Adaptive roles of SSY1 and SIR3 during cycles of growth and starvation in Saccharomyces cerevisiae populations enriched for quiescent or nonquiescent cells
Over its evolutionary history, Saccharomyces cerevisiae has evolved to be well-adapted to fluctuating nutrient availability. In the presence of sufficient nutrients, yeast cells continue to proliferate, but upon starvation haploid yeast cells enter stationary phase and differentiate into nonquiescent (NQ) and quiescent (Q) cells. Q cells survive stress better than NQ cells and show greater viability when nutrient-rich conditions are restored. To investigate the genes that may be involved in the differentiation of Q and NQ cells, we serially propagated yeast populations that were enriched for either only Q or only NQ cell types over many repeated growth–starvation cycles. After 30 cycles (equivalent to 300 generations), each enriched population produced a higher proportion of the enriched cell type compared to the starting population, suggestive of adaptive change. We also observed differences in each population’s fitness suggesting possible tradeoffs: clones from NQ lines were better adapted to logarithmic growth, while clones from Q lines were better adapted to starvation. Whole-genome sequencing of clones from Q- and NQ-enriched lines revealed mutations in genes involved in the stress response and survival in limiting nutrients (ECM21, RSP5, MSN1, SIR4, and IRA2) in both Q and NQ lines, but also differences between the two lines: NQ line clones had recurrent independent mutations affecting the Ssy1p-Ptr3p-Ssy5p (SPS) amino acid sensing pathway, while Q line clones had recurrent, independent mutations in SIR3 and FAS1. Our results suggest that both sets of enriched-cell type lines responded to common, as well as distinct, selective pressures
Transient atrioventricular block following catheter radiofrequency sinus node modification
Complete atrioventricular block, second-degree Mobitz type II and first-degree atrioventricular block with right bundle branch block were observed consecutively following successful radiofrequency ablation in close proximity to the sinus node. This resulted in the modification and disappearance of the inappropriate sinus tachycardia that had previously been present. Neither tachycardia nor conduction disturbances have been recorded in the 9-year follow-up, implying that ventricular pacing standby should also be considered in high atrial ablation cases

Andropauza i proces starzenia się a jakość życia mężczyzn po 50. roku życia
INTRODUCTION. Andropause and physiological aging of men are the natural stages of their lives. Nevertheless, these issues are often overlooked by men themselves as well as by medical personnel. The primary purpose of self-study is to determine the level of knowledge about the impact of andropause in the aging process of people over 50 years of age. The research is aimed at demonstrating whether the symptoms associated with andropause reduce the comfort of men’s life after the age of 50.
MATERIAL AND METHODS. The study was conducted on a group of 50 men aged 50 years. The author used a questionnaire that used the knowledge of the respondents on andropause, their symptoms and their effects on the well-being of men.
RESULTS. Men in the study showed varying degrees of knowledge about aging and andropause. At different frequencies, they also experienced symptoms characteristic for an dropause.
CONCLUSIONS. The analysis of the results obtained from the study of andropause and the physiological aging of the organism revealed that the level of knowledge of men in this area was low. Analyzes have also shown that men after 50 years of age experience different ailments characteristic for andropause, and the ailments themselves lessen the comfort of their lives.WSTĘP: Andropauza oraz fizjologiczne starzenie się mężczyzn są naturalnymi etapami ich życia. Niemniej jednak wspomniane zagadnienia są bardzo często pomijane, tak przez samych mężczyzn jak również przez personel medyczny.
CEL: Zasadniczym celem badań własnych jest określenie poziomu wiedzy na temat wpływu andropauzy w procesie starzenia się osób po 50 roku życia. Badania mają na celu wykazanie czy objawy towarzyszące andropauzie zmniejszają komfort życia mężczyzn po 50 roku życia.
MATERIAŁ I METODA: Badania przeprowadzone zostały na grupie 50 mężczyzn po 50 roku życia. Do ich przeprowadzenia wykorzystano autorski kwestionariusz ankiety, który odnosił się do wiedzy respondentów na temat andropauzy oraz występowania u nich objawów charakterystycznych dla tego stanu i ich wpływu na samopoczucie mężczyzn.
WYNIKI: Mężczyźni objęci badaniem wykazywali się różnym stopniem wiedzy na temat procesu starzenia się oraz andropauzy. Z różną częstotliwością odczuwali oni również objawy charakterystyczne dla andropauzy.
