methoden
Elektronische Befragung der teilnehmenden Zentren der NeoVitaA-Studie. Hauptergebnisparameter waren die gemeldeten Raten von UVC- und PICC-assoziierten Komplikationen (Infektion, Thrombose, Embolie, Organverletzung, Arrhythmie, Dislokation, Sonstiges).
Ergebnisse
Insgesamt wurden 20 neonatale Intensivstationen (NICU) mit maximaler Intensivpflege in Österreich und Deutschland (Level I) kontaktiert, wobei 12/20 (60 %) von leitenden Neonatologen beantwortet wurden. Die gemeldeten Raten für UVC mit einer Verweildauer von 1 bis 10 Tagen waren bakterielle Infektionen: 4,2 ± 3,4 % (Bereich: 0–10 %); Thrombose: 7,3 ± 7,1 % (0–20 %); Embolie: 0,9 ± 2,0 % (0–5 %); Organverletzung: 1,1 ± 1,9 % (0–5 %); Herzrhythmusstörungen: 2,2 ± 2,5 % (0–5 %); und Dislokation: 5,4 ± 8,7% (0–30 %); und bei PICC mit einer Verweildauer von 1 bis 14 Tagen bakterielle Infektionen: 15,0 ± 3,4 % (Bereich: 2,5–30 %); Thrombose: 4,3 ± 3,5 % (0–10 %); Embolie: 0,8 ± 1,6 % (0–5 %); Organverletzung: 1,5 ± 2,3 % (0–5 %); Herzrhythmusstörungen: 1,5 ± 2,3 % (0–5 %) und Verrenkungen: 8,5 ± 4,6 % (0–30 %).
Schlussfolgerung
Die in dieser Umfrage berichteten katheterbedingten Komplikationsraten unterschieden sich zwischen UVC und PICC und waren höher als die in der Literatur berichteten. Um zuverlässigere Daten zu diesem klinisch wichtigen Thema zu erhalten, ist eine große prospektive, multizentrische, randomisierte, kontrollierte Studie geplant, in der die Nichtunterlegenheit einer verlängerten UVC-Verweildauer (bis zu 10 Tage) gegenüber dem frühen Wechsel (bis zu 5 Tage) zu einem PICC untersucht werden soll
Effectiveness of a multidisciplinary care program on recovery and return to work of patients after gynaecological surgery; design of a randomized controlled trial
<p>Abstract</p> <p>Background</p> <p>Return to work after gynaecological surgery takes much longer than expected, irrespective of the level of invasiveness. In order to empower patients in recovery and return to work, a multidisciplinary care program consisting of an e-health intervention and integrated care management including participatory workplace intervention was developed.</p> <p>Methods/Design</p> <p>We designed a randomized controlled trial to assess the effect of the multidisciplinary care program on full sustainable return to work in patients after gynaecological surgery, compared to usual clinical care. Two hundred twelve women (18-65 years old) undergoing hysterectomy and/or laparoscopic adnexal surgery on benign indication in one of the 7 participating (university) hospitals in the Netherlands are expected to take part in this study at baseline. The primary outcome measure is sick leave duration until full sustainable return to work and is measured by a monthly calendar of sickness absence during 26 weeks after surgery. Secondary outcome measures are the effect of the care program on general recovery, quality of life, pain intensity and complications, and are assessed using questionnaires at baseline, 2, 6, 12 and 26 weeks after surgery.</p> <p>Discussion</p> <p>The discrepancy between expected physical recovery and actual return to work after gynaecological surgery contributes to the relevance of this study. There is strong evidence that long periods of sick leave can result in work disability, poorer general health and increased risk of mental health problems. We expect that this multidisciplinary care program will improve peri-operative care, contribute to a faster return to work of patients after gynaecological surgery and, as a consequence, will reduce societal costs considerably.</p> <p>Trial registration</p> <p>Netherlands Trial Register (NTR): <a href="http://www.trialregister.nl/trialreg/admin/rctview.asp?TC=2087">NTR2087</a></p
