Is health care getting safer?
Patient safety has been high on the national and international agenda in health care for almost a decade. In the United Kingdom, reviews of case records have shown that over 10% of patients experience an adverse event while in hospital,1 2 a figure reflected in similar studies around the world.3 Considerable efforts have been made to improve safety, and it is natural to ask whether these efforts have been well directed. Are patients any safer? The answer to this simple question is curiously elusive. Although some aspects of safety are difficult to measure for technical reasons (defining preventability for instance), the main problem is that measurement and evaluation have not been high on the agenda. We believe that the lack of reliable information on safety and quality of care is hindering improvement in safety across the world.
The principal approach to patient safety in the UK, United States, and many other countries has been to establish local and national reporting systems; these systems invite voluntary reporting of unspecified safety incidents with the aim of learning lessons and feeding back the findings into the system. However, these reporting systems do not effectively detect adverse events. In the most recent comparison, reporting systems detected only about 6% of adverse events found by systematic review of records.2 Reporting systems are a valuable component of a safety system, but they are essentially systems for warning and communication inside an organisation and, if large scale, of detecting rare events not easily detectable by other means. They cannot and never will act as a measurement system for safety.
Here, we use the example of the UK National Health Service to determine whether it is possible to assess change in several core areas that reflect the safety of health care and, if so, what changes are apparent. We focus on measures of outcome, in the sense of definable events that happen to patients (infections, morbidity, mortality) and on key measures of process (such as drug errors). We have not considered concepts such as culture or resilience that are held to reflect safety but are not proved indices of clinical process or outcome. Defining safety is itself a challenge, and we do not pretend that the indicators can provide more than a crude measure of overall levels of safety. Universal Vectorial and Ultrasensitive Nanomechanical Force Field Sensor
Miniaturization of force probes into nanomechanical oscillators enables
ultrasensitive investigations of forces on dimensions smaller than their
characteristic length scale. Meanwhile it also unravels the force field
vectorial character and how its topology impacts the measurement. Here we
expose an ultrasensitive method to image 2D vectorial force fields by
optomechanically following the bidimensional Brownian motion of a singly
clamped nanowire. This novel approach relies on angular and spectral tomography
of its quasi frequency-degenerated transverse mechanical polarizations:
immersing the nanoresonator in a vectorial force field does not only shift its
eigenfrequencies but also rotate eigenmodes orientation as a nano-compass. This
universal method is employed to map a tunable electrostatic force field whose
spatial gradients can even take precedence over the intrinsic nanowire
properties. Enabling vectorial force fields imaging with demonstrated
sensitivities of attonewton variations over the nanoprobe Brownian trajectory
will have strong impact on scientific exploration at the nanoscale
The role of Endobronchial ultrasound guided transbronchial needle aspiration (EBUS-TBNA) for qualitative diagnosis of mediastinal and hilar lymphadenopathy: a prospective analysis
<p>Abstract</p> <p>Background</p> <p>Recently EBUS-TBNA, which has a sensitivity of 94.6%, specificity of 100% and diagnostic accuracy rate of 96.3% as previously reported, has been widely used for patients with mediastinal and hilar lymphadenopathy or suspected lung cancer to get accurate diagnosis. The purpose of the current study was to evaluate the usefulness of EBUS-TBNA in obtaining cytological and histological diagnosis of mediastinal and hilar lymph nodes compared to the results obtained with conventional mediastinoscopy as previously reported, and to assess the relationship of diagnostic accuracy and number of passes and size of lymph nodes.</p> <p>Methods</p> <p>101 patients with mediastinal and hilar lymphadenopathy or suspected lung cancer in our institution were included in this prospective study. EBUS-TBNA was performed in all cases. The final diagnosis was confirmed by cytology, surgical results, and/or clinical follow-up for at least 6 months. Sensitivity, specificity, accuracy, and positive and negative predictive values were calculated using standard formulas.</p> <p>Results</p> <p>In 101 patients, EBUS-TBNA was successfully performed to obtain samples from 225 lymph nodes, 7 lung masses, 1 mediastinal mass and 2 esophageal masses. 63 malignant tumors and 38 benign diseases were confirmed. Epidermal growth factor receptor mutation was detected in 10 biopsy samples, and epidermal growth factor receptor mutation was detected in 4 cases. With respect to the correct diagnosis of mediastinal and hilar lymphadenopathy, EBUS-TBNA had a sensitivity of 95.08%, specificity of 100%, positive predictive value of 100%, negative predictive value of 93.02%, and overall accuracy of 97.02%. The relationship of diagnostic accuracy and number of lymph node passes or size of lymph nodes was both insignificant (p = 0.27; p = 0.23). The procedure was uneventful without complications.</p> <p>Conclusions</p> <p>EBUS-TBNA is an accurate and safe tool in diagnosis of mediastinal and hilar lymphadenopathy. It cannot completely replace mediastinoscopy, it may indeed reduce the number of mediastinoscopy procedures. In some cases, it can necessarily be the first-line procedure before mediastinoscopy.</p

Consistent phenological shifts in the making of a biodiversity hotspot: the Cape flora
Background
The best documented survival responses of organisms to past climate change on short (glacial-interglacial) timescales are distributional shifts. Despite ample evidence on such timescales for local adaptations of populations at specific sites, the long-term impacts of such changes on evolutionary significant units in response to past climatic change have been little documented. Here we use phylogenies to reconstruct changes in distribution and flowering ecology of the Cape flora - South Africa's biodiversity hotspot - through a period of past (Neogene and Quaternary) changes in the seasonality of rainfall over a timescale of several million years.
