Knowledge, belief and misconceptions of the parents abaou fever
AMAÇ: Ebeveynlerin acil servise en sık başvuru nedenlerinden biri çocuklardaki ateştir. Ailelerin çoğu ateşi bir belirtiden çok hastalık olarak algılamaktadır. Bu çalışma bir eğitim ve araştırma hastanesi çocuk acil servisine başvuran olguların ebeveynlerine ateşi bir bulgu mu yoksa bir hastalık olarak mı gördükleri ve nasıl yönettiklerini saptama amacıyla yapılmıştır.
GEREÇ ve YÖNTEM: Çalışma, 10 Ocak-20 Şubat 2011 tarihleri arasında çocuk acil servise yüksek ateş şikayeti ile başvuran 238 olgunun ebeveynleri ile yapılmıştır. Verilerin toplanmasında yüksek ateşe ilişkin bilgi ve tutumlarının sorgulandığı toplam 28 veri içeren bir anket formu kullanılmıştır. Elde edilen veriler SPSS 18.0 paket programı ile değerlendirilmiştir.
BULGULAR: Araştırmadan elde edilen sonuçlara göre çocukların %86.1'ine hastaneye gelmeden önce ateş düşürücü bir işlem uygulandığı belirlenmiştir. Ailelerin %44.5'i ateşi düşürmenin doğrudan hastalığı da iyileştirmek demek olduğunu, sadece ateşi düşüremediği için hastaneye geldiği, hatta düşürmek için kolonya, alkol ve sirkeli su gibi yöntemleri sıklıkla uyguladıklarını belirtmişlerdir. Ebeveynlerin ateşe ilişkin bilgi düzeyi değerlendirildiğinde anne mesleği, eğitim düzeyi ve yaşı arasında ilişki gözlenmezken, baba yaşı ile bilgi düzeyi arasında ilişki görülmüştür.
SONUÇ: Eğitim seviyesi ve ateşle ilgili bilgi düzeyi arasında bir bağlantı olmadığı, yönetimin hatalı olduğu ortaya çıkmaktadır. Bu konuda kamuoyuna ateşin yararları ve zararları anlatılarak toplumu bilinçlendirmek gerekmektedir
Evaluation of premature newborn with patent ductus arteriosus
Prematüre bebeklerde, önemli soldan sağa şanta neden olan patent duktus arteriozus (PDA), artmış neonatal morbidite ve mortalite ile ilişkilidir. Bu çalışmada, hastanemiz yenidoğan yoğun bakım ünitesinde
izlenen ve PDA tanısı alan prematüre bebekler özellikle PDA boyutunun prognoza etkisi açısından geriye dönük
olarak değerlendirildi.
GEREÇ ve YÖNTEM: 2005-2009 yılları arasında Adnan Menderes Üniversitesi Tıp Fakültesi Yenidoğan
Yoğun Bakım Ünitesi'nde toplam 407 prematüre bebek izlendi ve 33 bebekte hemodinamik olarak anlamlı PDA
saptandı. PDA tanısı alan bebeklerin demografik verileri, klinik bulguları, tedaviye yanıtları ve gözlenen
komplikasyonlar incelendi.
BULGULAR: Ortalama gebelik yaşı 30,1±2,8 hafta ve ortalama doğum ağırlığı 1602±553 g idi. Prematüre
bebeklerde semptomatik PDA %8,1 sıklıkta saptandı. Olguların %27'sinde PDA spontan kapandı (hepsinde PDA
2,5 mm'nin altındaydı). 24 bebeğe ibuprofen(19 intravenöz, 5 per oral) verildi, 13' ünde (%54) kapanma gözlendi.
Cerrahi ligasyon, semptomatik PDA'lı olguların %12'sinde gerekti. PDA <2,5 mm olanlarda ligasyon gerekmedi,
PDA ≥2,5 mm olanların %24'üne ligasyon uygulandı. Ligasyon yapılan bir olguda şilotoraks ve bir olguda da
lokal enfeksiyon gözlendi.
SONUÇ: Bu çalışmada PDA tanısı alan prematüre bebeklerin klinik izlemlerine etki eden faktörler incelenmiştir.
