    Rule behind the silk curtain: the Sultanahs of Aceh 1641-1699.

    PhDThis thesis is about the kingdom of Aceh Dar al-Salam in the latter half of the seventeenth century when four women ruled in succession: Sultanah Tajul Alam Safiatuddin Syah (1641-1675), Sultanah Nur Alam Naqiatuddin Syah (1675-1678), Sultanah Inayat Zakiatuddin Syah (1678-1688) and Sultanah Iamalat Zainatuddin Syah (1688-1699). How and why these queens came to power, and how they exercised it, are problems that have fascinated enquirers, prompting a range of comments and observations, especially the assertion that the queens were mere figureheads, during whose reigns the male elite (orangkaya) captured power. The Sultanahs were held responsible for the erosion of royal power and the kingdom's decline in the seventeenth century. Yet no in-depth study has ever investigated these claims. The main objective of this thesis is to evaluate the received views on these female sovereigns. The thesis also seeks to examine the origin, nature and impact of these Sultanahs. Female rule seems a curious phenomenon in a Muslim state. Furthermore, in a largely patriarchal kingdom such as Aceh, queens seemed to be strangely out of place. This unique episode in Aceh's history happened when European Companies - the Dutch VOC (Veerinigde Ooost-Indische Compagnie) and the English East India Company - were gradually increasing their commercial hold and flexing their military muscles in the region. Indigenous polities suffered increasing interference and pressure from Westerners. Most Malay and Muslim coastal polities in maritime Southeast Asia fell into European intruders' hands. By exploring the circumstances and arguments surrounding female accession, and examining some key episodes that show how power worked in Aceh at the time, I hope to approach a new understanding of how and why the male elite of Aceh placed the fate of the kingdom in the hands of women, and with what effects

    Finasteride as a model for personalized medicine

    The side effects of Finasteride are currently a subject of controversy. Some studies report minor or acceptable adverse effects, which decrease after a variable period of time so that they do not necessitate terminating Finasteride administration. However, several clinical and neuro-endocrine studies show that some adverse effects persist indefinitely in the form of post-Finasteride syndrome, even after the drug cessation. This paper presents a possible explanation for these inconsistent findings. First, the study design of either informing or not informing patients prior therapy about possible adverse effects can influence the incidence and magnitude of reported adverse effects. Second, structural and information dichotomies of the brain generate four distinct neuronal networks, which are activated through specific cerebral neuromodulators and that are able to support four distinct minds within an individual body. As a conclusion, the “mind psychophysiology” and the corresponding mental impairments differ across individuals, such that not only the prediction of adverse effects should be addressed from a more individualized medical perspective, but also the therapeutic strategies could be tailored to the four distinct mental profiles described. It is a personalized approach that would be applicable to several interrelated domains of neuroscience, like psychology, psychiatry and sexuality. Finally, this perspective may represent a starting point for a more individualized understanding of mental events, perhaps even a step forward in the understanding of the mind-body problem

    Androgenic alopecia; the risk–benefit ratio of Finasteride

    Finasteride is currently approved and largely used as a therapeutic option for androgenetic alopecia. Apparently a safe drug and effective at the onset of its application, several concerns have since appeared over the years regarding the frequency and magnitude of finasteride adverse effects, which in some cases appear irreversible even after drug termination. This paper discusses the use of finasteride for androgenic alopecia from two distinct perspectives. On the one hand, androgenic alopecia is a condition that especially affects a person’s self-image and esteem, aspects that are subjectively-constructed and thus relative and changeable. On the other hand, this condition involves a multifactorial etiology, with androgens being only partly responsible. Because androgens have important and unique physiological roles within the body, any procedure that results in androgenic suppression should be advised with caution. Furthermore, adverse effects induced by finasteride are neither fully documented nor easily treated. Finally, as alternative therapeutic approaches (such as topical finasteride) become available, the oral administration of finasteride for androgenic alopecia should, in our opinion, be reevaluated. Due to such concerns, a detailed and informed discussion should take place with patients considering therapy with finasteride for androgenic alopecia