Results
Forty-three distributional and phenological shifts consistent with past climatic change occur across the flora, and a comparable number of clades underwent adaptive changes in their flowering phenology (9 clades; half of the clades investigated) as underwent distributional shifts (12 clades; two thirds of the clades investigated). Of extant Cape angiosperm species, 14-41% have been contributed by lineages that show distributional shifts consistent with past climate change, yet a similar proportion (14-55%) arose from lineages that shifted flowering phenology.
Conclusions
Adaptive changes in ecology at the scale we uncover in the Cape and consistent with past climatic change have not been documented for other floras. Shifts in climate tolerance appear to have been more important in this flora than is currently appreciated, and lineages that underwent such shifts went on to contribute a high proportion of the flora's extant species diversity. That shifts in phenology, on an evolutionary timescale and on such a scale, have not yet been detected for other floras is likely a result of the method used; shifts in flowering phenology cannot be detected in the fossil record
Emergent complex neural dynamics
A large repertoire of spatiotemporal activity patterns in the brain is the
basis for adaptive behaviour. Understanding the mechanism by which the brain's
hundred billion neurons and hundred trillion synapses manage to produce such a
range of cortical configurations in a flexible manner remains a fundamental
problem in neuroscience. One plausible solution is the involvement of universal
mechanisms of emergent complex phenomena evident in dynamical systems poised
near a critical point of a second-order phase transition. We review recent
theoretical and empirical results supporting the notion that the brain is
naturally poised near criticality, as well as its implications for better
understanding of the brain
Attitudes and behaviors of Japanese physicians concerning withholding and withdrawal of life-sustaining treatment for end-of-life patients: results from an Internet survey
<p>Abstract</p> <p>Background</p> <p>Evidence concerning how Japanese physicians think and behave in specific clinical situations that involve withholding or withdrawal of medical interventions for end-of-life or frail elderly patients is yet insufficient.</p> <p>Methods</p> <p>To analyze decisions and actions concerning the withholding/withdrawal of life-support care by Japanese physicians, we conducted cross-sectional web-based internet survey presenting three scenarios involving an elderly comatose patient following a severe stroke. Volunteer physicians were recruited for the survey through mailing lists and medical journals. The respondents answered questions concerning attitudes and behaviors regarding decision-making for the withholding/withdrawal of life-support care, namely, the initiation/withdrawal of tube feeding and respirator attachment.</p> <p>Results</p> <p>Of the 304 responses analyzed, a majority felt that tube feeding should be initiated in these scenarios. Only 18% felt that a respirator should be attached when the patient had severe pneumonia and respiratory failure. Over half the respondents felt that tube feeding should not be withdrawn when the coma extended beyond 6 months. Only 11% responded that they actually withdrew tube feeding. Half the respondents perceived tube feeding in such a patient as a "life-sustaining treatment," whereas the other half disagreed. Physicians seeking clinical ethics consultation supported the withdrawal of tube feeding (OR, 6.4; 95% CI, 2.5–16.3; P < 0.001).</p> <p>Conclusion</p> <p>Physicians tend to harbor greater negative attitudes toward the withdrawal of life-support care than its withholding. On the other hand, they favor withholding invasive life-sustaining treatments such as the attachment of a respirator over less invasive and long-term treatments such as tube feeding. Discrepancies were demonstrated between attitudes and actual behaviors. Physicians may need systematic support for appropriate decision-making for end-of-life care.</p

Influenza Research Database: An integrated bioinformatics resource for influenza virus research
The Influenza Research Database (IRD) is a U.S. National Institute of Allergy and Infectious Diseases (NIAID)-sponsored Bioinformatics Resource Center dedicated to providing bioinformatics support for influenza virus research. IRD facilitates the research and development of vaccines, diagnostics and therapeutics against influenza virus by providing a comprehensive collection of influenza-related data integrated from various sources, a growing suite of analysis and visualization tools for data mining and hypothesis generation, personal workbench spaces for data storage and sharing, and active user community support. Here, we describe the recent improvements in IRD including the use of cloud and high performance computing resources, analysis and visualization of user-provided sequence data with associated metadata, predictions of novel variant proteins, annotations of phenotype-associated sequence markers and their predicted phenotypic effects, hemagglutinin (HA) clade classifications, an automated tool for HA subtype numbering conversion, linkouts to disease event data and the addition of host factor and antiviral drug components.