WNIOSKI: Analiza wyników uzyskanych po przeprowadzeniu badań odnoszących się do zagadnienia andropauzy i fizjologicznego starzenia się organizmu pozwoliły na stwierdzenie, że poziom wiedzy mężczyzn w tym zakresie jest niski. Analizy wykazały również, że mężczyźni po 50 roku życia z różną częstotliwością odczuwają dolegliwości charakterystyczne dla andropauzy, a same dolegliwości zmniejszają komfort ich życia
Leukotriene biosynthesis in coronary artery disease : results of the Leukotrienes and Thromboxane In Myocardial Infarction (LTIMI) study
Leukotrienes (LTs) may be involved in atherosclerosis and may contribute to cardiovascular outcomes in CAD. We aimed to compare the baseline LT production in patients with stable CAD (sCAD) and myocardial infarction (MI), and to assess whether an increased LT production is associated with major adverse cardiovascular events (MACEs) at 1 year after MI. LTIMI (Leukotrienes and Thromboxane In Myocardial Infarction) was a single‑center, prospective, observational study of patients with stable sCAD and MI. Urinary leukotriene () levels were measured on admission, at 1 month, and at 1 year, using high‑performance liquid chromatography tandem mass spectrometry. Of the 404 patients screened, 289 were enrolled (110 with sCAD and 179 with MI;
mean [SD] age, 63.9 [ years). Patients with MI had higher median
(interquartile range [IQR]) levels of log‑transformed ()
than those with sCAD (4.74 pg/mg creatinine [ vs 4.51 pg/mg creatinine
[3.99=4.86], respectively; P <0.001). Median (IQR) levels in patients
with MI significantly decreased at 1 month to 4.37 pg/mg creatinine (3.81-4.95),
and at 1 year to 4.16 pg/mg creatinine (3.55-4.85). The baseline urinary
levels were similar in patients with MACEs and those without MACEs
(median [IQR], 4.78 pg/mg creatinine [4.01-5.56]) and 4.68 pg/mg creatinine
[3.97-5.28], respectively; P >0.05). Multiple regression showed no relation
between levels and the incidence of MACEs. LT production assessed by urinary excretion is higher in patients with MI
than in those with sCAD; however, levels at baseline do not differ between
patients with and without MACEs at 1 year after MI
Submassive pulmonary embolism as a mask of acute coronary syndrome
The paper presents a case of a 77-year-old man, who was admitted to hospital suffering from
chest pain. The ECG showed horizontal ST segment depression in the V4-V6 leads. Non-ST
segment elevation acute coronary syndrome was diagnosed. However, transthoracic
echocardiography revealed signs of pulmonary embolism. The present case indicates the need to
consider pulmonary embolism in the differential diagnosis of acute coronary syndromes and
underlines the role of echocardiography, which should be performed at the earliest possible stage.

Predictors of sinus rhythm return during defibrillation testing in patients with permanent atrial fibrillation undergoing implantation of a cardioverter-defibrillator
Background: Atrial fibrillation (AF) is present in a significant proportion of patients treated with an implantable cardioverter-defibrillator (ICD). Defibrillation testing may lead to sinus rhythm (SR) restoration which may be hazardous due to the
increased risk of thromboembolic complications in these patients.
Aim: To identify predictors of SR restoration during defibrillation testing in patients with permanent AF undergoing ICD
implantation.
Methods: Permanent AF was present in 79 (12%) of 671 consecutive patients who received ICD in our institution between
2005 and 2010. In this group, 47 patients (mean age 64 ± 12 years, 38 males) underwent defibrillation testing during the
implantation procedure and in the remaining 32 patients defibrillation testing was not performed due to various contraindications.
Sinus rhythm was restored in 17 (36%) patients, while AF was still present after defibrillation testing in the remaining
30 patients. We analysed demographic, clinical, echocardiographic and electrophysiological parameters which could identify
those patients in whom SR was restored, using univariate and multivariate analysis as well as constructing ROC curves.
Results: Demographic parameters and clinical history were similar in both groups. Patients in whom SR was restored had
smaller left atrial diameter (49.9 ± 5.9 vs 59.4 ± 6.1 mm, p < 0.001), more often received a double-coil defibrillation lead
(83% vs 10%, p < 0.001), had less advanced heart failure as assessed using NYHA classification (p < 0.05), and were more
frequently treated with amiodarone (47% vs 23%, p < 0.025). The chance of SR return was increased 11 times in patients
receiving amiodarone, 6 times in patients with a double-coil lead, 2 times in patients with lower NYHA class, and 1.36 times
in patients with smaller left atrium diameter (for each 1 mm increase). The ROC curves showed that using the cut-off value for
the left atrial diameter of 47 mm, patients prone to SR restoration were identified with a sensitivity of 65%, specificity of
100%, positive predictive value of 83%, and negative predictive value of 90% (area under curve 0.904, 95% CI 0.809–1.0).
