Livelihood Risk from HIV in Semi-Arid Tropics of Rural Andhra Pradesh
This paper discusses the livelihood dynamics in the fragile landscape of the semi-arid tropics (SAT) of Andhra Pradesh. SAT is home to the poorest of the poor who live in conditions of persistent drought, subsistence agriculture and poor access to markets. This paper is a case study focusing particularly on labour migration, its role in influencing the health risk behaviour of migrants and in the spread of the HIV epidemic among SAT rural households. The most vulnerable population in these drought prone regions are the migrant labourers, and their vulnerability is influenced by three major factors—the vulnerability and unstable productivity in the degraded and marginal landscape, the caste system that has traditionally kept them backward and vulnerable, and experiences in the external environment to which they migrate. This study is based on a theoretical framework, whereby livelihood risks lead to health risks, particularly HIV infection—outlines the process that causes a further deterioration of the household and the occurrence of cyclical health risk. The paper calls for a multisectoral approach to tackle the issue of migrant vulnerability, and for interventions with a more migrant-need sensitive approach.

Experiments on the culture of Penaeus monodon in the salt pan area at Tuticorin
The paper embodies the results of growth and production of Penaeus monodon cultured during 1988-89 in the salt pan
area at Tuticorin. The overall growth rate of the hatchery produced seeds stocked in ponds varied from 25.1 mm/5.85
g to 35.2 mm/9.8g per month and the best growth was associated with low stocking density. The post-larvae attained
marketable size of 32 g in about 140 days. Supplementary feed consisted of 7-9 ingredients with protein value of 38%.
The conversion ratio varied from 1:1:5 to 1:1.91
Prospecting Period Measurements with LSST - Low Mass X-ray Binaries as a Test Case
The Large Synoptic Survey Telescope (LSST) will provide for unbiased sampling
of variability properties of objects with mag 24. This should allow for
those objects whose variations reveal their orbital periods (), such
as low mass X-ray binaries (LMXBs) and related objects, to be examined in much
greater detail and with uniform systematic sampling. However, the baseline LSST
observing strategy has temporal sampling that is not optimised for such work in
the Galaxy. Here we assess four candidate observing strategies for measurement
of in the range 10 minutes to 50 days. We simulate multi-filter
quiescent LMXB lightcurves including ellipsoidal modulation and stochastic
flaring, and then sample these using LSST's operations simulator (OpSim) over
the (mag, ) parameter space, and over five sightlines sampling a range
of possible reddening values. The percentage of simulated parameter space with
correctly returned periods ranges from 23 %, for the current baseline
strategy, to 70 % for the two simulated specialist strategies. Convolving
these results with a distribution, a modelled Galactic spatial
distribution and reddening maps, we conservatively estimate that the most
recent version of the LSST baseline strategy will allow determination
for 18 % of the Milky Way's LMXB population, whereas strategies that do
not reduce observations of the Galactic Plane can improve this dramatically to
32 %. This increase would allow characterisation of the full binary
population by breaking degeneracies between suggested distributions
in the literature. Our results can be used in the ongoing assessment of the

Secukinumab versus adalimumab for psoriatic arthritis: comparative effectiveness up to 48 weeks using a matching-adjusted indirect comparison
Secukinumab and adalimumab are approved for adults with active psoriatic arthritis (PsA). In the absence of direct randomized controlled trial (RCT) data, matching-adjusted indirect comparison can estimate the comparative effectiveness in anti-tumor necrosis factor (TNF)-naïve populations. Individual patient data from the FUTURE 2 RCT (secukinumab vs. placebo; N = 299) were adjusted to match baseline characteristics of the ADEPT RCT (adalimumab vs. placebo; N = 313). Logistic regression determined adjustment weights for age, body weight, sex, race, methotrexate use, psoriasis affecting ≥ 3% of body surface area, Psoriasis Area and Severity Index score, Health Assessment Questionnaire Disability Index score, presence of dactylitis and enthesitis, and previous anti-TNF therapy. Recalculated secukinumab outcomes were compared with adalimumab outcomes at weeks 12 (placebo-adjusted), 16, 24, and 48 (nonplacebo-adjusted). After matching, the effective sample size for FUTURE 2 was 101. Week 12 American College of Rheumatology (ACR) response rates were not significantly different between secukinumab and adalimumab. Week 16 ACR 20 and 50 response rates were higher for secukinumab 150 mg than for adalimumab (P = 0.017, P = 0.033), as was ACR 50 for secukinumab 300 mg (P = 0.030). Week 24 ACR 20 and 50 were higher for secukinumab 150 mg than for adalimumab (P = 0.001, P = 0.019), as was ACR 20 for secukinumab 300 mg (P = 0.048). Week 48 ACR 20 was higher for secukinumab 150 and 300 mg than for adalimumab (P = 0.002, P = 0.027), as was ACR 50 for secukinumab 300 mg (P = 0.032). In our analysis, patients with PsA receiving secukinumab were more likely to achieve higher ACR responses through 1 year (weeks 16-48) than those treated with adalimumab. Although informative, these observations rely on a subgroup of patients from FUTURE 2 and thus should be considered interim until the ongoing head-to-head RCT EXCEED can validate these findings. Novel methods to estimate antiretroviral adherence: protocol for a longitudinal study.
BackgroundThere is currently no gold standard for assessing antiretroviral (ARV) adherence, so researchers often resort to the most feasible and cost-effective methods possible (eg, self-report), which may be biased or inaccurate. The goal of our study was to evaluate the feasibility and acceptability of innovative and remote methods to estimate ARV adherence, which can potentially be conducted with less time and financial resources in a wide range of clinic and research settings. Here, we describe the research protocol for studying these novel methods and some lessons learned.MethodsThe 6-month pilot study aimed to examine the feasibility and acceptability of a remotely conducted study to evaluate the correlation between: 1) text-messaged photographs of pharmacy refill dates for refill-based adherence; 2) text-messaged photographs of pills for pill count-based adherence; and 3) home-collected hair sample measures of ARV concentration for pharmacologic-based adherence. Participants were sent monthly automated text messages to collect refill dates and pill counts that were taken and sent via mobile telephone photographs, and hair collection kits every 2 months by mail. At the study end, feasibility was calculated by specific metrics, such as the receipt of hair samples and responses to text messages. Participants completed a quantitative survey and qualitative exit interviews to examine the acceptability of these adherence evaluation methods. The relationship between the 3 novel metrics of adherence and self-reported adherence will be assessed.DiscussionInvestigators conducting adherence research are often limited to using either self-reported adherence, which is subjective, biased, and often overestimated, or other more complex methods. Here, we describe the protocol for evaluating the feasibility and acceptability of 3 novel and remote methods of estimating adherence, with the aim of evaluating the relationships between them. Additionally, we note the lessons learned from the protocol implementation to date. We expect that these novel measures will be feasible and acceptable. The implications of this research will be the identification and evaluation of innovative and accurate metrics of ARV adherence for future implementation
