    Vulval swellings have always been a case of a difficult and a puzzling situation and more so when they are huge in size. Vulvar swellings are of various types such as Bartholin cyst, sebaceous cyst, cyst of canal of nuck, inguino-labial hernia and vulval varicosities. Most common vulvar cysts are epidermal inclusion cysts. Usual location is beneath the epidermis. An alternative histogenesis is embryonic remnants or occlusion of pilosebaceous ducts of sweat glands. Cutaneous cysts which are lined by ciliated epithelium are very rare, and authors present a rare case of a cyst arising from a left labium majus resembling a hydrocele seen in males with histopathology suggestive of ciliated cyst of the vulva

    Postmenopausal patient presenting with pain and lump in abdomen should be considered as a complication caused by gynaecological malignancy unless proved otherwise pyometra is most common in postmenopausal women but it can affect women of child bearing age as well. Usually it is caused by impairment of natural drainage of the cervix due to malignant diseases of genital tract and consequences of their treatment with radiotherapy. Authors report one such case with cervical intraepithelial neoplasia with uterus markedly enlarged to 26-28 weeks size with Vaginal stenosis in an eighty-year-old postmenopausal patient

    Successful management of a near miss case of eclampsia with intracranial haemorrhage requiring craniotomy

    Large population-based studies on stroke report that intracerebral haemorrhage is rare in young women however it is a grave and disturbing complication of pregnancy. Preeclampsia is usually clinically silent, but may cause symptoms of neurological dysfunction such as headache, visual disturbances and impairment of consciousness. Eclampsia is in the occurrence of seizures in the context of preeclampsia and is often, but not always, preceded by the above neurological symptoms. Most published data support the view that preeclampsia and eclampsia are important causative factors for pregnancy related ICH. Sparse data is available with respect to the management of such cases. Identification of near miss cases is an important step in reducing mortality. Herein we report a near miss case of eclampsia with intracranial haemorrhage requiring craniotomy

    Unusual presentation of a case of ovarian endometrioma

    Ca-125 is a cell surface glycoprotein present in epithelial ovarian tumors. It is called as a biomarker because it provides information about the biological state of a disease. It is also considered a “tumor associated protein” because elevated CA 125 levels do not always indicate ovarian cancer. Pelvic inflammatory disease irritable bowel disease, tuberculosis pancreatitis and endometriosis can also increase the Ca125 levels in these benign conditions the Ca 125 levels are rarely markedly. Here authors report a case of ovarian endometrioma with markedly elevated levels of Ca 125 who was diagnosed as a case of ovarian malignancy due to presence of ascites and a large ovarian mass of 20x20 cm size arising from the pelvis

    The ALTCRISS project on board the International Space Station

    The Altcriss project aims to perform a long term survey of the radiation environment on board the International Space Station. Measurements are being performed with active and passive devices in different locations and orientations of the Russian segment of the station. The goal is to perform a detailed evaluation of the differences in particle fluence and nuclear composition due to different shielding material and attitude of the station. The Sileye-3/Alteino detector is used to identify nuclei up to Iron in the energy range above 60 MeV/n. Several passive dosimeters (TLDs, CR39) are also placed in the same location of Sileye-3 detector. Polyethylene shielding is periodically interposed in front of the detectors to evaluate the effectiveness of shielding on the nuclear component of the cosmic radiation. The project was submitted to ESA in reply to the AO in the Life and Physical Science of 2004 and data taking began in December 2005. Dosimeters and data cards are rotated every six months: up to now three launches of dosimeters and data cards have been performed and have been returned with the end of expedition 12 and 13.Comment: Accepted for publication on Advances in Space Research http://dx.doi.org/10.1016/j.asr.2007.04.03

    Parametrizations of Inclusive Cross Sections for Pion Production in Proton-Proton Collisions

    Accurate knowledge of cross sections for pion production in proton-proton collisions finds wide application in particle physics, astrophysics, cosmic ray physics and space radiation problems, especially in situations where an incident proton is transported through some medium, and one requires knowledge of the output particle spectrum given the input spectrum. In such cases accurate parametrizations of the cross sections are desired. In this paper we review much of the experimental data and compare to a wide variety of different cross section parametrizations. In so doing, we provide parametrizations of neutral and charged pion cross sections which provide a very accurate description of the experimental data. Lorentz invariant differential cross sections, spectral distributions and total cross section parametrizations are presented.Comment: 32 pages with 15 figures.     A Composite Metric for Benchmarking Site Performance in TAVR: Results from the STS/ACC TVT Registry

    Background: Transcatheter aortic valve replacement (TAVR) is a transformative therapy for aortic stenosis. Despite rapid improvements in technology and techniques, serious complications remain relatively common and are not well described by single outcome measures. The purpose of this study was to determine if there is site-level variation in TAVR outcomes in the United States using a novel 30-day composite measure. Methods: We performed a retrospective cohort study using data from the STS/ACC TVT Registry to develop a novel ranked composite performance measure that incorporates mortality and serious complications. The selection and rank order of the complications for the composite was determined by their adjusted association with 1-year outcomes. Sites whose risk-adjusted outcomes were significantly more or less frequent than the national average based on a 95% probability interval were classified as performing worse or better than expected. Results: The development cohort consisted of 52,561 patients who underwent TAVR between January 1, 2015 and December 31, 2017. Based on the associations with 1-year risk-adjusted mortality and health status, we identified four periprocedural complications to include in the composite risk model in addition to mortality. Ranked empirically according to severity, these included stroke, major, life-threatening or disabling bleeding, stage III acute kidney injury, and moderate or severe peri-valvular regurgitation. Based on these ranked outcomes, we found that there was significant site-level variation in quality of care in TAVR in the United States. Overall, better than expected site performance was observed in 25/301 (8%) of sites; performance as expected was observed in 242/301 sites (80%); and worse than expected performance was observed in 34/301 (11%) of sites. Thirty-day mortality, stroke, major, life-threatening or disabling bleeding, and moderate or severe peri-valvular leak were each substantially more common in sites with worse than expected performance as compared with other sites. There was good aggregate reliability of the model. Conclusions: There are substantial variations in the quality of TAVR care received in the United States, and 11% of sites were identified as providing care below the average level of performance. Further study is necessary to determine structural, process-related, and technical factors associated with high- and low-performing sites

    Search for antihelium in cosmic rays

    The Alpha Magnetic Spectrometer (AMS) was flown on the space shuttle Discovery during flight STS-91 in a 51.7 degree orbit at altitudes between 320 and 390 km. A total of 2.86 * 10^6 helium nuclei were observed in the rigidity range 1 to 140 GV. No antihelium nuclei were detected at any rigidity. An upper limit on the flux ratio of antihelium to helium of < 1.1 * 10^-6 is obtained.Comment: 18 pages, Latex, 9 .eps figure

    Graphene-based ultrathin flat lenses

    Flat lenses when compared to curved surface lenses have the advantages of being aberration free and they offer a compact design necessary for a myriad of electro-optical applications. In this paper we present flat and ultra-thin lenses based on graphene, the world’s thinnest known material. Monolayers and low number multilayers of graphene were fabricated into Fresnel zones to produce Fresnel zone plates which utilize the reflection and transmission properties of graphene for their operation. The working of the lens and their performance in the visible and terahertz regimes was analyzed computationally. Experimental measurements were also performed to characterize the lens in the visible regime and a good agreement was obtained with the simulations. The work demonstrates the principle of atom thick graphene-based lenses, with perspectives for ultra-compact integration.HB would like to thank The Leverhulme Trust for the research funding.
