    Should We Be Afraid Of Neuroendocrine Appendix Tumors?

    Introduction: Appendix neoplasms are rarely seen and usually detected by chance after appendectomy. Carcinoid tumors, also called neuroendocrine tumors, make up more than 50% of appendix tumors. Since the diagnosis is usually made as a result of examination of postoperative pathology specimens, patients should be followed up well after appendectomy. Material and Method: 820 appendectomies performed between 2016 and 2020 in Eskisehir Osmangazi University General Surgery Clinic were examined and the patients whose pathology results were appearing as appendix neoplasm were retrospectively analyzed. Results: Neoplastic lesions were detected in 22 of 820 patients. The most common tumoral lesions in the appendix are neuroendocrine tumors and were seen in 8 patients (0.9%) in our series. The average age of patients is 44.75. It was determined that 6 of the patients were male and 2 were female. The average tumor diameter was found to be 4.9 mm. Discussion: Among the appendix malignant tumors, neuroendocrine tumors are the most common. Apart from this, although rare, goblet cell carcinoma, lymphoma, mucocele, primary adenocarcinoma and mucinous cystadenocarcinoma are also seen. In our study, the incidence of neuroendocrine tumors in patients with appendectomy was 0.9%. In the literature, the rates on this issue range from 0.3% to 0.9%. In our study, the average age of patients with neuroendocrine tumors was determined to be 44.75 and is consistent with the literature. Appendix neuroendocrine tumors rarely metastasize. Carcinoid syndrome due to appendix carcinoid tumor is rare and is often associated with liver metastasis. Although serosal and intramural lymphatic invasion, tumors smaller than 2 cm in diameter can be treated with appendectomy and mesoapendix resection. Conclusion: Appendix neoplasms are generally diagnosed after examination of pathology specimens after appendectomy. Although their prognosis is good, they should be followed closely since the presence of synchronous and metachronous tumor is high.Keywords: appendix, tumor, incidentalDOI: 10.7176/JHMN/78 0

    Primary Multiple Giant Hydatid Cyst of the Liver: A Case Report

    Hydatid cyst is a parasitic infection of liver. One of frequent reason of liver mass is hydatid cyst intropical and rural countries. In some cases rapidly grow up of cyst mimic liver masses. A sixty yearsold female admitted our hospital with abdominal pain. Abdominal ultrasound showed 74x75 mm insegment two, 115x91 mm in segment six and 115x62 mm in segment four type four hydatid cysts.Patient was admitted for surgery; pericystectomy and omentoplasty performed. In multiple cystssurgical management is sometimes difficult. It is better to drain all the cysts at the same time. Surgicalinterventions can prevent complications and spread of Echinococcosis also gives chance of curativetreatment.</p

    Evaluation of mean platelet volume as a predictor of gastric disorders

    Aim: Gastric disorders present in a wide range, including malignant and benign diseases of the upper gastrointestinal system. Increased MPV levels are associated with inflammation. The goal of this study was to determine the diagnostic importance of MPV in gastric disorders and evaluate level of MPV in patients that underwent upper gastrointestinal endoscopy. Materials and Methods: Patients who had undergone endoscopy and shown to have a gastric disorder with blood count performed were included in the study. Only one surgeon performed all of the endoscopies. MPV value, platelet count, hemoglobin and white blood cell count levels were analyzed. Results: 116 patients were included in the study. Mean age of all patients was 47,8+/-16,4. Mean value of MPV was determined to be 7,79+/-1,21, within the range of 5,85 to 12,5 fL. There was no significant correlation between diagnoses and MPV levels (P>0,05). Additionally, there was no significant difference in MPV levels in histopathological diagnoses groups (p>0,05). There was a highly negative correlation between platelet count and MPV levels in a Scatter Plot correlation graph (r=0,083). Conclusions: MPV is a frequently used hematological parameter that indicates platelet function and activity affected by inflammation. It was hypothesized that changes in MPV levels could be associated with gastric disorders. Statistical analysis of the data revealed there was no association between MPV and gastric disorders. It is suggested that MPV is not a suitable marker to determine gastric disorders, however further larger studies can be useful to determine the importance of MPV such a context.
