Case study: ENVRI science demonstrators with D4Science
Whenever a community of practice starts developing an IT solution for its use case(s) it has to face the issue of carefully selecting âthe platformâ to use. Such a platform should match the requirements and the overall settings resulting from the specific application context (including legacy technologies and solutions to be integrated and reused, costs of adoption and operation, easiness in acquir- ing skills and competencies). There is no one-size-fits-all solution that is suitable for all application context, and this is particularly true for scientific communities and their cases because of the wide heterogeneity characterising them. However, there is a large consensus that solutions from scratch are inefficient and services that facilitate the development and maintenance of scientific community-specific solutions do exist. This chapter describes how a set of diverse communities of practice efficiently developed their science demonstrators (on analysing and pro- ducing user-defined atmosphere data products, greenhouse gases fluxes, particle formation, mosquito diseases) by leveraging the services offered by the D4Science infrastructure. It shows that the D4Science design decisions aiming at streamlin- ing implementations are effective. The chapter discusses the added value injected in the science demonstrators and resulting from the reuse of D4Science services, especially regarding Open Science practices and overall quality of service
Racial and Socioeconomic Disparities in Cancer-Related Knowledge, Beliefs, and Behaviors in Indiana
Background: This statewide survey examined differences in cancer-related knowledge, beliefs, and behaviors between racial and socioeconomic groups in select counties in Indiana.
Methods: A stratified random sample of 7,979 people aged 18â75 who lived in one of 34 Indiana counties with higher cancer mortality rates than the state average, and were seen at least once in the past year in a statewide health system were mailed surveys.
Results: Completed surveys were returned by 970 participants, yielding a 12% response rate. Black respondents were less likely to perceive they were at risk for cancer and less worried about getting cancer. Individuals most likely to perceive that they were unlikely to get cancer were more often black, with low incomes (less than 50,000 or more), or less than a high school degree. Black women were greater than six times more likely to be adherent to cervical cancer screening. Higher income was associated with receiving a sigmoidoscopy in the last 5 years and a lung scan in the past year. Those with the highest incomes were more likely to engage in physical activity. Both income and education were inversely related to smoking.
Conclusions: Socioeconomic and racial disparities were observed in health behaviors and receipt of cancer screening. Black individuals had less worry about cancer.
Impact: Understanding populations for whom cancer disparities exist and geographic areas where the cancer burden is disproportionately high is essential to decision-making about research priorities and the use of public health resources
Dynamical frustration in ANNNI model and annealing
Zero temperature quench in the Axial Next Nearest Neighbour Ising (ANNNI)
model fails to bring it to its ground state for a certain range of values of
the frustration parameter , the ratio of the next nearest neighbour
antiferromagnetic interaction strength to the nearest neighbour one. We apply
several annealing methods, both classical and quantum, and observe that the
behaviour of the residual energy and the order parameter depends on the value
of strongly. Classical or thermal annealing is found to be adequate
for small values of .
However, neither classical nor quantum annealing is effective at values of
close to the fully frustrated point , where the residual

Cluster Monte Carlo study of multi-component fluids of the Stillinger-Helfand and Widom-Rowlinson type
Phase transitions of fluid mixtures of the type introduced by Stillinger and
Helfand are studied using a continuum version of the invaded cluster algorithm.
Particles of the same species do not interact, but particles of different types
interact with each other via a repulsive potential. Examples of interactions
include the Gaussian molecule potential and a repulsive step potential.
Accurate values of the critical density, fugacity and magnetic exponent are
found in two and three dimensions for the two-species model. The effect of
varying the number of species and of introducing quenched impurities is also
investigated. In all the cases studied, mixtures of -species are found to
have properties similar to -state Potts models.Comment: 25 pages, 5 figure
Contribution of patient, physician, and environmental factors to demographic and health variation in colonoscopy follow-up for abnormal colorectal cancer screening test results
BACKGROUND:
Patient, physician, and environmental factors were identified, and the authors examined the contribution of these factors to demographic and health variation in colonoscopy follow-up after a positive fecal occult blood test/fecal immunochemical test (FOBT/FIT) screening.
