Radiation therapy combined with intracerebral administration of carboplatin for the treatment of brain tumors
Background: In this study we determined if treatment combining radiation therapy (RT) with intracerebral (i.c.) administration of carboplatin to F98 glioma bearing rats could improve survival over that previously reported by us with a 15 Gy dose (5 Gy × 3) of 6 MV photons.Methods: First, in order to reduce tumor interstitial pressure, a biodistribution study was carried out to determine if pretreatment with dexamethasone alone or in combination with mannitol and furosemide (DMF) would increase carboplatin uptake following convection enhanced delivery (CED). Next, therapy studies were carried out in rats that had received carboplatin either by CED over 30 min (20 μg) or by Alzet pumps over 7 d (84 μg), followed by RT using a LINAC to deliver either 20 Gy (5 Gy × 4) or 15 Gy (7.5 Gy × 2) dose at 6 or 24 hrs after drug administration. Finally, a study was carried out to determine if efficacy could be improved by decreasing the time interval between drug administration and RT.Results: Tumor carboplatin values for D and DMF-treated rats were 9.4 ±4.4 and 12.4 ±3.2 μg/g, respectively, which were not significantly different (P = 0.14). The best survival data were obtained by combining pump delivery with 5 Gy × 4 of X-irradiation with a mean survival time (MST) of 107.7 d and a 43% cure rate vs. 83.6 d with CED vs. 30-35 d for RT alone and 24.6 d for untreated controls. Treatment-related mortality was observed when RT was initiated 6 h after CED of carboplatin and RT was started 7 d after tumor implantation. Dividing carboplatin into two 10 μg doses and RT into two 7.5 Gy fractions, administered 24 hrs later, yielded survival data (MST 82.1 d with a 25% cure rate) equivalent to that previously reported with 5 Gy × 3 and 20 μg of carboplatin.Conclusions: Although the best survival data were obtained by pump delivery, CED was highly effective in combination with 20 Gy, or as previously reported, 15 Gy, and the latter would be preferable since it would produce less late tissue effects.peer-reviewe

A comparison of mechanisms generating vertical transport in the Brazilian coastal upwelling regions
Several mechanisms can drive vertical velocities in the coastal ocean, including wind-forcing and through gradients in the vorticity field generated by flow-topography interactions. A two-layer, steady, wind-driven, analytical model is applied to the major upwelling systems of Brazil : Cabo Frio (CF) and Cabo de Santa Marta (CSM) regions. Comparisons are made between the relative roles of wind and flow-topography interaction in inducing upwelling over these regions. Ekman pumping is the weakest mechanism over the shelf, but does influence the along-shelf temperature in the CF area. Away from coastline irregularities, wind-driven upwelling (Ekman transport) dominates over all mechanisms. However, in the vicinity of capes and coastal features, topographically driven upwelling plays a significant role, and its transports may vary from 43% to 94% of wind-driven upwelling. Upstream of capes, topographically driven vertical motions are downwelling favorable and act against the wind-driven coastal upwelling, while downstream of capes, they are upwelling favorable, where all mechanisms add up to create strong upwelling. Peaks in total upwelling in the CF region are about twice as large as those in the CSM area because the CF region has stronger winds and larger coastline perturbations than in the CSM region. Observed sea surface temperatures (SST) agree well with variability in the vertical transports where upwelling peaks are in phase with low temperature peaks along the coast. Results suggest that on larger scales, the SST variability along the coast is mainly controlled by wind-driven upwelling, while upwelling due to flow-topography interaction is responsible for the smaller scale SST variability.Keywords: Flow-topography interaction, Brazil, Cabo Frio, Upwelling, Cabo de Santa Mart
Cryptosporidium, Enterocytozoon, and Cyclospora Infections in Pediatric and Adult Patients with Diarrhea in Tanzania.
Cryptosporidiosis, microsporidiosis, and cyclosporiasis were studied in four groups of Tanzanian inpatients: adults with AIDS-associated diarrhea, children with chronic diarrhea (of whom 23 of 59 were positive [+] for human immunodeficiency virus [HIV]), children with acute diarrhea (of whom 15 of 55 were HIV+), and HIV control children without diarrhea. Cryptosporidium was identified in specimens from 6/86 adults, 5/59 children with chronic diarrhea (3/5, HIV+), 7/55 children with acute diarrhea (0/7, HIV+), and 0/20 control children. Among children with acute diarrhea, 7/7 with cryptosporidiosis were malnourished, compared with 10/48 without cryptosporidiosis (P < .01). Enterocytozoon was identified in specimens from 3/86 adults, 2/59 children with chronic diarrhea (1 HIV+), 0/55 children with acute diarrhea, and 4/20 control children. All four controls were underweight (P < .01). Cyclospora was identified in specimens from one adult and one child with acute diarrhea (HIV-). Thus, Cryptosporidium was the most frequent and Cyclospora the least frequent pathogen identified. Cryptosporidium and Enterocytozoon were associated with malnutrition. Asymptomatic fecal shedding of Enterocytozoon in otherwise healthy, HIV children has not been described previously
