    The initial stages of the femtosecond (fs) laser ablation process of gold, platinum, and tungsten were observed by single-shot soft x-ray imaging technique. The formation and evolution of soft x-ray Newton’s rings (NRs) were found for the first time. The soft x-ray NRs are caused by the interference between the bulk ablated surface and nanometer-scale thin spallation layer; they originate from the metal surface at pump energy fluence of around 1 J/cm2 and work as a flying soft x-ray beam splitter

    Background: Cervical auscultation is a technique frequently used for the screening of dysphagia. However, this method is difficult to evaluate objectively and it is unclear how sound is generated during the swallowing process. The aim of this study was to analyze the waveform of swallowing sound and clarify the sound production process using recordings of swallowing sounds and ultrasound images (USI), performed simultaneously. Materials and Methods: Commercial natural spring water and natural carbonated water were used in experiments 1 and 2, respectively. In experiment 1, a microphone was attached to the skin of the neck of 20 young participants and swallowing sounds were recorded and analyzed. In experiment 2, swallowing processes in three participants were recorded by a medical ultrasonography apparatus. The ultrasonic probe was placed on the skin over one of the thyroid cartilages or the thyroid gland. Results: The swallowing sound wave (SSW) was divided into three sectional periods. The mean duration of the first, second, and third SSW was 210 ± 147 ms, 458 ± 113 ms, and 91 ± 61 ms, respectively. The mean intensity ratio of the first, second, and third SSW was 7.8 ± 5.2, 29.2 ± 16.5, and 5.8 ± 5.1, respectively. When the ultrasonic probe was placed on the skin over one of the thyroid cartilages, in the phase between the production of the second SSW and the silent period, the USI revealed an accumulation of swallowed material around the valleculae and oropharynx. In the silent period of the second SSW, the swallowed material accumulated around the hypopharynx. When the ultrasonic probe was placed on the skin over the thyroid gland, in the silent period of the second SSW, the USI revealed that the swallowed material had passed through esophagus. Conclusion: Waveform and USI findings from this study suggest that swallowing sound can be divided into three sectional periods: an oral phase, a pharyngeal phase, and a repositioning phase

    High-pressure synthesis, crystal structure determination, and a Ca substitution study of the metallic rhodium oxide NaRh2O4

    The sodium rhodate NaRh2O4 was synthesized for the first time and characterized by neutron and X-ray diffraction studies and measurements of magnetic susceptibility, specific heat, electrical resistivity, and the Seebeck coefficient. NaRh2O4 crystallizes in the CaFe2O4-type structure, which is comprised of a characteristic RhO6 octahedral network. The compound is metallic in nature, probably reflecting the 1:1 mixed valence character of Rh(III) and Rh(IV) in the network. For further studies of the compound, the Rh valence was varied significantly by means of an aliovalent substitution: the full-range solid solution between NaRh2O4 and CaRh2O 4 was achieved and characterized as well. The metallic state was dramatically altered, and a peculiar magnetism developed in the low Na concentration range

    Characterization of Swallowing Sound : Preliminary Investigation of Normal Subjects

    Objective The purpose of this study was to characterize the swallowing sound and identify the process of sound generation during swallowing in young healthy adults. Methods Thirty-three healthy volunteers were enrolled and allocated into three experimental groups. In experiment 1, a microphone was attached to one of eight cervical sites in 20 subjects, participants swallowed 5 ml water, and the sound waveform was recorded. In experiment 2, 10 subjects swallowed either 0, 5, 10, or 15 ml water during audio recording. In addition, participants consumed the 5 ml bolus in two different cervical postures. In experiment 3, the sound waveform and videofluoroscopy were simultaneously recorded while the three participants consumed 5 ml iopamidol solution. The duration and peak intensity ratio of the waveform were analyzed in all experimental groups. Results The acoustic analysis of the waveforms and videofluoroscopy suggested that the swallowing sound could be divided into three periods, each associated with a stage of the swallowing movement: the oral phase comprising posterior tongue and hyoid bone movement; the pharyngeal phase comprising larynx movement, hyoid bone elevation, epiglottis closure, and passage of the bolus through the esophagus orifice; and the repositioning phase comprising the return of the hyoid bone and larynx to their resting positions, and reopening of the epiglottis. Conclusion Acoustic analysis of swallowing sounds and videofluoroscopy suggests that the swallowing sound could be divided into three periods associated with each process of the swallowing movement: the oral phase comprising the posterior movement of the tongue and hyoid bone; the pharyngeal phase comprising the laryngeal movement, hyoid bone elevation, epiglottis closure, and the bolus passage to the esophagus orifice; and the repositioning phase comprising the repositioning of the hyoid bone and larynx, and reopening of the epiglottis