METHODS:
In total, 76,243 FOBT/FIT-positive patients were identified from 120 Veterans Health Administration (VHA) facilities between August 16, 2009 and March 20, 2011 and were followed for 6 months. Patient demographic (race/ethnicity, sex, age, marital status) and health characteristics (comorbidities), physician characteristics (training level, whether primary care provider) and behaviors (inappropriate FOBT/FIT screening), and environmental factors (geographic access, facility type) were identified from VHA administrative records. Patient behaviors (refusal, private sector colonoscopy use) were estimated with statistical text mining conducted on clinic notes, and follow-up predictors and adjusted rates were estimated using hierarchical logistic regression.
RESULTS:
Roughly 50% of individuals completed a colonoscopy at a VHA facility within 6 months. Age and comorbidity score were negatively associated with follow-up. Blacks were more likely to receive follow-up than whites. Environmental factors attenuated but did not fully account for these differences. Patient behaviors (refusal, private sector colonoscopy use) and physician behaviors (inappropriate screening) fully accounted for the small reverse race disparity and attenuated variation by age and comorbidity score. Patient behaviors (refusal and private sector colonoscopy use) contributed more to variation in follow-up rates than physician behaviors (inappropriate screening).
CONCLUSIONS:
In the VHA, blacks are more likely to receive colonoscopy follow-up for positive FOBT/FIT results than whites, and follow-up rates markedly decline with advancing age and comorbidity burden. Patient and physician behaviors explain race variation in follow-up rates and contribute to variation by age and comorbidity burden. Cancer 2017;123:3502-12. Published 2017. Effect of Pimobendan in Dogs with Preclinical Myxomatous Mitral Valve Disease and Cardiomegaly: The EPIC Study - A Randomized Clinical Trial
Background: Pimobendan is effective in treatment of dogs with congestive heart failure (CHF) secondary to myxomatous mitral valve disease (MMVD). Its effect on dogs before the onset of CHF is unknown. Hypothesis/Objectives: Administration of pimobendan (0.4-0.6 mg/kg/d in divided doses) to dogs with increased heart size secondary to preclinical MMVD, not receiving other cardiovascular medications, will delay the onset of signs of CHF, cardiac-related death, or euthanasia. Animals: 360 client-owned dogs with MMVD with left atrial-to-aortic ratio >= 1.6, normalized left ventricular internal diameter in diastole >= 1.7, and vertebral heart sum >10.5. Methods: Prospective, randomized, placebo-controlled, blinded, multicenter clinical trial. Primary outcome variable was time to a composite of the onset of CHF, cardiac-related death, or euthanasia. Results: Median time to primary endpoint was 1228 days (95% CI: 856-NA) in the pimobendan group and 766 days (95% CI: 667-875) in the placebo group (P = .0038). Hazard ratio for the pimobendan group was 0.64 (95% CI: 0.47-0.87) compared with the placebo group. The benefit persisted after adjustment for other variables. Adverse events were not different between treatment groups. Dogs in the pimobendan group lived longer (median survival time was 1059 days (95% CI: 952-NA) in the pimobendan group and 902 days (95% CI: 747-1061) in the placebo group) (P = .012). Conclusions and Clinical Importance: Administration of pimobendan to dogs with MMVD and echocardiographic and radiographic evidence of cardiomegaly results in prolongation of preclinical period and is safe and well tolerated. Prolongation of preclinical period by approximately 15 months represents substantial clinical benefit
Which veterans enroll in a VA health information exchange program?
Objective: To characterize patients who voluntarily enrolled in an electronic health information exchange (HIE) program designed to share data between Veterans Health Administration (VHA) and non-VHA institutions.
Materials and Methods: Patients who agreed to participate in the HIE program were compared to those who did not. Patient characteristics associated with HIE enrollment were examined using a multivariable logistic regression model. Variables selected for inclusion were guided by a health care utilization model adapted to explain HIE enrollment. Data about patientsâ sociodemographics (age, gender), comorbidity (Charlson index score), utilization (primary and specialty care visits), and access (distance to VHA medical center, insurance, VHA benefits) were obtained from VHA and HIE electronic health records.
Results: Among 57â072 patients, 6627 (12%) enrolled in the HIE program during its first year. The likelihood of HIE enrollment increased among patients ages 50â64, of female gender, with higher comorbidity, and with increasing utilization. Living in a rural area and being unmarried were associated with decreased likelihood of enrollment.
Discussion and Conclusion: Enrollment in HIE is complex, with several factors involved in a patientâs decision to enroll. To broaden HIE participation, populations less likely to enroll should be targeted with tailored recruitment and educational strategies. Moreover, inclusion of special populations, such as patients with higher comorbidity or high utilizers, may help refine the definition of success with respect to HIE implementation
