    Impact of COVID-19 Pandemic on TAVR Activity: A Worldwide Registry

    Background: The COVID-19 pandemic had a considerable impact on the provision of structural heart intervention worldwide. Our objectives were: 1) to assess the impact of the COVID-19 pandemic on transcatheter aortic valve replacement (TAVR) activity globally; and 2) to determine the differences in the impact according to geographic region and the demographic, development, and economic status of diverse international health care systems. Methods: We developed a multinational registry of global TAVR activity and invited individual TAVR sites to submit TAVR implant data before and during the COVID-19 pandemic. Specifically, the number of TAVR procedures performed monthly from January 2019 to December 2021 was collected. The adaptive measures to maintain TAVR activity by each site were recorded, as was a variety of indices relating to type of health care system and national economic indices. The primary subject of interest was the impact on TAVR activity during each of the pandemic waves (2020 and 2021) compared with the same period pre–COVID-19 (2019). Results: Data were received from 130 centers from 61 countries, with 14 subcontinents and 5 continents participating in the study. Overall, TAVR activity increased by 16.7% (2,337 procedures) between 2018 and 2019 (ie, before the pandemic), but between 2019 and 2020 (ie, first year of the pandemic), there was no significant growth (–0.1%; –10 procedures). In contrast, activity again increased by 18.9% (3,085 procedures) between 2020 and 2021 (ie, second year of the pandemic). During the first pandemic wave, there was a reduction of 18.9% (945 procedures) in TAVR activity among participating sites, while during the second and third waves, there was an increase of 6.7% (489 procedures) and 15.9% (1,042 procedures), respectively. Further analysis and results of this study are ongoing and will be available at the time of the congress. Conclusion: The COVID-19 pandemic initially led to a reduction in the number of patients undergoing TAVR worldwide, although health care systems subsequently adapted, and the number of TAVR recipients continued to grow in subsequent COVID-19 pandemic waves.     Vienas svarbiausių medaus gavybos procesų yra medaus sukimas. Mokslininkai, kurie tyrė medaus gavybą, didesnį dėmesį skyrė įvairiems medaus gavybos metodams, siekiant užtikrinti medaus gavimo našumą, tačiau detalesnių medaus sukimo metodų tyrimų, medsukio parametrų tyrimų neatliko. Svarbu nustatyti geriausiai tinkantį medaus sukimo metodą priklausomai nuo medaus rūšies ir optimalius medsukio darbo rėžimus. Baigiamajame darbe pateikti 2020 metais atlikti universalaus medsukio darbo parametrų tyrimų rezultatai, sukant rapsų, mišrų, liepų ir grikių medų. Medus buvo sukamas ašiniu ir tangentiniu metodais, sukant tangentiniu metodu korių padėties kampas buvo nulinis arba 23º. Tyrimais sustatyta, kad didžiausias medaus kiekis yra išsukamas, kai universalaus medsukio rotoriaus sūkiai palaipsniui didinami nuo 273 min-1 pasiekia 819 min-1. Toliau sukant medų tikslinga universalaus medsukio rotoriaus sūkius palaipsniui didinti iki 2184 min-1, kad neišsukto medaus nuostoliai būtų minimalūs. Dar daugiau didinti universalaus medsukio sūkius negalima, nes pradeda irti koriai. Bet kurios rūšies medų tikslingiau sukti tangentiniu metodu, nes neišsukto medaus nuostoliai mažesni, negu sukant medų ašiniu metodu. Rapsų, mišrų ir grikių medų geriausia sukti, kai nustatytas nulinis korių padėties kampas, o liepų medų tikslingiau sukti korių padėties kampą nustačius 23º, nes tuomet gaunami patys mažiausi nuostoliai. Darbo tikslas - nustatyti geriausiai tinkantį medaus sukimo metodą priklausomai nuo medaus rūšies ir optimalius universalaus medsukio darbo rėžimus.One of the most important processes for honey extraction is the turning of honey. Scientists who explored honey extraction paid more attention to various methods of honey extraction to assure the productivity of honey extraction, but they did not carry out more detailed research of honey turning methods, parameters of the honey centrifuge. It is important to identify the best method for honey turning depending on the species of honey and the optimal working regimes of the honey centrifuge. In the final work, the results of studies on the work parameters of universal honey centrifuge that was performed in 2020, turning canola, mixed, lime-blossom, and buckwheat honey are presented. The honey was turned using axial and tangential methods, and with the tangential method, the angle of the honeycomb position was zero or 23º. Studies identified that the maximum number of honey is rectified when the spins of the universal honey centrifuge rotor are gradually increased from 273 min-1 to 819 min-1. When the honey is further turned, it is purposeful to increase gradually the rotor speed of the universal honey centrifuge to 2184 min-1 so that the loss of unrectified honey would be minimal. It is impossible to increase the speed of the universal honey centrifuge more because the honeycombs begin to decay. It is more purposeful to turn any species of honey using the tangential method because the loss of unrectified honey is lower than by turning the honey using the axial method. Canola, mixed, and buckwheat honey is best turned at zero honeycomb position angle, and lime-blossom honey is best turned at 23º honeycomb angle because losses are the lowest then.     Straipsnyje pateikti 2020 metais atlikti universalaus medsukio darbo parametrų tyrimų rezultatai, sukant skirtingų rūšių medų. Rapsų, poliflorinį, liepų ir grikių medus buvo sukamas ašiniu ir tangentiniu metodais, sukant tangentiniu metodu korių padėties kampas buvo nulinis arba 23º . Sukant medų tangentiniu metodu, korių padėties kampas turi įtakos neišsukto medaus nuostoliams. Rapsų, poliflorinį ir grikių medų geriausia sukti, kai nustatytas nulinis korių padėties kampas, o liepų medų tikslingiau sukti korių padėties kampą nustačius 23º, nes tuomet gaunami patys mažiausi nuostoliai iš atliktų tyrimų. Last year, meteorological conditions were very unfavorable for beekeeping, and the number of honey that was carried decreased severely. In honey extraction, it is very important to rectify the honeycombs as efficiently as possible to gain the maximum number of honey. It is important to identify the best method for the honey rotation depending on the species of honey and the optimal regimens for the honeycomb work. In the article, the results from the study on the parameters of universal honeycomb work, which was accomplished in 2020, when canola, mixed, lime-blossom, and buckwheat honey are rotating, are presented. Relative humidity was identified for each species of honey. The study was carried out by rotating honey by using three different methods: axial; tangential at zero honeycomb angle; tangential at an angle of 23 ° in the position of honeycombs. In the beginning, all honeycombs are attacked, weighed, and put in the honeycomb. It was rotated 12 honeycombs by using each method. Other researchers identified that in the beginning, it is necessary to rotate the honey at a lower speed, and at every particular time interval, the speed should be increased. Concerning the technical characteristics of the universal honeycomb, the initial speed of the honeycomb twists during the study was minimal 273 min-1 , the time interval was chosen to increase the frequency every 2 min, and the honeycomb was twisted at even 7 different rotation speeds increasing every 273 min-1 : 546, 819, 1092, 1365, 1638, 1911, 2184 min-1 . Twisting at each speed, the number of honey rectified and its moisture was identified by weighing.