Multivariate analysis showed that NYHA class, amiodarone usage, type of defibrillating lead and left atrial diameter were
independent predictors of SR restoration.
Conclusions: Atrial fibrillation was present in 12% of consecutive patients undergoing ICD implantation and was terminated
by defibrillation testing in 36% of those who underwent this test. The NYHA class, amiodarone usage, type of defibrillating
lead and left atrial diameter were independent predictors of SR restoration.
Kardiol Pol 2011; 69, 1: 17-22Wstęp: Wśród pacjentów poddawanych zabiegowi implantacji kardiowertera-defibrylatora (ICD) są także chorzy z utrwalonym
migotaniem przedsionków (AF). U tych osób istnieje możliwość niespodziewanego powrotu rytmu zatokowego (SR)
podczas testu defibrylacji migotania komór (VF) wykonywanego w czasie wszczepiania ICD.
Cel: Celem pracy była identyfikacja czynników determinujących powrót SR w czasie testu defibrylacji VF u chorych z utrwalonym
AF poddanych wszczepieniu ICD w ramach prewencji wtórnej lub pierwotnej nagłego zgonu sercowego.
Metody: W grupie 671 chorych (śr. wieku 62,3 roku, 501 mężczyzn) poddanych implantacji ICD u 79 (12%) osób rozpoznano
utrwalone AF. W tej grupie u 47 pacjentów (śr. wieku 63,9 roku, 38 mężczyzn) wykonano test defibrylacji VF przy
wszczepieniu ICD, natomiast u pozostałych 32 odstąpiono od wykonania tego testu z powodu przeciwwskazań. Podczas
testu u 17 (36%) pacjentów (grupa A, śr. wieku 64,2 roku, 14 mężczyzn) stwierdzono powrót SR, a u pozostałych 30 chorych
(grupa B, śr. wieku 63,5 roku, 24 mężczyzn) nadal rejestrowano AF. Obie grupy porównano pod względem wskazań
i technicznych szczegółów implantacji, a także danych klinicznych echokardiograficznych, stosowanego leczenia i parametrów
elektrofizjologicznych. Przeprowadzono analizę jednoczynnikową i wieloczynnikową w celu ustalenia, które parametry
były niezależnie związane z powrotem SR oraz wykreślono krzywe ROC dla tych zmiennych.
Wyniki: Wiek, czas trwania AF, rozkład głównych kardiologicznych schorzeń i towarzyszących chorób, wskazań do ICD, stosowanych
leków (beta-adrenolityków, inhibitorów ACE, diuretyków, antykoagulacyjnych, antyagregacyjnych) oraz wartości parametrów
elektrofizjologicznych (elektrycznych) nie różniły się znacząco między grupami A i B. U chorych z grupy A zarejestrowano
istotnie niższe klasy niewydolności serca wg NYHA (p < 0,05), mniejszy wymiar lewego przedsionka (p < 0,001),
częstsze stosowanie elektrody dwupierścieniowej (p < 0,001) oraz częstsze podawanie amiodaronu (p < 0,025). Szansa
odzyskania SR przez osoby leczone amiodaronem była 10,77-krotnie większa niż przez osoby nieleczone, a przy stosowaniu
elektrody dwukoilowej - 6,24-krotnie większa niż w przypadku elektrody jednokoilowej. Zmniejszenie klasy NYHA z III lub
IV do II zwiększało szansę odzyskania SR 2,29-krotnie. Mniejszy o 1 jednostkę (1 mm) wymiar lewego przedsionka powodował
zwiększenie szansy odzyskania SR 1,36-krotnie. W krzywej ROC optymalny punkt odcięcia dla wymiaru lewego przedsionka
w celu identyfikacji chorych, u których AF ustąpiło podczas defibrylacji, wyniósł 47 mm.
Wnioski: Wśród osób z ICD utrwalone AF stwierdzono u 12% chorych i u 36% z nich po teście defibrylacji obserwowano
powrót SR. Czynnikami sprzyjającymi ustąpieniu AF były konfiguracja spiral elektrody defibrylującej, mniejszy wymiar lewego
przedsionka, niższa klasa NYHA oraz stosowanie amiodaronu.

Ablacja RF dodatkowych szlaków przedsionkowo-komorowych z dostępu przez zatokę wieńcową
Ablacja szlaków dodatkowych przez CS
Cel pracy: Ocena skuteczności ablacji RF dodatkowych szlaków tylnoprzegrodowych z dostępu przez zatokę wieńcową.
