Neuromusculoskeletal disorders in the neck and upper extremities among drivers of all-terrain vehicles – a case series
<p>Abstract</p> <p>Background</p> <p>The purpose of this study was to investigate whether professional drivers of all-terrain vehicles (ATVs) with neck pain have a different array of neuromusculoskeletal disorders in the neck and upper extremities than a referent group with neck pain from the general population. It is hypothesized that exposure to shock-type vibration and unfavorable working postures in ATVs have the capacity to cause peripheral nervous lesions.</p> <p>Methods</p> <p>This study was based on a case series analyzed according to a case-case comparison design. The study population consisted of 60 male subjects, including professional drivers of forest machines (n = 15), snowmobiles (n = 15), snowgroomers (n = 15) and referents from the general population (n = 15) all of whom had reported neck pain in a questionnaire and underwent an extensive physical examination of the neck and upper extremities. Based on symptom history, symptoms and signs, and in some cases chemical, electroneurographical and radiological findings, subjects were classified as having a nociceptive or neuropathic disorder or a mix of these types.</p> <p>Results</p> <p>The occurrence of asymmetrical and focal neuropathies (peripheral nervous lesion), pure or in a mix with a nociceptive disorder was common among cases in the ATV driver groups (47%–79%). This contrasted with the referents that were less often classified as having asymmetrical and focal neuropathy (27%), but instead had more nociceptive disorders. The difference was most pronounced among drivers of snowgroomers, while drivers of forest machines were more frequently classified as having a nociceptive disorder originating in the muscles.</p> <p>Conclusion</p> <p>This study found a high prevalence of assymetrical and focal neuropathies among drivers with pain in the neck, operating various ATVs. It seems as if exposure to shock-type whole-body vibration (WBV) and appurtenant unfavorable postures in ATVs may be associated to peripheral nervous lesions.</p
Dosing of Erythropoiesis-Stimulating Agents Can Be Reduced by a New Administration Regimen
Background/Aims:At our hemodialysis (HD) unit, we noted a drop in the treatment dose of erythropoietin-stimulating agents (ESAs) when the frequency of dose adjustment was reduced from weekly, where doses were withheld if hemoglobin was >130 g/l, to monthly, where doses were not withheld. The aim of this study was to find an explanation for this reduction in ESA requirement. Methods: This is a retrospective study on 18 stable HD patients. Comparable follow-up periods of 6 months with the two different ESA adjustment regimens were established and data on ESA dose, hemoglobin and known predictors of ESA response were collected. Results: With the new ESA administration regimen, a 22.5% drop in the total ESA dose was noted. The corresponding fall in the erythropoietin resistance index was 20.0%. Simultaneously, the dialysis dose and transferrin saturation increased significantly. However, in a multivariate linear regression model, changes in these factors did not significantly predict changes in ESA requirement. No relevant changes were noted in other erythropoiesis-modulating factors. Conclusion: Frequent dose adjustments and the current ESA administration practice of withholding ESA doses does not seem to reduce ESA demand. On the contrary, such practice is likely to increase ESA requirement over time
Decompositions of Gelfand-Shilov kernels into kernels of similar class
We prove that any linear operator with kernel in a Gelfand-Shilov space is a
composition of two operators with kernels in the same Gelfand-Shilov space. We
also give links on numerical approximations for such compositions. We apply
these composition rules to establish Schatten-von Neumann properties for such

Hemodiafiltration Improves Plasma 25-Hepcidin Levels: A Prospective, Randomized, Blinded, Cross-Over Study Comparing Hemodialysis and Hemodiafiltration
Background/Aims: Data from studies comparing the effect of hemodiafiltration (HDF) and conventional hemodialysis (HD) on clinically important outcomes are insufficient to support superiority of HDF. None of these studies has been participant-blinded. Methods: We performed a prospective, randomized, and patient-blinded cross-over study. Twenty patients on chronic HD received either HD for 2 months followed by post-dilution HDF for 2 months or in opposite order. A range of clinical parameters, as well as markers of inflammation, oxidative stress and iron metabolism was measured. Results: The two treatments were similar with respect to dialysis-related complications, quality of life, and the biomarkers of oxidative stress and inflammation. Compared to HD, 25-hepcidin and β2-microglobulin were 38 and 32%, respectively, lower after 60 days of HDF (p Conclusion: In short term, HDF is not superior to HD regarding dialysis-related complications. The higher ESA consumption observed with HDF can be explained by blood clotting in tubing and dialyzers, as more anticoagulation was needed with post-dilution HDF. In a longer perspective, lowering serum hepcidin levels may improve pathological iron homeostasis