    注意障害事例の行動の特徴と機能評価との関連性:観察記録のテキストマイニングによる分析とTrail Making Test の検討より

    研究報告Original Articles 本研究は,注意機能の評価として広く一般的に行われているTrail Making Test(TMT)の評価結果と,日常における行動の特徴との関連性を明らかにすることを目的とした探索的研究である.対象は,日本における医学論文情報のインターネット検索サービスである医中誌WEB に1999 年~2012 年までの13 年間に登録された文献のうち,“ 作業療法” と,“ 注意障害” をキーワードとして検索した(2013 年5 月検索).その結果31 例の作業療法士が報告する症例を得たが,それらの内でTMT を評価として用いていた17 症例を対象とした.17 症例の報告から,34 組の行動の観察記録とTMT の評価結果を抽出し,行動の観察記録をテキストマイニング手法により分析した後,TMT の結果と照らし合わせて比較検討した.検討の結果,4 段階に分かれたTMT の遂行レベルについて,各々の段階の行動の特徴が明らかとなった. The present exploratory study was aimed to examine the relationship between the results of the trail-making test (TMT), a common test to assess attentional function, and the behavioral characteristics in people with attentional deficits in daily life. A search of documents for a thirteen-year period, between 1999 and 2012, was conducted in May 2013, using Ichushi-Web, an Internet search service containing information on Japanese medical papers, with the keywords : “occupational therapy” and “attention deficits”. There were 31 papers written by occupational therapists, and 17 which adopted the TMT for assessment were selected. Observation records of the behaviors of 34 subjects and TMT assessment results were extracted from the 17 papers. The observation records were analyzed using text-mining, and the analysis results were compared with those of TMT assessment. The TMT assessment results were classified into four levels, and the characteristics of the behaviors at each level were identified

    Cost-effectiveness of gargling for the prevention of upper respiratory tract infections

    <p>Abstract</p> <p>Background</p> <p>In Japan, gargling is a generally accepted way of preventing upper respiratory tract infection (URTI). The effectiveness of gargling for preventing URTI has been shown in a randomized controlled trial that compared incidences of URTI between gargling and control groups. From the perspective of the third-party payer, gargling is dominant due to the fact that the costs of gargling are borne by the participant. However, the cost-effectiveness of gargling from a societal perspective should be considered. In this study, economic evaluation alongside a randomized controlled trial was performed to evaluate the cost-effectiveness of gargling for preventing URTI from a societal perspective.</p> <p>Methods</p> <p>Among participants in the gargling trial, 122 water-gargling and 130 control subjects were involved in the economic analysis. Sixty-day cumulative follow-up costs and effectiveness measured by quality-adjusted life days (QALD) were compared between groups on an intention-to-treat basis. Incremental cost-effectiveness ratio (ICER) was converted to dollars per quality-adjusted life years (QALY). The 95% confidence interval (95%CI) and probability of gargling being cost-effective were estimated by bootstrapping.</p> <p>Results</p> <p>After 60 days, QALD was increased by 0.43 and costs were 37.1higherinthegarglinggroupthaninthecontrolgroup.ICERofthegarglinggroupwas37.1 higher in the gargling group than in the control group. ICER of the gargling group was 31,800/QALY (95%CI, 1,9001,900–248,100). Although this resembles many acceptable forms of medical intervention, including URTI preventive measures such as influenza vaccination, the broad confidence interval indicates uncertainty surrounding our results. In addition, one-way sensitivity analysis also indicated that careful evaluation is required for the cost of gargling and the utility of moderate URTI. The major limitation of this study was that this trial was conducted in winter, at a time when URTI is prevalent. Care must be taken when applying the results to a season when URTI is not prevalent, since the ICER will increase due to decreases in incidence.</p> <p>Conclusion</p> <p>This study suggests gargling as a cost-effective preventive strategy for URTI that is acceptable from perspectives of both the third-party payer and society.</p

    DOCK2 is involved in the host genetics and biology of severe COVID-19

    「コロナ制圧タスクフォース」COVID-19疾患感受性遺伝子DOCK2の重症化機序を解明 --アジア最大のバイオレポジトリーでCOVID-19の治療標的を発見--. 京都大学プレスリリース. 2022-08-10.Identifying the host genetic factors underlying severe COVID-19 is an emerging challenge. Here we conducted a genome-wide association study (GWAS) involving 2, 393 cases of COVID-19 in a cohort of Japanese individuals collected during the initial waves of the pandemic, with 3, 289 unaffected controls. We identified a variant on chromosome 5 at 5q35 (rs60200309-A), close to the dedicator of cytokinesis 2 gene (DOCK2), which was associated with severe COVID-19 in patients less than 65 years of age. This risk allele was prevalent in East Asian individuals but rare in Europeans, highlighting the value of genome-wide association studies in non-European populations. RNA-sequencing analysis of 473 bulk peripheral blood samples identified decreased expression of DOCK2 associated with the risk allele in these younger patients. DOCK2 expression was suppressed in patients with severe cases of COVID-19. Single-cell RNA-sequencing analysis (n = 61 individuals) identified cell-type-specific downregulation of DOCK2 and a COVID-19-specific decreasing effect of the risk allele on DOCK2 expression in non-classical monocytes. Immunohistochemistry of lung specimens from patients with severe COVID-19 pneumonia showed suppressed DOCK2 expression. Moreover, inhibition of DOCK2 function with CPYPP increased the severity of pneumonia in a Syrian hamster model of SARS-CoV-2 infection, characterized by weight loss, lung oedema, enhanced viral loads, impaired macrophage recruitment and dysregulated type I interferon responses. We conclude that DOCK2 has an important role in the host immune response to SARS-CoV-2 infection and the development of severe COVID-19, and could be further explored as a potential biomarker and/or therapeutic target