Materiał i metody: Badana grupa obejmowała 10 pacjentów (3M,7K) w wieku 19–69 lat (średnio 44,4 lat), u których ablację RF dodatkowego szlaku przedsionkowo-komorowego o położeniu tylnoprzegrodowym wykonano z dostępu przez zatokę wieńcową (CS, coronary sinus). W powierzchniowych zapisach EKG oceniano polaryzację fali delta i morfologię QRS. Analizowano dokumentację z badań elektrofizjologicznych i ablacji RF. Pacjentów poddawano kontroli po miesiącu od ablacji, a następnie co 6 miesięcy. Badanie kontrolne obejmowało wywiad, EKG oraz badanie echokardiograficzne.
Wyniki: U 8 pacjentów w powierzchniowym EKG stwierdzono jawną preekscytację z ujemną falą delta i zespołem QS w III i aVF. U 3 chorych, u których wykonano ablację z żyły średniej serca (MCV) rejestrowano ujemną falę delta w II, tworzącą kąt > 45º z linią izoelektryczną. Skuteczną ablację wykonano u 7 pacjentów z CS między jej ujściem a MCV, zaś u 3 pozostałych z MCV. Potencjał drogi dodatkowej rejestrowano u 8 chorych w miejscu skutecznej ablacji. U 7 pacjentów, w tym u 5 z jawną preekscytacją i u 2 z utajoną drogą dodatkową, dostęp przez CS był pierwszym i skutecznym miejscem ablacji. U 1 chorego wystąpił blok przedsionkowo-komorowy Iº po skutecznej ablacji. Nie stwierdzono innych powikłań zabiegu w badanej grupie. Czas obserwacji wynosił 4–26 miesięcy (średnio 18,7 miesięcy). U 1 pacjenta po 6 miesiącach od ablacji powróciło przewodzenie w drodze dodatkowej. Powtórną, skuteczną ablację wykonano w ujściu CS.
Wnioski: Ablacja RF szlaków tylnoprzegrodowych z dostępu przez CS jest metodą skuteczną i bezpieczną. Potencjał drogi dodatkowej ułatwia odnalezienie miejsca skutecznej ablacji. Analiza powierzchniowego EKG jest przydatna w kwalifikacji do ablacji z dostępu od CS
Zależność między poablacyjnymi zmianami wskaźników uszkodzenia miokardium i parametrami ablacji RF
Wstęp: Celem niniejszej pracy była ocena dynamiki poablacyjnych zmian wartości wskaźników
uszkodzenia mięśnia sercowego [kinazy kreatynowej (CK) i jej izoenzymu MB (CK-MB),
aminotransferazy asparginowej (GOT), aminotransferazy alaninowej (GPT), troponiny I oraz
mioglobiny] i ich korelacji z wartością użytej energii prądu o częstotliwości fal radiowych (RF).
Materiał i metody: Badaniem objęto 108 pacjentów (62 kobiet, 46 mężczyzn) w wieku 16–78 lat (śr. 50,2 roku), u których skutecznie wykonano ablację RF (WPW A - 20, WPW B - 13, AVNRT - 33, AF - 24, AFl - 8, VT z RVOT - 8, inne - 2). Pomiaru osoczowego
stężenia wymienionych wskaźników dokonano przed zabiegiem i 20 godzin po jego zakończeniu.
Wyniki: Uśrednione wartości parametrów ablacyjnych wynosiły: liczba aplikacji - 10,3,
całkowita użyta energia RF - 23 139 J. Uwagę zwraca spodziewana najmniejsza liczba
aplikacji oraz użytej energii RF w grupie, w której wykonano ablacje łącza przedsionkowokomorowego.
Najwięcej aplikacji wymagały ablacje prawostronnego pęczka Kenta, a nieco
mniej ektopii prawokomorowej. Największą ilość aplikowanej energii RF notowano w ablacjach
arytmii złożonych i zespołu Wolffa-Parkinsona-White’a typu A. Stwierdzono znamienny
poablacyjny wzrost stężenia troponiny I i mioglobiny, a w mniejszym stopniu GOT i CK; GPT i CK-MB wykazały tendencję spadkową. Ujawniono także istotną statystycznie korelację
między wzrostem troponiny I oraz mioglobiny a parametrami dostarczonej energii RF - liczbą
aplikacji i energią RF.
Wniosek: Troponina I, mioglobina oraz w mniejszym stopniu CK i GOT mogą być przydatnymi
klinicznie wskaźnikami nawet niewielkich rozmiarów uszkodzeń mięśnia sercowego