    Coulomb distortion and medium corrections in nucleon-removal reactions

    One-nucleon removal reactions at or above the Fermi energy are important tools to explore the single-particle structure of exotic nuclei. Experimental data must be compared with calculations to extract structure information, evaluate correlation effects in nuclei or determine reaction rates for nuclear astrophysics. However, there is insufficient knowledge to calculate accurately the cross sections for these reactions. We evaluate the contributions of the final state interaction (FSI) and of the medium modifications of the nucleon-nucleon interactions and obtain the shapes and magnitudes of momentum distributions. Such effects have been often neglected in the literature. Calculations for reactions at energies 35 - 1000 MeV/nucleon are reported and compared to published data. For consistency, the state-of-the-art eikonal method for stripping and diffraction dissociation is used. We find that the two effects are important and their relative contributions vary with the energy and with the atomic and mass number of the projectile involved. These two often neglected effects modify considerably the one-nucleon removal cross sections. As expected, the effect are largest at lower energies, around 50 MeV/nucleon and on heavy targets.Comment: 12 pages, 10 figures.     The postfinasteride syndrome; an overview

    As a 5-α reductase inhibitor, Finasteride has proven effective in ameliorating two conditions documented to be androgen dependent, namely male androgenic alopecia and benign prostatic hyperplasia. Therapeutic results are maintained as long as the drug is administered, with treatment cessation generally leading to the return of symptomatology for each condition. In addition, during the therapeutic phase, several adverse effects have been reported, some of which persist long or indefinitely after treatment cessation, known as “post-finasteride syndrome.” Herein we present and discuss the most common finasteride side effects, along with a psycho-neuroendocrine rationale that could explain the persistence of many adverse effects after treatment cessation. Moreover, we argue that finasteride adverse effects occurring during finasteride administration should be delineated from postfinasteride side effects (encountered after treatment cessation), suggesting the need to be addressed separately within a therapeutic perspective. Until a tailored therapeutic approach of postfinasteride syndrome becomes available, we have noted that hand preference and sexual orientation seem to be useful as possible predicting factors for finasteride side effects and postfinasteride syndrome. Finally, even though finasteride administration is considered relatively safe, literature data urges prudence. Specifically, recent studies report that some subjects receiving finasteride develop severe depressive episodes including suicidal thoughts, in part due to persistent sexual side effects

    Three-body decay of 6^{6}Be

    Three-body correlations for the ground-state decay of the lightest two-proton emitter 6^{6}Be are studied both theoretically and experimentally. Theoretical studies are performed in a three-body hyperspherical-harmonics cluster model. In the experimental studies, the ground state of 6^{6}Be was formed following the α\alpha decay of a 10^{10}C beam inelastically excited through interactions with Be and C targets. Excellent agreement between theory and experiment is obtained demonstrating the existence of complicated correlation patterns which can elucidate the structure of 6^{6}Be and, possibly, of the A=6 isobar.Comment: 17 pages, 21 figures, 5 table

    RECLAIMING HAWWA’ IN ISLAMIC CIVILIZATION AND DISCOURSE

    The ‘woman question’ in Islamic discourse and history is perhaps one of the most emotive and contested issues for Muslims and non-Muslims. In the western imaginary, nourished by vivid media images of shadowed, black-cloaked silent women coupled with sensational stories about honor killings and stoning, domestic oppression, political and legal disenfranchisement, women are symbols of the east-west civilizational conflict. In international politics, the idea of culture has gained prominence and Muslims are presented as a homogenized and troubled culture and Muslim women represent how alien this civilization is. For some Muslims, a patriarchal interpretation of the ‘rib of Adam’ narrative asserts that a woman is secondary to a man as a human being. Yet, in the course of Muslim civilizational history and tradition, women were heads of states in Muslim societies, philosophers, preachers, intellectuals and freedom fighters. This paper examines this disjuncture and argues that one needs to re-think the ‘woman question’ not only in the Quran and hadith literature but to re-visit Islamic history and civilization to reclaim the place of women in contemporary Islam and its imaginaries