PDA çapı <2,5 mm olanlarda prognozun daha iyi olduğu gözlenmiştir.OBJECTIVE: Patent ductus arteriosus (PDA) with significant left to right shunt is related to increased neonatal
morbidity and mortality in premature babies. In this retrospective study, premature newborns with PDA followed
in our unit were evaluated especially with regard to effect of PDA diameter on the outcome.
MATERIALS and METHODS: Premature babies (n=407; gestational age <37 weeks) followed in Adnan
Menderes University Faculty of Medicine NICU between years 2005-2009 were included in the study. Thirtythree
of them with haemodynamically significant (hs) PDA were determined. Demographic data, clinical
features, respond to therapy and complications were investigated.
RESULTS: Mean gestational age was 30.1±2.8 weeks and mean birth weight was 1602±553 g. Symptomatic
PDA incidence among premature babies was 8.1%. Spontaneous closure was observed in 27% of the cases (all
had ductus diameter of <2.5 mm). Twenty-four babies were given ibuprofen (19 intravenously, 5 per oral), closure
was obtained in 13 cases. Success rate of ibuprofen therapy was 54%. Surgical ligation was needed in 12% of the
cases. None of the cases with <2.5 mm PDA required surgery, whereas surgical ligation was performed in 24% of
cases with ≥2.5 mm PDA. Local infection in one case and chylothorax in another case were seen as surgical
complications.
CONCLUSION: Factors affecting the clinical outcome of premature infants suffering from PDA were
investigated in this study. Better prognosis was seen when ductus diameter is <2.5 mm
Pseudobartter and hypertriglyceridemia in an ınfant presenting with afebrile convulsion: a case of cystic fibrosis
Kistik fibrozis otozomal resesif geçiş gösteren, beyaz ırkta daha sık görülen multisistemik bir hastalıktır. Ülkemizdeki
gerçek sıklığı tam olarak bilinmemekle birlikte 1/2000-1/4000 arasında olduğu tahmin edilmektedir. Birçok sistemi
tutabilen kistik fibrozis değişik klinik tablolarla gelebilmektedir. Bu tablolardan biri olan psödo-Bartter sendromu sıcak
havalarda terle aşırı miktarda tuz kaybı ile birlikte daha sık görülebilmektedir. Elektrolit bozukluğuna bağlı konvülziyon,
ölüm gibi ciddi komplikasyonlara yol açabildiği için gözden kaçırılmaması gereken metabolik bir bozukluktur. Bu
yazıda, yaz mevsiminde acil servise dehidratasyon, konvülziyon ve uykuya eğilim tablosuyla gelen, hipertrigliseridemi,
hiponatremi, hipokalemi ve hipokloremik alkaloz saptanan 4 aylık bir olgu sunulmuştur. Bu vaka ile infantil dönemde
hiponatremiye bağlı konvülziyonun kistik fibrozisin geliş kliniği olabileceği vurgulanmak istenmiştir.Cystic fibrosis is a multisystemic disorder which is mostly seen in white people and its inheritance is autosomal recessive. The precise incidence is not known but it is estimated between 1/2000-1/4000. Cystic fibrosis can effect many of the organs, so it can be presented with different features. Pseudo-Bartter's syndrome is one of these features and can be with the salt loss in hot weather. In this paper, a four-month-old infant presented with dehydration, convulsion, dullness and detected hypertrigliceridemia, hyponatremia, hypocalemia, hypochloremia in the summer time. With this case we would like to emphasize that seizure due to sodium depletion can be a presentation of cystic fibrosis during infancy
Abusive head trauma: two case reports
Abusive head trauma is a serious form of child abuse and mostly seen in infants below the age of two years as a result of a strong shaking by the caregiver who aims to stop the infant’s crying. Characteristic symptoms include subdural hematomas, encephalopathy, retinal hemorrhages and fractures of the long bones. When physically examined, there are generally no externally visible signs. For this reason, it can be underdiagnosed, if it is not considered in the differential diagnosis. When the information provided from the parents is inconsistent and contradictory with the clinical picture of the patient, this form of abuse must be suspected and retinal hemorrhages should be searched. In this article, two patients who were admitted to our emergency department and diagnosed with physical child abuse are reported. One of these patients had a history of minor head trauma after falling down from the sofa and the other one had a history of breathlessness and loss of consciousness as a result of excessive crying