Electromagnetic dispersion modeling and measurements for HVDC power cables
This paper provides a general framework for electromagnetic modeling, computation and measurements regarding the wave propagation characteristics of High-Voltage Direct Current (HVDC) power cables. The modeling is focused on very long (10 km or more) HVDC power cables and the relevant frequency range is therefore in the low-frequency regime of about 0-100 kHz. An exact dispersion relation is formulated together with a discussion on practical aspects regarding the computation of the propagation constant and the related characteristic impedance. Experimental time-domain measurement data from an 80 km long HVDC power cable is used to validate the model. It is concluded that a single-mode transmission line model is not adequate to account for the mismatch between the power cable and the instrumentation. A mismatch calibration procedure is therefore devised to account for the connection between the measurement equipment and the cable. A dispersion model is thus obtained that is accurate for early times of pulse arrival. To highlight the potential of accurate electromagnetic modeling, an example of high-resolution length-estimation is discussed and analyzed using statistical methods based on the Cramer-Rao lower bound. The analysis reveals that the estimation accuracy based on the present model (and its related model error) is in the order of 100 m for an 80 km long power cable, and that the potential accuracy using a perfect model based on the given measurement data is in the order of centimeters
Proteoglycans contribute to the functional integrity of the glomerular endothelial cell surface layer and are regulated in diabetic kidney disease
All capillary endothelia, including those of the glomeruli, have a luminal cell surface layer (ESL) consisting of glycoproteins, glycolipids, proteoglycans (PGs) and glycosaminoglycans. Previous results have demonstrated that an intact ESL is necessary for a normal filtration barrier and damage to the ESL coupled to proteinuria is seen for example in diabetic kidney disease (DKD). We used the principles of ion exchange chromatography in vivo to elute the highly negatively charged components of the ESL with a 1 M NaCl solution in rats. Ultrastructural morphology and renal function were analyzed and 17 PGs and hyaluronan were identified in the ESL. The high salt solution reduced the glomerular ESL thickness, led to albuminuria and reduced GFR. To assess the relevance of ESL in renal disease the expression of PGs in glomeruli from DKD patients in a next generation sequencing cohort was investigated. We found that seven of the homologues of the PGs identified in the ESL from rats were differently regulated in patients with DKD compared to healthy subjects. The results show that proteoglycans and glycosaminoglycans are essential components of the ESL, maintaining the permselective properties of the glomerular barrier and thus preventing proteinuria.publishedVersio
Classical signal model reproducing quantum probabilities for single and coincidence detections
We present a simple classical (random) signal model reproducing Born's rule.
The crucial point of our approach is that the presence of detector's threshold
and calibration procedure have to be treated not as simply experimental
technicalities, but as the basic counterparts of the theoretical model. We call
this approach threshold signal detection model (TSD). The experiment on
coincidence detection which was done by Grangier in 1986 \cite{Grangier} played
a crucial role in rejection of (semi-)classical field models in favor of
quantum mechanics (QM): impossibility to resolve the wave-particle duality in
favor of a purely wave model. QM predicts that the relative probability of
coincidence detection, the coefficient is zero (for one photon
states), but in (semi-)classical models In TSD the
coefficient decreases as where is
the detection threshold. Hence, by increasing this threshold an experimenter
can make the coefficient essentially less than 1. The
TSD-prediction can be tested experimentally in new Grangier type experiments
presenting a detailed monitoring of dependence of the coefficient
on the detection threshold
