PD-L1 expression is associated with the spontaneous regression of patients with methotrexate-associated lymphoproliferative disorders

Background Most patients with methotrexate-associated lymphoproliferative disorder (MTX-LPD) show diffuse large B-cell lymphoma (DLBCL) or classic Hodgkin lymphoma (CHL) types. Patients with MTX-LPD often have spontaneous remission after MTX discontinuation, but chemotherapeutic intervention is frequently required in patients with CHL-type MTX-LPD. In this study, we examined whether programmed cell death-ligand 1 (PD-L1) expression levels were associated with the prognosis of MTX-LPD after MTX discontinuation. Methods A total of 72 Japanese patients diagnosed with MTX-LPD were clinicopathologically analyzed, and immunohistochemical staining of PD-L1 was performed in 20 DLBCL-type and 24 CHL-type MTX-LPD cases to compare with the clinical course. Results PD-L1 was expressed in 5.0% (1/20) of patients with DLBCL-type MTX-LPD, whereas it was expressed in 66.7% (16/24) of the patients with CHL-type MTX-LPD in more than 51% of tumor cells. Most CHL-type MTX-LPD patients with high PD-L1 expression required chemotherapy owing to exacerbations or relapses after MTX discontinuation. However, no significant differences in clinicopathologic findings at diagnosis were observed between PD-L1 high- and low-expression CHL-type MTX-LPD. Conclusion PD-L1 expression was significantly higher in patients with CHL-type than DLBCL-type MTX-LPD, suggesting the need for chemotherapy in addition to MTX discontinuation in CHL-type MTX-LPD patients to achieve complete remission. No association was observed between PD-L1 expression levels and clinical findings at diagnosis, suggesting that PD-L1 expression in tumor cells influences the pathogenesis of CHL-type MTX-LPD after MTX discontinuation

Successful management of refractory pleural effusion due to systemic immunoglobulin light chain amyloidosis by vincristine adriamycin dexamethasone chemotherapy: a case report

<p>Abstract</p> <p>Introduction</p> <p>Refractory pleural effusion in systemic immunoglobulin light chain amyloidosis without cardiac decompensation is rarely reported and has a poor prognosis in general (a median survival of 1.6 months). Moreover, the optimum treatment for this condition is still undecided. This is the first report on the successful use of vincristine, adriamycin and dexamethasone chemotherapy for refractory pleural effusion due to systemic immunoglobulin light chain amyloidosis without cardiac decompensation.</p> <p>Case presentation</p> <p>We report the case of a 68-year old Japanese male with systemic immunoglobulin light chain amyloidosis presenting with bilateral pleural effusion (more severe on the right side) in the absence of cardiac decompensation that was refractory to diuretic therapy. The patient was admitted for fatigue, exertional dyspnea, and bilateral lower extremity edema. He had been receiving intermittent melphalan and prednisone chemotherapy for seven years. One month before admission, his dyspnea had got worse, and his chest radiograph showed bilateral pleural effusion; the pleural effusion was ascertained to be a transudate. The conventionally used therapeutic measures, including diuretics and thoracocentesis, failed to control pleural effusion. Administration of vincristine, adriamycin, and dexamethasone chemotherapy led to successful resolution of the effusion.</p> <p>Conclusion</p> <p>Treatment with vincristine, adriamycin, and dexamethasone chemotherapy was effective for the refractory pleural effusion in systemic immunoglobulin light chain amyloidosis without cardiac decompensation and appears to be associated with improvement in our patient's prognosis.</p

本研究の目的は，中学生の保健体育授業への動機づけにかかわる，フロー体験と基本的心理欲求の充足の関連について検討することである。研究Ⅰでは，フロー体験尺度および基本的心理欲求尺度を開発し，中学校１・２年生の合計４９２名を対象に，運動・保健体育授業への意識調査と合わせ，質問紙調査を実施した。因子分析の結果，フロー体験尺度では「フロー体験」の１因子，基本的心理欲求尺度では「有能感」「関係性」「自律性」の３因子で尺度が作成された。研究Ⅱでは，共分散構造分析によってこれらの関連を検討した。フロー体験は，運動や体育の授業への意欲に正の関連を示した。基本的心理欲求の３因子「有能感」「関係性」「自律性」はフロー体験に正の関連を示した。この結果から，体育授業における基本的心理欲求の充足とフロー体験の関連が明らかとなり，運動時のフロー体験が，運動や体育の授業への意欲向上を促すことが示唆された。The purposes of this study were to examine the motivation of junior high school students on physical education and the relation between the flow experience and the basic psychological needs. This study is separated two parts. In the first part of this study named ‘study 1’, the flow experience scale and the basic psychological needs scale were developed by ourselves. After that, 492students of junior high school was investigated by these developed questionnaire. From the results by factor analysis of these questionnaire, the flow experience scale was consisted with the “flow experience factor”, and the basic psychological needs scale was consisted with the “competence”, the “relationship” and the “autonomy”. In the second part of this study named ‘study 2’, the structural equation modeling has been indicated. Flow experience is a positive association was observed in the willingness to exercise or physical education class. Basic psychological needs 3 factors, “Competence”, “relationship”, “autonomy” had a positive relationship with the flow experience. As a result, the flow experiences during sports have been shown to lead to improvement in willingness to exercise or physical education. Interferon-alpha-induced mTOR activation is an anti-hepatitis C virus signal via the phosphatidylinositol 3-kinase-Akt-independent pathway.

OBJECT: The interferon-induced Jak-STAT signal alone is not sufficient to explain all the biological effects of IFN. The PI3-K pathways have emerged as a critical additional component of IFN-induced signaling. This study attempted to clarify that relationship between IFN-induced PI3-K-Akt-mTOR activity and anti-viral action. RESULT: When the human normal hepatocyte derived cell line was treated with rapamycin (rapa) before accretion of IFN-alpha, tyrosine phosphorylation of STAT-1 was diminished. Pretreatment of rapa had an inhibitory effect on the IFN-alpha-induced expression of PKR and p48 in a dose dependent manner. Rapa inhibited the IFN-alpha inducible IFN-stimulated regulatory element luciferase activity in a dose-dependent manner. However, wortmannin, LY294002 and Akt inhibitor did not influence IFN-alpha inducible luciferase activity. To examine the effect of PI3-K-Akt-mTOR on the anti-HCV action of IFN-alpha, the full-length HCV replication system, OR6 cells were used. The pretreatment of rapa attenuated its anti-HCV replication effect in comparison to IFN-alpha alone, whereas the pretreatment with PI3-K inhibitors, wortmannin and LY294002 and Akt inhibitor did not influence IFN-induced anti-HCV replication. CONCLUSION: IFN-induced mTOR activity, independent of PI3K and Akt, is the critical factor for its anti-HCV activity. Jak independent mTOR activity involved STAT-1 phosphorylation and nuclear location, and then PKR is expressed in hepatocytes

Modes of transmission and stability of Rice yellow mottle virus

Rice yellow mottle virus (RYMV) is the most important rice virus in Africa. We examined RYMV transmission via soil and water contaminated with RYMV-infected rice plants and by serial cutting with RYMV-contaminated scissors. Transmission of RYMV via dried rice straw kept at 27°C was also examined. The results showed the virus could be transmitted via soil and water, and by scissors. Rice straw that was RYMV-infected was not infective if it was dried and was kept longer than 42 days. By insect transmission experiments and ELISA, long-horned grasshoppers (Conocephalus spp.) were found to be a possible vector of RYMV in Uganda