    Positive influence of short message service and voice call interventions on adherence and health outcomes in case of chronic disease care: a systematic review

    Background: Chronic diseases have emerged as a serious threat for health, as well as for global development. They endenger considerably increased health care costs and diminish the productivity of the adult population group and, therefore, create a burden on health, as well as on the global economy. As the management of chronic diseases involves long-term care, often lifelong patient adherence is the key for better health outcomes. We carried out a systematic literature review on the impact of mobile health interventions -mobile phone texts and/or voice messages- in high, middle and low income countries to ascertain the impact on patients’ adherence to medical advice, as well as the impact on health outcomes in cases of chronic diseases. Methods: The review identified fourteen related studies following the defined inclusion and exclusion criteria, in PubMed, Cochrane Library, the Library of Congress, and Web Sciences. All the interventions were critically analysed according to the study design, sample size, duration, tools used, and the statistical methods used for analysing the primary data. Impacts of the different interventions on outcomes of interest were also analysed. Results: The findings showed evidence of improved adherence, as well as health outcomes in disease management, using mobile Short Message Systems and/or Voice Calls. Significant improvement has been found on adherence with taking medicine, following diet and physical activity advice, as well as improvement in clinical parameters like HbA1c, blood glucose, blood cholesterol and control of blood pressure and asthma. Conclusions: Though studies showed positive impacts on adherence and health outcomes, three caveats should be considered, (i) there was no clear understanding of the processes through which interventions worked; (ii) none of the studies showed cost data for the m-health interventions and (iii) only short term impacts were captured, it remains unclear whether the effects are sustained. More research is needed in these three areas before drawing concrete conclusions and making suggestions to policy makers for further decision and implementation

    Performance evaluation of novel square-bordered position-sensitive silicon detectors with four-corner readout

    We report on a recently developed novel type of large area (62 mm x 62 mm) position sensitive silicon detector with four-corner readout. It consists of a square-shaped ion-implanted resistive anode framed by additional low-resistivity strips with resistances smaller than the anode surface resistance by a factor of 2. The detector position linearity, position resolution, and energy resolution were measured with alpha-particles and heavy ions. In-beam experimental results reveal a position resolution below 1 mm (FWHM) and a very good non-linearity of less than 1% (rms). The energy resolution determined from 228Th alpha source measurements is around 2% (FWHM).Comment: 13 pages, 10 figures, submitted to Nucl. Instr. and Meth.

    Astrophysical S factor for the radiative capture 12N(p,gamma)13O determined from the 14N(12N,13O)13C proton transfer reaction

    The cross section of the radiative proton capture reaction on the drip line nucleus 12N was investigated using the Asymptotic Normalization Coefficient (ANC) method. We have used the 14N(12N,13O)13C proton transfer reaction at 12 MeV/nucleon to extract the ANC for 13O -> 12N + p and calculate from it the direct component of the astrophysical S factor of the 12N(p,gamma)13O reaction. The optical potentials used and the DWBA analysis of the proton transfer reaction are discussed. For the entrance channel, the optical potential was inferred from an elastic scattering measurement carried out at the same time with the transfer measurement. From the transfer, we determined the square of the ANC, C^2(13Og.s.) = 2.53 +/- 0.30 fm-1, and hence a value of 0.33(4) keVb was obtained for the direct astrophysical S factor at zero energy. Constructive interference at low energies between the direct and resonant captures leads to an enhancement of Stotal(0) = 0.42(5) keVb. The 12N(p,gamma)13O reaction was investigated in relation to the evolution of hydrogen-rich massive Population III stars, for the role that it may play in the hot pp-chain nuclear burning processes, possibly occurring in such objects.Comment: 15 pages, 10 figures, 3 tables submitted to Phys.
