Computed tomography measures of nutrition in patients with end-stage liver disease provide a novel approach to characterize deficits
Aim
Patients with cirrhosis and end-stage liver disease (ESLD) develop severe nutrition deficits that impact on morbidity and mortality. Laboratory measures of nutrition fail to fully assess clinical deficits in muscle mass and fat stores. This study employs computed tomography imaging to assess muscle mass and subcutaneous and visceral fat stores in patients with ESLD.
Methods
This 1:1 case-control study design compares ESLD patients with healthy controls. Study patients were selected from a database of ESLD patients using a stratified method to assure a representative sample based on age, body mass index (BMI), gender, and model for end-stage liver disease score (MELD). Control patients were trauma patients with a low injury severity score (<10) who had a CT scan during evaluation. Cases and controls were matched for age +/- 5 years, gender, and BMI +/- 2.
Results
There were 90 subjects and 90 controls. ESLD patients had lower albumin levels (p<0.001), but similar total protein levels (p=0.72). ESLD patients had a deficit in muscle mass (-19%, p<0.001) and visceral fat (-13%, p<0.001), but similar subcutaneous fat (-1%, p=0.35). ESLD patients at highest risk for sarcopenia included those over age 60, BMI< 25.0, and female gender. We found degree of sarcopenia to be independent of MELD score.
Conclusions
These results support previous research demonstrating substantial nutrition deficits in ESLD patients that are not adequately measured by laboratory testing. Patients with ESLD have significant deficits of muscle and visceral fat stores, but a similar amount of subcutaneous fat
Study on investment pattern and organizational structure of Super markets in India-An empirical evidence of Karnataka
Retail business has shown tremendous growth in the World in the recent past. In India, the organized food retailing sector is on the verge of a boom and expected to undergo further change with prospective new domestic and global foreign entrants and the takeover or exit of some existing participants. Among the Indian states, Karnataka is far more forward looking as far as food retailing is concerned. Therefore, it is imperative to study the entire business aspects of organized food retailing. The study indicated that majority of the organized supermarkets has multi outlets operating under the single management. The un-organized supermarkets were formed by the big local and conventional store owners. The average area of the supermarket outlets in Bangalore (4333 Sqft) and Mysore (3133 Sqft) is more compared to other cities of Karnataka like Mangalore (2833 Sqft), Hubli-Dharwad (1833 Sqft) and Belgaum (1200 Sqft) as the supermarkets in these cities were recently converted from the conventional ‘kirana’ format. The average total fixed cost per outlet is Rs. 78.42 lakh for the state as a whole and total costs required to establish a supermarket in the Karnataka is Rs. 261.33 lakh. The investments in establishment, modernization and expansion are capital intensive in supermarkets. In all the supermarkets, the liquid assets to total assets ratio indicated organized supermarkets seems to better than other unorganized supermarkets in terms of assets liquid ratio. Further, organized supermarkets/supermarkets in multi outlet (chain of supermarkets) firms/companies followed line, line and staff, and the functional categories of organizational structures. Doping Dependence of Thermal Oxidation on n-type 4H-SiC
The doping dependence of dry thermal oxidation rates in n-type 4H-SiC was
investigated. The oxidation was performed in the temperature range 1000C to
1200C for samples with nitrogen doping in the range of 6.5e15/cm3 to
9.3e18/cm3, showing a clear doping dependence. Samples with higher doping
concentrations displayed higher oxidation rates. The results were interpreted
using a modified Deal-Grove model. Linear and parabolic rate constants and
activation energies were extracted. Increasing nitrogen led to an increase in
linear rate constant pre-exponential factor from 10-6m/s to 10-2m/s and the
parabolic rate constant pre-exponential factor from 10e9m2/s to 10e6m2/s. The
increase in linear rate constant was attributed to defects from doping-induced
lattice mismatch, which tend to be more reactive than bulk crystal regions. The
increase in the diffusion-limited parabolic rate constant was attributed to
degradation in oxide quality originating from the doping-induced lattice
mismatch. This degradation was confirmed by the observation of a decrease in
optical density of the grown oxide films from 1.4 to 1.24. The linear
activation energy varied from 1.6eV to 2.8eV, while the parabolic activation
energy varied from 2.7eV to 3.3eV, increasing with doping concentration. These
increased activation energies were attributed to higher nitrogen content,
leading to an increase in effective bond energy stemming from the difference in
C-Si (2.82eV) and Si-N (4.26eV) binding energies. This work provides crucial
information in the engineering of SiO2 dielectrics for SiC MOS structures,
which typically involve regions of very different doping concentrations, and
suggests that thermal oxidation at high doping concentrations in SiC may be

Correlation between associating factors of obstructive airway disease with obstructive sleep apnoea
Background: Several researchers reported that OSA may contribute to asthma symptoms and severity. But, the relation between associating factors of obstructive airway disease with obstructive sleep apnoea was not well documented. Therefore, the present study has been undertaken to investigate the relation between associating factors of OAD with obstructive sleep apnoea.Methods: Pulmonary Function Test was done for assessing air flows and measuring absolute and predicted values of FEV1, FVC, FEV1/FVC and MMEF. PFT was interpreted as normal if FEV1/FVC >70 as per GOLD and >75 as per GINA, with normal FEV1 and normal FVC. Decreased FEV1, Decreased FVC, Decreased FEV1/ FVC, predicted (0.05). The Sleep Stages in the study group (OAD and No OAD group) was also statistically insignificant except for Stage III. The PFT-FEV1was found to be statistically significant (p=0.043) when compared between OAD group and in the No OAD group.Conclusions: Due to the consequences of the overlap syndrome, it is recommended to actively search for existence of OSA, and to treat it with continuous positive airway pressure (CPAP) concurrently with oxygen and optimal pharmacological treatment
Neuroprotective role of agmatine in prenatal acute ethanol exposure induce alterations in rats
The present study investigates the impact of agmatine, a neuromodulator with neuroprotective and anxiolytic properties, on behavioral changes associated with prenatal ethanol exposure in rats. The research focuses on the vulnerability of adolescents to alcohol-related problems and explores the potential link between prenatal alcohol exposure, anxiety, and adolescent alcohol use. The study also delves into the neurotoxic effects of ethanol on social behavior, cognitive function, and emotional regulation. The pregnant (GD12) Sprague Dawley rats were exposed to ethanol 2.5 g/kg, 20% v/v followed by a second i.p. injection of 1.25 g/kg ethanol and administered agmatine, along with its modulators, during adolescence. The results reveal that prenatal ethanol exposure induces behavioral changes such as increased locomotor activity, anxiety, social interaction deficits, and depression-like behavior. Agmatine administration, particularly at doses of 40 and 80 mg/kg, mitigates these effects, indicating its potential therapeutic role. Moreover, agmatine treatment improves recognition memory impaired by ethanol exposure and reduces oxidative stress, emphasizing its neuroprotective properties. In conclusion, the study suggests that agmatine holds promise in addressing the behavioural and neurochemical alterations induced by prenatal ethanol exposure during adolescence. The findings contribute to understanding the potential therapeutic capabilities of agmatine in mitigating the adverse consequences of early alcohol exposure on brain function and behaviour
Community based rehabilitation by paraplegic - a case report
A journey of a young man started from day one in the hospital. When he came with the traumatic paraplegia, no doubt a paraplegic has to face a lot of challenges in his life when he came to know that his both legs are not working. He became bedridden and his moral is also become down for his future planning. But we accept the challenge, a team work by an orthopaedician and a physio-occupational therapist make it little easier. Postoperatively we prefer to offer rehabilitation through community base programme. Inspiration of hospital environment and equipment are going to help him for such kind of programme he accepts the challenge, basic aim of providing this programme is to reduce duration as well as incurred monitor losses. Special attraction of this case report is homemade bicycle, which served the purpose of providing repetitive rhythmic mobilisation of bilateral lower limbs which facilitates normal pattern of ambulation. Now he is ambulatory and goes back to his work. So, a person with spinal cord injury involved with their community with a little modification in his activity and equipment is worth for him. Traumatic paraplegia offers a big challenge to physio- occupational therapist due to long term recovery it takes. For Such patients on regular follow up till complete recovery, we preferred community based rehabilitation. This made him independent and incurred reduced monitory losses
